AI-powered ECG screening applies machine-learning software to an electrocardiogram (ECG) to flag electrical patterns associated with a defined clinical target. The clearest FDA-labeled example screens adults at risk for a weak pumping function—left ventricular ejection fraction (LVEF) of 40% or less—so a clinician can decide whether echocardiography or other evaluation is needed. It is a screening aid, not a diagnosis: a positive result requires clinical confirmation, and a negative result does not rule out further testing when a patient remains high risk.

What is AI-powered ECG screening?

A conventional ECG records the heart’s electrical activity through electrodes placed on the skin. AI-ECG software processes that signal with a machine-learning model and returns a likelihood, risk flag or screening result for a particular target condition. The model may identify combinations of waveform features that are difficult to recognize consistently by eye, but the software does not replace a clinician’s history, examination or confirmatory testing.

The target is product-specific. The strongest regulatory and clinical evidence covered here concerns reduced left ventricular ejection fraction, a finding associated with heart failure. It would be inaccurate to describe every AI-ECG system as a general detector for arrhythmias, heart attacks, valve disease or all forms of heart failure. Each model needs its own intended-use statement, validation evidence and regulatory authorization.

How does AI ECG screening work?

1. A standard ECG supplies the input

The cited low-EF use case starts with a clinical ECG, not a special scan. The FDA product classification describes software that uses non-invasive physiological input and machine learning to suggest the likelihood of reduced ejection fraction for referral or diagnostic follow-up. The reviewed label specifies ECG input and prescription use. It does not establish that a particular consumer watch, handheld recorder or home electrode set is compatible.

2. Software analyzes the signal

After acquisition, software examines the ECG data for patterns associated with the model’s target. Public labeling and the sources reviewed do not disclose the model architecture, training dataset or complete signal-processing pipeline. Therefore, claims that a particular neural-network design is used, or that the program directly calculates ejection fraction from the tracing, go beyond the available evidence. The software infers a screening signal from the ECG; ejection fraction still has to be assessed by an appropriate diagnostic method.

3. The result is presented as decision support

The output can be made available to a clinician alongside routine-care ECG results. In the randomized primary-care study discussed below, clinicians received the AI result as decision support. The result was intended to influence whether they pursued evaluation, not to make an autonomous diagnosis or replace echocardiography.

4. Clinical evaluation confirms or rejects the concern

A positive screen generally prompts review of symptoms, medical history and the original ECG, followed by a suitable non-invasive test when indicated. Echocardiography is a common way to measure LVEF, although the clinician chooses the confirmatory pathway. The AI output is one piece of information in that decision.

What can an AI ECG detect?

FDA-labeled low-ejection-fraction screening

One FDA-labeled algorithm is intended to screen adults at risk for heart failure for LVEF less than or equal to 40%. LVEF is the percentage of blood the left ventricle ejects with each contraction; a value at or below 40% indicates substantially reduced pumping function and can change treatment decisions. The indication is a screening use, not a diagnosis of heart failure by itself.

The label gives examples of people who may be considered at risk:

  • People with cardiomyopathies.
  • People who have had a myocardial infarction (heart attack).
  • People with aortic stenosis.
  • People with chronic atrial fibrillation.
  • People receiving potentially cardiotoxic pharmaceutical therapies.
  • Postpartum women.

The same indication excludes ECGs with a paced rhythm. A pacing spike or paced rhythm can alter the signal in ways that are outside the validated use, so the result should not be treated as applicable in that situation.

What the evidence does not establish

The available sources support the low-EF use case. They do not show that one AI-ECG product detects every arrhythmia, coronary blockage, myocardial infarction, structural abnormality or cardiac condition. A different product may have a different target, lead configuration, population or threshold. Its performance cannot be inferred from the low-EF evidence.

Can an ECG detect a weak heart?

An ECG can contain patterns associated with reduced LVEF, and an AI model can use those patterns to identify people who merit further evaluation. However, an ECG—AI-assisted or interpreted by a clinician—does not directly measure the ventricle’s ejection percentage. A positive screen indicates that the model found a pattern associated with the target; an imaging or other diagnostic assessment is needed to determine the actual LVEF.

The FDA’s product classification describes this type of software as suggesting the likelihood of reduced ejection fraction for referral or diagnostic follow-up. That wording matters: the intended outcome is appropriate referral, not an automatic diagnosis or treatment decision.

What does a positive or negative AI-ECG result mean?

Positive result

A positive result means the ECG pattern met the software’s screening criteria for the specified target. It should trigger clinician review and, when appropriate, confirmatory testing. It does not prove that the patient has heart failure or that LVEF is at or below the product’s threshold. Symptoms, examination findings, prior imaging, medications and other risk factors remain relevant.

Negative result

A negative result lowers concern according to the model’s validated use, but it does not end evaluation for a person who remains high risk or has concerning symptoms. The FDA label specifically cautions that a negative result should not rule out further non-invasive evaluation in a high-risk patient.

Not a monitoring device

The software is not intended for continuous patient monitoring. It is used with an ECG recording obtained for clinical care, and the clinician must apply judgment to the result. The FDA’s official Indications for Use states:

“Anumana Low Ejection Fraction AI-ECG Algorithm is not intended to be a stand-alone diagnostic device for cardiac conditions, should not be used for monitoring of patients, and should not be used on ECGs with a paced rhythm.”

