mail() returning true does not prove that a message reached an inbox. It only means PHP handed the message to the local delivery transport and that transport accepted the handoff. On Unix-like servers, the usual break is a missing or misconfigured sendmail-compatible program, an incorrect sendmail_path, or a web server using a different php.ini than the one you edited. On Windows, the critical settings are the configured SMTP host and port unless sendmail_path overrides them.

Fix the problem in stages: verify the configuration used by the web process, send a controlled message with valid headers, confirm that the local MTA queues it, then follow the MTA logs through remote delivery. If you have no local MTA or need useful connection, TLS, bounce, and provider-response diagnostics, move to authenticated SMTP through a maintained library such as PHPMailer.

What PHP mail() actually does

PHP’s mail() function is a handoff interface, not a complete mail-delivery service. Your script supplies recipients, a subject, a body, and headers. PHP then passes that message to a local transport:

  • On Unix-like systems, PHP invokes a sendmail-compatible executable specified by sendmail_path. That executable may be Sendmail itself, a Postfix or qmail wrapper, or another compatible transport.
  • On Windows, PHP can submit directly to the SMTP host named by SMTP and the port in smtp_port, unless a sendmail_path command is configured.

The Boolean result describes only that local acceptance step. A false result indicates that PHP could not complete the handoff. A true result means the local transport accepted the message for further processing; it does not establish that the remote server accepted it, that the recipient provider placed it in the inbox, or that the recipient saw it.

That distinction determines your troubleshooting path. If there is no queue entry or local log event, stay in PHP and server configuration. If the message is queued, deferred, rejected, or bounced, PHP has already done its part and the MTA or remote provider is the next place to investigate.

Run a controlled diagnostic before changing code

Use one fixed recipient mailbox that you can inspect and a sender address on a domain you control. Do not begin with a public form, a visitor-supplied sender, HTML templates, or multiple recipients. A minimal test removes variables and gives every later log entry the same subject.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Record the return value. Log the result of mail() and any PHP error generated at the call site. A log line showing true is evidence of local acceptance only.
  2. Identify the web process configuration. The command-line output from php --ini describes the CLI SAPI. Your web server may load a different configuration. Create a temporary page containing phpinfo(), open it through the same virtual host that runs the form, note the loaded configuration file and mail settings, and remove the page immediately afterward.
  3. Check the active mail settings. On Unix-like systems inspect sendmail_path and mail.log. On Windows inspect SMTP, smtp_port, sendmail_from, and whether sendmail_path is set.
  4. Send one plain-text message. Use a domain-owned sender and CRLF line endings. Avoid user data in every header while diagnosing.
  5. Watch the local transport while testing. Review the MTA queue and system mail logs immediately after each attempt. The timing lets you match the PHP test to the queue, connection, rejection, or bounce event.
  6. Check the destination deliberately. Look in spam and quarantine, then test a second mailbox at another provider. A single empty inbox is not enough evidence to locate the fault.

Unix-like servers: repair the sendmail handoff

Confirm that a compatible executable exists

Unix-like PHP installations require a sendmail-compatible binary or wrapper that the web process can execute. PHP documents /usr/sbin/sendmail -t -i as the default form, but the correct path depends on the installed mail transport. Systems using Postfix or qmail need the wrapper supplied by that system rather than an assumed Sendmail binary.

Check all three links in the chain:

  • The file named by sendmail_path exists on the server.
  • The web-server account has permission to execute it and to access any files or sockets it requires.
  • The local MTA behind the wrapper is installed, configured, and running.

A shell test performed as an administrator is not conclusive if the web server runs under a restricted account. Permissions, service environments, and paths can differ between an interactive shell and the web process.

Set the active php.ini values

In the configuration loaded by the web SAPI, use a path that matches the installed MTA. The following is the documented pattern for a conventional sendmail-compatible interface:

; Unix-like systems; adjust the executable to your installed MTA
sendmail_path = "/usr/sbin/sendmail -t -i"
mail.log = "/var/log/php-mail.log"

-t tells the transport to read recipients from the message headers, and -i prevents a line containing only a period from prematurely terminating message input. Do not copy the path blindly: if your server provides a different wrapper, use that wrapper’s actual location and options.

