Hives are raised, itchy welts on the skin’s surface. Angioedema is swelling deeper under the skin, commonly around the lips, eyelids, face, hands, feet, or throat. The two can occur together, but either one can also appear alone. Because swelling in the mouth, tongue, or throat can obstruct the airway, treat breathing trouble, throat tightness, fainting, wheezing, or sudden mouth and throat swelling as an emergency.

This guide explains the typical differences and the limits of comparing symptoms at home. It cannot identify the cause of an individual episode. Medication-related, hereditary, infection-related, and unexplained forms exist, so a first or recurring episode may need clinical assessment.

Hives and angioedema at a glance

The most useful first distinction is where the reaction is occurring. Hives (urticaria) affect the superficial skin and produce visible welts. Angioedema affects deeper skin and tissue layers, producing swelling that may be easier to feel than to see as a sharply outlined rash.

Feature Hives (urticaria) Angioedema
Depth Raised welts in the surface skin Swelling in deeper skin and tissue layers
Typical locations May appear anywhere on the body Often lips, eyelids, face, hands, feet, or throat
How it feels Usually itchy May feel painful or tight and is often not itchy
Appearance Pink, red, or skin-colored raised patches that vary in size Diffuse puffiness or enlargement rather than discrete welts
Can it occur alone? Yes Yes; the absence of hives does not exclude it
Typical time course Individual welts commonly fade within 24 hours without marks or bruising Duration varies with the cause and may require medical evaluation
Urgency Usually not an emergency unless airway or severe systemic symptoms occur Airway, breathing, or fainting symptoms require emergency care

These are patterns, not a diagnostic test. A person can have itchy hives and deeper swelling at the same time, and a person with angioedema may have no visible hives.

What hives look and feel like

Raised, itchy surface welts

Hives are raised areas on the skin that can be small or extensive and can develop on any body area. Individual lesions often change shape or location as others appear. Itching is the characteristic sensation, although the surrounding skin may also feel warm or irritated.

How long one hive lasts

The American Academy of Allergy, Asthma & Immunology (AAAAI) notes that an individual hive usually fades within 24 hours and does not leave a mark or bruise. New welts can appear while earlier ones disappear, making an episode seem continuous even though each lesion is transient. A spot that remains fixed, bruises, or causes persistent pain deserves medical review rather than being assumed to be an ordinary hive.

When hives become chronic urticaria

When hives persist or recur for more than six weeks, clinicians use the term chronic urticaria. That label describes the duration; it does not by itself identify the trigger. Recurrent symptoms that interfere with sleep, work, school, or eating are a reason to arrange an appointment even when each individual welt resolves within a day.

What angioedema means

Swelling under the skin

Angioedema is swelling in deeper tissue layers. It commonly affects the lips, eyelids, face, hands, feet, or throat. The area can feel tight, tender, or painful and is often not itchy. Because the swelling is deeper, the skin may look only mildly changed while the tissue underneath feels enlarged.

Angioedema with or without hives

Angioedema can accompany hives, but it can also occur by itself. Therefore, the presence of welts does not prove that deeper swelling is absent, and clear skin does not rule angioedema out. Tell a clinician about swelling that developed without a rash, especially if it is recurrent or unexplained.

Why the throat matters

Swelling involving the mouth, tongue, or throat can narrow the airway. The AAAAI states: “Throat swelling can be life threatening and requires immediate medical attention.” A person can deteriorate even if the skin symptoms look limited, so airway symptoms take priority over identifying whether the episode began as hives or angioedema.

When to seek emergency help

Call your local emergency number now for these symptoms

  • Difficulty breathing, shortness of breath, or struggling to breathe
  • Wheezing or noisy breathing
  • Throat tightness or a sensation that the throat is closing
  • Fainting, collapse, or feeling about to lose consciousness
  • Sudden swelling of the lips, mouth, tongue, or throat

The NHS advises immediate emergency action for sudden swelling of the lips, mouth, throat, or tongue and for difficulty breathing. Use the emergency number for your location; numbers differ between countries. Do not wait to see whether an over-the-counter product works when the airway, breathing, or consciousness is affected.

If another person is affected

Stay with them while emergency help is contacted, describe the onset and symptoms clearly, and report any known medicine, food, insect, or other recent exposure. If they become faint or have worsening breathing, tell the dispatcher immediately. Do not let a reassuring-looking rash delay help for throat or breathing symptoms.

If symptoms improve while help is on the way

Improvement does not establish that the danger has passed. Report what happened and follow the emergency service’s instructions. Airway swelling can be serious even when the visible swelling is changing.

