Lifespan is how long a person lives. Healthspan is how many of those years are spent in good health, with the function and well-being needed for daily life. They are related, but they are not interchangeable: a population can add years to life without adding the same number of years in full health.
For population statistics, life expectancy is the usual measure of length of life, while the World Health Organization’s healthy life expectancy (HALE) estimates years lived in full health after accounting for disease, disability and the severity of health states. Understanding the difference helps you read health statistics accurately and set more realistic goals for ageing well.
Lifespan, life expectancy and healthspan: the basic distinction
These terms describe different outcomes and operate at different levels.
Lifespan is an individual’s total time alive
An individual’s lifespan runs from birth until death. It is known only after that person’s life has ended. Saying that someone “has a lifespan of 84 years” describes an observed life, not a forecast.

Life expectancy is a population estimate
Public-health statistics usually use life expectancy when comparing how long people live. The World Health Organization defines life expectancy as “the average number of years a person is expected to live.” The estimate is tied to a specified population, place, year and starting age, and it reflects mortality conditions in that setting. Life expectancy at birth and life expectancy at age 65 answer different questions.
Life expectancy is therefore not a personal deadline. It is a statistical average derived from the death rates used for the selected population and period. An individual may live substantially shorter or longer than the reported average.
Healthspan describes healthy, functional years
Healthspan is a reader-friendly term for the portion of life spent in good health and with acceptable function and well-being. Unlike life expectancy, it does not have one universally accepted clinical cutoff. One study might define healthy years by the absence of a particular disease; another might use disability, cognitive function, physical capability or self-rated health.

HALE is the main standardized population measure
The closest standardized counterpart to healthspan is healthy life expectancy, abbreviated HALE. WHO defines it as the “average number of years that a person can expect to live in ‘full health’.” HALE combines mortality information with morbidity information and adjusts for the distribution and severity of different health states.
HALE is still a population estimate. It does not predict the exact number of healthy years available to one person, and it should not be read as a guarantee that a particular individual will remain healthy until that age.
How the measures compare
| Question | Lifespan or life expectancy | Healthspan or HALE |
|---|---|---|
| What is counted? | Total time alive; life expectancy summarizes expected years of life. | Years lived in full or good health, with health states weighted by their severity in HALE. |
| What level does it describe? | Lifespan is individual; life expectancy is population-level. | Healthspan can describe an individual, but HALE is a population-level estimate. |
| How are illness and disability treated? | Every year alive counts equally toward length of life. | Years affected by disease or disability are discounted according to the measure’s health-state definitions and severity. |
| Is function included? | Not directly. A person’s years count whether or not they can perform valued activities. | Yes, depending on the definition. HALE incorporates health states; broader healthy-ageing frameworks also consider function, independence and well-being. |
| Can it predict one person’s future? | No. Averages describe groups, not an individual’s exact outcome. | No. HALE is a modeled average and individual healthspan depends on personal and environmental circumstances. |
Why living longer does not always mean living better
Two people can reach the same age while experiencing very different years of illness, disability or loss of independence. If one person remains able to move around, think clearly, maintain relationships and manage daily activities for most of later life, that person has a different experience from someone who spends many years with severe limitations—even if both have the same lifespan.

Life expectancy counts survival. Healthspan asks about the quality and capability of the years that survival produces. A rise in life expectancy can therefore coincide with a larger, smaller or unchanged gap between total years and healthy years. That gap is often called years lived in less than full health, but its size depends on the definition and data used.
This distinction matters for more than personal planning. Health systems use mortality and health-status information to estimate future needs for chronic-care services, rehabilitation, accessibility and support for independent living. Governments, employers and communities may also evaluate whether added years are accompanied by the ability to participate and contribute.
What HALE measures and how it is calculated
Mortality and morbidity are combined
HALE starts with information about how many people die at each age and combines it with information about disease, disability and other health states. The calculation estimates how much time a population spends in each state and adjusts years according to the severity distribution of those states. In simplified terms, years lived with a serious limitation contribute less than years lived in full health.
The result is not a simple survey average and not merely life expectancy minus the number of people with a diagnosis. It is a modeled estimate that integrates mortality and morbidity data using standardized methods.

