AI models predict how cells respond to drugs by learning patterns in experiments that measure cells before and after treatment. Researchers provide data about the cell context, the drug or other perturbation, and the observed outcome; a model then estimates an outcome for a condition it has not been asked to predict during training. In single-cell studies, that outcome may describe gene expression in individual cells or how treatment shifts the mix of cell states. The prediction is a research hypothesis—not proof that a drug will benefit a patient.

What does “predicting a cell’s response” mean?

A cell’s response to a drug is not one universal number. Depending on the experiment, researchers may ask whether particular genes become more or less active, whether a treatment changes the average gene-expression profile, whether it shifts the distribution of cell states, or whether a drug produces a response pattern associated with a particular class. A model can also rank candidate drugs for a defined research question.

These are related, but not interchangeable, outputs. A predicted expression profile is not automatically a prediction of which genes changed because of treatment. A response class or drug ranking is a different kind of result again. To understand a model’s claim, identify both the output it predicts and the conditions under which it was evaluated.

For single-cell RNA sequencing, researchers measure gene-expression profiles one cell at a time. This can reveal that cells in the same sample do not all respond alike: a treatment might affect some cell states more strongly than others, while the average across all cells conceals that variation. The data are also high-dimensional, sparse and noisy, and can vary for technical reasons between experiments.

How the prediction workflow works

At a high level, a model learns from examples linking a biological context and a treatment to measured outcomes. The goal is to use those learned relationships to predict outcomes in a condition withheld from training, then compare the predictions with what an experiment actually observed.

  1. Measure a baseline or control. Researchers collect gene-expression data from cells without the treatment or under a defined control condition. This gives a reference for interpreting the treated cells.
  2. Measure the treated condition. Cells are exposed to a drug or another perturbation, and their responses are measured. The experiment also records relevant context, such as cell identity and treatment information.
  3. Represent the inputs. The model receives information describing the cell context and the drug or perturbation. The exact representation depends on the model and study; the essential point is that it has to connect treatment and biological context to observed outcomes.
  4. Fit the model to examples. The model estimates patterns associated with the treatment by learning from the measured examples. It is learning from the data it was given, not observing a future experiment directly.
  5. Test on a held-out condition. Researchers ask the model to predict a condition withheld from fitting. What is held out matters: it could be a perturbation, a cell type, a dataset, or a cell-line–drug combination.
  6. Compare prediction with experiment. Researchers use one or more metrics to assess agreement. A result can look strong under one metric and weaker under another, so the metric and test design are part of the result, not minor technical details.

For example, suppose a study has control and drug-treated measurements in one cell context and wants to predict a treatment outcome in another condition. A model might output expected gene-expression values, estimated treatment-induced changes, or a predicted distribution of cell states. These answer different questions even when they use the same experimental data.

What researchers measure to judge a prediction

There is no single metric that captures every meaning of “accurate.” The right evaluation depends on the scientific question and the output. Expression-level agreement can be misleading if the treatment causes only a small shift: a prediction close to the control profile may resemble the observed values overall while missing the treatment-induced change that researchers care about.

Evaluation perspective What it asks Why it matters
Expression-level agreement Do predicted gene-expression values resemble the observed values? Assesses profile similarity, but may reward predictions close to control when treatment effects are small.
Delta or change Does the model predict the difference between treated and control conditions? Focuses on the treatment-associated shift rather than similarity to the overall expression profile.
Direction accuracy Does the model correctly predict whether affected genes go up or down? Tests whether the predicted response has the right direction, even if its magnitude differs.
Differential-expression recovery Does the model identify genes that change under treatment? Checks recovery of the treatment-associated gene set, rather than only profile-wide agreement.
Distributional similarity Do predicted and observed distributions of cells resemble one another? Can assess changes across cell states that a comparison of average profiles alone may miss.

These measures can disagree without any one of them being inherently invalid: they describe distinct aspects of the prediction. A claim that a model is “most accurate” is incomplete unless it specifies the target, metric, and held-out test design.

What recent benchmarks reveal

A broad comparison across datasets

The authors of the 2026 study A systematic comparison of single-cell perturbation response prediction models compared 13 representative models and baselines across 25 datasets. They reported that performance depended on perturbation effect size and on how performance was measured. Expression-level agreement was strongest for small-effect perturbations that resembled controls, while delta and differential-expression metrics gave clearer signals for larger effects. No method consistently generalized across cell types.

