Astronauts begin readjusting to gravity on landing day, but recovery does not happen all at once. The return to Earth can challenge blood-pressure regulation, balance, coordination, movement, muscle strength, aerobic fitness, and bone health. NASA says most astronauts regain their preflight fitness baseline within 45 days; bone mineral density can take much longer, and some crew members need extended rehabilitation.
Why returning to gravity affects several body systems
On Earth, gravity constantly loads the body. When a person stands, it also pulls fluid toward the lower body, so the cardiovascular system must adjust blood pressure to keep the person functioning upright. In microgravity, fluids shift toward the head and the body adapts to reduced loading. After a long spaceflight, returning to Earth means adapting again—to gravity, upright posture, and the physical demands that come with them.
These changes are related, but they are not a single condition with one symptom or recovery clock. An astronaut may regain strength or aerobic fitness while still needing time to recover bone mineral density. Another may have difficulty with balance or standing even as other abilities return. NASA does not establish one universal sequence of symptoms or a day-by-day schedule that applies to every astronaut.
Blood pressure and standing
NASA calls difficulty maintaining blood pressure when standing after spaceflight postflight orthostatic intolerance. It can cause lightheadedness or fainting. The practical issue is not simply that the astronaut has landed: the body must once again manage the effects of gravity when upright. Some returning astronauts experience these symptoms, but they are not inevitable for every crew member.

This is why standing is a distinct challenge from lying down or sitting. When the astronaut is upright, the cardiovascular system has to regulate blood pressure in the presence of gravity’s pull on fluid. Orthostatic intolerance describes difficulty with that regulation; it does not mean that every returning astronaut faints, nor does it describe all the other changes that may follow a mission.
Balance, orientation, and movement
Gravity transitions also affect spatial orientation, balance, locomotion, and coordination between the eyes, head, and hands. A returning astronaut may need to reacquire a dependable sense of body position and movement while standing and moving on Earth. Some crew members experience motion sickness. These effects help explain why an astronaut may need a supervised return to ordinary movement even when able to perform some tasks.
Balance and orientation are related but different parts of the adjustment. Spatial orientation concerns interpreting where the body is and how it is moving; balance and locomotion matter when controlling posture and walking. Eye-hand and head-eye coordination are also among the functions affected by the transition. NASA includes the ability to sense body position and movement among the areas its reconditioning program works on, rather than treating the return as a matter of strength alone.

Long-duration spaceflight can affect aerobic capacity, muscle strength, and bone strength. NASA reports that weight-bearing bones lose an average of 1% to 1.5% of mineral density per month during spaceflight. That is an average reported for spaceflight, not a prediction for a particular astronaut or a guaranteed loss rate for every bone.
Functional fitness and bone mineral density should not be treated as interchangeable measures. NASA says many crew members may need about a year for bone mineral density to return to preflight values, and it cautions that rehabilitation may not completely correct bone loss. An astronaut can therefore reach a preflight fitness baseline without every physiological measure having returned to its preflight value.
The comparison matters because “recovery” can refer to several endpoints. A fitness baseline is assessed through measures such as aerobic fitness, agility, strength, and endurance. Functional ability concerns safely performing everyday movement, while mission-task capability is another distinct measure. Bone mineral density is a separate physiological measure with a potentially longer recovery period. Reaching one milestone does not automatically establish that all the others have been reached.

