Short answer: an AI agent cannot delete a file by producing text alone. The model must choose a tool or command, the agent harness must approve it under its permission policy, and a permitted process must then perform the filesystem operation. To understand or prevent damage, audit that complete chain: the original request, model-generated tool call, approval decision, command result, file change, and any network-policy event.

Use narrowly scoped writable directories, explicit approval for destructive and sensitive-file operations, an isolated Git worktree or disposable environment, and a review gate before a commit, merge, deployment, or pull request. Those controls reduce the blast radius without pretending that an autonomous agent is infallible.

What actually happens when an agent deletes or overwrites a file?

The visible symptom—an old file is gone or its contents changed—is the final step of a longer decision path. The language model proposes an action, but a tool-capable runtime decides whether that action can execute. A terminal process, file-edit API, MCP action, or child process performs the write, rename, or delete only after the runtime’s rules allow it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Request interpretation. The model turns the user’s task and the repository context into a proposed edit. Ambiguous wording such as “clean up the old configuration” can become a deletion plan even when the user intended an archive.
  2. Tool selection. The model chooses a capability that can change state: a file-edit tool, a shell command such as rm or mv, an MCP operation, or a child process started by a script.
  3. Policy evaluation. The harness checks the permission mode, command or tool approval rules, sensitive-file protections, and the sandbox boundary. Approval and sandboxing are separate controls: approval determines whether a proposed call may run automatically, while sandboxing limits where a supported process can read, write, or connect.
  4. Execution. If permitted, the operating-system process performs the operation. A network-enabled command can create effects outside the original file change, including uploading data or fetching an untrusted script.
  5. Evidence capture. The runtime or surrounding security tooling records the prompt, tool call, approval result, command output, affected paths, before-and-after hashes or a patch, and relevant network-policy decisions.
  6. Human integration. Someone reviews the diff and checkpoint in an isolated worktree before the change is committed, merged, deployed, or submitted as a pull request.

This sequence is a synthesis of the control mechanisms documented by OpenAI and Microsoft Visual Studio Code. It is not a claim that every agent has identical internal reasoning or identical log fields.

Why an apparently harmless task can destroy the wrong file

Natural-language intent is underspecified

Models optimize for a plausible interpretation of the task, not for an unstated safety preference. “Replace the old build setup” might mean edit one file, remove several files, or regenerate a directory. If the prompt does not define the exact paths, allowed operations, and stop conditions, the model can select a broader change than the user expected.

Shell commands have destructive semantics

A command can affect more data than its prose description suggests. Recursive flags, shell globbing, command substitution, redirection, and scripts that follow symlinks all change the scope. A generated command that is valid in the current directory may be catastrophic if the working directory differs from the model’s assumption. The audit record therefore needs the exact command and arguments, not a summary such as “cleaned files.”

Context can contain hostile instructions

Repository files, issue text, documentation, or fetched web content may contain instructions that the model treats as part of the task. The nd7 project describes the risk plainly: “One wrong command, or a malicious instruction hidden in a file the agent reads, can delete a project, leak a key or push to the wrong branch.” Treat untrusted repository content as data to inspect, not as authority to change permissions or run commands.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Path assumptions and symlinks expand the target

An agent may infer a relative path, follow a symlink, or operate in a worktree different from the one the user is viewing. A path outside the intended repository should be rejected by the sandbox or require an explicit approval. Logging the resolved path and worktree identity makes this distinction visible during an investigation.

Network access adds a second side-effect channel

Installing a package, running a build hook, or executing a downloaded script can alter files and transmit data. File auditing without network-policy events cannot show the full causal chain. Record every allow, deny, and approval decision for outbound connections, including the process that requested it.

Permissions and sandboxing: related, but not interchangeable

Many incidents happen because teams configure one control and assume it provides the other. A permission prompt can stop an individual command but does not define the maximum filesystem reach of a command that is approved. A sandbox can restrict filesystem and network access but does not tell a reviewer why the model asked for a destructive operation.

