Focused ultrasound and deep brain stimulation (DBS) are not interchangeable treatments. Focused ultrasound uses MRI-guided sound energy to create a permanent lesion in a precisely chosen brain area. DBS places electrodes and a pulse generator that deliver adjustable electrical stimulation. Neither cures Parkinson’s disease or stops its progression; the better option depends on the symptom being treated, the target, medication response, cognition, surgical fitness, and the applicable regulatory indication.

The key practical difference is reversibility: clinicians can reprogram DBS, while a focused-ultrasound lesion cannot be removed or adjusted after it is made. A movement-disorders team must determine whether either procedure is appropriate for a particular person.

Focused ultrasound and DBS at a glance

Comparison point Focused ultrasound Deep brain stimulation
Basic action MRI and specialized equipment focus sound waves through the skull to heat a small target and create a lesion. Implanted leads and a battery-powered pulse generator deliver electrical stimulation to a selected brain target.
Implant No implanted stimulator. Requires brain leads, connecting extensions and an implanted pulse generator.
Adjustability The lesion is permanent; it cannot be reprogrammed if benefit is incomplete or a side effect develops. Clinicians can change stimulation settings without another brain operation.
Common Parkinson’s targets Thalamus for tremor; globus pallidus internus (GPi) for pallidotomy; and, in specified settings, the subthalamic nucleus or pallidothalamic tract. Subthalamic nucleus (STN) and GPi are common targets.
Typical treatment pattern Usually a unilateral lesion for an approved indication; bilateral lesioning can increase side-effect risk. One or both sides may be stimulated, with programming changed over time.
Main ongoing burden Specialized MRI-guided treatment and follow-up, without device maintenance. Programming visits, battery or generator management, medication management and device-related follow-up.
Important risks Headache, dizziness, nausea, balance or gait problems, sensory changes, weakness, speech or swallowing effects; rarely stroke or seizure. Brain-surgery risks such as bleeding, infection, seizures and postsurgical pain, plus stimulation or hardware complications.

This is a framework, not a prediction of which treatment will work better. The available authoritative material does not establish a head-to-head complication rate or universal efficacy advantage for either approach.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The comparison is also not simply “non-surgical versus surgical.” Focused ultrasound avoids an implanted lead but intentionally changes brain tissue through a procedure. DBS requires implantation, yet its electrical settings can be changed as symptoms and medication effects evolve. Both require specialist assessment, realistic goals and follow-up.

How focused ultrasound works

MRI-guided lesioning

During focused ultrasound (FUS, also called MR-guided focused ultrasound or MRgFUS), MRI helps the treatment team locate the intended target and monitor the procedure. Concentrated ultrasound energy passes through the skull and heats a small area until a therapeutic lesion is produced. The treatment avoids implanting a stimulator, but it is still an advanced medical procedure that requires specialized equipment, imaging and a target that can be reached safely through the skull.

The treatment is therefore dependent on both the clinical target and the technical path through the skull. A center must determine whether the proposed area can be reached and whether the planned side and indication fit current device labeling. “No implant” does not mean “no procedure,” “no risk” or “no follow-up.”

The lesion is permanent

The central trade-off is permanence. Once tissue has been lesioned, settings cannot be turned down, moved or reprogrammed. If the result is weaker than expected, or a lasting side effect occurs, there is no DBS-style programming session that can reverse the tissue change. That is why target selection, laterality and a discussion of possible speech, swallowing, gait and sensory effects are critical before treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Permanence also affects future planning. Parkinson’s symptoms can change, and a symptom that is most disabling today may not remain the main problem later. Ask how the proposed lesion fits the expected course of care, what other symptoms will still need medication or rehabilitation, and what options remain if benefit is incomplete.

Targets depend on the symptom

The Parkinson’s Foundation describes several lesion targets:

  • Thalamus: thalamotomy may reduce medication-refractory tremor.
  • Globus pallidus internus (GPi): pallidotomy may be considered for slow movement, stiffness, dystonia, tremor, “off” time or dyskinesia, depending on the clinical situation.
  • Subthalamic region: subthalamotomy has been discussed for Parkinson’s symptoms, but the Parkinson’s Foundation identifies it as experimental and not FDA-approved in the context described there.
  • Pallidothalamic tract: specific U.S. device indications cover unilateral tractotomy for certain advanced cases.

A lesion aimed at tremor should not be assumed to improve every other Parkinson’s symptom. The intended target and expected symptom response must be stated explicitly by the treating team. The same word—“focused ultrasound”—can describe different targets with different goals, so comparing procedures without naming the target can be misleading.