What happens when astronauts return
Recovery starts on the first day back, not after a fixed waiting period. NASA describes medical evaluation and repeat testing against preflight baselines. Before flight, those baselines can include aerobic fitness, agility, strength, and endurance. Comparing postflight results with those earlier measurements helps specialists assess what has changed and tailor a suitable program.
NASA describes the post-mission process as phased and individualized. It begins with crew egress at landing, and its goals include a safe return to functional ability, pre-mission fitness, and flight readiness. The astronaut’s condition, test results, medical status, mission type, and mission duration all inform the plan. Astronaut Strength and Conditioning specialists tailor and monitor progressive exercise rather than applying one identical routine to everyone.
In practical terms, the process has a sequence: establish a preflight reference, assess the astronaut after landing, compare test results and medical status with that reference, and then progress exercise in response to the individual’s needs. It is not simply a set number of days of exercise followed by an assumption that recovery is complete. Specialist monitoring lets the program be adjusted or extended when the astronaut’s status calls for it.
What the program works on
Depending on individual needs, the program can address:
- Mobility and basic movement, including the ability to walk and move safely.
- Balance, flexibility, and the ability to sense body position and movement.
- Aerobic conditioning, strength, and endurance.
These are training targets, not a checklist that every astronaut must complete in the same order or at the same pace. Medical evaluation and testing guide progression. NASA’s description is of an astronaut-specific, monitored program; it is not a consumer rehabilitation plan for readers to copy.

The range of targets reflects the range of demands involved in returning to Earth. Basic ambulation addresses movement in gravity; aerobic conditioning and endurance address sustained physical capacity; strength addresses muscular demands; and balance, flexibility, and proprioception support controlled movement. NASA does not present these as isolated problems with one universal sequence. They are parts of a phased program, progressed according to medical status and test results.

How long does recovery take?
There is no single recovery duration that covers every system. NASA’s public guidance says most astronauts regain their preflight fitness baseline within the first 45 days, with extensions when needed. That figure concerns the fitness baseline; it does not mean every symptom has ended or that all body systems, including bone, have returned to preflight condition.
| Period | What NASA describes | How to interpret it |
|---|---|---|
| Landing day | Medical evaluation and the start of postflight reconditioning, with testing compared with preflight baselines. | Recovery begins immediately, but the plan and progression are individualized. |
| First 45 days | NASA says most crew members reach their preflight fitness baseline within this period; extensions may be needed. | This is a general fitness milestone, not a guarantee of complete physiological recovery for every astronaut. |
| About a year for many crew members | NASA’s exercise overview says many may take this long to recover bone mineral density to preflight values. | Bone recovery can continue beyond the typical fitness-reconditioning period; rehabilitation may not completely correct bone loss. |
The timeline is therefore best understood by measure, not as a countdown to feeling exactly as before flight. A fitness baseline, safe functional movement, mission-task capability, and bone mineral density describe different outcomes. The sources do not establish a universal date when every astronaut is symptom-free or every physiological measure has recovered.
The distinction also helps answer “How long does it take astronauts to recover after returning from space?” The supported general answer is that the program starts on landing day, most regain a preflight fitness baseline within 45 days, and bone mineral density may take many crew members about a year to return to preflight values. Those timeframes refer to different outcomes. Neither should be used as a promise about an individual astronaut or as a date by which all effects must have ended.

NASA’s current public FAQ, updated in 2025, says most astronauts recover their preflight fitness baseline within the first 45 days and that the program can be extended as needed. NASA’s standards overview describes a rehabilitation period of up to 45 days for aerobic capacity and muscle strength, while separately noting that bone mineral density can take many crew members about a year to recover. Read together, these statements point to a bounded fitness goal, not a universal deadline for total recovery.
A separate historical detail should not be mistaken for a present-day prescription. A 2011 NASA Technical Reports Server abstract described a U.S. astronaut reconditioning program scheduled for two hours per day, seven days per week, for 45 days. It said the program was tailored to the astronaut and that early activities emphasized basic ambulation, cardiovascular endurance, strength, balance, flexibility, and proprioception—the sense of body position and movement. That schedule belongs to the program described in the 2011 abstract; it does not establish a universal current schedule for every astronaut.
It is useful to separate that historical schedule from NASA’s broader current public description. The 2011 abstract gives a specific reported program structure; the current guidance describes individualized, phased reconditioning and a general 45-day fitness milestone. The historical two-hours-a-day schedule should not be presented as the prescription followed by every astronaut today.