Control What it decides What it cannot replace Practical configuration
Approval policy Whether a tool call or command runs automatically, waits for confirmation, or is blocked Filesystem isolation or a complete audit trail Use per-call or scoped approval for deletes, overwrites, sensitive files, and network-capable commands
Sandbox boundary Which paths and network destinations a supported process can access Human review of whether an allowed change is correct Set the writable root to only the repository or subdirectory required for the task; deny unnecessary outbound access
Sensitive-file rules Which high-impact files require an extra approval or are blocked Protection for every ordinary source file Protect environment configuration, deployment manifests, credentials, production exports, and other locally defined secrets
Version-control isolation How easily changes can be inspected, reverted, or discarded Prevention of a process writing to an active checkout Use a disposable container or separate Git worktree and checkpoint before integration
Telemetry What investigators can reconstruct after or during an action Prevention of a mistaken command Export structured events to a central log or SIEM and correlate intent, action, policy, result, and review

Microsoft’s Visual Studio Code documentation (October 14, 2026) explicitly separates approval levels from sandboxing and recommends reviewing generated code before committing. OpenAI’s 2026 security guidance likewise treats sandboxing, approvals, network policy, and agent-aware telemetry as distinct parts of the control system.

How to configure a safer operating model

1. Define the writable scope

Start with the smallest directory that contains the task. A documentation-only task may need one subdirectory; a build change may need the repository but not a parent directory containing other projects. Keep credentials, unrelated repositories, production data exports, and system paths outside the writable root. If the agent must read a file without modifying it, provide read access separately rather than widening write access.

2. Protect high-impact paths

Require an explicit approval for environment files, deployment manifests, credential stores, infrastructure state, migration scripts, and production data. The exact list is organization-specific, but the principle is stable: a file’s sensitivity should increase the review requirement, not merely the size of the diff. Make the protected-path policy visible in the agent’s effective-policy report so a reviewer can verify what was in force.

3. Keep destructive operations out of automatic approval

Prefer per-call or narrowly scoped approvals for deletion, recursive moves, broad replacements, permission changes, and commands that invoke another interpreter. Modes labelled Allow all, YOLO, or Autopilot trade interruption for autonomy and should be treated as elevated-risk settings. If an autonomous mode is necessary for a disposable environment, keep the environment disposable and preserve the complete telemetry.

4. Enforce OS-level isolation

Use a disposable container, virtual machine, or a separate Git worktree when the agent can run arbitrary terminal commands. The isolation must cover both filesystem and network access for terminal commands and child processes; a UI-level promise is not enough if a spawned process escapes that boundary. Deny outbound network access unless the task needs it, then allow only the required destinations and log the decision.

5. Establish a checkpoint before the first write

Create a clean commit or another recoverable checkpoint before starting. In a worktree, record the branch, commit identifier, and uncommitted state. This gives the reviewer a precise “before” image and lets the team discard the entire worktree instead of attempting to repair a partially overwritten active checkout.

6. Review before integration

Inspect the complete diff, not only the files the agent mentions. Look for unexpected deletions, generated files, permission changes, binary replacements, and edits outside the requested scope. Run the relevant tests, then decide whether to commit, merge, deploy, or reject the work. Microsoft states the rule directly: “Always review AI-generated code before committing.”

Comparing manual approval, assisted review, and autonomous modes

Choose an operating model by looking at more than interruption rate. The important questions are who approves an action, whether the sandbox still constrains it, how sensitive files are handled, how much network reach exists, how reversible the result is, and whether telemetry is complete.

Operating model Approval decision Sandbox and sensitive files Reversibility and review Best fit
Manual or per-call approval A person confirms each risky tool call or command Sandbox remains a separate boundary; protected files can require an additional confirmation Highest synchronous scrutiny; still needs a diff and checkpoint Active repositories, production-adjacent work, unfamiliar code, and destructive tasks
Assisted review The agent runs within policy and a reviewer examines a batch or checkpoint before integration Scoped writable roots and network restrictions remain mandatory Good balance for routine changes when the worktree is disposable Repeatable maintenance and test-driven changes with clear task boundaries
Autonomous or Allow all Commands may run without synchronous confirmation Only the sandbox and policy boundary constrain execution; a broad root creates a broad blast radius Relies on checkpoints, post-run diff review, and reliable telemetry Disposable sandboxes, low-value experiments, and tightly bounded batch jobs
Auto-review at the sandbox boundary A separate reviewer model evaluates actions instead of stopping a person every time It can enforce a sandbox boundary, but it does not replace sound permissions or scoped access Review is asynchronous or automated; retain human integration control for important repositories High-volume workflows that need fewer synchronous interruptions

OpenAI Alignment reported an internal Auto-review result of “roughly 200x less often” human stops and “around 99%” approval of reviewed actions. Those are provider-reported internal figures, not a universal benchmark or a guarantee for another agent, model, repository, or policy.

What a useful audit record must contain

An audit log should let an investigator answer five questions: What did the user ask? What did the model request? Which policy allowed or blocked it? What process actually ran? What changed, and who accepted the result?

