Bootstrap gives you the form controls and layout tools to build a survey, but a template is only the front end: it does not collect, store, or analyze responses on its own. The example below uses Bootstrap 5 and shows how to label questions, choose suitable controls, and prepare the form for connection to a submission endpoint or form service.
Choose a survey layout that fits your questions
Bootstrap’s form controls can be arranged into a questionnaire, and a Bootstrap 5 survey-form example is available from MDB. Bootstrap’s default form layout stacks fields vertically because its controls are generally block-level and full-width; see the Bootstrap 5.0 layout documentation.
A vertical layout is a practical starting point for mixed question types, longer prompts, and narrow screens. Group related questions and use columns only when the resulting labels and choices remain easy to follow at the viewport sizes your audience uses. Bootstrap documents form controls, layouts, and validation for version 5.3; use documentation for the same Bootstrap version as your project.
Bootstrap 5 survey form example
This standalone example provides a short questionnaire with a single-choice question, a multiple-choice question, a comment field, and a required consent choice. It is HTML for Bootstrap 5.3 and assumes Bootstrap’s CSS is already loaded by your page.

</form>Replace /survey with the route your application handles or the destination specified by your form service. The control names and values are what the receiving system uses to identify answers, so align them with that system’s expected fields.
Match each question to the right control
- One answer from a fixed set: use a select or radio buttons. Radio buttons make the available answers visible at once; a select can take less space when the list is longer.
- More than one answer: use checkboxes, and make clear that respondents may select multiple options.
- A written response: use a textarea when a comment or explanation is genuinely useful. Mark the question optional when an answer is not needed.
- Required answers: use the HTML
requiredattribute only for questions that must be answered, and explain requirements in the label or nearby instructions.
Bootstrap provides controls including selects, checks and radios, range inputs, input groups, floating labels, and validation styles in its Forms v5.3 documentation. Choose the simplest control that accurately represents the question rather than using a control for visual variety.

Make labels, instructions, and errors accessible
Every control needs an accessible name. Bootstrap explicitly advises that all form controls have an appropriate accessible name so assistive-technology users can understand their purpose; a visible label connected through matching for and id values is a straightforward approach. Do not rely on placeholder text as the only label.

For grouped radios or checkboxes, use a <fieldset> with a <legend> to give the choices a shared question. Associate field-specific instructions or error text with the relevant control using aria-describedby; Bootstrap describes this approach in its Forms v5.2 help-text guidance. If your interface inserts an error message after submission, ensure the message is associated with the input and that the error is not communicated by color alone.

Bootstrap’s default stacked layout is documented in its v5.0 layout guide. If you introduce multi-column rows, check that reading order and label-to-control relationships still make sense on small screens.
Validation depends on the Bootstrap version
Bootstrap’s validation presentation is not interchangeable across major versions. In its version 4.5 forms documentation, Bootstrap stated that its custom client-side validation styles and tooltips were not accessible because they were not exposed to assistive technologies. That is a specific historical warning about that version’s custom presentation, not a blanket statement about every Bootstrap validation method.

Use the validation guidance for the exact Bootstrap version in your project, and verify your implementation with keyboard navigation and assistive technology. Native HTML constraints such as required can help browsers identify missing values, but the server or receiving application must still validate submitted data.

Connect the form to a submission destination
The form markup defines questions and controls; it does not by itself provide response collection or analytics. A working survey needs a destination, such as an application endpoint that accepts the form submission or a form service configured to receive it. The action, method, field names, storage, and any confirmation experience depend on that choice.

- Decide where responses should go and configure the form’s
actionandmethodfor that destination. - Confirm the destination accepts the submitted field names and values, including multiple checkbox values.
- Implement the required server-side handling, storage, and validation; do not treat browser-side validation as a substitute.
- If you need consent, retention rules, or analytics, design and implement those separately. A Bootstrap template does not establish that these are included.
- Provide a useful success or error response so respondents know whether the submission was received.
Choosing and adapting a free template
Start with the simplest layout that fits your questionnaire. The MDB example is a Bootstrap 5 survey-form reference, while Bootstrap’s own versioned documentation is the authoritative guide to the controls and layout classes used in your selected release.
- Check that the example identifies its Bootstrap version and that it matches the version loaded by your site.
- Inspect labels, question grouping, and help text before changing colors or spacing.
- Check the example on small screens and with keyboard navigation; adapt the layout rather than forcing a desktop arrangement onto narrow viewports.
- Determine whether it is only front-end markup or includes an actual configured submission destination. Do not assume storage, consent handling, or reporting is part of a free code sample.
Bootstrap’s 2021 release post, dated May 5, 2021, said, “We’ve overhauled our Forms documentation and components.” That release context helps explain changes to Bootstrap’s form guidance, but it is not a substitute for checking the documentation for the version your project actually uses: Bootstrap 5 release post.
