Short answer: Exercise, particularly voluntary running, reliably increases adult hippocampal neurogenesis in many animal experiments. In humans, exercise studies find changes in hippocampal volume, blood flow, fitness or memory, but they usually do not count newly generated neurons. Those findings make exercise a plausible influence on human neurogenesis, not proof that exercise caused new-neuron formation. Whether substantial neurogenesis persists in the adult human hippocampus, how much occurs, and what it does remain active scientific questions.
What neurogenesis means in this question
Neurogenesis is the production of new neurons from neural stem and progenitor cells. In research on exercise, the hippocampus receives most attention because it is involved in learning, memory and spatial navigation, and because animal studies identify a population of stem cells in the dentate gyrus that can generate neurons.
A complete neurogenesis claim involves more than finding cells that divide. Researchers would need to establish that stem or progenitor cells proliferated, differentiated into neurons, survived, matured, formed connections with existing circuits and contributed to function. A larger hippocampus or better memory score does not identify which of those steps occurred.
Why running is such a strong result in animal research
A 2023 review by Gao, Syed and Zhao describes voluntary running as one of the strongest experimental stimuli for adult hippocampal neurogenesis. Across animal models, running has been associated with several stages of the process:

- Activation of previously quiescent neural stem cells.
- Proliferation of stem and progenitor cells.
- Differentiation and survival of newborn cells.
- Maturation of those cells into neurons.
- Integration into hippocampal circuitry.
These experiments can label dividing cells, identify neuronal markers and examine tissue directly. That gives them a level of cellular resolution that is generally unavailable in living human participants. The same review emphasizes that the detailed mechanisms linking running to each stage are not fully resolved. Candidate explanations include altered growth-factor signaling, blood-vessel changes, activity-dependent circuit effects and signals released elsewhere in the body.
The animal result should therefore be stated precisely: voluntary running strongly promotes hippocampal neurogenesis in experimental animals. It is not evidence that a particular human running schedule has been proven to produce new hippocampal neurons.
Is adult human hippocampal neurogenesis established?
Not conclusively. A 2023 review by Zanirati and colleagues examines evidence that neural stem cells may persist and generate neurons in the adult and aging human hippocampus, while noting that robust adult human neurogenesis remains unconfirmed.

Human tissue is scarce, often obtained after death, and affected by age, disease, medication, stress, tissue-processing delays and differences in laboratory methods. Studies have also disagreed about which cellular markers reliably identify newborn human neurons. As a result, researchers still need better answers to four separate questions:
- How many new neurons, if any, are produced at different adult ages?
- Do those cells survive for long periods?
- Do they integrate into functioning hippocampal circuits?
- Does increasing their production change memory or other behavior?
Evidence supporting some degree of adult neurogenesis does not settle its amount or importance. Conversely, uncertainty about measurement does not demonstrate that no new neurons exist. The scientifically defensible position is that human adult hippocampal neurogenesis is plausible and supported by some evidence, but its persistence, scale and function remain debated.

What human exercise studies actually measure
People cannot usually undergo the tissue-labeling procedures used in animal experiments. Human exercise trials therefore rely on outcomes that may be related to brain health but are not direct neuron counts.
| Outcome measured in people | What it can show | What it cannot establish by itself |
|---|---|---|
| Hippocampal volume on MRI | Whether volume changed after an intervention or differs between groups. | That new neurons were created, survived or caused the volume change. |
| Perfusion or blood-flow measures | Whether circulation in or near the hippocampus changed. | That any blood-flow change represents neurogenesis. |
| Fitness measures | Whether cardiorespiratory or other physical-fitness outcomes changed. | Which cellular pathway produced a brain change. |
| Memory or other cognitive tests | Whether measured performance changed or correlates with exercise exposure. | That newly born neurons caused the behavioral result. |
| Blood or hormone biomarkers | Whether candidate systemic signals changed with exercise. | That a signal reached the hippocampus and generated functional neurons. |
Exercise can affect several biological systems at once. A hippocampal change might involve neurogenesis, angiogenesis (new blood vessels), dendritic remodeling, neurotrophic factors, altered vascular health, reduced inflammation or reduced stress-related damage. These mechanisms are not mutually exclusive, and the same measured outcome can result from more than one of them.

A meta-analysis titled Exercise interventions preserve hippocampal volume: A meta-analysis combined 22 studies comprising 23 controlled trials and 1,204 participants. The underlying studies were published from 2010 through 2020. This is evidence about exercise and hippocampal volume, not a direct test of neurogenesis.
Across the included trials, the overall effect of exercise on hippocampal volume was significantly positive. The result supports the narrower statement that exercise interventions can affect a measurable hippocampal structural outcome in some study populations. It does not identify newly born neurons as the cause.
Subgroups were not a universal exercise prescription
The analysis found significant volume effects in older samples and in interventions lasting longer than 24 weeks. Effects were not significant in younger samples or when healthy samples were considered alone. These subgroup findings describe variation in the evidence base; they do not prove that age or duration alone determines a neurogenesis response.

