There is no single best treatment or definitive cure for endometriosis. The practical choice is usually between pain medicines, hormonal suppression, surgery, or combinations, selected according to symptoms, side effects, medical risks, cost and availability, and whether pregnancy is a current priority. The World Health Organization (WHO) and the UK National Institute for Health and Care Excellence (NICE) both emphasize shared decision-making rather than choosing treatment from the disease stage alone.

For many people, NICE supports a short, clinician-guided trial of paracetamol or an anti-inflammatory painkiller such as ibuprofen or naproxen, followed by reassessment if relief is inadequate. Hormonal treatment can reduce pain and does not permanently damage later fertility, but it is not a fertility treatment when you are actively trying to conceive. Laparoscopic surgery can remove or destroy visible disease and treat some endometriomas, yet it carries operative and recurrence risks. Persistent, severe, complex or fertility-related problems warrant specialist assessment.

How the options differ

Endometriosis is tissue similar to the uterine lining growing outside the uterus. It can cause period pain, pain between periods, pain during sex, bowel or bladder symptoms, heavy or irregular bleeding, fatigue and difficulty conceiving. Pain can also involve pelvic-floor, nerve, bowel or bladder mechanisms that do not disappear simply because a lesion is treated. That is why a scan, a surgical finding or a disease stage cannot predict one universal “best” treatment.

WHO reports that the average time to diagnosis is currently between 4 and 12 years (WHO, 2025). A delayed diagnosis does not mean you must tolerate escalating symptoms while waiting for surgery. A clinician can begin symptom treatment, investigate other causes and arrange referral in parallel.

Option Main purpose Reversible? Pregnancy planning Key cautions
Paracetamol or an NSAID Short-term pain relief; NSAIDs also reduce inflammation Yes; effect lasts while taken Check pregnancy status and timing with a clinician or pharmacist NSAIDs may be unsuitable with stomach ulcers, kidney disease, cardiovascular disease, bleeding risk or some pregnancies
Combined hormonal contraception Suppresses ovulation and bleeding-related cycles; reduces pain Usually; fertility generally returns after stopping, but timing varies Not used to improve spontaneous conception while trying Medical eligibility depends on migraine history, clot risk, blood pressure, smoking and other factors
Progestogen (pill, injection or hormonal IUD) Suppresses endometrial activity and often reduces bleeding and pain Yes, although injections can delay return of ovulation Not a treatment for active attempts to conceive Irregular bleeding, mood or other formulation-specific effects
GnRH-based medicines Stronger ovarian-hormone suppression for selected patients Reversible, with specialist monitoring Requires a plan for stopping before conception attempts Hot flushes, bone and low-estrogen effects; add-back therapy may be needed
Laparoscopic surgery Excision or ablation of visible lesions; treatment of some endometriomas No; removed tissue does not regrow, but disease can recur Ovarian reserve and the likely fertility benefit must be weighed individually Anesthesia, bleeding, organ injury, adhesions, recurrence and possible further surgery
Hysterectomy, with or without ovary removal Major option for selected people with persistent symptoms who do not plan pregnancy No Ends the ability to carry a pregnancy Does not guarantee pain relief; ovary removal can cause surgical menopause

The cited WHO and NICE guidance do not establish one percentage of pain relief or recurrence that can rank every medicine against surgery. Results vary with lesion location, coexisting pain conditions, treatment duration, age, prior operations and follow-up.

Pain medicines: a reasonable first step for many people

What NICE recommends

NICE advises discussing analgesic benefits and risks, considering other illnesses and personal preference, and considering a short trial—often about three months—of paracetamol or an NSAID such as ibuprofen or naproxen, alone or together. Use the dose and duration on the product label unless your prescriber gives different instructions. The aim is to test whether pain becomes manageable while the underlying diagnosis and longer-term plan are reviewed.

When an NSAID may be unsafe

Ibuprofen and naproxen can irritate the stomach and increase bleeding, kidney and cardiovascular risks. They may be unsuitable if you have had an ulcer or gastrointestinal bleeding, kidney disease, certain heart or blood-pressure conditions, a bleeding disorder, an NSAID allergy or particular stages of pregnancy. Do not combine multiple NSAIDs (for example, ibuprofen with naproxen) unless a clinician specifically tells you to. Ask a pharmacist or clinician to check interactions with anticoagulants, steroids, some blood-pressure medicines and other drugs.

If the trial does not work

Inadequate relief is a reason for reassessment, not for indefinitely increasing over-the-counter doses. Your clinician may review the diagnosis, look for adenomyosis or bowel, bladder, musculoskeletal and neuropathic contributors, and discuss hormonal treatment, pelvic-floor physiotherapy or a multidisciplinary pain service. Keep a record of pain, bleeding, bowel or bladder symptoms, medicines taken and side effects; that information helps determine whether a treatment is helping.

Hormonal treatment: suppressing the cycle to reduce symptoms

Combined hormonal contraception

A combined oral contraceptive can reduce the hormonal cycling that triggers endometriosis-related inflammation and pain. Some people use it continuously to avoid withdrawal bleeding; the exact regimen should follow the prescribed product and your medical history. It also provides contraception, but it is not appropriate for everyone. Migraine with aura, previous blood clots, uncontrolled hypertension, smoking at older ages and other conditions can change eligibility, so a clinician must assess risk.

