Neither glass ionomer nor composite resin is the best choice for every filling. Composite is commonly selected when a tooth-colored direct restoration is needed and the dentist can maintain a clean, dry field. Glass ionomer—either conventional or resin-modified—can be useful for selected cavities, higher caries risk, limited cooperation, or situations in which moisture control is difficult, provided the specific product is approved for that use.

The right material depends on the tooth, restoration class, chewing load, cavity size, appearance goals, caries risk, isolation, and the formulation’s instructions. A dentist must match the material and placement technique to those conditions rather than apply a universal rule.

What glass ionomer and composite resin are

“Glass ionomer” is a family of dental materials, not one identical product. Conventional glass-ionomer cement sets through an acid–base reaction between an ion-releasing glass powder and an aqueous acid. Resin-modified glass ionomer (RMGI) adds resin components and usually uses light activation or a combination of setting reactions. Their handling, strength, moisture tolerance, fluoride behavior, and approved indications can differ, so the dentist’s exact formulation matters.

The American Dental Association (ADA) describes glass ionomers primarily as cement or cavity-liner materials and generally as materials for non-load-bearing fillings. They chemically interact with tooth structure and can release and later recharge fluoride. That fluoride behavior may be useful in a caries-management strategy, but the degree to which it prevents future decay remains debated. Fluoride release does not make every glass ionomer suitable for a large or heavily loaded restoration.

Composite resin is a tooth-colored restorative material made from a resin matrix and inorganic filler particles. It is bonded to enamel and dentin with an adhesive system and placed directly into the prepared tooth. Shade matching and incremental placement can produce a natural appearance, but the procedure is technique-sensitive: contamination by saliva, blood, or crevicular fluid can compromise the bond.

Both categories include products with different compositions and approved uses. A product’s label and the dentist’s intended restoration—not the material’s name alone—determine whether it is appropriate.

Glass ionomer vs composite resin at a glance

Decision axis Glass ionomer Composite resin
Typical strengths Chemical adhesion to tooth structure; fluoride release and recharge; lower moisture sensitivity in certain uses Natural tooth color; adhesive bonding; broad direct-restoration indications
Main constraints Conventional types have relatively low stress-bearing properties and greater susceptibility to abrasive wear than resin composite Requires appropriate adhesive technique and control of moisture and contamination
Situations in which guidelines may allow it Selected small cavities, increased caries risk, limited compliance, or difficult dry isolation; permanent posterior use only when the product is approved for that indication Direct Class I and II restorations in permanent teeth; Class III and IV defects; Class V when contamination control and adhesive technique are adequate
Appearance Usually less translucent and less shade-matched than a well-finished composite, although appearance varies by formulation and site Designed to match tooth shade and translucency more closely
Longevity evidence No universal ranking; performance depends on formulation, site, technique, and indication No universal ranking; study results vary by tooth, cavity class, follow-up, and material generation

How a dentist decides which material fits

A dentist normally considers several factors together. One favorable property cannot compensate for a mismatch between the material and the restoration.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

1. Tooth and restoration class

The restoration class describes where the defect is located. Class I cavities are pits and fissures on chewing surfaces. Class II cavities involve contact areas between posterior teeth. Class III defects are on the proximal surfaces of front teeth without involving the incisal edge; Class IV defects include an incisal edge; Class V defects are near the gumline on the facial or lingual surface.

Location changes the forces, visibility, access, and ability to isolate the tooth. A material that is reasonable for a small cervical lesion may be a poor choice for a broad molar chewing surface. The ADA-supported guideline for permanent teeth supports direct composite for Class I and II restorations. It recognizes glass-ionomer cement as an alternative in specific circumstances, including smaller cavities, increased caries risk, or limited compliance, and specifies that the product must be approved by its manufacturer for permanent posterior use.