Why two astronauts may recover at different rates
NASA notes that the effects of spaceflight vary with personal factors and mission conditions. Mission type and duration are among the factors that inform reconditioning, alongside an astronaut’s medical status, testing, and observed condition. A plan can therefore change as the astronaut progresses rather than follow a fixed timetable set solely by the number of days since landing.

Different recovery measures also respond on different timelines. The 45-day fitness baseline and the longer bone-density recovery period are examples of why it is misleading to label an astronaut simply “recovered” or “not recovered” without saying what is being measured. Functional ability, physical conditioning, symptoms such as dizziness, and bone mineral density are not equivalent endpoints. NASA’s material does not support assuming that every astronaut will experience every listed effect, or that one individual’s course predicts another’s.
Mission duration is relevant because the body has spent that mission adapting to microgravity and reduced loading, but duration alone does not determine one exact recovery schedule. Personal factors, mission conditions, medical status, test results, and progression all matter. NASA’s approach is to tailor and monitor the program, not infer a universal recovery date from a single factor.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What this means for readers
For a reader wondering why astronauts can feel unsteady or dizzy after returning from space, the short answer is that the body has adapted to microgravity and must readjust to Earth’s gravitational loading. Blood-pressure regulation when standing, balance, coordination, and movement may all be involved. Astronauts are assessed and guided through a monitored, individualized program; the public timelines describe broad outcomes, not a self-treatment protocol or a promise about any one person.
NASA’s official FAQ summarizes the approach this way: “Recovery begins the first day back on Earth, with medical evaluations and progression through exercises that improve mobility, balance, flexibility, aerobic conditioning, strength, endurance, and the body’s ability to sense its position and movement.” The explanation is NASA’s description of postflight reconditioning, rather than a quotation attributed to a named individual.
Frequently Asked Questions
Why do astronauts feel dizzy when they return to Earth?
In microgravity, fluids shift toward the head and the body adapts to reduced loading. On Earth, gravity again pulls fluid downward when a person stands, and the cardiovascular system must maintain blood pressure upright. NASA calls difficulty doing so postflight orthostatic intolerance; it can cause lightheadedness or fainting. Some astronauts experience it, but it is not inevitable.

How long does it take astronauts to recover after a long spaceflight?
Recovery begins on landing day. NASA says most astronauts regain their preflight fitness baseline within the first 45 days, though programs can be extended. Bone mineral density follows a different timeline: NASA says many crew members may take about a year to return to preflight values.
Does the 45-day milestone mean an astronaut is completely recovered?
No. The 45-day figure refers to a general preflight fitness baseline, including goals related to aerobic capacity and muscle strength. It does not guarantee that every symptom has ended or that bone mineral density and every other physiological measure have returned to preflight values.
Do all astronauts experience dizziness, motion sickness, or balance problems?
No. NASA describes these as possible effects, not a checklist every crew member experiences. Individual outcomes vary with personal factors, mission conditions, medical status, and other factors; NASA does not establish one universal symptom sequence.
What does postflight reconditioning involve?
NASA describes medical evaluation and repeat testing against preflight baselines, followed by individualized, progressive exercise tailored and monitored by specialists. Depending on need, it can work on mobility, balance, flexibility, aerobic conditioning, strength, endurance, and sensing body position and movement.
Why can bone recovery take longer than fitness recovery?
They are different measures with different timelines. NASA reports an average loss of 1% to 1.5% of weight-bearing bone mineral density per month during spaceflight, and says many crew members may take about a year to recover bone mineral density to preflight values. Rehabilitation may not completely correct bone loss.
The Bottom Line
Astronauts start readjusting on landing day through medical evaluation and individualized, monitored reconditioning. NASA says most regain their preflight fitness baseline within 45 days, but that is not a deadline for complete recovery: balance and standing can pose challenges, and bone mineral density may take many crew members about a year to return to preflight values. The right timeline depends on the astronaut, the mission, and the measure being assessed.