After changing php.ini, reload or restart the PHP handler and web server as required by your deployment. Re-open the temporary phpinfo() page to verify that the web process now reports the intended values, then remove the page.

Use logs and the queue to locate the failing stage

Enable a writable mail.log during diagnosis and review the operating system’s MTA log at the same time. Interpret the evidence in this order:

  • No PHP mail-log entry and a false result: inspect the call, permissions, the loaded configuration, and the executable path.
  • A PHP handoff but no queue entry: the wrapper or MTA is not accepting the message, or the web account cannot use it. Stay on the server; remote DNS and recipient filtering are not involved yet.
  • A queued message: PHP has handed off successfully. Read the MTA’s delivery attempts, connection results, TLS messages, and retry schedule.
  • A rejection or bounce: use the recorded SMTP response and recipient address to correct the sender identity, DNS, policy, or destination problem indicated by that response.
  • A completed handoff with an empty mailbox: check spam or quarantine and test another provider, then investigate sender authentication and reputation.

Do not treat the absence of an inbox message as proof that PHP failed. Once the local queue contains the message, changing PHP syntax cannot fix a downstream rejection.

Windows servers: distinguish SMTP settings from sendmail_path

PHP’s direct SMTP mode on Windows uses the SMTP host, smtp_port (25 by default), and sendmail_from. Test the configured host and port from the server itself, not from your workstation. A firewall, hosting provider, or network policy may block unauthenticated connections on port 25.

If sendmail_path is configured, PHP executes that command instead and ignores the Windows SMTP, smtp_port, and sendmail_from settings for the mail() call. This precedence explains many apparently contradictory tests: an administrator edits the SMTP host while PHP is actually invoking a local command.

Some providers require authenticated submission on port 587 with STARTTLS rather than unauthenticated port 25. In that case, direct mail() settings are insufficient unless the configured transport supports the provider’s authentication and TLS requirements. Use the provider’s documented hostname, port, encryption mode, and credentials through an SMTP-capable library or relay.

Make the PHP call valid and safe

The function manual requires a From header and RFC-compliant addresses and headers. Use a fixed sender on your own domain while debugging, and put a visitor’s address in Reply-To instead of copying it into From. Every user-controlled header value must be validated and sanitized to prevent header injection.

<?php
$to = 'you@example.net';
$subject = 'PHP mail() diagnostic';
$body = "This is a plain-text delivery test.rn";
$headers = [
    'From' => 'webform@example.com',
    'Reply-To' => 'webform@example.com',
    'Content-Type' => 'text/plain; charset=UTF-8',
];

$ok = mail($to, $subject, $body, $headers);
error_log('mail() returned: ' . var_export($ok, true));

Why each detail matters

  • Domain-owned From: it gives the MTA and recipient provider a stable identity that you can authenticate with DNS.
  • Reply-To for visitors: replies still reach the visitor without allowing an untrusted address to impersonate the sending domain.
  • CRLF line endings: use rn in headers and normal message lines for RFC-compatible formatting.
  • Plain text first: add HTML only after transport works, and then set Content-Type explicitly, for example text/html; charset=UTF-8.
  • Validated addresses and headers: reject control characters and malformed values before constructing the header block. Never concatenate raw form fields into From, To, Cc, Bcc, or other headers.

Keep the diagnostic recipient, sender, and subject constant until you have identified the failing stage. Changing all three between attempts makes queue and provider logs difficult to correlate.

Follow the message after PHP hands it off

Once the test returns, inspect the queue and logs before attempting another code change. A useful investigation records the message identifier, recipient, queue status, and the latest remote response.