How deep brain stimulation works

Implanted leads and a pulse generator

DBS involves neurosurgical placement of one or more leads in a movement-related brain target. Extensions run under the skin to a battery-operated pulse generator, commonly implanted in the chest or another suitable location. The system delivers electrical stimulation that modulates abnormal activity associated with movement symptoms.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Common Parkinson’s targets include the STN and GPi. Target selection is not a simple ranking: the symptom pattern, levodopa response, medication complications and the clinician’s assessment all matter. The lead and generator become part of an ongoing treatment system, rather than a one-time lesion.

Programming creates flexibility

After implantation, a clinician programs stimulation settings and adjusts them as symptoms, medication effects and side effects change. Adjustments do not require another brain operation, although generator replacement or other hardware procedures may eventually be needed. DBS therefore introduces continuing device care rather than a one-time, irreversible tissue change.

Programming is not an instant guarantee of improvement. It may take follow-up visits to balance benefit against stimulation-related effects, and medication generally remains part of care. Ask the center how programming is organized, who provides it, and how device follow-up will work if you live far from the implanting team.

Medication usually continues

DBS is an adjunct to Parkinson’s treatment, not a replacement for all medication. Most recipients continue taking Parkinson’s medicines, although some people may be able to reduce medication under medical supervision. The amount of reduction cannot be promised in advance.

Because DBS does not stop neurodegeneration, it does not prevent future symptoms that are unrelated to the stimulated motor circuits. Medication, physical therapy, speech therapy, rehabilitation and treatment for non-motor symptoms may remain necessary.

Which symptoms can each approach address?

DBS and levodopa-responsive motor symptoms

DBS may help disabling motor symptoms such as tremor, stiffness, slowness, dystonia, dyskinesia and motor fluctuations. People whose symptoms still improve with levodopa often have better motor responses to DBS. That relationship is one reason a careful medication-response history and testing are central to candidacy.

DBS is not a reliable treatment for every non-motor problem. Dementia, depression, anxiety, balance difficulties and speech concerns may persist or require separate treatment. A procedure selected for limb tremor, for example, should not be presented as a treatment for cognitive decline or the full range of Parkinson’s symptoms.

Describe symptoms in practical terms: when they occur, whether they are present during “off” periods, how dyskinesia affects daily activities, and whether medication consistently changes them. This information helps the team distinguish a potentially responsive motor complication from a problem that surgery is unlikely to address.

Focused ultrasound is indication- and target-specific

FUS benefits depend on the lesion target and the approved or studied indication. Thalamic lesioning is aimed primarily at tremor. Pallidal or pallidothalamic procedures may be directed at combinations of dyskinesia, “off” periods, stiffness, slowness, dystonia or tremor in appropriately selected patients. The expected benefit is symptom management, not disease reversal.

The side being treated matters. A unilateral procedure may focus on symptoms on the opposite side of the body, while a bilateral lesion can carry greater side-effect risk. Do not assume that treating one side automatically treats symptoms on both sides.

Ask the specialist to identify which symptom is the treatment’s primary endpoint and which symptoms are unlikely to change. This prevents a successful treatment of one problem from being judged against an unrealistic expectation that all Parkinson’s symptoms will improve.

U.S. regulatory indications for focused ultrasound

U.S. Food and Drug Administration records for the Exablate Neuro MR-guided focused-ultrasound system describe specific Parkinson’s indications. They include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Unilateral thalamotomy for medication-refractory, tremor-dominant Parkinson’s disease.
  • Unilateral pallidotomy for advanced idiopathic Parkinson’s disease with medication-refractory moderate-to-severe motor complications, as an adjunct to medication.
  • Unilateral pallidothalamic tractotomy for advanced idiopathic Parkinson’s disease with medication-refractory moderate-to-severe motor complications.
  • A staged contralateral pallidothalamic tractotomy, for qualifying opposite-side symptoms, performed at least six months after the first procedure in the described device supplement.

These are device- and indication-specific U.S. regulatory records. They do not mean that every focused-ultrasound target, bilateral approach or research use is approved, nor do they establish that a particular patient qualifies. Device labeling, local availability and clinical practice can change, so a treatment center should confirm the current indication.

The six-month interval is a labeled minimum for the described staged contralateral tractotomy; it is not a general statistic about recovery, safety or how long everyone must wait between procedures. Other targets or devices may have different rules.

Who may be considered for DBS?

