APOE ε4 is inherited as one of the two APOE alleles you receive from your biological parents. It is associated with a higher risk of late-onset Alzheimer’s disease, but it is not a diagnosis and does not reliably predict whether—or when—you will develop the disease. One ε4 copy is generally associated with more risk than some other APOE combinations, and two ε4 copies with greater risk than one. Many ε4 carriers never develop Alzheimer’s, and many people diagnosed with Alzheimer’s do not carry ε4.
Genetic testing can identify your APOE genotype, usually by examining two DNA markers called rs429358 and rs7412. The result can help describe an inherited susceptibility, but it must be interpreted alongside age, family history, ancestry, health, lifestyle and environmental factors. Professional counseling before and after testing is especially important if the result could affect medical decisions, family conversations or emotional wellbeing.
What APOE4 means
APOE is the gene that provides instructions for apolipoprotein E, a protein involved in transporting fats and cholesterol. The gene has several common forms, called alleles: ε2, ε3 and ε4. Every person has two APOE copies, with one copy inherited from each biological parent. A genotype is written as a pair, such as ε3/ε3, ε3/ε4 or ε4/ε4.

APOE ε4 is a risk allele for late-onset Alzheimer’s disease. A risk allele changes the likelihood of a condition but does not cause it in a predictable, all-or-nothing way. MedlinePlus Genetics summarizes the distinction this way: “people with the APOE e4 allele inherit an increased risk of developing Alzheimer’s disease, not the disease itself.”

Late-onset Alzheimer’s generally refers to disease beginning in later adulthood. Its inheritance pattern is not established as a simple dominant or recessive pattern. APOE is one influence among many genetic, health and environmental factors, not a complete explanation of an individual’s outcome.
Is APOE4 inherited from your mother or father?
It can come from either biological parent. Each parent has two APOE alleles and passes one of them to each child. The child’s genotype combines the allele received from the mother with the allele received from the father.
If one parent has one ε4 allele
A parent with ε3/ε4, for example, has one ε4 copy and one ε3 copy. For each pregnancy, that parent can pass either allele. The chance of passing ε4 from that parent is approximately 50 percent; the chance of passing the other allele is approximately 50 percent. The other biological parent’s genotype determines the child’s final pair.

A parent with ε4/ε4 has only ε4 copies to transmit, so every biological child inherits an ε4 allele from that parent. The child may still have ε3/ε4, ε2/ε4 or ε4/ε4, depending on the allele transmitted by the other parent.

Why a parent’s result is not a certain prediction for a child
Knowing that a parent carries ε4 does not establish that every child will carry it, unless that parent has two ε4 copies. Even when a child inherits ε4, the result indicates susceptibility rather than certainty. Conversely, a child who does not inherit ε4 can still develop Alzheimer’s because other genetic and non-genetic factors contribute to risk.
The FDA review describing APOE testing lists possible two-allele combinations, but it does not provide a complete family-pedigree probability table for every parental pairing. A genetic counselor can calculate a specific family’s transmission possibilities from both parents’ confirmed genotypes.
APOE4 versus genes that directly cause some early-onset familial Alzheimer’s
The word “inherited” can refer to two very different situations. APOE ε4 is a common susceptibility allele associated mainly with late-onset risk. Rare disease-causing variants in APP, PSEN1 or PSEN2 can cause some forms of early-onset familial Alzheimer’s disease and may follow an autosomal dominant pattern.

When a biological parent carries a confirmed disease-causing variant in one of those genes, the National Institutes of Health states that each child has a 50 percent chance of inheriting the variant. That 50 percent figure applies to transmission of a specific rare variant; it is not the probability that a child inherits APOE ε4. It also should not be used to promise an identical age of onset or course for every carrier.

| Feature | APOE ε4 | Rare APP, PSEN1 or PSEN2 disease-causing variant |
|---|---|---|
| Role | Risk or susceptibility factor for late-onset Alzheimer’s | Can be a direct cause of some early-onset familial Alzheimer’s |
| Typical inheritance interpretation | Not a simple, certain disease-prediction pattern | Often autosomal dominant in families with a confirmed pathogenic variant |
| Transmission example | A parent with one ε4 copy can pass it or the other APOE allele | A parent carrying the relevant variant has a 50% chance of passing it to each child |
| Meaning of a positive result | Changed susceptibility; not a diagnosis | Requires specialist interpretation because disease risk and age of onset depend on the specific variant and family context |
These categories should not be conflated. A family history of dementia may justify a clinical genetics appointment, but it does not by itself show that APOE ε4 or an autosomal-dominant variant is present.
What does an APOE genetic test report?
An APOE test commonly examines the DNA variants rs429358 and rs7412. The combination of the two markers is used to determine whether each APOE copy is ε2, ε3 or ε4. A laboratory report may therefore list a genotype such as ε3/ε3, ε3/ε4 or ε4/ε4 and then describe an association with relative risk compared with a reference genotype.
Common genotype notation
| Reported genotype | Plain-language interpretation |
|---|---|
| ε2/ε2, ε2/ε3 or ε2/ε4 | Contains one or two ε2 alleles; the report’s interpretation depends on the laboratory and the comparison group |
| ε3/ε3 | Often used as a reference genotype in risk studies |
| ε3/ε4 | Contains one ε4 allele and is associated with higher risk than the reference in many studies |
| ε4/ε4 | Contains two ε4 alleles and is associated with greater risk than one ε4 allele in many studies |
Not every laboratory uses identical wording or reference categories. Read the laboratory’s explanation of the comparison group, the population studied and whether it is reporting a relative measure or an absolute probability.