Observed result Most likely stage Next action
mail() returns false PHP call, active configuration, permissions, or sendmail-compatible executable Verify the web SAPI’s php.ini, path, executable permissions, and PHP/MTA error logs.
true, but no local queue or MTA event Wrapper invocation or local transport handoff Confirm the wrapper path, options, web-account permissions, and that the intended MTA service is running.
Message remains queued or repeatedly deferred Local MTA connection, DNS, TLS, or remote availability Read the MTA’s retry reason, verify DNS and network access, and correct the stated transport error.
Remote server rejects or bounces it Recipient policy, sender identity, authentication, or address validity Use the rejection code and bounce text to fix the indicated domain, sender, authentication, or recipient issue.
MTA reports delivery, but the mailbox is empty Recipient filtering, spam placement, quarantine, or provider-specific policy Check spam and quarantine, test another provider, and review sender authentication and alignment.

Queue state is especially valuable because it separates local acceptance from remote delivery. A queue entry proves that the local transport received the message; it does not promise eventual delivery. A bounce or rejection is more actionable than a silent missing message because it identifies the next system and often supplies a reason.

Improve acceptance with DNS and sender authentication

Recipient providers evaluate more than the visible message. They can compare the envelope sender used for delivery, the visible From domain, the sending server’s IP address, and the domain’s authentication records. Misalignment can lead to rejection, quarantine, or spam placement even when your MTA reports a successful connection.

SPF

Publish an SPF policy for the domain used by the envelope sender. It should authorize the servers or relay services that actually send your messages. Keep the policy synchronized with any change in outbound infrastructure; an old record can authorize the wrong host or omit the new one.

DKIM

Configure the MTA or SMTP relay to sign outgoing messages with DKIM for the sending domain. Publish the corresponding public key in DNS and monitor verification results. Signing ties the message to a domain even when it passes through an intermediary.

DMARC

Publish DMARC for the visible From domain and begin with a reporting policy that lets you observe legitimate and unauthorized senders. Move to a stricter enforcement policy only after the reports show that your web server and relay are aligned and authenticating correctly.

Forward and reverse DNS

The public IP of a sending SMTP server should have a PTR record resolving to a hostname, and that hostname should resolve back to the same address. Make the MTA’s advertised identity, forward DNS, reverse DNS, and TLS certificate naming consistent. These checks do not guarantee inbox placement, but inconsistent identity creates an avoidable reason for providers to distrust the connection.

When SMTP and PHPMailer are the better path

Move away from bare mail() when there is no dependable local MTA, sendmail_path cannot be made correct, or the available diagnostics are too opaque for your operational needs. A maintained PHP library such as PHPMailer can open an authenticated SMTP connection directly to a provider or relay.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Direct SMTP gives you events that the basic Boolean API does not expose: hostname and port failures, TLS negotiation results, authentication errors, provider responses, queue identifiers, and bounce information. It does not remove the need for DNS authentication or reputation management, but it makes the transport stage observable.

  1. Choose a provider or relay that explicitly permits application sending.
  2. Create a sending identity and obtain the documented hostname, port, encryption mode, username, and password or token.
  3. Store credentials outside source control, such as in protected environment configuration, and restrict access to the web process.
  4. Configure PHPMailer or another SMTP-capable library to use authenticated submission, normally the provider’s documented encrypted port rather than an unauthenticated port 25 connection.
  5. Send the same fixed-recipient plain-text diagnostic used for the mail() test.
  6. Capture connection, TLS, authentication, and provider-response errors in a protected application log; do not expose credentials or verbose SMTP transcripts to visitors.
  7. Continue monitoring bounces, complaints, authentication reports, and mailbox placement after the provider accepts the SMTP transaction.

A successful SMTP DATA response means the receiving SMTP server accepted the message content for processing. It still is not proof of inbox placement, so retain bounce and provider-feedback monitoring.