Every figure needs its context
Read a HALE or life-expectancy number with all of its labels attached:
- Geography: country, region or other population covered.
- Reference year: the mortality and health conditions represented by the estimate.
- Starting age: at birth, at age 65 or another specified age.
- Sex: some statistics are reported separately for females and males, while others use a combined population.
- Method and data source: survey definitions, mortality records and model revisions can change the result.
WHO notes that modeled estimates may differ slightly from official national life tables. A difference does not automatically mean that one figure is wrong; the two sources may use different data, definitions or estimation methods.
A current example, with its limits
On WHO’s Europe dashboard, healthy life expectancy at birth rose from 62.7 years in 2000 to 66.7 years in 2023, a four-year increase. Those values describe the dashboard’s Europe geography and the years shown; they are not a timeless global average and do not predict any individual’s healthy years. Dashboard figures can be revised as methods and underlying data are updated.

Healthspan is broader than the absence of disease
A diagnosis-only definition can miss capabilities that determine whether life feels healthy. WHO defines healthy ageing as “the process of developing and maintaining the functional ability that enables well-being in older age.” Functional ability depends on three interacting elements:
- Intrinsic capacity: the person’s physical and mental capacities, such as movement, cognition, hearing and psychological ability.
- Environment: the physical, social and policy surroundings in which the person lives.
- The interaction between them: accessible transport, supportive relationships or assistive technologies can allow someone with a limitation to continue doing valued activities.
Under this framework, healthspan concerns more than whether a clinician has recorded a disease. Mobility, cognition, relationships, independence, the ability to meet basic needs and the ability to contribute all affect functional well-being. Someone with a chronic condition may retain substantial function, while someone without a formal diagnosis may still face disabling limitations.
How researchers operationalize “healthy years”
Because healthspan has no single universal endpoint, researchers choose a measure that fits the question. Common approaches include:
Healthy life expectancy (HALE)
HALE is designed for standardized comparisons between populations. It combines mortality and morbidity and accounts for the severity of health states, making it useful for monitoring broad trends.
Disability-free life expectancy
This measure estimates years expected without a defined disability. The result depends on the disability questionnaire, threshold and population used.
Disease-free or condition-free years
A study may count years without a specified disease or group of diseases. Such a measure can be precise for that condition but cannot stand in for overall healthspan.
Function- or cognition-based years
Some analyses focus on mobility, activities of daily living, cognitive performance or another functional endpoint. These measures can reveal changes that a diagnosis count misses.

Self-rated health and composite measures
Surveys may ask people to rate their health or combine several domains into an index. These approaches capture personal experience but can be affected by culture, expectations, wording and response patterns.
When comparing two “healthspan” studies, first check whether they measure the same endpoint. A higher value under one definition is not necessarily evidence that another population has more years of full health under a different definition.

