Ebola, Marburg, Lassa fever, Crimean-Congo haemorrhagic fever (CCHF), and yellow fever are not one disease. “Viral hemorrhagic fever” (VHF) is a clinical grouping for infections caused by several unrelated virus families. They differ in reservoirs, vectors, transmission routes, geography, vaccines, treatments, and risk. Ebola and Marburg are closely related filovirus diseases, while Lassa, CCHF, and yellow fever illustrate very different ways a VHF can spread. Symptoms overlap so extensively that bleeding, rash, or fever cannot identify the virus; suspected cases require professional assessment, exposure history, and pathogen-specific laboratory testing.
What “viral hemorrhagic fever” means
Viral hemorrhagic fever is an umbrella term for severe viral illnesses that can affect multiple organ systems and, in some patients, disrupt blood-clotting and blood-vessel function. The term describes a clinical pattern, not a single pathogen or a universal route of transmission. Some VHFs spread between people through body fluids, some begin with animal exposure, some require a tick or mosquito, and some are mainly associated with rodents.
For this comparison, the main diseases are Ebola disease, Marburg virus disease, Lassa fever, CCHF, and yellow fever. The U.S. Centers for Disease Control and Prevention (CDC) also groups South American hemorrhagic fevers among selected VHFs, while the World Health Organization (WHO) uses Rift Valley fever and yellow fever as additional examples. “Other VHFs” therefore includes several diseases with substantially different ecology and prevention requirements.

These infections can resemble malaria, typhoid, sepsis, meningitis, and other common illnesses. A person cannot identify the cause from symptoms alone, and visible bleeding is neither required nor reliably early. The comparisons below explain the major differences; they are not a diagnostic tool.

At-a-glance comparison
| Disease | Virus or group | Main exposure highlighted by WHO or CDC | Distinctive practical point | Prevention and treatment status |
|---|---|---|---|---|
| Ebola disease | Orthoebolaviruses, including Ebola, Sudan, and Bundibugyo viruses | Contact with infected animal tissue can introduce infection; person-to-person spread follows direct contact with infected body fluids or contaminated materials | Often starts abruptly; bleeding is less common than popular depictions suggest and may occur later | Early optimized supportive care; approved vaccines and therapeutics apply to Ebola virus disease, not Sudan or Bundibugyo disease |
| Marburg virus disease | Marburg virus or Ravn virus | Initial cases are associated with prolonged exposure to fruit-bat-inhabited mines or caves; human spread is through body fluids or contaminated materials | Clinically resembles Ebola, with abrupt onset and a 2–21-day incubation period | Early supportive care; WHO reports no currently approved vaccine or antiviral treatment |
| Lassa fever | Lassa virus | Usually exposure to infected rodents; limited human transmission occurs mainly in healthcare settings through body fluids or contaminated equipment | Onset is often gradual; hearing loss can persist after recovery | Supportive care; ribavirin has been used, but efficacy and dosing remain uncertain; no licensed vaccine or approved antiviral |
| CCHF | Crimean-Congo haemorrhagic fever virus | Tick bites, infected animal blood or tissue, and contact with an infected person’s body fluids | Incubation depends on the exposure route; livestock and tick contact are central risks | Early supportive care; no approved specific treatment, and WHO says ribavirin efficacy is uncertain |
| Yellow fever | Yellow fever virus | Infected mosquitoes | A mosquito-borne VHF with a highly effective vaccine | One vaccine dose offers lifelong protection according to WHO; treatment is primarily supportive |
How Ebola and Marburg differ from each other
Different viruses in the same filovirus family
Ebola and Marburg are both filovirus diseases, which explains their similar severe clinical presentations, but they are not caused by the same virus. WHO identifies Ebola virus, Sudan virus, and Bundibugyo virus as causes of large Ebola disease outbreaks. Marburg virus and Ravn virus cause Marburg virus disease. A test or treatment validated for one disease cannot automatically be applied to the other.
Similar person-to-person transmission
For both diseases, human transmission is associated with direct contact with infected body fluids or objects contaminated by those fluids, particularly after symptoms begin. Caregiving, funeral practices, handling contaminated clothing or bedding, and unsafe healthcare procedures can create exposure. CDC’s public explanation is explicit: “Ebola does NOT spread the same way as respiratory viruses like flu and COVID. You cannot get it from being near someone.” That statement concerns Ebola; it should not be turned into a claim that every VHF has identical transmission rules.
Different animal links, overlapping illness
WHO links some initial Ebola infections to contact with infected animals. Marburg introductions have been associated with prolonged visits to mines or caves inhabited by fruit bats. Once human transmission begins, the illnesses can look alike: fever, severe weakness, muscle pain, headache, gastrointestinal symptoms, and, in serious cases, organ dysfunction and bleeding.
Fatality figures are outbreak statistics, not an individual forecast
WHO reported in 2025 that Ebola disease has an average case-fatality rate of about 50%, with past outbreaks ranging from 25% to 90%. WHO gave Marburg virus disease an average of about 50%, with past outbreaks ranging from 24% to 88%. These ranges reflect different outbreaks, healthcare access, case definitions, and treatments. They do not predict what will happen to a particular patient or describe a single current outbreak.

