DT-109 is the experimental drug behind the headline. In studies of mice and nonhuman primates, the glycine-based tripeptide improved severe metabolic dysfunction-associated steatohepatitis (MASH) by repairing the intestinal barrier, lowering ammonia-producing bacteria and reducing gut-driven liver inflammation.
The finding is preclinical. DT-109 has not been tested in human clinical trials, approved for patients or shown to reverse MASH or liver fibrosis in people. Human safety, dose, effectiveness, side effects, price and availability remain unknown.
What DT-109 is—and what the study actually showed
Michigan Medicine researchers identified DT-109 as an investigational glycine-based tripeptide for severe MASH, the progressive form of fatty liver disease associated with metabolic dysfunction. Their report, by Pengxiang Qu and colleagues, appeared in the Journal of Clinical Investigation in 2026 under the title “Metabolic dysfunction–associated steatohepatitis exacerbated by Clostridium perfringens-derived ammonia is attenuated by tripeptide DT-109” (DOI 10.1172/JCI200522).
The experiments used animal models rather than people. The researchers reported improvement in severe MASH in mice and nonhuman primates, but the report says additional testing is needed before human clinical trials. An animal response cannot establish a human dose, benefit or safety profile.

Y. Eugene Chen, M.D., Ph.D., the study’s senior author, said the team saw evidence that DT-109 protects the gut epithelial barrier and reduces the flow of harmful microbial products into the bloodstream. Co-author Jifeng Zhang, Ph.D., described the compound as connecting microbiota modulation with liver protection by restoring gut-barrier integrity and limiting ammonia and other inflammatory products entering circulation.
How the proposed gut–liver mechanism works
1. Severe MASH changes the intestinal environment
The study linked severe MASH with overgrowth of Clostridium perfringens, a bacterium that can produce ammonia in the intestine. The investigators’ model proposes that this excess ammonia injures the intestinal lining.
2. A damaged barrier lets microbial products escape
When the gut barrier becomes more permeable, ammonia and other products made by intestinal microbes can cross into the circulation. This systemic exposure gives the immune system a route by which gut disturbances can amplify inflammation beyond the intestine.

3. The liver receives an inflammatory signal
The reported pathway includes activation of CD8+ T cells and inflammation in the liver. In this model, liver injury is not driven only by fat stored in liver cells; it is also intensified by signals arriving through the gut–liver axis.
4. DT-109 targets the chain upstream
In the animal experiments, DT-109 reduced C. perfringens and ammonia, strengthened the intestinal barrier and reduced liver inflammation. That is why the proposed treatment is described as gut-barrier repair and microbiota/ammonia reduction rather than as a drug that acts only on liver tissue.
Does this mean DT-109 reverses fatty liver disease in people?
No. The evidence currently supports an improvement in animal models of severe MASH. It does not establish reversal in human patients, resolution of human liver inflammation or regression of human fibrosis.

The cited report provides no human response rate, dosage, adverse-event rate or price. It also does not establish how long any benefit would last, whether the compound works across different causes or stages of MASH, or whether an effect seen in animals would translate to people with cirrhosis or other liver conditions.
The distinction matters because MASH can progress from fat accumulation and inflammation to scarring, advanced fibrosis and cirrhosis. A treatment that improves laboratory or tissue findings in an animal model still has to demonstrate meaningful clinical benefit and acceptable risks in carefully designed human trials.
Is DT-109 available yet?
No. DT-109 is an investigational compound and is not an approved prescription, over-the-counter medicine or established supplement. The researchers said further testing is required before human clinical trials. There is therefore no validated patient dose, prescribing pathway, pharmacy supply or consumer purchase option.

The report names Diapin Therapeutics as the licensee associated with the program, but the study does not establish a clinical-trial timetable or patient-access program. Any claim that DT-109 is currently available should be checked against an official clinical-trial listing or regulatory announcement.

DT-109 and Rezdiffra (resmetirom) are at different evidence and regulatory stages and use different biological strategies. Rezdiffra is an approved prescription treatment for eligible adults with noncirrhotic NASH/MASH and moderate to advanced fibrosis. The U.S. Food and Drug Administration based its 2024 efficacy approval on Trial 1, which included 888 adults with biopsy-confirmed disease.

What the prevalence figure means
Michigan Medicine’s 2026 institutional summary states that about 7% of people worldwide have MASH. That is a prevalence estimate describing the scale of the disease, not a response rate, trial result or prediction about DT-109. The estimate also does not mean that 7% of people have severe disease suitable for an experimental therapy.
What must happen before patients could use DT-109
- Candidate development: Researchers must complete additional laboratory and animal work to characterize dose, distribution, toxicity and durability.
- Regulatory permission for human testing: Investigators would need authorization to begin a first-in-human clinical trial.
- Early safety studies: Phase 1 testing would examine tolerability, pharmacokinetics and a safe dose in people.
- Controlled efficacy studies: Later trials would need to show whether DT-109 improves MASH activity, fibrosis or clinically important outcomes compared with an appropriate control.
- Regulatory review: Approval would require evidence that benefits outweigh risks for a defined patient population.
Until those steps occur, people with suspected or diagnosed MASH should not substitute an unapproved compound, probiotic or “gut-healing” supplement for evaluation and treatment from a qualified clinician. Generic supplements have not been shown in this study to reproduce DT-109’s effects.
What patients should take from the announcement
- DT-109 is the compound named by the headline.
- The reported benefit is limited to animal models so far.
- The proposed action is to repair the gut barrier and reduce microbiota-derived ammonia and inflammation.
- No human dose, safety rate, effectiveness rate or price has been reported.
- Rezdiffra is an approved but mechanistically different option for eligible noncirrhotic patients with moderate or advanced fibrosis.
Elliot Tapper, M.D., Academic Director of Hepatology at Michigan Medicine, said patients need a safe and effective therapy that improves both liver and heart health. DT-109 addresses an important biological hypothesis, but human trials will determine whether that promise becomes a usable treatment.