When to arrange prompt medical assessment

Seek timely clinical advice for:

  • A first episode of angioedema, even if it has settled
  • Severe swelling or swelling that does not respond to the treatment you were advised to use
  • Recurrent, unexplained, or progressively more frequent episodes
  • Symptoms that persist rather than resolving in the expected pattern
  • Hives that continue or recur beyond six weeks

MedlinePlus notes that the cause of angioedema is often not found. An evaluation can still matter because different mechanisms require different management, and some medicines—including ACE inhibitors—are associated with angioedema. Do not stop a prescribed medicine on your own; ask the prescriber or another qualified clinician how to proceed.

Why the causes are not interchangeable

Possible hives triggers

Hives may follow exposure to a food, medicine, insect bite, infection, or environmental trigger. They can also occur without an identifiable cause. Some longer-lasting cases are inducible: heat, cold, pressure, or rubbing can provoke welts. Finding a trigger from timing alone is unreliable, particularly when several exposures occurred close together.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Possible angioedema mechanisms

Angioedema can be associated with allergic reactions, medicines, hereditary conditions, or other mechanisms. It may occur with hives or independently. Because treatment depends on the mechanism, the same self-care approach should not be assumed to work for every type.

Why “allergy” is not a complete explanation

Calling every episode an allergy can hide important differences. Medication-associated and hereditary angioedema are examples in which the underlying process may not be the same as ordinary itchy hives. A clinician may need the full history, including medicine use and family pattern, to decide what evaluation is appropriate.

How clinicians sort out the pattern

Assessment usually starts with the history and an examination of the skin and swollen areas. Be ready to explain which symptom appeared first, how quickly it developed, and whether each welt disappeared within a day. A clinician will also want to know whether the swelling was itchy, painful, or tight and whether it involved the lips, tongue, mouth, eyelids, hands, feet, or throat.

Medication review is important even when a medicine has been taken for a long time. Include prescription drugs, over-the-counter products, and supplements, plus the date of any new or recently changed product. ACE inhibitors are one medication group associated with angioedema, but only a qualified clinician can decide whether a medicine is involved and what should replace it, if anything.

Family history can be useful when swelling occurs without hives or repeats in a similar pattern. Tell the clinician if relatives have had unexplained swelling or episodes that led to emergency care. Also mention infections, insect bites, foods, heat, cold, pressure, or rubbing that preceded symptoms. These details do not prove a cause, but they help determine what evaluation is appropriate.

Understanding the timeline

A moving pattern of hives

With hives, one welt commonly disappears within 24 hours while another appears elsewhere. Photographing a rash can help show a clinician what it looked like, but a photograph cannot determine its cause or prove that deeper swelling is absent.

Swelling that lasts or returns

Angioedema does not have one universal duration. Note when swelling began, whether it is expanding, and whether it affects the lips, tongue, mouth, throat, hands, feet, or face. Swelling that persists, recurs, or occurs without an obvious explanation should be discussed with a clinician.

The six-week boundary

The six-week threshold applies to the chronic urticaria category, not to a rule that every episode lasting less than six weeks is harmless. Emergency symptoms override duration. A single episode with breathing difficulty needs emergency care regardless of how long the rash has been present.

What to record before an appointment

A concise timeline can make an assessment more useful:

  • When the first welt or swelling appeared and how quickly it changed
  • Whether each welt moved or faded within a day
  • Exact locations, especially the lips, tongue, mouth, throat, eyelids, hands, or feet
  • Itching, pain, tightness, wheezing, faintness, or other associated sensations
  • Foods, medicines, insect bites, infections, heat, cold, pressure, or rubbing shortly before symptoms
  • Any previous episodes and whether a family member has similar unexplained swelling
  • All medicines and supplements being taken, including recently started prescriptions

Bring photographs if the skin has changed by the time you are seen. Keep the original timing and wording rather than trying to label the episode yourself.

Symptom relief while arranging care

Antihistamines for some hives

AAAAI describes antihistamines as initial medication options for hives and names cetirizine, fexofenadine, loratadine, and others. MedlinePlus also notes that a provider may suggest an antihistamine such as cetirizine or diphenhydramine. Use only a product suitable for you and follow the package directions or your clinician’s instructions.

What antihistamines cannot establish

Relief after an antihistamine does not prove that the cause was an allergy. Conversely, lack of relief does not identify a different diagnosis. Antihistamines do not treat every form of angioedema, and they are not a substitute for emergency care when the throat, breathing, or consciousness is affected.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medication safety

Do not combine products, exceed labeled directions, or change a prescribed medicine without professional advice. If swelling began after a prescription or over-the-counter medicine, tell the clinician exactly what was taken and when.