How Lassa fever differs
Rodents are the main reservoir
Lassa fever is primarily associated with infected rodents rather than bats or ticks. People can be exposed through contact with rodent urine, feces, saliva, blood, or materials contaminated by those fluids. Human-to-human spread occurs less often and is especially documented in healthcare settings when body fluids or inadequately decontaminated medical equipment transmit the virus.
Onset and long-term effects
Lassa fever often develops more gradually than Ebola or Marburg. Fever and weakness may be followed by headache, sore throat, chest or abdominal pain, vomiting, diarrhea, cough, and facial swelling. Severe illness can involve bleeding or shock, but bleeding is not required. Hearing loss is a notable post-infection complication: WHO reports that up to 25% of survivors have been reported to experience sudden hearing loss, which may be temporary or permanent.
What is known about treatment
Supportive care—maintaining fluids, oxygenation, blood pressure, and organ function—is central. Ribavirin has been used, but WHO states that its effectiveness and optimal dosing remain uncertain. No licensed Lassa vaccine or approved Lassa antiviral was identified by WHO.

Ticks, livestock, and occupational exposure
CCHF is mainly linked to Hyalomma and other ticks, infected livestock, and contact with animal blood or tissues. People can also acquire it from an infected person’s body fluids, making healthcare and household caregiving important secondary transmission settings. This ecology is different from the rodent focus of Lassa and the bat-associated introductions described for Marburg.

Where it occurs
CDC describes CCHF across Eastern and Southern Europe, the Mediterranean region, northwestern China, Central Asia, Africa, the Middle East, and the Indian subcontinent. WHO summarizes the endemic area more broadly as Africa, the Balkans, the Middle East, and Asian countries south of the 50th parallel north. These are broad geographic patterns, not a diagnosis: local risk depends on travel, season, occupation, tick activity, animal contact, and current surveillance.
Incubation and severity
The incubation period varies with the route of exposure. Tick-bite infections may begin sooner than infections acquired from blood or tissue, so a single time window cannot rule CCHF in or out. WHO’s key facts describe case-fatality rates of up to 40%, while its overview gives a 10–40% range. CDC separately says up to 50% of hospitalized patients die. Those figures use different populations and denominators and should not be merged into one estimate.
Reducing tick and animal exposure
In areas where CCHF occurs, CDC recommends tick-bite prevention measures that include a DEET-containing repellent. People handling animals should use gloves and protective clothing as appropriate for the local public-health guidance. These measures address tick and animal exposure for CCHF; they do not prevent Ebola, Marburg, Lassa, or every other VHF.
Why yellow fever belongs in the comparison
Yellow fever shows why the phrase VHF can mislead. It is transmitted by infected mosquitoes, not by ordinary proximity to a sick person. Its prevention strategy is therefore vaccination and mosquito-bite avoidance, rather than the body-fluid precautions central to Ebola or Marburg.

WHO says a safe, affordable yellow-fever vaccine provides lifelong protection with one dose. Treatment remains primarily supportive. WHO estimates 67,000–173,000 severe yellow-fever infections and 31,000–82,000 deaths each year in Africa and the Americas; these are regional annual estimates, not global totals or a prediction for a particular season.

