Air pollution harms both the respiratory and cardiovascular systems. Fine particulate matter can travel deep into the lungs, and particles 2.5 micrometres wide or smaller (PM2.5) can cross into the bloodstream. Short- and long-term exposure are associated with breathing disease, ischemic heart disease, stroke and premature death at the population level; an association does not prove that one exposure caused an illness in every individual.

The practical response is to identify the pollutant and exposure setting, follow current local air-quality advice, reduce time in polluted air, and improve indoor filtration during smoke events.

What counts as air pollution?

Air pollution is a mixture of gases and particles whose concentrations can damage health. The World Health Organization (WHO) 2021 global air quality guidelines provide health-based recommendations for six major pollutants:

Pollutant What it is and common sources Why it matters
PM2.5 Fine particles no wider than 2.5 micrometres; produced by vehicle and power-plant combustion, industry, wood burning, cooking and wildfire smoke. Can reach the deepest parts of the lungs and enter the bloodstream.
PM10 Particles up to 10 micrometres, including road dust, construction dust and combustion particles. Irritates airways and can carry toxic substances into the respiratory tract.
Ozone (O₃) A reactive gas formed when sunlight drives reactions between nitrogen oxides and volatile organic compounds. Inflames and narrows airways, especially during hot, sunny weather.
Nitrogen dioxide (NO₂) A combustion gas from traffic, gas appliances, power generation and industry. Aggravates airway inflammation and contributes to ozone and particle formation.
Sulfur dioxide (SO₂) Released mainly by burning sulfur-containing fuels and some industrial processes. Can trigger bronchial constriction and asthma symptoms.
Carbon monoxide (CO) An odorless gas from incomplete combustion, including vehicles, generators and indoor fires. Reduces blood oxygen delivery by binding to hemoglobin.

Sources and concentrations differ by geography and season. Transport corridors, energy production, factories, agricultural burning, wildfires and household cooking or heating can all contribute. The WHO explains pollutant types and health effects at its air-quality pollutant overview.

How polluted air reaches the lungs and circulation

When you inhale, larger particles are trapped in the nose and upper airways, while smaller particles move farther down the bronchial tree. PM2.5 can reach the alveoli, the tiny air sacs where oxygen enters the blood. Some components can pass through the lung barrier, helping explain why particle exposure is linked with effects beyond the respiratory system.

Airway irritation activates immune and inflammatory responses. Inflammation can increase mucus, swelling and airway reactivity, making breathing harder for people with asthma or chronic obstructive pulmonary disease (COPD). Systemic inflammation and changes in blood-vessel function can increase cardiovascular strain. These are biological pathways that support observed population associations, not a way to diagnose the cause of one person’s symptoms.

“Both long-term and short-term exposure to particulate matter is associated with morbidity and mortality from cardiovascular and respiratory diseases.”

This is the WHO’s summary of the evidence on its particulate-matter health page.

Respiratory effects: immediate symptoms and longer-term disease

Short-term responses

A sudden rise in particles or irritant gases can cause coughing, throat irritation, phlegm, wheezing, chest tightness and shortness of breath. People with asthma may need their reliever medicine more often, and those with COPD can experience an exacerbation. Ozone and nitrogen dioxide can make airways more reactive even when the pollution is not visibly smoky.

Wildfire-smoke symptoms

Wildfire smoke combines fine particles with gases from burning vegetation and buildings. The WHO’s wildfire fact sheet lists eye and respiratory-tract irritation, wheezing, chest pains, asthma attacks and shortness of breath among reported short-term effects. Symptoms can appear during exposure or after cumulative exposure over several hours. Smoke can also worsen existing heart and lung disease.

Long-term respiratory burden

Repeated or sustained exposure is associated with chronic respiratory disease, reduced lung function and increased risk of COPD, asthma complications and lung cancer. The WHO’s 2021 guideline background identifies COPD, asthma and cancer among conditions considered in the air-pollution evidence. These are risk relationships across populations; they do not establish that every exposed person will develop a particular disease.

Cardiovascular effects deserve equal attention

Heart and blood-vessel stress

Particles that reach the circulation can promote systemic inflammation, oxidative stress and impaired blood-vessel function. Exposure is also associated with changes in heart-rate regulation and a greater tendency toward clotting. These responses can add stress for people whose arteries or heart are already diseased.

Major outcomes linked with exposure

WHO evidence links both short- and long-term particulate exposure with ischemic heart disease and stroke. Short pollution spikes can coincide with more emergency visits and hospital admissions, while long-term exposure contributes to cumulative cardiovascular risk. During smoke events, WHO reports heart palpitations and cardiovascular-related hospital admissions among older populations. A population association is not an individual diagnosis, so new or severe chest symptoms require medical evaluation rather than an assumption about pollution.

Who faces higher risk?

Risk depends on concentration, duration, pollutant mix, ventilation, age and health status. WHO’s 2026 technical brief identifies these groups as especially vulnerable:

How large is the global health burden?

