Short answer: Current U.S. prescribing information for semaglutide (Wegovy) and tirzepatide (Zepbound) does not list nail disorder as an adverse reaction for either medicine. The labels therefore do not establish that one causes more nail problems than the other. They also do not prove that nail changes are impossible, because an uncommon or not-yet-recognized event can be absent from a label.
No head-to-head comparison of nail outcomes was identified. Both labels do discuss hair loss associated with weight reduction, but hair and nails are different symptoms, so those hair findings cannot be used to calculate or compare nail risk.
What the current labels actually say
The most precise comparison comes from the current U.S. labels rather than from anecdotes or from comparing unrelated trial percentages. The DailyMed prescribing information for Wegovy and the DailyMed prescribing information for Zepbound, both reviewed in 2026, do not identify nail disorder as a listed adverse reaction.

What label silence means
A label is a record of recognized safety information, not a guarantee that every possible symptom has been ruled out. The absence of “nail disorder” means the reviewed labeling does not establish a known, quantified nail adverse effect for either product. It does not demonstrate that no patient can experience a nail change while taking semaglutide or tirzepatide.

The evidence also does not show a nail-specific biological mechanism for either medicine. If a nail change begins after treatment, timing alone cannot prove that the drug caused it. A clinician must consider the complete clinical context.
Why hair-loss numbers do not answer a nail question
Hair loss and nail changes are separate outcomes. Both labels describe hair loss in the setting of weight reduction, but neither label converts that information into a nail incidence rate or a nail mechanism.

The figures also come from different clinical-trial programs, populations and conditions. The Wegovy labeling cautions that adverse-reaction rates from separate trials cannot be directly compared. Consequently, the 3.3% Wegovy hair figure and the sex-specific Zepbound figures cannot be treated as evidence that either medicine has a higher nail risk—or even a directly comparable hair risk.
Is there evidence that one medicine causes more nail changes?
No. The sources reviewed contain no head-to-head nail-outcome study and no nail-specific comparative statistic. A responsible comparison can say only that both current labels are silent on nail disorder and that neither label provides a numerical basis for ranking nail risk.
Reports on social media, an individual’s experience, or a sequence in which nail changes followed an injection can be useful information to discuss with a prescriber, but they cannot establish incidence or causality. Without a controlled comparison, it is not possible to separate a medication effect from weight change, nutrition, an underlying condition, another medicine, or an unrelated nail problem.

Can semaglutide or tirzepatide affect your nails?
The most accurate answer is that a nail change is possible during treatment, but current labeling does not establish either drug as its cause. Several explanations may need consideration at the same time.
Medication timing is a clue, not proof
Note when the change started, whether it followed a dose change, and whether it is continuing. That chronology helps a clinician investigate, but an event occurring after treatment does not by itself show that semaglutide or tirzepatide produced it.
Weight reduction may be part of the clinical context
Both labels associate reported hair loss with weight reduction. That association is not evidence of a nail effect, but the amount and pace of weight change are still relevant facts for a clinician reviewing any new hair, skin or nail concern.

Nutrition and other health factors may matter
The available evidence does not settle whether a particular nail change is medication-related, connected with nutrition, linked to an underlying condition, or caused by something else. A medical history that includes recent dietary changes, illnesses, other medicines and prior nail problems gives the prescriber or dermatologist a way to weigh those possibilities.
What to do if your nails change during treatment
- Describe the change precisely. Record which nails are involved, what you see (for example, a change in color, thickness, texture or breakage), when it began and whether it is worsening. Photographs taken in consistent lighting can help show progression.
- Contact the prescriber who manages the medicine. Tell the clinician about the nail finding and any recent weight change, dose adjustment, illness, dietary change or new product used on the nails. The clinician can decide whether an examination or additional evaluation is appropriate.
- Do not stop, reduce or switch the prescription on your own. The reviewed evidence does not establish a nail-specific adverse effect that would justify changing treatment without medical advice. Ask the prescriber how to proceed if the symptom is troublesome or progressing.
- Consider dermatology evaluation. The American Academy of Dermatology advises people with GLP-1-related skin, hair or nail concerns to communicate with their medical team and partner with a board-certified dermatologist. - Bring a complete product and medication list. Include prescription and nonprescription medicines, supplements and topical nail products. This helps the clinician look beyond the injection itself instead of assuming that the newest medicine is responsible.
How a clinician may assess the problem
An assessment starts with the nail pattern and the timeline. The clinician may ask whether one nail or many are affected, whether the change appeared on hands, feet or both, and whether similar changes occurred before treatment. The relationship to dose changes and weight reduction can then be considered alongside other health events.

A dermatologist can examine the nails directly and determine whether the appearance warrants further testing or observation.

Because no nail-specific mechanism is established in the reviewed labels, an evaluation that identifies another cause is not contradicting the patient’s experience. It is the way to distinguish coincidence, an indirect effect of changing health circumstances and a genuine medication reaction.
How to interpret questions about Ozempic, Mounjaro and other formulations
People often use brand names when asking whether “Ozempic,” “Wegovy,” “Mounjaro” or “Zepbound” can change nails. The evidence summarized here is specifically the current U.S. prescribing information for Wegovy (semaglutide) and Zepbound (tirzepatide). It should not be presented as a nail-outcome comparison for every formulation, dose or country.
Even when two products contain the same active ingredient, indications, doses, study populations and labeling can differ. A clinician or pharmacist can check the label for the exact product and indication being used.
What is known—and what remains unknown
- Established by the reviewed labels: Nail disorder is not listed as an adverse reaction for Wegovy or Zepbound.
- Established by the reviewed labels: Both products include hair-loss information associated with weight reduction.
- Not established: The incidence of nail changes among all users of either medicine.

Based on the current U.S. information reviewed, however, there is no evidence-based way to choose between semaglutide and tirzepatide by claiming that one has a higher nail-side-effect risk.