  • Identity and scope: session ID, agent or model identity, user identity, repository, branch, worktree, container or host, and effective policy version.
  • Intent: the original user prompt, relevant task metadata, and the repository or issue content supplied as context. Preserve enough context to identify an instruction injected through a file.
  • Tool request: the model-generated tool name, exact command and arguments, environment or working directory, target paths, and whether an MCP server was involved.
  • Decision: approval mode, approver identity or automated reviewer, allow/deny result, reason, sensitive-file rule that matched, and sandbox and network policy in force.
  • Execution: start and end times, process identity, exit code, standard output, standard error, and child-process relationships.
  • Filesystem evidence: resolved paths, operation type, file hashes before and after, patch or diff, file sizes, and metadata changes such as permissions or ownership.
  • Network evidence: destination, protocol, process, policy decision, and any approval associated with an outbound request.
  • Integration outcome: reviewer, tests run and results, checkpoint or commit ID, revert or restore action, and whether the change was merged, deployed, or rejected.

Hash or diff data should be captured as close to execution as practical. A later snapshot can prove that a file differs, but it may not prove which command caused the change or whether an intermediate secret was exposed.

How to investigate an unexpected deletion or overwrite

  1. Stop further writes. Pause the agent and any automation that shares the same worktree. Do not run cleanup commands that could destroy recoverable evidence.
  2. Record the current state. Capture the worktree, branch, commit, file listing, timestamps, and the agent’s effective permission, sandbox, and network policy.
  3. Locate the initiating session. Use the session ID, user, repository, and time window to find the original prompt and all tool calls, including rejected calls that show what the model attempted.
  4. Compare intent with execution. Check whether the model asked for the path that changed, whether a shell expanded a glob, and whether a child process or hook performed the final operation.
  5. Check policy decisions. Determine whether the action was manually approved, automatically allowed, blocked and retried, or reviewed by an Auto-review system. Confirm the writable root and any sensitive-file exception.
  6. Correlate network events. Look for package downloads, remote scripts, or uploads immediately before the file change. A network event can explain an unexpected child process that is absent from a superficial agent transcript.
  7. Recover into a new worktree. Restore from version control, a checkpoint, backup, or filesystem recovery mechanism into a separate location. Preserve the damaged worktree for analysis until the incident is understood.
  8. Test the remediation. Re-run only the intended change under a narrower policy, verify the diff, and document the control change before reopening the original workflow.

Recovery and prevention after the incident

When the file is tracked

Use the last known-good commit or checkpoint to restore the file in a separate worktree, compare it with the damaged copy, and bring back only the intended edits. Avoid a blanket reset if unrelated work was present; it can erase useful evidence and legitimate changes.

When the file is untracked

Check agent checkpoints, editor local history, filesystem snapshots, container layers, and backups. If none exists, preserve the current storage and timestamps and use an approved recovery tool; do not assume that an agent transcript contains the original bytes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When secrets may have been exposed

Rotate affected credentials, inspect network-policy logs, and identify every process that could read the sensitive path. A successful file restore does not undo a prior upload. Treat the event as both an integrity problem and a possible confidentiality incident.

When the same mistake can recur

Turn the lesson into a policy change: narrow the writable root, add the path to sensitive-file protection, disable automatic approval for the command class, deny the unnecessary network destination, or require a disposable worktree. Add a regression test to the agent’s operating procedure so the control is checked before the next run.

Central telemetry for teams and security operations

For more than one developer or repository, keep agent events in a central, tamper-evident system. OpenAI describes preserving “agent-native telemetry so we can understand and audit what the agent did.” In practice, export structured events through an OpenTelemetry-compatible pipeline or equivalent collector, attach a stable session and worktree ID, and send them to the same security analytics system used for command and network logs.

Design retention around investigation needs: keep the prompt and policy context long enough to explain a later file change, protect logs from modification by the agent, and restrict access because prompts and command output may contain secrets. Alert on combinations that are more meaningful than any single event, such as a first-time agent identity requesting a recursive delete followed by an outbound connection, or a protected deployment file changing outside a release window.

Limits of current evidence

There is no authoritative, cross-agent statistic establishing how often accidental deletion or overwriting occurs. Rates vary with model, prompt, tool design, repository, approval mode, sandbox, and user review, so a single percentage would be misleading. The reliable engineering response is to constrain reach, make destructive actions reviewable, preserve before-and-after evidence, and keep integration reversible.

The controls described here also do not prove that a model’s internal plan was safe. They make the transition from plan to effect observable and bounded. That distinction matters: a blocked command is evidence of a successful control, while an allowed but mistaken command is a workflow failure that version control and review should catch before integration.