WHO’s technical brief published 21 June 2026 describes air pollution as the leading environmental health risk factor and estimates about 6.6 million deaths globally every year. It reports that close to 84% of air-pollution-related deaths are from noncommunicable diseases, which include cardiovascular and chronic respiratory conditions. These are global estimates, not a count of deaths attributable to one person’s exposure in a particular year.

WHO also states that 99% of the global population breathe air exceeding its guideline limits. The topic-page figure is not assigned a data year there, so it should not be read as a newly measured 2026 statistic. Local readings can be better or worse than the global average and can change hour by hour.

Using WHO air-quality guidelines correctly

The 2021 WHO guidelines specify annual and short-term reference levels for PM2.5, PM10, ozone, nitrogen dioxide, sulfur dioxide and carbon monoxide. They help governments and the public judge how polluted air is, but they are not a guarantee that a concentration below a guideline is risk-free for every person.

Regulatory limits, alert categories and live air-quality indexes differ by country and can be revised. Check your local environmental or public-health authority for current readings, smoke advisories and evacuation instructions. During an emergency, follow those instructions even if a general app displays a lower number.

Reducing exposure during wildfire smoke or severe pollution

  1. Check current advice. Use local air-quality alerts and emergency notices. Conditions can change quickly with wind, fire behavior, heat and shifting smoke.
  2. Move strenuous activity indoors or postpone it. Exercise increases the volume of air you inhale and therefore the dose of particles.
  3. Create a cleaner-air room. Choose a room that can be closed off from outdoor air, keep windows and doors shut while smoke is heavy, and run filtration appropriate for the space.
  4. Stop adding particles indoors. Avoid smoking, candles, incense, frying or broiling, aerosol sprays and vacuuming that lacks effective particle control. EPA’s smoke-preparation guidance and cleaner-air-room advice provide practical steps.
  5. Use a cleaner-air location if needed. If your home cannot be kept clean or cool safely, ask local authorities about designated cleaner-air spaces. Do not compromise essential heat, cooling or medical equipment without professional advice.

Choosing indoor filtration that can actually help

Portable air cleaners

EPA recommends sizing a portable unit to the room’s area and using its smoke clean-air delivery rate (CADR). Its smoke-preparation guidance uses a rule of thumb: tobacco-smoke CADR should be at least two-thirds of the room area in square feet. For a 10-by-12-foot room (120 square feet), that means at least 80 tobacco-smoke CADR. Higher ceilings require a larger-rated unit.

Selection point What to check Limit to remember
Particle capacity Smoke CADR relative to room area and ceiling height. CADR describes particle delivery, not complete pollutant removal.
Ozone emissions Prefer low-ozone or CARB-certified devices, as advised by EPA’s indoor-filtration guide. Certification addresses ozone emissions; it is not a guarantee against every indoor pollutant.
Filter type and scope HEPA or other high-efficiency particle filtration; activated carbon may help with some gases. There is no widely used, standardized gas-removal rating system, and carbon performance varies with the amount and condition of carbon.
Noise and run time Choose a noise level you can tolerate so the unit can run for long periods; higher fan speeds generally move more air. A unit that is switched off or run at a very low setting filters less air.
Maintenance Confirm replacement-filter availability and the correct fit for the model. A loaded or dirty filter can perform poorly; follow the manufacturer’s replacement schedule.

EPA states, “No air cleaner or filter will eliminate all of the air pollutants in your home.” Portable cleaners reduce particles in the room they serve; they do not make outdoor smoke safe everywhere or remove every gas.

Central HVAC filtration

If a home has a compatible forced-air heating or cooling system, EPA recommends a MERV 13 filter or the highest efficiency the system can accommodate. Ask the system manufacturer or an HVAC professional before upgrading: a filter that is too restrictive can reduce airflow or strain equipment. Keep the fan running as advised for the system, and replace filters when they are dirty or overloaded. Details are in EPA’s indoor air-filtration guidance.

DIY box-fan cleaners

A properly assembled box-fan and filter cleaner may be a lower-cost option where commercial units are unavailable. EPA’s DIY air-cleaner guidance reports that shrouds, thicker filters and multiple filters can increase CADR, but performance varies by design and construction. Commercial units with a known CADR are generally easier to size. Never use a damaged fan, block its motor vents, or leave an improvised assembly unattended around children.

When symptoms need medical care

Follow your asthma or heart-disease action plan and take prescribed medicines as directed. Seek urgent medical help for severe trouble breathing, persistent chest pain or pressure, blue or gray lips, fainting, new confusion, or symptoms that rapidly worsen. During smoke events, palpitations, unusual weakness or repeated need for rescue medication also merit prompt clinical advice. Emergency instructions from local authorities take precedence over general online guidance.

What air-pollution evidence can—and cannot—tell you

Studies can show that disease and death rates rise as pollution exposure rises across groups or time periods. They cannot, without an individual exposure assessment and clinical evaluation, determine exactly how much pollution contributed to one person’s illness. Personal risk depends on the pollutant, measured concentration, duration, indoor protection, age, genetics and existing disease. Use live local data and professional medical advice for decisions about your own situation rather than treating a guideline value as a personal safety guarantee.
