AI safety refusals are learned behaviors reinforced by instructions—not guarantees that a model will always decline unsafe requests. A model can still be coaxed into unsafe compliance, or refuse harmless help that merely resembles a prohibited request. Understanding both failure modes requires looking at training, instruction priority, and how safety is evaluated.

What an AI safety refusal is—and what it is not

A refusal is a model response that declines to provide requested help, usually because the request conflicts with a safety rule or higher-priority instruction. But “refusal” is not the only safe outcome. Depending on the request, a model may decline the dangerous part while offering a safer alternative, answer a benign version of the question, or redirect the user toward non-harmful information. Those responses can be safe even when they do not use a standard refusal phrase.

Conversely, a refusal system is not a force field around a model. Its behavior is shaped by training and by instructions applied while it generates an answer. A model may follow those signals in one case and fail to follow them in another. The fact that a system has been trained to refuse certain content does not establish that it will never produce that content.

It helps to distinguish two meanings of “failure.” Unsafe compliance means the model provides disallowed help when it should not. Overrefusal means it declines a benign request it should have answered. A useful safety evaluation needs to look for both: a system that refuses everything might avoid some unsafe answers, but it would not be a good general-purpose assistant.

How refusals are produced

Training shapes the model’s response patterns

OpenAI describes training signals that reward refusals of disallowed content. Training also includes allowed examples, which matters because a model exposed only to examples of refusal in safety-related situations could learn a crude rule: refuse anything that sounds remotely risky. Allowed examples help teach the distinction between a harmful request and a legitimate question in a sensitive area.

This does not mean the model consults a perfect checklist before every answer. The relevant point is that training influences which responses the model is more likely to produce. A refusal can therefore be understood as one behavior the system has been taught to favor in particular circumstances, not as proof that every future case will be classified correctly.

Instructions constrain the answer at the time of use

Training is only part of the picture. Instructions supplied to a model can also govern how it answers a user. The OpenAI Model Spec describes instruction handling in terms of applicability and priority: when a lower-priority instruction conflicts with an applicable higher-priority one, the lower-priority instruction should not control the response.

OpenAI’s instruction-hierarchy research gives the ordering as system, developer, user, then tool content. For example, if a tutor’s developer instruction requires the assistant to teach rather than simply provide a requested answer, the model should follow that instruction when it conflicts with the user’s request. The principle is not that user instructions are unimportant; it is that a user cannot override a relevant higher-priority instruction merely by asking the model to ignore it.

In practical terms, the system may decline, provide a safe completion, or redirect. These outcomes are not interchangeable for every user or every automatic evaluator. A useful answer to a safety-adjacent request may involve explaining what is safe to discuss while withholding the dangerous details. Whether that answer is scored as a refusal depends on the evaluation’s definitions.

Why a model may provide an unsafe answer

Jailbreaks try to defeat the refusal behavior

A jailbreak is an adversarial attempt to get a model to produce content it is not supposed to provide. A prompt might frame a disallowed request in a way intended to evade the model’s safeguards, or combine multiple tactics. The important feature is the goal: the user is trying to make the model disregard the intended boundary.

Jailbreak tests are useful because they probe whether a model continues to follow its safety requirements when the request is phrased adversarially. But a result on one benchmark can only describe the tested prompts, model, and scoring method. It cannot establish resistance to every jailbreak technique or every deployment context.

Direct instruction conflicts target the hierarchy

A direct attack can simply tell the model to disregard an earlier, higher-priority instruction. The hierarchy is intended to prevent a lower-priority user message from overriding an applicable system or developer instruction. If the model follows the conflicting user request anyway, that is a failure to uphold the intended priority in that case.

Instruction hierarchy is a defense, not a guarantee. OpenAI reports very high results for particular models under a specific October 2026 evaluation, but those figures should be read as results for that test setup—not as proof that the models resist all instruction conflicts or attacks in real-world use.

Prompt injection arrives through third-party content

Prompt injection is related to instruction conflict, but the hostile text can arrive indirectly. In a tool-using or retrieval setting, a model might be asked to read a webpage or other outside material. That material could contain instructions aimed at the assistant rather than information relevant to the user’s task.

The model should treat such text as data to analyze, not as a command that takes precedence over the instructions governing the task. For example, a retrieved page might contain a sentence telling the assistant to ignore its prior instructions. The presence of that sentence in the page does not make it an authoritative instruction. OpenAI’s evaluation discussion distinguishes direct instruction-hierarchy attacks from indirect attacks embedded in external content, because they enter the interaction through different paths.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

That distinction matters when interpreting a system’s performance. A test of direct user prompts does not, by itself, show how well a model handles hostile instructions in retrieved material. A meaningful comparison needs to identify which path was tested.

