Short answer: Extra-virgin olive oil (EVOO) may reduce the early glucose rise after some meals, but there is no reliable evidence that it improves long-term blood sugar more than other olive oils for everyone. A very small type 1 diabetes study found a lower early glucose response when EVOO accompanied a high-glycemic-index meal. A 2025 randomized trial found lower insulin resistance after 24 weeks of a high-phenolic EVOO in adults with type 2 diabetes. However, the newest pooled analysis, covering 51 randomized trials and 4,334 participants, found no statistically significant overall improvement in blood glucose, insulin, HbA1c, or HOMA-IR from olive-oil interventions.
That apparent conflict is understandable once you separate a short-lived post-meal glucose curve from fasting glucose, HbA1c, and insulin resistance measured over weeks or months. EVOO is a nutritious fat choice, but these studies do not establish it as a diabetes treatment, a medication substitute, or a universally superior oil.
What does “better for blood sugar” actually mean?
Oil can affect several different measurements, and they are not interchangeable. A meal study may measure glucose for only a few hours after eating. A longer intervention may measure fasting glucose, insulin, HbA1c, or HOMA-IR. The first describes how a particular meal is handled; the others are used to assess glycemic status or insulin sensitivity over time.

It is strongly affected by the meal’s carbohydrate amount, glycemic index, and fat content.
- Fasting glucose: a blood-glucose measurement taken after fasting. It can change with medication, body weight, illness, sleep, and many other factors.
- HbA1c: a longer-term marker of average glucose exposure. It is not a direct measure of what happened after one meal.
- Insulin and HOMA-IR: measures used to examine insulin response or insulin resistance. A change in HOMA-IR is not the same as proven lower average blood glucose.
Therefore, a lower post-meal peak with EVOO cannot by itself prove better diabetes control, and a trial that improves HOMA-IR cannot automatically prove that every person will have lower HbA1c.

What the small meal study found
High-glycemic meals in adults with type 1 diabetes
Bozzetto and colleagues studied 13 adults with type 1 diabetes who used insulin pumps in a randomized crossover experiment. Participants ate high- or low-glycemic meals prepared with different fat conditions, including EVOO, low fat, and butter. After the high-glycemic meal, the 0–3-hour glucose incremental area under the curve was 198 ± 274 mmol/L × 180 min with EVOO, compared with 416 ± 329 with the low-fat meal and 398 ± 355 with butter. The differences were statistically significant.
The result was not universal across meals. When the meal had a low glycemic index, the type of fat did not produce a significant difference in post-meal glucose response. The authors emphasized that both carbohydrate quality and fat quality affected the observed glucose curve.
Why this does not establish long-term superiority
This experiment had only 13 participants, involved adults with type 1 diabetes, and observed glucose for three hours after specific meals. It does not show that EVOO lowers HbA1c, reduces insulin requirements, or improves long-term control. It also cannot be used to calculate an individual insulin dose or justify changing insulin timing. A person with diabetes should follow the dosing plan established with their clinical team.
What longer-term studies found
The 2025 OILDIABET randomized trial
The OILDIABET trial enrolled 116 adults with type 2 diabetes and followed them for 24 weeks. The intervention group consumed 30 mL per day of a phenolic-rich EVOO made from native Galician olives. The control advice was to minimize EVOO and favor refined olive-oil blends.

At 24 weeks, the intervention group had a significant reduction in HOMA-IR. The trial abstract states: “After 24 weeks, the Interventional group demonstrated significant reductions in insulin resistance (HOMA IR).” This is a result for that study population, oil, dose, and duration—not proof that every retail EVOO will produce the same effect.
The authors also reported favorable fasting-glucose and HbA1c findings in subgroups. Those subgroup analyses were underpowered, so they should be considered exploratory rather than settled benefits for all adults with type 2 diabetes. The study did not establish a generally recommended 30 mL dose, and it did not prove that high-phenolic EVOO lowers average glucose more effectively than every other olive oil.
The 2026 meta-analysis of 51 randomized trials
The broadest and newest synthesis identified 51 randomized controlled trials involving 4,334 participants. Its pooled analysis found no statistically significant overall changes associated with olive-oil consumption in blood glucose, insulin, HbA1c, or HOMA-IR.

