An unruptured brain aneurysm does not automatically need an operation. Doctors balance the aneurysm’s estimated chance of rupturing against the risks of an intervention, while considering its size, location, shape, growth, symptoms, your age and health, family and medical history, and your preferences. Some aneurysms are followed with imaging; others are treated by microsurgical clipping, endovascular coiling, or flow diversion.
Monitoring and treatment decisions should be made with a neurovascular team experienced in aneurysm care. A sudden, severe headache or new stroke-like symptoms are emergencies, whether or not an aneurysm has previously been diagnosed.
What an unruptured brain aneurysm is
A cerebral (brain) aneurysm is a weakened area in a brain artery that bulges outward. “Unruptured” means it has not leaked or burst. Many are discovered incidentally when imaging is performed for another reason. Others are found while evaluating symptoms or a family or medical history.
The central decision is not simply whether an aneurysm exists, but whether treating it now is safer than observing it. The National Institute of Neurological Disorders and Stroke (NINDS) states plainly that “Not all cerebral aneurysms need treatment.” NINDS patient guidance describes observation as an option when immediate treatment is not warranted.

Aneurysms that have ruptured are a different emergency. A rupture can cause subarachnoid hemorrhage and requires urgent hospital assessment; the elective monitoring approach described below is for aneurysms that have not ruptured.
How clinicians find and characterize an aneurysm
Imaging is used to confirm an aneurysm, measure it, map its relationship to nearby arteries, and look for changes over time. The AHA/ASA summary identifies CT angiography and MR angiography as the most sensitive methods for follow-up of unruptured aneurysms. Its clinical summary should be interpreted alongside advice from your treating specialists.
| Test | What it contributes | How it is commonly used |
|---|---|---|
| CT angiography (CTA) | Produces images of blood vessels so clinicians can identify and measure an aneurysm. | Detection and surveillance of an unruptured aneurysm, when appropriate for the patient. |
| MR angiography (MRA) | Shows the cerebral arteries and the aneurysm without using a catheter. | Detection and repeat imaging when clinicians select MR-based follow-up. |
| Catheter cerebral angiography | Provides detailed information about the aneurysm’s exact location, size, shape, and connected vessels. | Used when the team needs highly detailed vascular anatomy, often to clarify diagnosis or plan treatment. |
Your clinician will choose a test according to the question that needs answering, previous images, your health, and the information required for a possible procedure. Bring earlier scans when changing hospitals or specialists so the team can compare studies rather than relying on memory or a single report.

Who may be observed
Some very small, asymptomatic aneurysms may be monitored when the specialist judges that the near-term rupture risk is low and the risk of intervention is greater. “Small” does not mean “always safe,” and “large” does not mean “always treated.” Location, shape, growth, symptoms, and personal risk factors can change the balance.

What monitoring involves
Observation generally combines repeat vascular imaging with clinical review. The team looks for enlargement, a change in shape, new symptoms, or a change in your health that alters the treatment calculation. Keep a personal record of scan dates, the reported measurements, symptoms, blood-pressure discussions, and the name of the clinician responsible for follow-up.
Why there is no universal scan interval
There is no single follow-up schedule that fits every unruptured aneurysm. The appropriate timing depends on the aneurysm’s characteristics, the quality and comparability of previous images, your medical circumstances, and the specialist’s assessment. Do not assume that an appointment interval used for another patient applies to you.
NICE specifically advises that, after aneurysmal hemorrhage, follow-up imaging frequency and duration should take account of the initial treatment, other aneurysms, estimated risk of further bleeding, risks of additional investigations or intervention, and the person’s preference. NICE recommendations are UK guidance and do not establish one schedule for everyone. For an unruptured aneurysm, ask your team to document when the next review is due and what change would trigger earlier assessment.
What happens if the aneurysm enlarges
Growth is an important change because it can increase concern about future rupture. The AHA/ASA guideline summary says aneurysms with documented enlargement should be offered treatment when there are no comorbidities that change the decision. This is guideline context, not an automatic instruction: your team still has to weigh the anatomy, procedural risk, other illnesses, and your preferences. The summary refers to the 2015 guideline and should be considered with current specialist advice.

How specialists decide whether treatment is safer than observation
NICE recommends review of a non-culprit unruptured aneurysm by a multidisciplinary team that includes an interventional neuroradiologist and a neurosurgeon. The team should discuss the estimated lifetime rupture risk, the risk of each treatment, relevant illnesses, and your preferences. NINDS also identifies age, general health, family and medical history, and aneurysm characteristics as factors. NINDS and the NICE recommendations explain this individualized approach.
| Factor | Why it matters to the discussion |
|---|---|
| Size and location | They influence estimated rupture risk and whether a procedure can be performed safely. |
| Shape and neck anatomy | Irregular features or a wide connection to the artery can affect which technique is feasible. |
| Documented growth | Enlargement can shift the balance toward treatment. |
| Symptoms | Symptoms attributed to an aneurysm may make observation less suitable. |
| Age, health, and comorbidities | These affect both lifetime rupture risk and the chance of complications from anesthesia or the procedure. |
| Family and medical history | Prior aneurysmal hemorrhage or relevant family history can change the assessment of risk. |
| Your preferences | Some people prioritize avoiding an invasive procedure; others prioritize reducing a long-term rupture concern after discussing realistic risks. |
| Center experience | AHA/ASA guidance recommends treatment at high-volume centers with experience in aneurysm care. |
Ask the team to explain the estimated rupture risk in your particular case, the serious complications that could occur with each option, and what monitoring would look like if you defer treatment. A second opinion from another experienced neurovascular center can be reasonable when the decision is uncertain.
The three main treatment approaches
Clipping, coiling, and flow diversion are clinician-performed procedures. They are not interchangeable consumer treatments, and no method is best for every aneurysm. Anatomy and patient characteristics determine which options are technically possible and medically sensible.