2. Chewing force and cavity size

Posterior teeth transfer substantial compressive and shear forces. Conventional glass ionomer has lower stress-bearing capacity than composite and is more vulnerable to abrasive wear, so it is not appropriate for every high-stress site. A small, well-contained lesion may impose a very different demand from a large restoration replacing a cusp or a substantial portion of a marginal ridge.

The dentist also evaluates remaining enamel and dentin, cusp thickness, occlusion, bruxism or clenching, and whether the restoration will contact an opposing tooth. If the defect is large or the tooth is structurally weakened, the discussion may involve a different restoration design rather than simply choosing between two direct filling materials.

3. Caries risk and fluoride strategy

Glass ionomer can release fluoride and take up fluoride again from sources such as toothpaste or professional applications. This makes it attractive in selected patients with recurrent decay risk, root-surface lesions, or difficulty maintaining ideal plaque control. However, fluoride release should be viewed as one part of prevention, not a guarantee that decay will not recur. The ADA notes that the amount of caries protection provided by this property remains uncertain.

Daily fluoride toothpaste, dietary habits, saliva flow, oral hygiene, recall intervals, and the seal of the restoration still matter. A high caries risk may support choosing a glass-ionomer option where its mechanical demands are acceptable, but it does not override the need for a material approved for the specific tooth and site.

4. Moisture and contamination control

Composite bonding depends on a clean, controlled interface. Rubber dam isolation, cotton rolls, suction, retraction, and careful sequencing may be used to keep saliva and blood away. Deep margins near the gum, partially erupted teeth, a strong gag reflex, or limited ability to keep the mouth open can make this difficult.

Some glass-ionomer uses are less moisture-sensitive than composite placement, particularly when a completely dry field cannot be maintained. “Less moisture-sensitive” does not mean moisture is harmless: excess water, saliva contamination, or early dehydration can still damage a setting glass ionomer. The dentist follows the product’s instructions for conditioning, mixing, placement, protection, and finishing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Appearance expectations

Composite is generally favored when the restoration is visible and the patient wants close matching of shade, translucency, and surface anatomy. It can be layered in different opacities and polished to a glossy finish.

Glass ionomer can be tooth-colored, but conventional formulations commonly have less optical depth and may be more noticeable, especially on front teeth. In a posterior tooth, at the gumline, or in a situation where caries control is the priority, a small appearance difference may be an acceptable trade-off. The dentist should show or describe the likely result for the exact product and location.

6. Product indication and formulation

Not every glass ionomer is approved for permanent posterior restorations, and not every composite is intended for every technique or thickness. Ask whether the proposed material is conventional glass ionomer, resin-modified glass ionomer, or composite; which restoration class it is approved for; and whether the manufacturer’s instructions require a liner, conditioner, adhesive, light curing, or a protective coating.

Where glass ionomer may be appropriate

Small or conservatively prepared cavities

When a cavity is small and the expected load is modest, a glass-ionomer restoration may provide chemical adhesion with less reliance on a highly moisture-sensitive bonding protocol. This is especially relevant when preserving tooth structure and managing caries risk are central goals.

Higher caries risk

For a patient with frequent new lesions, exposed root surfaces, reduced saliva, or difficulty maintaining plaque control, fluoride release and recharge may be considered among the benefits. The choice still depends on wear, occlusion, access, and the product’s approved indication.

Limited cooperation or difficult isolation

Children, patients with a strong gag reflex, people who cannot keep their mouth open for long, and teeth near the gumline can present practical challenges. In selected cases, the dentist may favor a glass-ionomer approach because it is more forgiving of limited dryness than a composite procedure. The material is not automatically suitable simply because isolation is difficult; the dentist must confirm that the restoration will tolerate the forces at that site.

Class V and cervical lesions

For Class V defects, guidelines allow glass ionomer or modified glass ionomer as alternatives. These lesions may be affected by root-surface anatomy, gum moisture, flexure, and abrasion from brushing. A separate review of non-carious cervical lesions found similar performance across the outcomes it evaluated, but results varied with follow-up period, operators, lesion characteristics, brands, and definitions of success. Those findings apply to cervical lesions and should not be generalized to every carious cavity.