Why a model may refuse harmless help

Overrefusal occurs when a model declines a benign request because it resembles a risky one. A question can mention a sensitive topic for a legitimate reason—such as explanation or prevention—without asking for disallowed assistance. If the model reacts to keywords or broad subject matter instead of the request’s actual intent, it may block useful help.

This is why safety evaluation cannot be reduced to counting refusals to harmful prompts. OpenAI’s evaluation descriptions include benign prompts in safety-relevant contexts and track overrefusal separately from unsafe completion. The goal is not maximal refusal; it is to refuse the requests that should be refused while remaining useful on requests that are allowed.

For a reader, a refusal does not automatically prove that the question was unsafe. It may be an appropriate boundary, a cautious interpretation, or a mistaken refusal. The distinction depends on what the user asked and what the applicable policy permits—not simply on whether the model declined.

What the published evaluation figures do—and do not—show

Safety figures are meaningful only with their model, metric, benchmark, and evaluation context attached. They are not universal rates that predict how often any AI system will be safe in everyday use. The following results come from different evaluations and should not be treated as a single leaderboard.

Source and evaluation Reported result What the figure means
OpenAI, o3 and o4-mini System Card; standard refusal evaluation Aggregate “not_overrefuse” was 0.84 for o3, 0.81 for o4-mini, and 0.86 for o1. These are results on that evaluation’s metric and set, not population-wide success rates or a measure of every safety behavior.
OpenAI, o3 and o4-mini System Card; standard refusal evaluation The Chain of Thought summarizer for both o3 and o4-mini scored 0.95 on “not_unsafe.” This is the stated metric result for those summarizers in that evaluation, not a guarantee that unsafe output is impossible.
OpenAI, GPT-4 Research; comparison with GPT-3.5 OpenAI reported an 82% decrease in GPT-4’s tendency to respond to disallowed requests compared with GPT-3.5, and said GPT-4 responded to sensitive requests in accordance with its policies 29% more often. These are OpenAI’s reported comparative results in its stated research context, not a universal refusal rate or a comparison that can be directly combined with the other rows.
OpenAI, GPT-6 Sol and GPT-6 Luna October 2026 update; instruction-hierarchy robustness evaluation 99.99% for GPT-6 Sol and 99.79% for GPT-6 Luna. These figures apply to the update’s evaluation setup. They do not establish resistance to all prompt injection or real-world attacks.

Metrics also depend on what the evaluator counts as success. A strict grader might expect a particular refusal style and mark a safe redirection as a miss. A grader might also mistake a nuanced refusal for an answer that complies. In a joint OpenAI–Anthropic evaluation, manual inspection found that automated graders sometimes misread nuanced refusals and safe answers. That is a reason to examine the rubric and examples behind a score, not just its headline percentage.

When comparing two systems, keep the questions separate: how often does each produce unsafe completions on disallowed prompts; how often does it overrefuse benign prompts; how does it handle direct jailbreaks; how does it handle malicious instructions embedded in external content; and how strong is the evaluation itself? Results from different models or benchmarks should not be combined unless their methods are demonstrably comparable.

How to interpret a refusal or a safety claim

  • Look at the actual request. A refusal may be warranted, or it may be an overrefusal of a benign question. The response alone does not settle that distinction.
  • Separate the attack path. A user directly asking the model to ignore instructions is different from hostile text arriving inside a webpage, document, or other external content.
  • Ask what the metric measures. “Not unsafe” and “not overrefuse” describe different evaluation dimensions. A result for one should not be presented as proof of the other.
  • Keep the test context attached to a percentage. Name the model, benchmark or evaluation, metric, and any stated comparison. Do not turn a benchmark score into a general safety rate.
  • Check how answers were graded. Rubrics, examples, and manual review matter when a safe response can be a refusal, a redirection, or a carefully limited answer.
  • Do not treat a high score as invulnerability. Evaluation provides evidence about tested conditions. It cannot establish that every future prompt, outside document, or deployment will be handled correctly.

The practical takeaway

AI safety refusals are produced by learned behavior and reinforced by instruction priority. That design is intended to prevent unsafe compliance while preserving useful answers to benign requests. It can fail in either direction: a jailbreak or instruction conflict may elicit an unsafe answer, while an overly cautious interpretation may block harmless help. Evaluation figures can show how a particular model performed on a particular test, but their meaning depends on the prompts, metric, grader, and attack path. Treat them as bounded evidence—not a promise of perfect refusal behavior.