The 2021 EVOO-specific review
Dehghani and colleagues analyzed 13 trials with 633 participants. This EVOO-focused meta-analysis found no significant pooled effect on fasting blood sugar, insulin, or HOMA-IR. Its conclusion is consistent with the later pooled analysis: evidence for a durable glucose-lowering effect remains uncertain.
An earlier small high-phenolic study
Santangelo and colleagues studied 11 overweight adults with type 2 diabetes. Participants consumed refined olive oil for four weeks and then high-polyphenol EVOO at 25 mL per day for four weeks. Fasting glucose and HbA1c were lower after the EVOO phase.
That finding is preliminary because the sample was very small and the phases occurred in a sequential, non-randomized order. Without a randomized parallel comparison, changes could reflect time, behavior, treatment, or other factors rather than the oil alone.
How the oils and study designs differ
| Evidence or oil condition | Participants and duration | Main finding | What it can and cannot show |
|---|---|---|---|
| EVOO with a high-glycemic meal | 13 adults with type 1 diabetes; single-meal crossover | Lower 0–3-hour glucose area under the curve than low fat or butter | Shows a meal-specific acute response; does not establish long-term control |
| High-phenolic Galician EVOO | 116 adults with type 2 diabetes; 30 mL/day for 24 weeks | Significant reduction in HOMA-IR; subgroup glucose and HbA1c results were underpowered | Promising for insulin resistance in this trial; not a universal dose or brand claim |
| Olive-oil interventions overall | 51 randomized trials; 4,334 participants | No statistically significant pooled change in glucose, insulin, HbA1c, or HOMA-IR | Best broad estimate of average effect, but individual oils and populations varied |
| EVOO trials pooled | 13 trials; 633 participants | No significant pooled effect on fasting blood sugar, insulin, or HOMA-IR | Supports uncertainty about durable glucose lowering |
| Refined olive oil followed by high-polyphenol EVOO | 11 adults with type 2 diabetes; four weeks per phase | Lower fasting glucose and HbA1c after the EVOO phase | Preliminary because the order was sequential and not randomized |
These interventions are not interchangeable. Ordinary EVOO, a phenolic-rich EVOO, refined olive oil, and an olive-oil blend can differ in phenolic content and processing. The OILDIABET result came from one high-phenolic Galician oil; it does not validate a particular supermarket brand or prove that all bottles labeled EVOO match the tested composition.

Why EVOO might change a meal response without changing HbA1c
Meal composition can dominate the short-term result
The type of carbohydrate and its glycemic index changed the interpretation of the type 1 diabetes experiment. A high-glycemic meal produced a measurable difference between fat conditions, whereas low-glycemic meals did not. This means the same oil may appear helpful in one meal pattern and neutral in another.
Phenolic content is not the same as the EVOO label
High-phenolic EVOO is a specific intervention characteristic, not a guarantee attached to every bottle of extra-virgin olive oil. Phenolic concentration can vary by olive variety, harvest, processing, storage, and time. None of the cited evidence establishes that a named retail product will reproduce the OILDIABET result.
Small studies can produce unstable estimates
The 13-person meal trial and 11-person sequential study are useful for generating hypotheses, but their results are more sensitive to individual variation than large randomized trials. A favorable small study should be weighed alongside the 2021 and 2026 pooled analyses rather than treated as definitive.
Practical guidance for choosing and using olive oil
Use EVOO as a food choice, not as glucose therapy
If you enjoy EVOO, it can be part of a balanced eating pattern. Choose it for flavor and its culinary qualities, not because a bottle promises to lower blood sugar. The evidence does not support replacing prescribed medication, insulin, or a clinician-designed eating plan with EVOO.

Do not copy the trial dose as a prescription
The 30 mL per day used in OILDIABET was a research protocol. It is not an established diabetes dose or a recommendation for every person. Olive oil is energy-dense, so the appropriate amount depends on a person’s overall diet, health goals, and clinical advice.
Keep meal-specific findings in context
If you monitor glucose, your own readings may show that meals differ, but do not change insulin timing or dose based on the 2016 experiment. Discuss repeated patterns with your diabetes clinician, who can account for carbohydrate amounts, medications, activity, and hypoglycemia risk.
How to evaluate a claim that one olive oil is best
- Identify the outcome. Is the claim about a three-hour post-meal curve, fasting glucose, HbA1c, insulin, or HOMA-IR?
- Check the population. Results in type 1 diabetes do not automatically apply to type 2 diabetes, and neither group represents everyone at risk of diabetes.
- Look for the meal and dose. The acute result depended on a high-glycemic meal, while the OILDIABET protocol used 30 mL per day for 24 weeks.
- Check the oil description. High-phenolic EVOO, ordinary EVOO, refined oil, and blends are different interventions.
- Prioritize study design. A randomized trial or pooled analysis generally provides stronger evidence than a tiny sequential study.
- Separate statistical significance from clinical advice. A statistically significant result in one study does not create a universal treatment recommendation.
What can reasonably be concluded?
EVOO may blunt an early glucose response when paired with a high-glycemic meal, as shown in a small type 1 diabetes crossover study. A 2025 randomized trial also raises the possibility that a particular high-phenolic EVOO can improve insulin resistance in adults with type 2 diabetes over 24 weeks. Those findings are specific to their populations, oils, meals, and designs.
For the broader question—whether EVOO reliably improves long-term blood sugar more than other olive oils—the answer remains no. The 2026 analysis of 51 trials found no significant average improvement in glucose, insulin, HbA1c, or HOMA-IR, and the 2021 EVOO review reached a similarly uncertain conclusion. EVOO can be a sensible culinary fat, but current evidence does not justify calling it a blood-sugar treatment or assuming that a particular bottle will outperform refined olive oil for diabetes.