Common mistakes to avoid

  • Waiting because the rash is small: airway risk depends on the location and symptoms, not the size of a skin rash.
  • Assuming no hives means no angioedema: deeper swelling can occur with clear skin.
  • Assuming all swelling is allergic: medicine-related, hereditary, and other forms exist.
  • Stopping a prescription abruptly: discuss suspected medicine reactions with a clinician instead.
  • Using symptom duration as a safety test: emergency signs require action immediately, regardless of whether the episode is new or longstanding.
  • Diagnosing from a photograph: images can document change but cannot determine the mechanism or rule out airway involvement.

A practical decision path

  1. Check airway and breathing first. Look for throat tightness, mouth or tongue swelling, wheezing, difficulty breathing, or fainting.
  2. If any emergency sign is present, call your local emergency number and seek emergency care immediately.
  3. If there is swelling without emergency symptoms, arrange prompt medical advice, particularly for a first, severe, persistent, recurrent, or unexplained episode.
  4. If there are itchy surface welts only, observe the timing and pattern, avoid suspected irritants when practical, and follow clinician or package directions for any antihistamine you use.
  5. If hives last or recur for more than six weeks, book an assessment for chronic urticaria.
  6. At every stage, record medicines and exposures and do not infer a cause solely from whether an antihistamine helped.

Examples of how the distinction changes urgency

Itchy welts that move and fade

Raised, itchy patches that appear on an arm, fade, and reappear elsewhere within a day fit the usual surface pattern of hives. If breathing, throat, or fainting symptoms develop, the situation becomes an emergency regardless of the skin pattern.

A swollen lip without a rash

A suddenly enlarged lip with otherwise clear skin can be angioedema. Absence of hives does not make it harmless. If the tongue, mouth, or throat is involved, or breathing is difficult, call the local emergency number immediately. Even without those signs, a first or unexplained episode warrants prompt assessment.

Repeated episodes over months

Recurring welts or swelling need a history-based evaluation rather than repeated self-diagnosis. The clinician can consider timing, medicines, family history, and possible physical triggers such as heat, cold, pressure, or rubbing.

How common are hives?

AAAAI states that about one quarter of the general population may experience hives during their lifetime. That frequency does not make a particular episode predictable or remove the need to act on airway symptoms.

Key points to remember

  • Hives are superficial, raised, and usually itchy; an individual welt commonly fades within 24 hours without bruising.
  • Angioedema is deeper swelling that may be painful and often is not itchy, frequently affecting the lips, eyelids, face, hands, feet, or throat.
  • The conditions can overlap, and either can occur without the other.
  • Breathing difficulty, wheezing, fainting, throat tightness, or sudden swelling of the lips, mouth, tongue, or throat requires emergency help.
  • First episodes, persistent or recurrent swelling, unexplained symptoms, and hives lasting beyond six weeks deserve medical advice.

Frequently Asked Questions

Can you have hives and angioedema at the same time?

Yes. Hives affect the surface skin and angioedema affects deeper tissue, so they may appear together. Hives also can occur without angioedema, and angioedema can occur with clear skin.

Is angioedema always caused by an allergy?

No. Allergic reactions are one possibility, but medicines such as ACE inhibitors, hereditary conditions, and other mechanisms can cause angioedema. A clinician may need your medication and family history to assess the cause.

When is lip swelling an emergency?

Call your local emergency number immediately if lip swelling is sudden and involves the mouth, tongue, or throat, or if there is trouble breathing, wheezing, throat tightness, fainting, or collapse. Do not wait for an antihistamine to work.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How long should an individual hive last?

AAAAI says an individual hive usually fades within 24 hours without leaving a mark or bruise. New welts can appear as old ones disappear, making the episode seem continuous.

What does chronic urticaria mean?

Chronic urticaria means hives persist or recur for more than six weeks. It describes the time pattern, not a specific trigger, and merits clinical discussion, especially when symptoms disrupt daily life.

Should I stop a medicine that might have caused swelling?

Do not stop a prescribed medicine on your own. Contact the prescriber or another qualified clinician, explain what you took and when, and ask how to proceed. ACE inhibitors are among medicines associated with angioedema.

Can an antihistamine treat angioedema?

Antihistamines are commonly used for some hives, but they do not treat every form of angioedema. They never replace emergency care for throat, mouth, tongue, breathing, wheezing, or fainting symptoms.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What information should I bring to an appointment?

Record onset and progression, whether welts moved or faded within a day, locations of swelling, itching or pain, recent foods, medicines, insect bites or infections, physical triggers, prior episodes, and any family history. Photographs can document changes.

The Bottom Line

Hives are itchy, raised surface welts; angioedema is deeper swelling that may be painful and may occur without a rash. The distinction matters less than the warning signs: sudden mouth, tongue, throat, or lip swelling, breathing difficulty, wheezing, throat tightness, or fainting requires immediate emergency help. Arrange prompt medical advice for a first, persistent, recurrent, severe, or unexplained swelling episode, and for hives lasting more than six weeks.