Progestogens and hormonal IUDs

Progestogen treatment is available in tablets, injections and some intrauterine systems. WHO includes progestins, a hormonal IUD and depot medroxyprogesterone among options. Benefits can include less pain and lighter or absent bleeding. Unscheduled bleeding, acne, mood changes, headaches or weight changes vary by formulation. An injection can delay the return of ovulation after it is stopped, which matters if pregnancy is planned soon.

GnRH medicines and newer specialist options

GnRH analogues produce a low-estrogen state and are generally reserved for selected patients when other treatment is ineffective, unsuitable or not tolerated. Hot flushes, sleep or mood changes, vaginal dryness and loss of bone density are important concerns. “Add-back” hormonal therapy can reduce low-estrogen effects and help protect bone, but it must be prescribed and monitored.

Fertility after hormonal treatment

NICE states that hormonal treatment for endometriosis does not have a permanent negative effect on subsequent fertility. It does not, however, increase spontaneous pregnancy rates while you are actively trying to conceive. A clinician can help plan when to stop contraception, how long a particular medicine may delay ovulation and when to refer for fertility care.

Laparoscopic surgery: when and what it can do

What happens

Laparoscopy uses small abdominal incisions and a camera, usually under general anesthesia. NICE recommends offering laparoscopic surgery unless contraindicated and says visible peritoneal disease and uncomplicated endometriomas may be treated during the procedure. Treatment can involve excision (cutting out lesions) or ablation (destroying their surface). The surgeon may also divide adhesions or treat an endometrioma, depending on its size, location and complexity.

Questions to settle before consenting

Ask what the surgeon expects to find, which lesions can be treated safely, whether treatment might involve the bowel, bladder, ureter or ovary, and what symptoms are realistically likely to improve. Counseling should cover anesthesia, bleeding, infection, adhesions, organ injury, conversion to a larger operation, recurrence and the possibility of another operation. A normal-looking pelvis does not guarantee that every pain mechanism has been eliminated.

Endometriomas and ovarian reserve

For an ovarian endometrioma, the choice between excision and ablation must account for fertility wishes and ovarian reserve. Surgery on ovarian tissue can reduce reserve, particularly after repeat procedures or treatment of both ovaries, while leaving an endometrioma can also create symptoms or complicate fertility treatment. The decision should be individualized with an endometriosis surgeon and, when pregnancy matters, a fertility specialist. Ovarian-reserve testing can inform the discussion but cannot predict pregnancy with certainty.

Hysterectomy and removal of the ovaries

Hysterectomy is major surgery generally considered only for selected people with persistent symptoms who do not plan a future pregnancy and who have discussed less invasive options. Removing the uterus ends menstrual bleeding and the ability to carry a pregnancy, but it does not guarantee that endometriosis-related pain will disappear. Disease outside the uterus, pelvic-floor pain, adhesions or nerve sensitization can remain.

Removing both ovaries causes immediate surgical menopause. The discussion should include hot flushes, bone and cardiovascular health, sexual symptoms and whether hormone-replacement therapy is appropriate. Keeping or removing an ovary is a separate decision from removing the uterus and depends on disease, age, cancer risk and personal priorities.

If pregnancy is the priority

Do not use hormonal treatment alone, or hormonal treatment combined with surgery, to improve spontaneous pregnancy rates while actively trying to conceive. NICE recommends fertility-specialist involvement and an individualized discussion of laparoscopic treatment, ovarian reserve and alternatives. Depending on age, ovarian reserve, tubal status, semen factors, disease location and prior treatment, options may include timed attempts, assisted conception or surgery before fertility treatment. A specialist can also explain how long it is reasonable to try after surgery and when further intervention is more useful than waiting.

Non-drug and coordinated care

WHO says multidisciplinary pain management can reduce pain and improve quality of life. Pelvic-floor physiotherapy may help muscle overactivity, painful intercourse and movement-related pain; cognitive behavioral therapy can support coping, sleep and function without implying that pain is psychological. These approaches complement—not replace—assessment for lesions and other medical causes.

NICE emphasizes coordinated services that may include specialist nursing, imaging expertise, multidisciplinary pain services and fertility services. Ask who will coordinate prescriptions, imaging, follow-up and referrals so that treatment is not fragmented.

A practical decision path

  1. Define the priority. Decide whether the immediate goal is pain control, lighter bleeding, avoiding hormones, preserving fertility, achieving pregnancy or treating a mass or organ problem.
  2. Check safety. Review pregnancy possibility, contraception needs, migraine, clot history, blood pressure, stomach or kidney disease, other medicines and mental-health concerns before choosing an analgesic or hormone.
  3. Try the least invasive suitable option. A documented short analgesic trial or hormonal treatment is often reasonable when there is no urgent surgical indication.
  4. Measure the result. Track pain days, bleeding, sleep, activity, sex, bowel or bladder symptoms and side effects rather than judging treatment from one difficult cycle.
  5. Escalate deliberately. Seek specialist review for persistent disabling pain, an endometrioma or deep disease, suspected organ involvement, medication intolerance, uncertain diagnosis or fertility concerns.
  6. Revisit the plan. Endometriosis can recur and priorities change. A treatment that is appropriate while avoiding pregnancy may not be appropriate during conception attempts.

Claims to approach cautiously

No medicine, operation or supplement is a guaranteed cure. NICE states that evidence does not support traditional Chinese medicines or herbal supplements for treating endometriosis. “Natural” products can also interact with prescriptions or be unsafe in pregnancy. Be wary of programs promising a universal pain-relief percentage, permanent prevention of recurrence or restored fertility without accounting for disease location and individual risk.
