Cancer can pass between identical twins before birth when they share a placenta with connected blood vessels. Cells from a cancer-associated clone in one fetus may cross those connections and enter the other twin’s circulation. The best-established examples involve leukemia; a peer-reviewed 2026 report also describes a rare soft-tissue cancer that spread between twins in the womb. This is a transfer of cells, not ordinary inheritance from a parent—and it does not mean that every identical-twin pair shares cancer cells.
What “cancer passing” between twins means
The phrase can sound as if cancer behaves like an infection. It does not. In the cases described here, the proposed route is the movement of cells through blood-vessel connections in a shared placenta. A cancer-associated cell population that developed in one fetus can cross to the co-twin; it is not a contagious disease spreading through contact.
The cells may belong to a clone: a group descended from one original cell and carrying particular acquired genetic changes. If cells from that clone are found in both twins, they provide evidence that the twins shared a cellular ancestor before birth. The clone’s presence is not automatically the same thing as both babies having clinically apparent cancer. A shared prenatal clone and later, overt disease are distinct biological stages.
“Acquired” is important here. It describes changes that arose in cells during development, rather than a change present in the egg or sperm and passed down from a parent. The evidence points to movement of a population of cells between fetuses, not to cancer itself being inherited or transmitted like an ordinary infection.

Why the placenta matters
Some identical twins share a placenta; others do not
Identical, or monozygotic, twins originate from one fertilized egg that divides. Their placental arrangement depends on when that division occurs. Some monozygotic twins have a shared placenta, called a monochorionic placenta; others have separate placentas. So “identical twins” alone does not establish that a route for cell transfer existed.
In a monochorionic pregnancy, blood-vessel connections within the shared placenta can allow blood cells to move between the fetuses. If a preleukemic or cancerous clone forms in one fetus, cells may travel through those connections and enter the other twin’s bloodstream. The relevant feature is the shared circulation, not genetic identity by itself. A twin pair with separate placentas does not fit this particular mechanism merely because the babies are identical.
This is why placental anatomy matters when interpreting a report about twins. The fact that two children are monozygotic is not enough to infer that their fetal blood could mix. Conversely, a shared placenta supplies a possible route for cells to cross, but it does not mean that cancer cells necessarily formed, crossed, or caused disease in both babies. The proposed mechanism depends on the combination of a prenatal cell clone and vascular connections that permit exchange.

A cell clone can arise and cross the placenta before birth, while the timing of clinical cancer is a separate question. Additional mutations or other changes may accumulate later and help determine whether, when, and how leukemia becomes apparent in each child. Thus, finding a shared prenatal origin does not, by itself, prove that both newborns already had overt leukemia.
This distinction helps explain why the twins’ later outcomes need not be identical even when their cancer cells share an early origin. The initial clone is one part of the story; subsequent biological changes can differ. The evidence of a shared clone supports prenatal cell transfer, but it does not alone specify the exact course or timing of illness in either child.

In other words, there can be a shared beginning without identical disease histories. Cells descended from the same prenatal clone may be present in both twins, but later events can shape whether clinically apparent cancer develops and when. A genetic match between cancer cells is therefore evidence about their ancestry; it is not, on its own, a prediction of what will happen to either child.
How scientists identify a shared prenatal cancer clone
Leukemia: matching acquired genetic markers
In twin leukemia, researchers have found the same unusual, clone-specific genetic alterations in both twins’ leukemia cells. Such matching changes are consistent with a single prenatal clone arising in one fetus and cells from it reaching the other through placental blood-vessel connections. This is stronger evidence of a shared cellular origin than simply observing that both children developed the same broad type of cancer.
These alterations are acquired in the cell population; they are not necessarily germline changes inherited from a parent and present throughout both children’s bodies. The distinction matters: sharing cells from a prenatal clone is biologically different from inheriting a cancer predisposition through the egg or sperm. A 2024 review of cross-individual cancer transmission discusses evidence and proposed routes in gestational and perinatal settings, including twin leukemia (Arakawa et al., Cancer Science).
The logic is that the same rare, acquired marker in both twins’ cancer-cell populations is consistent with descent from one prenatal clone. By contrast, the fact that two children have leukemia in itself would not establish that their cells came from a common source. Matching clone-specific changes add evidence of shared ancestry and fit the known route through connected vessels in a monochorionic placenta.
A soft-tissue cancer: tracing the route across the placenta
Evidence beyond leukemia remains rare. In a paper published online on 6 October 2026, Walkowiak, Baskind, Dave and colleagues used whole-genome sequencing of multiple tumor, normal, and placental samples to reconstruct a soft-tissue cancer in a monozygotic twin pair. Their analysis traced the tumor’s origin to one twin, its diversification, and a transfer to the other twin across the placenta.