Overlapping early symptoms
Fever, fatigue, headache, muscle aches, weakness, nausea, vomiting, diarrhea, abdominal pain, and sore throat can occur in several VHFs. Malaria and typhoid can produce similar early syndromes. Rash, conjunctival changes, dehydration, or bleeding may occur in severe disease, but none is specific enough to identify a pathogen.
Incubation ranges overlap
WHO gives incubation periods of 2–21 days for Ebola, Marburg, and Lassa. CCHF timing varies according to whether exposure came from a tick bite, infected animal tissue, or another person. Because these ranges overlap, counting days since travel or contact cannot provide a reliable self-diagnosis.
Bleeding is not a screening test
The name “hemorrhagic fever” encourages the mistaken idea that visible bleeding is required. Bleeding can be absent, late, or clinically minor, while severe disease can occur without dramatic external bleeding. Do not wait for bleeding before seeking medical advice after a credible exposure.

How professionals diagnose a suspected VHF
Clinicians combine the illness pattern with detailed exposure and travel history: dates and locations, contact with sick people or animals, tick or mosquito exposure, cave or mine visits, healthcare work, funeral contact, and possible contact with blood or other body fluids. Laboratory confirmation may use RT-PCR or other pathogen-specific methods. The same-day differential may include malaria, typhoid, bacterial sepsis, and noninfectious conditions.
Testing is a specialized safety operation. WHO warns that non-inactivated specimens can pose an extreme biohazard risk. In U.S. healthcare settings, CDC guidance calls for VHF-specific infection-control practices and appropriate personal protective equipment. A presumptive negative result alone may not release a patient from special-pathogen isolation; confirmatory reference testing can be required. Those CDC procedures are written for U.S. healthcare facilities and are not a universal do-it-yourself protocol.
What to do after a possible exposure
- Contact local emergency or public-health services promptly. Tell the dispatcher or clinician about the illness, travel dates, animal or tick contact, healthcare or funeral exposure, and any contact with body fluids.
- Follow the instructions you receive before going to a clinic. A facility may arrange a controlled evaluation or advise how to arrive without exposing others.
- Do not attempt home testing, self-prescribed ribavirin or other medicines, or improvised PPE. Treatment and isolation decisions depend on the suspected pathogen and local protocols.
- Do not handle blood, vomit, diarrhea, needles, clothing, bedding, or remains from a potentially infected person without professional direction.
For Ebola specifically, simply being near someone is not the described route of transmission. Risk assessment still depends on whether direct contact with infectious fluids or contaminated materials occurred.

Which treatments and vaccines apply to which disease?
| Disease | Core care | Specific medical options stated by WHO |
|---|---|---|
| Ebola virus disease | Early optimized supportive care | Approved vaccines and therapeutics exist for Ebola virus disease; they are not approved for Sudan or Bundibugyo disease |
| Marburg virus disease | Early supportive care | No vaccine or antiviral treatment currently approved, according to WHO |
| Lassa fever | Supportive care | Ribavirin has been used, but efficacy and dosing are uncertain; no licensed vaccine or approved antiviral |
| CCHF | Early supportive care | No approved specific treatment; WHO says ribavirin efficacy is uncertain |
| Yellow fever | Supportive care | One vaccine dose offers lifelong protection, according to WHO |
“Supportive care” is active hospital treatment, not merely observation. It can include fluids, oxygen, blood-pressure support, management of electrolyte disturbances, treatment of secondary infections when indicated, and organ support. The appropriate approach depends on the patient and the suspected infection.
The practical bottom line
Ebola and Marburg are the closest pair: both are filoviruses, can spread through infected body fluids after symptoms begin, and can cause abrupt severe illness. Lassa is primarily rodent-associated and often more gradual, CCHF is strongly connected to ticks and livestock, and yellow fever is mosquito-borne and vaccine-preventable. Incubation periods and symptoms overlap, so neither bleeding nor a travel timeline can identify the cause. A credible exposure or compatible severe illness warrants prompt contact with qualified clinicians and public-health services, not symptom matching or home treatment.