Limits in high-stress sites

Conventional glass ionomer’s lower stress-bearing properties and greater abrasive wear susceptibility limit its use in large posterior restorations, heavy occlusal contacts, and other high-load locations. Resin modification can change handling and physical behavior, but it does not turn every RMGI into a universal substitute for composite. The dentist must verify the exact product’s indication.

Where composite resin may be appropriate

Posterior Class I and II restorations

For permanent teeth, the direct-restoration guideline supports composite for Class I and Class II cavities. Composite can be bonded to prepared enamel and dentin and shaped to restore contact, marginal ridges, and occlusal anatomy. The procedure may require incremental placement and curing to manage polymerization shrinkage and achieve complete light activation.

The result depends on isolation, adhesive selection, curing-light access, preparation design, and finishing. A composite restoration placed in a contaminated field may not perform as intended, even if the material itself is well suited to the tooth.

Front-tooth Class III and IV defects

Composite is supported for direct Class III and Class IV restorations, where shade, translucency, and the ability to sculpt an incisal edge are important. The guideline advises against glass ionomer for permanent restoration of these defects. A dentist may still discuss other treatment designs when a defect is extensive or the remaining tooth structure is compromised.

Class V when isolation is achievable

Composite can be used for Class V lesions when the dentist can control contamination and apply the adhesive system correctly. If the margin is persistently wet or bleeding, a glass-ionomer or modified glass-ionomer alternative may be considered instead, depending on the lesion and product indication.

What the evidence says about longevity

Longevity comparisons are less definitive than many online summaries suggest. An ADA-supported systematic review published in 2023 assessed 38 randomized controlled trials involving primary and permanent teeth and several restoration classes. Because failure events were infrequent, the review concluded that there was limited evidence to establish important overall differences among direct restorative materials. The certainty of evidence varied across comparisons.

The ADA materials overview states: “Overall, there is limited evidence to indicate a significant difference among restorative materials in longevity.” This is not a promise that every material lasts the same length of time. It means available trials have not consistently produced a reliable, universal ranking that applies to every tooth and cavity.

A 2023 review focused on posterior permanent teeth included 10 studies. Its analyses involved 849 high-viscosity glass-ionomer restorations and 800 composite restorations. Clinical performance was comparable through 36 months in the evaluated comparisons. The only reported poorer result for high-viscosity glass ionomer relative to composite was greater wear in Class I restorations at 24 months. The 36-month follow-up is a bounded study period, not evidence of equal lifetime performance.

Another review of non-carious cervical lesions also reported similar performance across its evaluated outcomes, while emphasizing variation in follow-up, operators, lesion characteristics, brands, and outcome criteria. Because cervical lesions differ from many carious cavities, those results cannot be transferred automatically to posterior Class I or II restorations.

What happens during placement

Composite procedure

  1. Assessment and shade selection: The dentist examines the tooth, checks the bite, and may select a composite shade before the tooth becomes dehydrated.
  2. Isolation: The tooth is kept dry with a rubber dam or other retraction and suction methods.
  3. Preparation: Decayed or unsupported structure is removed while preserving sound tooth where possible.
  4. Conditioning and bonding: Etching or a self-etch protocol, primer, and adhesive are applied according to the system’s instructions.
  5. Incremental placement and curing: Composite is placed in controlled increments and light-cured. Each increment must receive adequate light exposure.
  6. Finishing and bite adjustment: The dentist shapes the anatomy, polishes the surface, checks the contact, and adjusts any high spot in the bite.