The practical lesson is that a score needs context. If a treatment creates a small shift, a model may appear to match expression values while failing to capture the shift itself. If the main interest is a treatment-driven change, a delta-oriented or differential-expression measure may be more informative. Neither conclusion can be generalized to all targets and settings from one metric alone.

scDrugMap and the difference between pooled and external evaluation

The 2026 scDrugMap study evaluated ten models: eight single-cell foundation models and two general-purpose language models. Its curated collections contained 495,237 single cells. The primary collection included 326,751 cells from 36 single-cell datasets across 23 studies; the external validation collection included 168,486 cells from 24 datasets. Within its primary collection, the study covered 14 cancer types, three therapy types, five tissue types and 21 treatment regimens. These are counts for the study’s collections, not a census of all single-cell drug-response research.

scDrugMap reported better performance in pooled-data evaluation than in cross-data evaluation. That gap matters because mixing data can produce a more familiar evaluation setting than testing on a separate dataset. A result on pooled or familiar data may therefore overstate how well a model transfers to an independent experiment. Technical differences between datasets and batch effects can contribute to the challenge, although the performance gap itself does not identify one cause in every case.

The study frames model choice as dependent on data characteristics and resource constraints. Its results do not establish that foundation models are automatically superior: the broader benchmark found task-dependent performance, and scDrugMap’s own evaluation varied with the test setting.

Known drugs and unprofiled compounds are different tests

A 2026 Nature Machine Intelligence paper describes MAP, a mechanism-aware predictor evaluated on unseen cell-line–drug combinations and entirely unprofiled compounds. Those test settings represent different generalization challenges. Predicting a known drug in a new cell context is not the same as predicting a compound for which the model has no prior response profile.

MAP’s evaluation uses complementary gene-level and distributional metrics. This illustrates why the question “Can the model generalize?” needs a qualifier: generalize to what new context, and according to which measure? A study can support a claim about the particular held-out setting it tested without proving performance in every other setting.

How to interpret a result or model comparison

When reading a paper, abstract, or model comparison, check the test setup before treating a score as evidence of broad predictive ability. The same model can face an easier or harder task depending on what data were available during fitting and what was reserved for evaluation.

  • Identify the held-out unit. Was the test condition a new perturbation, cell type, dataset, study, cell-line–drug pair, or entirely unprofiled drug? These are not equivalent forms of generalization.
  • Match the metric to the claim. A score for expression resemblance does not by itself establish accurate treatment-induced changes, correct direction, recovery of differentially expressed genes, or a faithful cell-state distribution.
  • Check the biological scope. Note the cell types, drugs, regimens, tissues and assay data represented. A prediction for a new dose, duration, cell type, or assay may be less reliable than one inside familiar conditions.
  • Look for genuinely separate validation. Performance on pooled or related data should not be treated as equivalent to performance on an external dataset. The scDrugMap comparison makes this distinction particularly concrete.
  • Separate research utility from clinical evidence. A model can help generate hypotheses, analyze candidate biomarkers, or guide drug-discovery experiments without showing that its prediction selects an effective treatment for an individual.

For a fair comparison between models, read the prediction setting, output, metric, data coverage, validation design, and practical requirements together. A headline ranking without those details can hide that models were tested on different tasks or that a favorable score reflects one evaluation perspective.

Why predictions can fail to transfer

Cell context changes the response

Drug response depends on the biological context represented in the experiments. A pattern learned in one cell type does not guarantee the same response in another. The 2026 comparison across 25 datasets found no model that consistently generalized across cell types, so strong performance in one context should not be presented as a universal rule.

Datasets are not interchangeable

Single-cell measurements can differ technically between experiments, and batch effects can make two datasets harder to compare. scDrugMap’s better pooled-data than cross-data performance shows why evaluation on a separate collection is important: success when data are combined does not guarantee the same performance after transfer to another dataset.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Small effects and noisy measurements complicate scoring

Single-cell data are sparse and noisy, and a drug may alter only a subset of cells or cause a modest expression effect. When the treatment-associated shift is small, the overall profile can look similar to the control even if the model does not capture the particular changes under study. That is why the evaluation metric should reflect whether the goal is profile resemblance, treatment-induced change, gene-level direction, or distributional response.

A prediction does not establish patient benefit

These models predict molecular or cellular outcomes under defined experimental conditions. The benchmark studies described here do not establish that model outputs alone can choose effective treatment for an individual patient. Any clinical implication requires independent experimental and clinical validation; a model-generated response should be treated as a basis for investigation, not a treatment recommendation.

What this technology is useful for—and what it cannot yet settle

Used carefully, prediction models can help researchers prioritize hypotheses, investigate candidate biomarkers, and decide which experimental conditions merit follow-up. Single-cell methods can make variation among cells visible, and computational predictions can help organize that complex response space. Their value depends on whether the available training data resemble the question being asked and whether the evaluation actually tests the kind of transfer required.

They cannot turn incomplete or mismatched data into certainty. A predicted profile is not the same as a measured one; an association learned from examples is not proof of a biological mechanism; and success on a held-out condition of one kind is not proof of success on a different kind. The strongest interpretation stays close to the experiment: state what was predicted, which context was held out, how the outcome was scored, and where independent validation is still needed.