What did the clinical trial show?

A pragmatic, cluster-randomized trial published in Nature Medicine in 2021 tested whether making an AI-ECG result visible to primary-care clinicians changed care. It enrolled 22,641 adults without a prior diagnosis of heart failure across 45 clinics or hospitals. The primary endpoint was a new diagnosis of low EF—defined in that study as LVEF less than or equal to 50%—within 90 days of an ECG.

Trial measure Result How to interpret it
Participants 22,641 adults without prior heart failure Population studied in routine primary-care settings
Clinical sites 45 clinics or hospitals Cluster-randomized implementation across care sites
New low-EF diagnoses, AI result available 2.1% Diagnosis rate during the 90-day follow-up
New low-EF diagnoses, control 1.6% Diagnosis rate without the intervention
Effect estimate Odds ratio 1.32; 95% confidence interval 1.01–1.61; P=0.007 More new diagnoses were recorded when clinicians had the AI decision-support result
Among participants with a positive AI-ECG 19.5% versus 14.5% Diagnosis rates in the study’s positive-result comparison

These figures are workflow outcomes, not the sensitivity of the algorithm and not an individual patient’s probability of disease. The trial’s endpoint threshold of 50% also differs from the FDA-labeled example that screens for LVEF of 40% or less. You should not transfer the trial’s percentages or threshold to another product without evidence for that product’s intended use.

How is AI-ECG software regulated?

Regulation attaches to a specific device, indication and jurisdiction. The FDA recorded a substantial-equivalence 510(k) decision dated July 28, 2025, for an ECG-AI Low Ejection Fraction 12-Lead algorithm (1010), record K250652. A 510(k) decision means the device met the applicable premarket requirements for its stated use; it is not a general endorsement of AI-ECG technology and does not authorize self-screening.

How clinicians should use an AI-ECG result

  1. Confirm applicability. Check that the patient fits the labeled population and that the ECG is not from a paced rhythm when the product excludes paced rhythms.
  2. Review the complete clinical picture. Consider breathlessness, swelling, fatigue, prior infarction, cardiomyopathy, valve disease, atrial fibrillation, cancer therapies and postpartum status alongside the result.
  3. Inspect the ECG and data quality. Artifact, lead problems or an unsuitable recording can undermine interpretation; follow the device’s quality checks and labeling.
  4. Arrange confirmation when indicated. A positive screen commonly leads to echocardiography or another clinician-selected non-invasive assessment of LVEF.
  5. Do not stop because of a negative result when risk remains high. Persistent symptoms or strong risk factors can justify further evaluation despite a negative screen.
  6. Document the decision. Record the software result, clinical reasoning, follow-up test and final diagnosis so the screening step is not mistaken for the confirmatory result.

How to compare AI-ECG products or studies

Marketing labels such as “AI ECG” are not enough to establish that two systems are interchangeable. Compare the following items in the device labeling and peer-reviewed evidence:

Comparison point Questions to ask
Target and threshold Is the model screening for LVEF ≤40%, ≤50% or another condition and cutoff?
Population and exclusions Which ages, risk groups, rhythms, medications and clinical settings were validated? Are paced rhythms excluded?
Input Does it require a 12-lead ECG, and what recording quality or duration is required?
Validation setting Were data collected in primary care, hospital care, emergency settings or another population?
Performance reporting Are sensitivity, specificity, calibration and confidence intervals reported at the intended threshold?
Regulatory status What authorization applies, in which country, and for which exact intended use?
Follow-up pathway What test or specialist assessment is recommended after a positive result?

The evidence summarized here does not provide head-to-head testing that would justify ranking products. A study showing that decision support increased diagnoses also does not prove that every implementation will produce the same benefit.

Limitations and common mistakes

  • Confusing screening with diagnosis: the model flags a likelihood; it does not establish heart failure or measure LVEF directly.
  • Applying the wrong threshold: the FDA example uses LVEF ≤40%, while the 2021 trial used ≤50% for its primary endpoint.
  • Generalizing across products: different models can have different training populations, exclusions, lead requirements and error rates.
  • Ignoring clinical risk after a negative result: high-risk patients may still need imaging or other assessment.
  • Using an excluded rhythm: a paced ECG is outside the cited labeled use.
  • Treating authorization as a performance guarantee: a regulatory decision applies to the stated device and indication, not to every AI claim or every clinical scenario.
  • Assuming a home device is compatible: the evidence reviewed does not verify that general-purpose consumer ECG monitors can run the cited clinical algorithm.

Bottom line for patients and care teams

AI-powered ECG screening is best understood as an early-warning and referral tool. In the best-supported example, machine-learning software analyzes a clinical ECG to identify at-risk adults who may have LVEF of 40% or less. A positive result should lead to clinician assessment and, when appropriate, confirmatory imaging; a negative result does not overrule symptoms or substantial risk. The technology’s value depends on using the exact product within its labeled population and connecting the result to a reliable diagnostic follow-up pathway.