DBS candidacy is individualized rather than determined by a single age, symptom or disease-duration cutoff. The Parkinson’s Foundation highlights several considerations:

  • A confirmed Parkinson’s diagnosis and disabling tremor, dyskinesia or severe motor fluctuations despite appropriate medication adjustments.
  • Continued meaningful response to levodopa, which can help predict motor benefit.
  • Ability to tolerate an invasive operation and the follow-up required for programming and device care.
  • No severe cognitive problem that would make surgery, programming or postoperative management unsafe or ineffective.

Evaluation commonly involves a movement-disorders neurologist, neurosurgeon and neuropsychological testing. Neuropsychological assessment is strongly recommended because cognition, mood, impulse-control symptoms and the ability to manage a complex treatment pathway can affect the risk-benefit judgment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

These points are considerations, not a universal checklist. A person can have a levodopa-responsive symptom yet still be a poor candidate because of cognition, overall health, inability to attend follow-up or a mismatch between the proposed target and the main disability.

Who may be considered for focused ultrasound?

FUS evaluation must match the person’s symptoms and anatomy to a specific target and indication. The center will consider the disease type, medication response, severity and refractoriness of the target symptom, whether treatment is unilateral or staged, and whether imaging shows a safe path for the ultrasound energy. The FDA indication for one device cannot be used as a blanket eligibility checklist for every center or target.

Because a lesion cannot be reprogrammed, the consultation should address permanence more explicitly than a routine medication change. Ask what happens if the treated symptom returns, another symptom becomes dominant, or a speech, swallowing or balance problem appears after treatment.

Access is also center-dependent. Specialized MRI equipment, trained clinicians and an appropriate regulatory indication are required. Local availability, reimbursement and labeling can change, so the treating center should explain what is currently offered and under which indication.

Risks, limitations and recovery issues

Focused-ultrasound risks

Reported or discussed effects include headache, dizziness, nausea, balance or walking problems, numbness or tingling, weakness, and speech or swallowing difficulties. Some gait or balance problems may improve, but they can persist. Rare serious risks include stroke and seizure. Bilateral lesioning carries greater side-effect risk than a unilateral approach, which is one reason staged procedures and laterality matter.

Avoid judging risk solely by whether a procedure is described as non-invasive. The absence of an implanted stimulator does not remove the possibility of a permanent neurological effect. Ask which symptoms the center monitors immediately, which may emerge later, and whom to contact if they occur.

DBS risks

DBS requires invasive brain surgery. Risks described by the National Institute of Neurological Disorders and Stroke include bleeding or infection in the brain, headache, seizures and temporary postsurgical pain. Stimulation can affect tissue near the target, and hardware introduces additional potential complications. Programming is adjustable, but adjustability does not eliminate surgical risk.

DBS also creates practical obligations: programming appointments, medication changes, monitoring of the implanted system and eventual battery or generator management. Ask how these are handled locally and what happens if a device-related problem requires a procedure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Neither procedure stops neurodegeneration

Both treatments are symptom-management strategies. They do not cure Parkinson’s disease or stop the underlying neurodegeneration. New symptoms may develop over time, and medication, rehabilitation, speech therapy, physical therapy or other care may remain necessary.

There is no reliable basis in the cited material for telling a patient that one option has a lower overall complication rate than the other. Risks depend on the target, technique, patient characteristics and center experience. A specialist should explain the risks that are most relevant to the proposed target and the individual’s health.

Costs, access and practical burden

Neither procedure is an ordinary consumer purchase, and a meaningful price cannot be inferred from the treatment name alone. Costs, insurance coverage, reimbursement, travel and follow-up arrangements vary by location, device, indication and center. Ask the treating program for a current estimate of procedure, imaging, hospitalization, programming, medication and possible hardware-related expenses, as well as coverage requirements.

Focused ultrasound may avoid implanted-hardware maintenance but requires access to a specialized MRI-guided program. DBS may require repeated programming and later generator or battery management. The “lighter” option depends on which burden—permanent lesioning, invasive implantation, travel, follow-up or device care—is most consequential for the individual.

