In spot-check figures reported by The Independent in October 2026, 45 of 203 primary expert reviews of summaries produced by the Home Office’s Asylum Case Summarisation (ACS) tool failed accuracy standards—about 22%, or roughly one in five. Another 19 summaries were flagged for further review. These figures describe a checked subset of outputs, not the error rate for every summary the tool generated.
What the Home Office asylum AI tools do
The Home Office trialled two distinct AI tools between May and December 2024. Neither was designed to make asylum decisions.
Asylum Case Summarisation (ACS)
ACS uses a large language model to summarise asylum interview transcripts. In the pilot, decision-makers could use its summaries as an aid to familiarising themselves with a case.

APS is a chat-based search assistant for country policy information, including Country Policy Information Notes and Country of Origin Information Requests. Pilot users were expected to consult the underlying source material rather than treat the assistant’s summary as a substitute for it.

- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
The Home Office’s 2025 evaluation said the tools were intended to improve efficiency and “do not, and cannot, replace any part of the decision-making process.”
What the one-in-five figure means
The 45 failures came from 203 primary reviews of ACS summaries, according to The Independent’s October 2026 reporting on material obtained by Open Rights Group (ORG). That is about 22% of the primary reviews. A further 19 summaries were flagged for additional review; they are not part of the 45 failures reported in the primary-review count.

This is a result from spot-checks, not a census of all outputs. It does not establish that 22% of every ACS summary is inaccurate, or how many asylum decisions were affected. The reporting does not provide a full public account of the operational spot-check sampling method, and the available sources do not include a comparable independent audit of all live summaries.

Second reviews are a separate set
The same reporting said 30 of 56 summaries reviewed a second time were judged sub-standard. Those 56 second reviews are a separate set of review events; they must not be added to the 203 primary reviews to create a single failure rate. The reporting also describes 259 checks in total—203 primary reviews and 56 second reviews—which does not necessarily mean 259 unique cases.

Why the 2024 pilot’s 9% figure is different
The Home Office’s 2025 evaluation said that 9% of ACS summaries produced in the pilot were judged inaccurate or to have missing information and removed. Those cases then proceeded through the usual process. The pilot’s review process and period differ from the later operational spot-checks reported in 2026, so 9% and about 22% are not like-for-like measurements.
| Measure | What was counted | What it shows—and does not show |
|---|---|---|
| 2024 pilot: 9% | ACS summaries judged inaccurate or incomplete and removed after technical specialists reviewed them before use, according to the Home Office’s 2025 evaluation. | A pilot finding under a screening process that excluded flagged summaries; it is not a direct comparison with later spot-checks. |
| Later primary spot-checks: 45 of 203 | Primary expert reviews reported by The Independent in October 2026 from material obtained by ORG. | About 22% of that checked subset failed accuracy standards; it is not an error rate for all generated summaries. |
| Later second reviews: 30 of 56 | A separate set of second reviews reported by The Independent. | Sub-standard results in that separate set; not an overall failure rate and not to be combined with the primary-review denominator. |
For the pilot, 23% of users who received ACS summaries said they were not fully confident in the information. The evaluation also cautioned that the trial was small, the decision-making units were not selected to represent all units, and some measures relied on self-reporting. It found no apparent positive or negative impact on decision quality during the pilots, but that result cannot establish the effect of later operational use.

What is known about rollout and human oversight
The dates matter. In April 2026, a parliamentary answer said ACS had not yet been operationally rolled out. The Independent later reported that it was made available to all decision-makers from April 2026. The earlier answer and later account describe different points in time, not necessarily a contradiction.
ORG’s October 2026 briefing said its investigation found that only 0.3% of summaries generated between April and July had been checked by Home Office subject matter experts. ORG also said it found no quality assurance for generated ACS summaries or APS text, drawing on parliamentary answers, ministerial letters and Freedom of Information responses. Those are findings attributed to ORG’s investigation, not conclusions of the Home Office’s 2025 pilot evaluation.

The Home Office told The Independent: “Asylum decisions are made by trained decision-makers, not artificial intelligence.” That describes the department’s stated allocation of decision-making responsibility; it does not resolve how frequently summaries are checked or how errors in them are caught.
Efficiency estimates do not establish decision quality
In the 2024 pilot, ACS users reviewed transcripts an average of 23 minutes faster than the comparison group. APS users saved an average of 37 minutes per case researching country policy information. These are pilot estimates, not proof that decisions improved or that the same time savings—or outcomes—apply at scale.
The practical distinction is important: a tool can save time while still requiring effective checks on its output. The reported spot-check failures raise questions about accuracy and oversight, but the available figures alone do not show whether an error changed an asylum decision.

What remains unanswered
- How the operational spot-check sample was selected, and whether it represents the wider volume of ACS summaries.
- How many generated summaries were checked, by whom, and what corrective process followed when a check found a problem.
- Whether errors in summaries affected decisions, and how that effect was assessed.
- How the performance of live tools compares with the pilot under a consistent and independently auditable method.
The Independent reported that about 7,000 AI summaries were generated per month on average at the time of its October 2026 article, with use left to decision-makers. An internal impact assessment from October 2025 had predicted 17,515 cases a month. The first is a reported later average; the second is an earlier forecast, so they are not equivalent measures.