How to read a healthspan statistic without being misled
- Identify the measure. Is the number lifespan, life expectancy, HALE, disability-free expectancy or a disease-specific estimate?
- Check the starting point. “At birth” and “at age 65” are not interchangeable.
- Check the population and year. A national estimate from one year cannot be treated as a current figure for every country.
- Read the health definition. Look for the diseases, disabilities, functional tests or self-rated categories included.
- Look for sex-specific reporting. Combined values can conceal differences between groups.
- Check whether the value is modeled. Modeled estimates may differ from official life tables and may be revised.
- Separate averages from predictions. The statistic describes a population under stated conditions, not your personal forecast.
- Compare like with like. Trends are most meaningful when geography, age, sex, definition and method remain consistent.
What the gap between life expectancy and HALE can tell you
The difference between life expectancy and HALE is sometimes used as a rough indicator of years lived with less than full health. It is useful for asking whether added survival is accompanied by added healthy function, but it is not a complete description of lived experience.
A gap can reflect many conditions: multiple chronic diseases, temporary illness, permanent disability, pain, cognitive impairment or limitations in daily activities. HALE’s weighting method determines how strongly each state affects the estimate. A change in the gap may therefore result from real health changes, revised prevalence data, a new severity distribution or a methodological revision.
Do not interpret a larger gap as proof that every extra year is severely disabling, or a smaller gap as proof that all added years are free of symptoms. The statistic compresses varied health states into a population average.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why the distinction matters to individuals
It clarifies what a goal actually means
“Live longer” and “stay capable longer” are different goals. A person thinking about ageing may care about walking safely, managing personal tasks, maintaining relationships, continuing meaningful work or hobbies, and making decisions independently. Those are healthspan and functional-ability outcomes, not simply survival outcomes.
It prevents false precision
No lifestyle action can guarantee a fixed number of additional healthy years. Genetics, prior illness, exposures, access to care, social conditions and the built environment all influence outcomes. Population statistics can inform choices and planning, but they cannot assign an exact personal healthspan.

It supports better conversations with professionals
When discussing an estimate, ask which outcome is being measured: survival, absence of a disease, freedom from disability, physical function, cognition or overall well-being. A clear endpoint helps distinguish a realistic target from a vague promise.
Why the distinction matters to communities and policy
Longer survival changes the number of people who may need health and social services, but the type of need depends on functional ability. Planning based only on life expectancy can miss demand for rehabilitation, accessible housing, transportation, caregiving, dementia support and adaptations that enable independence.
WHO’s healthy-ageing framework emphasizes that environments can either constrain or expand functional ability. A community with accessible buildings, safe streets, social connection and appropriate services may allow people with the same intrinsic capacity to do more. Healthspan is therefore partly a property of surroundings and systems, not only an attribute of an individual body.

“Healthspan is just life expectancy minus sick years.”
Not exactly. Some healthspan measures subtract or weight unhealthy states, but the result depends on definitions and severity assumptions. HALE is a modeled measure that combines mortality and morbidity rather than a simple subtraction of everyone’s diagnosed conditions.
“HALE tells me how many healthy years I personally have.”
No. HALE is an average for a specified population, place, year, age and often sex. Individual outcomes vary widely around that average.
“A chronic diagnosis means healthspan has ended.”
A diagnosis alone does not define functional ability. People can live with chronic conditions while retaining mobility, cognition, relationships and independence. Conversely, functional limitations can exist without a single defining diagnosis.
“The newest number is always directly comparable with an older one.”
Only when the geography, age, sex, endpoint, data and method are sufficiently consistent. Revisions can change historical estimates, and different countries may use different national definitions.

Life expectancy and HALE can move differently. A population may survive longer while spending more years with limitations, or it may gain both survival and healthy function. The two measures must be examined together.
A practical checklist for reporting or sharing these numbers
- Use “lifespan” for an individual’s completed time alive and “life expectancy” for a population average.
- Define “healthspan” or name the specific measure instead of presenting it as a universal clinical number.
- Write the geography, year, starting age and sex alongside every statistic.
- State whether the estimate is modeled and acknowledge possible differences from national life tables.
- Explain what counts as less than full health and how severity is handled.
- Avoid turning a population average into a personal promise.
- Include function, independence and environment when discussing healthy ageing.
The bottom line for readers
Lifespan answers “How long are people alive?” Healthspan answers “How many of those years are lived with good health and usable function?” Life expectancy is the standard population measure for length of life; HALE is the standardized population measure that estimates years in full health. Neither number can predict one person’s future, and neither captures every dimension of well-being by itself.
The most informative view places the measures side by side, with their definitions and context. Longer lives matter, but so do the mobility, cognition, relationships, independence and supportive environments that make those years meaningful.