The authors report that the spread in this pair occurred only once. That is a finding about the reconstructed history of this case, not a rate or probability for twin pregnancies. The paper is an early-access accepted version; the publisher says it will be replaced by the final version of record. Its result broadens the documented evidence beyond leukemia, but a single reported pair does not show that solid tumors commonly pass between twins. This case and the leukemia examples provide different kinds of evidence. The leukemia literature describes shared acquired, clone-specific changes in the twins’ cancer cells, supporting a prenatal common origin. The 2026 report used whole-genome data from tumor, normal, and placental samples to reconstruct the sequence of events in one soft-tissue cancer case. Both support the possibility of cell transfer, but the solid-tumor report has a narrower scope: one reconstructed pair does not establish how often this occurs.

A 2018 review of cancer-cell transmission via the placenta summarized the then-known evidence by saying, “There is no evidence that paediatric solid tumour cells, though often of embryonic or foetal origin, spread between twins in this fashion.” That statement reflected the literature reviewed at the time; the 2026 report is newer evidence of a rare solid-tumor case, rather than proof that such spread is common. The review is available as “Cancer cell transmission via the placenta” in Evolution, Medicine, and Public Health.
Across these sources, no population-level statistic gives the incidence or probability of cancer-cell transfer specifically between identical twins. Case reports and genetic reconstructions can establish that transfer occurred in a particular pair, but they cannot by themselves tell a family how likely it is in another pregnancy. Figures for other phenomena, such as transmission from a pregnant parent to a fetus, should not be substituted for this missing twin-specific estimate.
It is also helpful to distinguish “possible” from “common.” The leukemia findings support a biological route and shared prenatal origins in described twin cases. The soft-tissue report shows that a solid tumor can also cross in a documented pair. Neither type of evidence provides a population-wide rate, and the 2026 paper’s finding that transfer occurred once refers to the reconstructed events in that pair, not to a measured frequency among pregnancies.

How to interpret a report about twins and cancer
When reading a claim that cancer “passed” between twins, separate four questions that are often blurred together:
- Was there a shared placenta? The proposed route depends on vascular connections; identical status alone does not answer this.
- What evidence links the tumors? Shared clone-specific genetic changes can support a common prenatal cell origin. In the 2026 solid-tumor case, whole-genome analysis of tumor, normal, and placental samples was used to reconstruct the history.
- What stage is being described? A prenatal clone may be present before either or both children have clinically apparent cancer. Later changes can contribute to overt disease.
- How far can the finding be generalized? Evidence from twin leukemia and one reconstructed solid-tumor case supports biological possibility, not a general frequency estimate or a claim that all identical twins share risk in the same way.
These distinctions also prevent a common misunderstanding: a shared cancer-cell clone does not show that one twin infected the other, nor does it automatically mean the twins inherited a cancer-causing change from a parent. It points to a possible transfer of cells between fetuses through placental circulation.

Does a shared cancer clone mean both twins already had cancer at birth?
No. The prenatal clone’s origin and transfer are distinct from the later onset of overt disease. Additional mutations or other changes can accumulate later and may affect whether and when leukemia becomes clinically apparent in each child.
Is cancer transfer between twins the same as inheriting cancer from a parent?
No. The twin cases discussed involve sharing cells from an acquired prenatal clone. That differs from inheriting a germline cancer-predisposition change through the egg or sperm, which would be present throughout the body.
Can a solid tumor pass between twins, or is this only known for leukemia?
Leukemia is the best-established setting in the twin literature. A 2026 whole-genome study reported one soft-tissue cancer reconstructed as originating in one twin and spreading across the placenta once to the other.
