Yes, gut microbiome composition is associated with how strongly people respond to some vaccines, but current human evidence does not show that the microbiome causes those differences or that changing it will improve your protection. The best evidence is a 2024 systematic review of 30 publications (representing 26 studies). Twenty-six publications reported an association between intestinal microbiota and antibody or cellular vaccine responses, yet the studies used very different populations, vaccines, sampling methods and outcome measures.

That makes the finding an important research signal—not a validated test of an individual’s response and not a reason to take a particular probiotic, buy a microbiome test or delay vaccination.

What the strongest human evidence shows

The most comprehensive source is a systematic review published in npj Vaccines on 20 December 2024. The authors searched MEDLINE and Embase through June 2024 for original human studies that measured intestinal microbiota alongside humoral (antibody) or cellular vaccine responses. They included 30 publications covering 26 studies: 14 studies in infants and 12 in adults. Twenty-six of the 30 publications reported some association between microbiota composition and vaccine response.

That count is not a vaccine-effectiveness percentage. It does not mean that 87% of people with a particular microbiome respond better, nor does it prove that changing bacteria changes protection. It counts publications whose results linked a microbiome measurement with an immune measurement. The review authors emphasized that methods and findings were heterogeneous and that larger, better-designed studies are needed.

The review covered oral and injected vaccines and measured both antibody and cellular responses. It did not find a clear overall difference in the frequency of reported associations between oral and parenteral vaccines. Individual results still varied substantially because the vaccines, participants, laboratory methods and timing were not the same.

Which microbiome patterns recur?

Across the heterogeneous studies, one broad pattern appeared repeatedly:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Actinomycetota, particularly Bifidobacterium, was often associated with stronger measured vaccine responses.
  • Pseudomonadota, particularly Gammaproteobacteria, was often associated with weaker measured responses.

These are group-level correlations, not a recipe for an individual. A stool sample containing more Bifidobacterium cannot currently predict whether a person will make enough antibody, need a booster or be protected from infection. The review did not identify a microbiome profile validated for that purpose.

Measures of overall community diversity also did not provide a simple answer. Alpha diversity (within-sample variety) and beta diversity (differences between samples) were not strongly associated with response differences across the review as a whole. A more diverse microbiome is therefore not automatically a better vaccine-response microbiome.

Why study results are difficult to compare

A result from one microbiome-vaccine study cannot be transferred directly to another. The review reports variation in the factors below, each of which can change the apparent association.

Comparison factor What varied Why it matters
Age and health Infants and adults, with different medical and developmental backgrounds The developing infant microbiome and immune system are not equivalent to those of an adult.
Vaccine Different vaccine products and antigen targets Immune correlates and response thresholds differ by vaccine.
Route Oral and injected (parenteral) administration Gut exposure may be more directly relevant to an oral vaccine, but the review still found associations reported for both routes.
Microbiome sample Stool collected at different times before or after vaccination Microbiota change over time; a single sample may not represent a person’s usual state or the state at immune priming.
Laboratory and analysis method Different sequencing, taxonomic classification and statistical approaches Methods can identify different organisms or produce results that are not directly interchangeable.
Immune outcome Antibody concentrations, seroconversion, cellular assays and other endpoints A change in one laboratory endpoint does not necessarily equal a change in clinical protection.
Measurement timing Different intervals between vaccination and immune testing Early, peak and waning responses answer different biological questions.
Study design Mostly observational associations, with uneven control of confounding Without randomization or careful adjustment, a third factor may explain both the microbiome and the response.

These differences explain why a headline such as “good bacteria improve vaccines” overstates what the evidence can support. The studies are informative when considered together, but their measurements are not interchangeable.

How the microbiome could influence immunity

Biology offers several plausible explanations for an association. A 2021 review in Nature Reviews Immunology discusses three broad possibilities:

Microbial molecules as natural adjuvants

Components of bacteria can stimulate innate immune sensors. Those signals may help activate antigen-presenting cells, which in turn shape the adaptive response generated after vaccination. This could affect the strength or quality of antibody and T-cell responses.

Metabolites that alter immune-cell behavior

Gut microbes produce metabolites that can circulate or act locally. Such compounds may influence antigen-presenting cells, inflammatory signaling and the balance of immune-cell populations involved in vaccine responses.

Cross-reactive microbial antigens

Some microbial antigens may resemble portions of vaccine targets. Immune cells trained by previous microbial exposure could then respond differently when they encounter a vaccine antigen. Cross-reactivity could theoretically enhance or interfere with a response, depending on the antigens and context.