Glass-ionomer procedure

  1. Material selection: The dentist chooses a conventional or resin-modified product approved for the restoration’s location and tooth type.
  2. Isolation: The field is controlled even though some uses tolerate limited moisture better than composite. Saliva pooling and blood contamination still need to be prevented.
  3. Tooth conditioning: A conditioner may be applied if required by the product instructions.
  4. Mixing or dispensing: Hand-mixed powder and liquid or a capsule system is used within the manufacturer’s working time.
  5. Placement and protection: The material is adapted to the cavity and may need a coating or surface protection during the early set.
  6. Finishing and occlusion: The dentist removes excess, checks contacts and bite, and finishes the restoration when the material has reached the appropriate stage.

These are material-dependent protocols, not do-it-yourself instructions. Product instructions, curing equipment, isolation, and the operator’s training affect the result.

Trade-offs patients should discuss

  • Strength versus moisture tolerance: Composite offers broad direct indications but needs strict adhesive and moisture control. Glass ionomer may be more practical in selected damp or difficult-to-isolate situations but has lower wear and load resistance in conventional forms.
  • Appearance versus caries-management features: Composite usually provides the closest cosmetic match. Glass ionomer’s fluoride release may be attractive for a high-caries-risk patient, although its protective effect is not guaranteed.
  • Immediate function versus maintenance: A restoration exposed to heavy chewing, grinding, or brushing abrasion may require a material and design with adequate wear resistance. Any restoration can chip, wear, leak, or develop recurrent decay.
  • Preservation of tooth structure: Both materials can be used in conservative direct restorations, but the preparation and bonding strategy differ. The dentist chooses the design that leaves the tooth adequately supported.
  • Repairability: Small defects may sometimes be repaired or polished rather than replaced, depending on their cause and extent. The dentist must determine whether the problem is surface wear, marginal leakage, recurrent decay, fracture, or bite-related damage.

How to prepare for the decision

Bring a focused set of questions to the appointment:

  • Which tooth and restoration class are being treated?
  • Will this restoration carry substantial chewing force or contact an opposing tooth heavily?
  • Is the proposed material conventional glass ionomer, resin-modified glass ionomer, or composite?
  • Is that specific product approved for this tooth and restoration site, including permanent posterior use if relevant?
  • How do my caries risk, saliva flow, oral hygiene, or previous fillings affect the choice?
  • Can the area be isolated well enough for the proposed adhesive technique?
  • What cosmetic difference should I expect at this location?
  • What wear, fracture, staining, repair, or replacement issues are most likely for this restoration?
  • How should I protect the restoration with brushing, fluoride use, diet, and follow-up visits?

After the restoration: maintenance and warning signs

Brush twice daily with fluoride toothpaste, clean between teeth, and follow any additional fluoride or diet advice from the dental team. Preventive care matters for both materials because a sound restoration does not protect the surrounding tooth from new plaque-related decay.

Contact the dentist if you develop persistent sensitivity, pain on biting, a sharp edge, a loose or missing piece, food trapping, a change in the bite, or visible darkening around a margin. Do not try to glue, file, or fill the restoration yourself. The appropriate treatment depends on whether the cause is early postoperative sensitivity, a high bite, wear, fracture, loss of adhesion, or recurrent decay.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Regular examinations allow the dentist to monitor margins, contacts, wear, and new disease. A restoration’s expected service depends on the tooth, material, placement conditions, habits such as clenching, and ongoing caries risk, so no study average can predict one individual’s exact lifespan.

Is glass ionomer better than composite?

It is better only for particular clinical circumstances. Glass ionomer may be the more suitable option when a selected small or cervical restoration has elevated caries risk, limited cooperation, or difficult isolation and the product is indicated for that use. Composite is often the better fit for tooth-colored restorations, permanent Class I and II cavities when isolation is reliable, and Class III or IV defects in permanent teeth.

The decision is not a contest between brand names or a promise of maximum longevity. It is a risk-and-site match: restoration class, load, moisture control, caries activity, appearance, formulation, and the dentist’s technique all matter. The most useful answer for a patient is the material recommendation tied to those specific findings.