How to compare the options with a specialist

  1. Define the main problem. Record the symptom that is most disabling, when it occurs, how medication changes it and whether it is the problem you want the procedure to address.
  2. Document medication response. Ask how levodopa response, “on” and “off” periods, dyskinesia and medication side effects influence the recommendation.
  3. Identify the proposed target and side. Obtain the exact target—such as thalamus, GPi, STN or pallidothalamic tract—and whether treatment is unilateral, bilateral or staged.
  4. Review cognition and mood. Discuss neuropsychological results, depression, anxiety, impulse-control symptoms, balance and speech before choosing a procedure.
  5. Compare permanence with device care. For FUS, ask what cannot be reversed. For DBS, ask about programming schedule, battery life, generator replacement and hardware follow-up.
  6. Verify the indication. Confirm that the proposed treatment and target match the current regulatory labeling and the center’s actual experience; an experimental use should be identified as such.
  7. Plan for outcomes and complications. Ask what improvement is realistic, which symptoms are unlikely to change, how side effects are managed and when urgent contact is needed.
  8. Consider a second opinion. A consultation with another experienced movement-disorders team can clarify whether the recommendation reflects symptom goals, anatomy, cognition and local expertise.

Questions to take to the appointment

  • Which specific symptom is this procedure intended to improve, and which symptoms are unlikely to improve?
  • Why is this target and laterality preferable to the alternatives?
  • Does my levodopa response, cognitive profile, mood or balance affect candidacy?
  • What are the center’s experience, follow-up schedule and plan if symptoms or side effects change?
  • For focused ultrasound, what is permanent and what is the plan if the result is incomplete?
  • For DBS, how often will programming occur, and what device maintenance will be required?
  • Which current regulatory indication applies to me, and is any part of the proposed treatment investigational?
  • What costs and coverage questions should I resolve before scheduling, including travel and follow-up?

Bottom line

Focused ultrasound trades an implanted device for an irreversible lesion; DBS trades an invasive implantation and ongoing device care for adjustable stimulation. Focused ultrasound may be attractive when a specific unilateral symptom and target fit an approved indication and the person values avoiding implanted hardware. DBS may be preferable when long-term programmability, bilateral treatment or adjustment as symptoms evolve is especially important. Neither is universally better, and neither replaces individualized assessment by a movement-disorders team.

Frequently Asked Questions

Is focused ultrasound better than DBS for Parkinson’s?

Neither is universally better. The choice depends on the symptom, target, laterality, levodopa response, cognition, surgical fitness, permanence concerns and the current indication available at an experienced center.

What is the main difference between focused ultrasound and DBS?

Focused ultrasound heats a selected area to create a permanent lesion. DBS implants leads and a pulse generator that deliver electrical stimulation clinicians can reprogram.

Can focused ultrasound be reversed or reprogrammed?

No. Once the lesion is created, it cannot be removed, turned down or reprogrammed. This makes target and side-effect counseling especially important.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does DBS cure Parkinson’s disease or stop its progression?

No. DBS can improve selected movement symptoms, but it does not stop neurodegeneration. Most people continue Parkinson’s medication and may need rehabilitation or other treatments.

Which Parkinson’s symptoms may respond to DBS?

DBS may help tremor, stiffness, slowness, dystonia, dyskinesia and motor fluctuations, particularly when the relevant symptoms still respond to levodopa. Dementia, mood problems, balance difficulties and speech concerns may not reliably improve.

What focused-ultrasound targets are used for Parkinson’s?

The Parkinson’s Foundation describes thalamic treatment for tremor and GPi lesioning for problems such as stiffness, slowness, dystonia, tremor, “off” time or dyskinesia. Specific U.S. device indications also describe pallidothalamic tractotomy; subthalamotomy is identified as experimental in the cited context.

How long must someone wait between staged contralateral pallidothalamic tractotomies?

The FDA device supplement describing that indication specifies at least six months between the first procedure and the qualifying contralateral procedure. This is a labeled interval for that indication, not a general rule for every focused-ultrasound treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should a DBS evaluation include?

A movement-disorders neurologist and surgical team generally review diagnosis, disabling medication-resistant motor symptoms, levodopa response, surgical fitness, cognition and mood. Neuropsychological testing is strongly recommended by the Parkinson’s Foundation.

Does focused ultrasound avoid all surgery-related risk?

No. It avoids an implanted stimulator but remains an advanced brain procedure. Possible effects include headache, dizziness, nausea, gait or balance problems, sensory changes, weakness, speech or swallowing problems, and rarely stroke or seizure.

Are there reliable cost or complication-rate comparisons?

The cited authoritative sources do not provide a direct head-to-head complication rate or a universal cost comparison. Coverage, reimbursement, device labeling and follow-up requirements vary by center and location, so ask the treating program for current details.

The Bottom Line

Focused ultrasound creates a permanent, target-specific lesion without implanted hardware. DBS uses implanted hardware but offers adjustable stimulation and the option to change settings as symptoms evolve. Target, symptom pattern, levodopa response, cognition, risks, follow-up and current regulatory indication should guide an individualized decision with a movement-disorders team.
