A useful food and symptom diary pairs what you ate and drank with when symptoms appeared, plus relevant context such as medication. It can help you spot patterns to discuss with a healthcare professional, but it cannot prove that a food caused a symptom or diagnose a condition. For a journal intended to run fully offline, also verify where entries are stored and whether backups or optional features send data elsewhere.
What should you write in a food and symptom diary?
Keep the record simple enough to maintain. For each meal or snack, note what you ate or drank and the approximate time. Record symptoms and when they began, rather than only marking that they occurred on the same day. Context can help make entries more useful at an appointment.
- Food and drink: note the meal, snack, or drink in enough detail to recognize it later.
- Timing: record when you ate and when a symptom started.
- Symptoms: describe what happened and, if useful, its severity or duration.
- Context: consider noting bowel habits, medication, exercise, and emotions when relevant. An IBS diary template in NIDDK-hosted study materials includes these kinds of fields. NIDDK-hosted IBS study materials
University Hospitals of Leicester NHS Trust advises that symptoms linked to a trigger food may appear immediately or within 48 hours. That window is a reason to log timing carefully, not a rule that establishes a food as the cause. The Trust says a diary can be useful for comparing symptoms with foods eaten. How should you use the diary without jumping to conclusions?
Look for repeated timing patterns and take the record to a healthcare professional rather than treating one entry as proof. A diary captures observations; it does not establish causation. Do not cut out whole food groups without speaking with a healthcare provider or dietitian, as the Leicester Trust specifically cautions.

There is also a difference between a useful record and evidence that an app improves health outcomes. In its summary of the ADAPT study, the Health Research Authority reports that 20 patients enrolled. Participants were assigned to online 24-hour dietary recall or usual interview and followed for six months, but low recruitment meant the study could not measure effects on symptoms, consultation length, or quality of care. What does “fully offline” need to mean?
The phrase should be checked feature by feature. Before entering sensitive health information, find out whether the journal needs an account or connection, where entries are saved, and what happens when you use export, backup, sync, or other optional features. “Offline” may describe diary entry without describing every data flow in the app.

Store listings also illustrate why the exact disclosure matters. TummyTrail’s Google Play listing, updated 16 August 2026, says logging works offline and entries stay on the phone, while its data-safety section lists possible collection of financial information and device or other IDs. Those statements concern different kinds of data and are developer-provided listing disclosures, not an independent audit. Lunaire’s Google Play listing makes its own local-only and no-data-collected claims; those claims likewise apply to that app and listing. TummyTrail on Google Play Lunaire IBS Tracker on Google Play

Is a paper journal a better fit?
Paper works without a device or network and keeps entries out of app storage, but it can be less convenient to search, organize, or share. An app may make repeated logging or exporting easier, but its privacy depends on how it is built and configured. Neither format is established here as clinically superior. Choose the one your sister can use consistently, and consider whether she will want to print or share records at appointments.