An unexpected pattern appeared when trials were grouped by weekly exercise time: studies prescribing less than 150 minutes per week showed a significant volume effect, whereas those prescribing at least 150 minutes did not. The authors suggested that adherence or exercise intensity might help explain the pattern. The analysis could not establish exercise intensity as a moderator, and the result should not be converted into a claim that less than 150 minutes is an optimal dose for making human neurons.
Subgroup results can also be unstable because they involve fewer participants and different protocols. They are useful for generating hypotheses about who may respond and under what conditions, not for proving a dose of exercise that increases neurogenesis.
Can MRI show neurogenesis?
MRI is valuable because it can be repeated in living people, but conventional MRI does not display individual newborn neurons. It estimates properties such as tissue volume, structure, diffusion or blood flow. A volume increase may reflect changes in water content, blood vessels, glial cells, dendrites, synapses or other tissue components, alone or in combination.
A review titled In vivo imaging of adult human hippocampal neurogenesis: progress, pitfalls and promise discusses indirect imaging strategies. Some animal studies have linked an imaging signal with neurogenesis measured in the same animals. Researchers can then ask whether a similar signal in humans might be informative. That cross-species correspondence is a hypothesis-generating bridge, not a direct human cell count.
For that reason, “MRI showed new neurons” is too strong unless a specific study directly validates that claim with an appropriate cellular measure. The accurate wording is that MRI showed a structural or physiological change that could have several causes, one of which might be neurogenesis.

How to evaluate a human exercise-and-neurogenesis claim
When reading a study, check the following features before accepting its conclusion:
| Question | Why it matters |
|---|---|
| Who participated? | Age, health status, medication use and baseline fitness can alter hippocampal measures and the ability to generalize results. |
| What was the exercise? | Walking, cycling, resistance training and mixed programs impose different physiological demands; “exercise” is not one biological treatment. |
| How long and how often? | Duration, weekly minutes, intensity and adherence determine the exposure actually received. |
| Was the study randomized? | Randomization helps separate intervention effects from pre-existing differences. Observational associations cannot establish causation. |
| What was measured? | Direct cellular evidence is fundamentally different from MRI volume, perfusion, biomarkers or cognition. |
| Were alternative mechanisms tested? | A study that measures only volume cannot determine whether neurogenesis, blood vessels, dendrites or other pathways caused the change. |
The key distinction is between “exercise changed hippocampal volume” and “exercise caused neurogenesis.” The first can be tested with repeated imaging. The second requires evidence about newly generated neurons and their fate that most human intervention studies do not collect.
Does exercise create new brain cells?
In animal experiments, exercise can increase the generation and persistence of new hippocampal neurons. In humans, available exercise trials do not generally demonstrate that outcome directly. They show associations or intervention-related changes in measurable proxies.
“Brain cells” is also broader than “neurons.” Exercise may influence blood-vessel cells, glia, synapses and dendritic branches in addition to any effect on neural stem cells. A change in memory or MRI structure could therefore be real and useful without being caused by newly born neurons.
Can running grow new neurons in people?
Running is the human activity most naturally connected to the animal voluntary-running literature, but that analogy has limits. Animal studies control genetics, housing, running access and tissue collection more tightly than human trials can. Human running programs also differ in pace, terrain, supervision, adherence and participant health.

No source reviewed here establishes a human running type, speed, weekly schedule or duration that is proven to increase hippocampal neurogenesis. Running may be a reasonable way to obtain the established general benefits of physical activity, while its specific effect on human neuron birth remains unconfirmed.
What biological pathways might connect exercise to the hippocampus?
Mechanistic reviews discuss several plausible routes rather than one confirmed pathway. Exercise changes circulation and can support angiogenesis. It can alter neurotrophic and growth-factor signaling, influence stress and inflammatory systems, and change peripheral hormone responses. Remodeling of dendrites and synapses may alter hippocampal function without requiring additional neurons.
A 2023 review by Kraemer and Kraemer focuses on peripheral hormone responses as candidate influences on adult hippocampal neurogenesis. Such pathways are biologically plausible, but a change in a circulating hormone is not proof that it generated, preserved or integrated a new human hippocampal neuron. Mechanistic proposals need confirmation with cellular and functional measurements.
What human studies can—and cannot—tell us
They can tell us
- Whether an exercise intervention is associated with a change in hippocampal volume, perfusion, fitness or cognition.
- Whether effects differ across age groups, health conditions or intervention lengths.
- Whether an exercise protocol is feasible and whether participants adhere to it.
- Which biological mechanisms deserve more direct testing.
They cannot tell us from indirect outcomes alone
- That new neurons were generated.
- That the cells survived and integrated into existing circuits.
- That neurogenesis caused a structural or cognitive change.
- That one exercise mode or dose is optimal for human neurogenesis.
Answering those questions will require better-validated cellular markers, carefully characterized human tissue, imaging methods that are demonstrably specific, and studies that connect cellular findings to behavior.
What should readers do with this evidence?
Do not wait for a neurogenesis claim to justify exercise. Physical activity can be evaluated for outcomes that human trials actually measure, such as fitness, mobility, cardiovascular health and—depending on the intervention—cognitive or structural brain outcomes. Those benefits should not be relabeled as proof of new-neuron formation.
If a program is described as “proven to grow new brain cells,” ask which species was studied, whether the work was randomized, what outcome was measured, how long the intervention lasted, and whether the investigators directly counted neurons. A precise answer may still be encouraging: exercise can change hippocampal measures, while the cellular explanation remains unresolved.
The evidence-based bottom line
Exercise—especially voluntary running—has strong neurogenesis effects in animal models. Human studies provide credible evidence that exercise can influence hippocampal structure and function, including volume in some populations, but the usual measurements are indirect. Adult human hippocampal neurogenesis itself remains debated, and no human exercise type or dose has been proven to increase it. The most accurate conclusion is that exercise is a plausible contributor to neurogenesis-related biology, not an established cause of new-neuron formation in people.