These mechanisms make the observed associations biologically credible. They do not demonstrate that adding a bacterium, removing a food or taking a commercial supplement will improve protection. Mechanistic plausibility is not an intervention result.

Mechanism overview: “Modulation of immune responses to vaccination by the microbiota: implications and potential mechanisms”.

Association is not proof that the microbiome causes a weak response

The 2024 review cautions that a microbiome pattern could be a marker of another condition rather than the cause of a vaccine-response difference. Potential confounders include age, underlying health, diet, medication use, antibiotic exposure and other features of the study population. For example, an illness or treatment could alter both gut bacteria and immune function, creating a correlation even if the bacteria themselves are not driving the response.

Observational studies are useful for finding signals, but they cannot by themselves establish what would happen if the microbiome were deliberately changed. A causal claim would require stronger designs, such as adequately powered randomized interventions with standardized microbiome measurements and clinically meaningful immune or infection outcomes. The systematic-review authors state that “Further adequately powered studies are needed to confirm this association and inform potential microbiota-targeted interventions to optimise vaccine responses.”

Should you take probiotics before a vaccine?

Current evidence does not support a generally effective probiotic product, dose or schedule for improving an individual’s vaccine response. An earlier systematic review examined 26 probiotic studies involving 3,812 participants. About half of the studies reported a beneficial effect, with the strongest signal for oral vaccines and injected influenza vaccines. However, the trials differed greatly in probiotic strain, dose, viability, purity, duration and timing.

Because probiotic effects are strain-specific and intervention-specific, a positive result for one formulation and vaccine cannot be generalized to every product or vaccination. The review did not identify a regimen that clinicians can recommend broadly. The 2024 microbiome review likewise calls for more adequately powered intervention research before microbiota-targeted strategies can be used to optimize vaccination.

Read the probiotic evidence review: Zimmermann and Curtis, “The influence of probiotics on vaccine responses – A systematic review”.

What this means in practice

  • Do not delay, skip or alter a recommended vaccine because of your gut microbiome.
  • Do not assume that a supplement marketed for “immune” or “gut” support will improve vaccine protection.
  • If you already use a probiotic for another medical reason, ask your clinician whether it is appropriate for you; that is a separate question from vaccine enhancement.
  • Follow the vaccine schedule and preparation instructions provided by your healthcare professional or public-health authority.

What about changing your diet?

A balanced diet supports general health, but the cited human evidence does not establish a microbiome-directed diet that improves vaccine responses. Studies associating food patterns, bacteria and immunity do not prove that a short-term dietary change before an appointment will produce a stronger or more protective response. Extreme restriction, unregulated supplements or abrupt diet changes can create risks of their own, especially for children, older adults and people with medical conditions.

Can a microbiome test tell whether a vaccine will work?

Not currently. Consumer stool tests may report bacterial names or diversity scores, but the 2024 review did not establish a validated profile that predicts an individual’s vaccine response. Results can also depend on when and how the sample was collected, which sequencing method was used and how organisms were classified.

A test result therefore cannot tell you that a vaccine will fail, that you need an extra dose or that a particular supplement will correct a supposed imbalance. Decisions about vaccination, boosters or evaluation of an inadequate response should use established clinical guidance and, when appropriate, validated antibody or cellular tests—not a consumer microbiome score.

What research would answer the remaining questions?

Future studies need to make their comparisons more reliable and their outcomes more useful to patients. Important design features include:

  • Large, adequately powered cohorts that include both infants and adults where appropriate.
  • Standardized stool collection, sequencing and taxonomic analysis, with repeated samples to capture change over time.
  • Clear recording of antibiotics, other medicines, diet, illness, age and health conditions that could confound results.
  • Pre-specified vaccine, route, immune endpoint and measurement time.
  • Randomized trials of a defined microbiota intervention, rather than observational comparisons alone.
  • Outcomes that connect laboratory responses with clinically meaningful protection, not just a change in an antibody number.

The literature summarized in the 2024 review runs through June 2024. Findings from later studies could refine the picture, but they would need to overcome the same problems of inconsistent methods and confounding before supporting a consumer recommendation.

The practical conclusion

Gut bacteria appear to be one of many factors associated with vaccine immune responses. Bifidobacterium-rich patterns are often linked with stronger responses and Gammaproteobacteria with weaker ones, but those are population-level signals from diverse studies. They do not provide a personal prediction, prove causation or identify a proven supplement or diet.

For now, the evidence-based action is straightforward: get recommended vaccinations on schedule and discuss individual medical concerns with a qualified clinician. Treat microbiome-targeted vaccine strategies as an active research area, not as established preventive care.
