Gene therapy and retinal implants are different treatments for different patient groups—not interchangeable options for inherited blindness. In the United States, LUXTURNA is indicated for retinal dystrophy caused by confirmed mutations in both copies of the RPE65 gene, provided enough retinal cells remain. The Argus II retinal prosthesis was intended for a narrowly selected group of adults with severe-to-profound retinitis pigmentosa and requires surgery, device fitting, follow-up, and visual rehabilitation. Neither should be understood as a way to restore normal sight for everyone with inherited vision loss.
What is the key difference?
LUXTURNA is a prescription gene therapy for a specific genetic cause of retinal dystrophy. Argus II is an implanted electronic retinal prosthesis, not gene therapy. The first pathway depends on confirming a particular genetic diagnosis and assessing whether enough retinal cells remain; the second involves a device-based intervention for a narrowly defined group of adults with severe-to-profound retinitis pigmentosa.
That distinction matters because “inherited blindness” covers many conditions. The U.S. Food and Drug Administration notes in its January 2020 guidance, Human Gene Therapy for Retinal Disorders: Guidance for Industry, that these disorders vary in cause, prevalence, diagnosis, and management. A diagnosis such as retinitis pigmentosa alone does not establish eligibility for either option.

Who may be eligible for LUXTURNA?
Genetic confirmation comes first
The FDA indication is limited to retinal dystrophy associated with confirmed mutations in both copies of RPE65. Genetic testing is needed to establish whether the cause matches that indication; a broad diagnosis of inherited retinal disease is not enough.

Enough retinal cells must remain
Even when the genetic result matches, the manufacturer says a healthcare professional must determine whether enough retinal cells remain for treatment. This makes specialist assessment of retinal structure part of the decision, alongside the genetic result.
Age information comes from manufacturer material
The manufacturer’s patient information describes LUXTURNA for patients aged 12 months or older. Because this age detail is from manufacturer material, confirm current regulatory labeling and the treating specialist’s assessment rather than treating it as a universal rule for every jurisdiction.

Who may be eligible for an Argus II implant?
The FDA Humanitarian Device Exemption record describes use in a selected group of adults aged 25 or older with severe-to-profound retinitis pigmentosa. The record includes visual-function requirements, including prior useful form vision and lens-related criteria, as well as the ability and willingness to complete clinical follow-up, device fitting, and visual rehabilitation. A lack of residual light perception does not automatically settle candidacy: the record calls for confirmation of intact inner retinal function in that circumstance.
These are specialized device criteria, not a general route for anyone with inherited blindness or any form of retinitis pigmentosa. A specialist experienced with retinal prostheses would need to assess candidacy and the practical care pathway.

What are the care burdens and known risks?
LUXTURNA
The manufacturer’s patient information warns of serious potential eye infection, including endophthalmitis that may lead to blindness. Discuss the risks, procedure, monitoring, and follow-up with the treating retinal specialist, using current product labeling for clinical decisions.
Argus II
The FDA record establishes that the pathway involves implantation, fitting, follow-up, and visual rehabilitation. The sources reviewed here do not establish a complete, current complication profile for Argus II or a balanced safety comparison with LUXTURNA. Ask the implant center for procedure-specific risks and what follow-up and device support it can provide.

Can one treatment be called more effective?
No head-to-head comparison between LUXTURNA and a retinal prosthesis is established by the available evidence described here. The treatments address different patient groups and have different eligibility requirements, so a simple ranking would be misleading. The sources also do not provide a matched comparison of outcomes that would show which approach is better for a particular person.

Neither option should be presented as a restoration of normal sight. A clinician can discuss realistic functional goals for an individual based on the diagnosis, remaining retinal function, overall health, and the treatment being considered.

What should patients check before pursuing either pathway?
- Get the precise diagnosis. Ask which gene and variants were identified, and whether the result confirms mutations in both copies of RPE65 if LUXTURNA is being considered.
- Ask whether the treatment’s criteria fit. For LUXTURNA, discuss the genetic indication and whether enough retinal cells remain. For a prosthesis, ask how the FDA record’s age, visual-function, and follow-up requirements apply to the individual case.
- Discuss expected function, not just the treatment name. Ask what visual or practical outcome is realistic for this person and what evidence supports that expectation.
- Review the full care pathway. Clarify procedure-specific risks, follow-up appointments, rehabilitation needs, and—for an implant—who can provide device fitting and ongoing support locally.
- Verify local access. Check with a retinal specialist and the relevant national regulator. An FDA record or the termination status of a post-approval study does not answer whether an implant is currently available or supported in a particular country.
- Ask about other appropriate options. A specialist can say whether a clinical trial or another care approach is relevant to the specific diagnosis.