- It cannot diagnose Alzheimer’s disease or prove that symptoms are caused by Alzheimer’s.
- It cannot reliably tell you whether you will develop Alzheimer’s.
- It cannot reliably predict the age at which disease would begin.
- It cannot rule out Alzheimer’s when ε4 is absent.
- It does not measure all genetic variants, medical conditions or life factors that influence cognition.
How to interpret the published risk numbers
The FDA’s 2019 review cites study-derived likelihood ratios of 1.9 for ε3/ε4 and 8.2 for ε4/ε4 among the cited data from people of European descent. These are not universal multipliers and are not an individual’s absolute lifetime probabilities.

A likelihood ratio changes how evidence is weighed against a baseline. Turning one into a personal probability would require a suitable baseline incidence and additional information such as age, sex, family history, ancestry and the model being used. Estimates differ across ancestry groups, and the studies cited in the FDA review provide different values for some other ancestry groupings. Quoting 1.9 or 8.2 without that context can make a population statistic sound like a personal forecast.
Risk comparisons also depend on the reference genotype. Saying that ε3/ε4 has a higher relative risk than ε3/ε3 does not mean that every ε3/ε4 carrier has the same absolute risk, nor that the increase is the same at every age.
How APOE testing is performed
The practical process depends on whether testing is ordered through a healthcare professional or purchased directly by a consumer, but the decision points are similar.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →- Clarify the reason for testing. Decide whether the goal is family planning, understanding a research finding, evaluating a family history or informing a medical discussion. APOE status alone is not a diagnostic work-up for memory symptoms.
- Review the test’s scope. Confirm that the laboratory tests the APOE markers needed to distinguish ε2, ε3 and ε4, and ask whether it provides clinical oversight and an explanation of limitations.
- Discuss consent and privacy. Before sending a sample or uploading raw data, read how the provider stores, uses and deletes genetic information, whether data may be shared for research and whether you can withdraw permission.
- Consider counseling before testing. A genetic counselor or other qualified healthcare professional can explain possible results, family implications and what the test will not answer.
- Receive and verify the report. Check that your name, specimen and genotype are correctly matched. If a direct-to-consumer report gives an unexpected result, ask whether confirmatory testing in a clinical laboratory is appropriate.
- Interpret the result in context. Review ancestry, age, family history, cardiovascular and metabolic health, sleep, activity, smoking, alcohol use and other relevant factors with a professional rather than treating the genotype as a standalone forecast.
Should you get tested?
Testing is a personal decision, and an APOE result can be difficult to use if there is no clear medical or planning question behind it. Consider speaking with a genetic counselor or healthcare professional first, particularly if you have memory symptoms, a strong or unusually early family history, anxiety about dementia, or concerns about how results could affect relatives.

Situations in which counseling is especially useful
- You want to understand whether a family pattern suggests a rare familial Alzheimer’s variant rather than APOE-related susceptibility.
- You are considering sharing a result with children, siblings or other relatives.
- You are deciding whether an uncertain risk result would change screening, health planning or research participation.
- You have received a direct-to-consumer result without a clinician’s interpretation.
- You are worried about privacy, discrimination, data retention or secondary use of genetic information.
There is no general basis for treating an ε4 result as a reason for a guaranteed preventive treatment. Likewise, an ε2 or ε3 result should not be taken as permission to ignore symptoms or modifiable health risks.
Limits of direct-to-consumer results
Direct-to-consumer tests may examine only selected variants and may not cover every genetic change associated with Alzheimer’s disease or other causes of cognitive impairment. A raw genotype file can be hard to interpret, and uploading it to another service creates additional privacy considerations. A result labeled “increased risk” may reflect a statistical association rather than a clinically validated individual estimate.
Before relying on a consumer report, ask:
- Which exact variants were tested?
- Was the test validated for the population in which it is being used?
- What reference genotype and ancestry group support the stated risk?
- Is the result intended for medical use, or is it informational only?
- Is confirmatory testing available through a qualified clinical laboratory?
- Who can access the sample and resulting data, and for how long?
What else influences Alzheimer’s risk?
APOE is only one part of the picture. NIH and MedlinePlus identify additional genetic, lifestyle and environmental influences. Examples include physical inactivity, an unhealthy diet, alcohol misuse, smoking, poor sleep and social isolation. These factors do not cancel out inherited susceptibility, and healthy habits cannot guarantee that Alzheimer’s will be prevented. They are relevant because risk is multifactorial and because many are also important to cardiovascular and general health.
Symptoms still require medical evaluation regardless of genotype. Memory loss, confusion, changes in language or difficulties with daily tasks can have many causes, some treatable. An APOE result should not replace an assessment based on symptoms, examination and appropriate diagnostic testing.
Questions to ask after receiving an APOE result
- What exact genotype was found, and was the result confirmed?
- What reference genotype was used for the comparison?
- Which ancestry group and studies support the stated association?
- Is the number a relative-risk measure, likelihood ratio or absolute probability?
- Does my family history suggest testing for APP, PSEN1, PSEN2 or another condition instead?
- What privacy choices apply to my sample and data?
- What practical health or planning decisions, if any, should follow from this result?
The most accurate conclusion is limited but useful: APOE testing can show which APOE alleles you inherited and can indicate how your genotype is associated with Alzheimer’s risk in studied populations. It cannot tell you with certainty whether or when you will develop Alzheimer’s. Professional interpretation helps keep the result in proportion and prevents a susceptibility finding from being mistaken for a diagnosis.