Choosing a delivery architecture

Architecture Installation and administration Observability Authentication and DNS Limits and reputation Best fit
PHP mail() with an existing local transport Low application effort, but dependent on correct server configuration and permissions. Basic Boolean result; detail comes from PHP and MTA logs and queues. Depends on the local MTA and your DNS configuration. You own sender reputation and must understand host and provider limits. Low-volume notifications on a server that already has a functioning MTA.
Local Postfix, Sendmail, or another MTA Highest administration burden: service configuration, queues, TLS, DNS, updates, and monitoring. Strong local queue and log visibility, including retries, bounces, and connection errors. Flexible, but you must configure SPF, DKIM, DMARC, PTR, and MTA identity correctly. You manage throttling, reputation, abuse prevention, and remote policy responses. Organizations that need control and have mail-operations expertise.
Authenticated SMTP relay through PHPMailer or another library Less local infrastructure; application credentials and provider settings still require careful management. Detailed connection, TLS, authentication, provider-response, and relay logs. The relay supplies submission authentication; your domain still needs aligned SPF, DKIM, and DMARC. Provider limits and policies apply, while the provider may supply reputation and bounce tools. Web forms and transactional notifications where dependable diagnostics matter more than running an MTA.

For a small internal form on a correctly managed Unix server, fixing the existing handoff can be sufficient. For a new deployment, a server without an MTA, or an application that must explain failures quickly, authenticated SMTP usually offers a clearer operational boundary.

Common failure patterns and precise fixes

The message works from a shell script but not from the website

The CLI and web server are probably loading different PHP configurations or running under different accounts. Compare php --ini with the temporary web phpinfo() output, then check the web account’s permission to execute the configured wrapper and write the diagnostic log.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The website reports true, but the MTA has no message

Inspect the active sendmail_path, including its executable and options. Confirm that the path exists and that the web account can execute it. If the command points to a wrapper, verify that the corresponding Postfix, qmail, or other local service is installed and running.

The queue grows and every attempt is deferred

Read the latest retry reason rather than repeatedly resubmitting the form. DNS lookup failures, blocked connections, TLS negotiation problems, and temporary remote failures require different fixes. Correct the condition identified by the MTA, then allow the queue to retry or resubmit one controlled test.

The recipient provider rejects the message

Use the exact rejection or bounce response to determine whether the address is invalid, the sender is unauthorized, the IP is blocked, or authentication is missing. Verify SPF authorization, DKIM signing, DMARC alignment, and the server’s forward and reverse DNS before changing application code.

The message is accepted but lands in spam

Check spam and quarantine at the affected provider and compare results with a second mailbox. Confirm that the envelope sender and visible From domain are aligned, DKIM passes, SPF authorizes the actual sender, and DMARC reports show legitimate traffic. Review sending behavior and complaint or bounce signals instead of assuming that a PHP return value can influence filtering.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Windows connections time out on port 25

Test the configured host and port from the server and check outbound firewall or hosting restrictions. If the provider requires authenticated submission with STARTTLS on port 587, configure an SMTP-capable library or relay using those documented requirements. Also verify that sendmail_path is not overriding the Windows SMTP settings you changed.

Production checklist

  • Use the php.ini loaded by the web SAPI, not only the CLI configuration.
  • On Unix-like systems, point sendmail_path to an existing, executable sendmail-compatible wrapper and keep the local MTA running.
  • On Windows, verify SMTP, smtp_port, and sendmail_from, while checking whether sendmail_path takes precedence.
  • Send from a stable domain-owned address; place visitor addresses in Reply-To.
  • Validate every recipient and header value, reject control characters, and use CRLF formatting.
  • Log the return value, but use PHP logs, MTA logs, and queue state to diagnose the actual failure.
  • Configure SPF, DKIM, DMARC, forward DNS, and reverse DNS for the sending identity.
  • Keep SMTP credentials out of source control and out of user-visible error messages.
  • Monitor bounces, deferrals, authentication reports, and spam placement after deployment.
  • Prefer authenticated SMTP through a maintained library when local handoff is unavailable or insufficiently observable.

The reliable way to fix PHP mail delivery is to identify the boundary where the message stops. Correct the PHP call and active configuration first, prove that the local transport queues the message, then use MTA and recipient-provider evidence to resolve DNS, authentication, network, or filtering problems. If that chain is difficult to operate, authenticated SMTP provides the visibility and controls that a bare mail() Boolean cannot.