During open surgery, a neurosurgeon places a small metal clip across the aneurysm’s neck so blood can no longer enter the bulge. NINDS describes clipping as potentially effective depending on the aneurysm’s location, size, and shape. It requires an operation and carries procedural risks. Aneurysms that are completely clipped generally do not recur, although your team may still recommend imaging based on the overall situation.

Endovascular coiling
For coiling, an interventional specialist guides a catheter through an artery to the aneurysm and places detachable platinum coils inside it. The coils reduce blood flow into the sac and promote clotting. Coiling is less invasive than open clipping, but a coiled aneurysm can sometimes recur and may need another procedure. Follow-up imaging is therefore part of the discussion.
Flow diversion
A flow-diverting device is a small stent-like implant placed in the parent artery. It redirects blood away from the aneurysm so the aneurysm can seal over time. NINDS describes flow diversion for very large aneurysms and for aneurysms not amenable to clipping or coiling. Suitability depends on the artery, aneurysm anatomy, and your health; the treating specialist must explain device-specific risks and medication requirements.
| Approach | How it works | Important trade-offs to discuss |
|---|---|---|
| Clipping | Open surgery places a clip across the aneurysm neck. | More invasive than catheter procedures; complete clipping generally prevents recurrence, but surgery has procedural risks. |
| Coiling | Catheter-delivered platinum coils reduce flow and encourage clotting inside the aneurysm. | Less invasive than clipping; recurrence and retreatment are possible, so imaging follow-up may be needed. |
| Flow diversion | A stent-like device redirects blood along the parent artery. | Useful for selected large or otherwise difficult aneurysms; anatomy and patient-specific factors determine suitability. |
The AHA/ASA 2023 guideline addresses aneurysmal subarachnoid hemorrhage rather than routine elective management of an incidental unruptured aneurysm. It recommends that specialists with endovascular and surgical expertise evaluate treatment risks and benefits according to the patient and aneurysm. Read the 2023 guideline for the ruptured-aneurysm context.

Blood pressure
High blood pressure is a modifiable concern identified in guidance on unruptured aneurysms. Work with your healthcare professional on measurement, medication, activity, diet, and follow-up targets that are appropriate for you. A home blood-pressure monitor can be an optional way to record readings for a clinician; the cited guidance does not establish that a particular device or home monitoring itself treats an aneurysm or prevents rupture.

Smoking and stimulant drugs
NINDS advises people with unruptured aneurysms to make a plan to stop smoking and to avoid drugs such as cocaine. Smoking and hypertension are identified as modifiable risk factors in AHA/ASA materials. Ask for evidence-based cessation support, medication options, and help with substance use if needed. Lifestyle changes reduce important health risks but do not eliminate the possibility of aneurysm growth or rupture.
Symptoms that require emergency help
Call your local emergency number immediately for a sudden, severe headache—often described as a “thunderclap” headache—or for new weakness or numbness on one side, trouble speaking, or loss or blurring of vision. These symptoms can indicate bleeding or another stroke emergency. The NHS advises not driving yourself to emergency care; use emergency medical services or have someone call for help. NHS emergency guidance is written for the UK, so use your local emergency number elsewhere.
Do not wait for a scheduled scan, take a consumer product marketed as an aneurysm treatment, or attempt to determine at home whether symptoms are serious. Emergency clinicians can perform the appropriate examination and imaging.

- Where exactly is the aneurysm, and what are its measured size, shape, and neck characteristics?
- Has it changed compared with every prior scan, and are the images directly comparable?
- What is my estimated rupture risk over the relevant time period, and which factors drive that estimate?
- What is the serious complication risk of clipping, coiling, or flow diversion for my anatomy?
- If observation is recommended, what imaging test will be used, when is the next review, and what symptoms or scan changes should prompt an earlier visit?
- Would another specialist or a high-volume aneurysm center provide a useful second opinion?
- Could any of my medicines, blood-pressure readings, smoking, or other health conditions affect the plan?
- If treatment is recommended, why is that method preferred and what follow-up imaging will be required?
- What would make you change from monitoring to treatment?
- Which emergency symptoms should my family recognize, and whom should they call?
What to expect from shared decision-making
A good consultation should leave you with a clear explanation of the competing risks, alternatives, and uncertainties. Ask for the plan in writing, including the next imaging date or the reason a particular interval was selected. Tell the team about prior brain bleeding, relatives with aneurysms, other medical conditions, pregnancy plans when relevant, medicines, and your priorities. Decisions can be revisited if the aneurysm grows, symptoms develop, your health changes, or better anatomical information becomes available.
