No blanket claim is justified. A 2026 systematic review of nine randomized trials in children aged 24 months or younger found no statistically significant reduction in upper-respiratory infections or respiratory infections overall from probiotic or synbiotic supplements. One strain, Bifidobacterium lactis BB-12, reduced infections in two individual infant reports, while another formula trial found no respiratory benefit. The result depends on the exact strain, dose, formula, infant population and outcome measured—not simply whether a label says “probiotic.”

What the best pooled evidence found

The 2026 systematic review and meta-analysis combined nine randomized trials involving children aged 24 months or younger. For upper-respiratory-tract infections, the pooled odds ratio was 0.95 (95% confidence interval 0.47–1.95; I²=78%). For any respiratory infection, it was 0.66 (95% confidence interval 0.35–1.25; I²=69%). Because both confidence intervals include no effect, neither result is statistically significant.

The high I² values indicate substantial variation between studies. The review’s conclusion was: “Current evidence does not support the routine use of probiotic or synbiotic supplementation for the prevention of respiratory infections in children aged ≤24 months.” That is an evidence statement about routine prevention, not proof that every probiotic is ineffective.

Why individual studies do not all agree

Infant probiotic trials are not interchangeable. They can differ in:

What the main BB-12 studies actually showed

Study Population and product Respiratory result How to interpret it
Taipale et al., British Journal of Nutrition (2011) Infants receiving BB-12; the available report summary does not establish that its dose and formula matrix match commercial products. Respiratory infections occurred in 65% of BB-12 infants versus 94% of controls; risk ratio 0.69 (95% CI 0.53–0.89; P=0.014). A statistically significant finding from one trial, not a class-wide effect.
Taipale et al., Pediatric Research (2016) Infants receiving BB-12. Respiratory-tract infections occurred in 87% of recipients versus 100% of controls; risk ratio 0.87 (95% CI 0.76–1.00; P=0.033). Again supportive for BB-12 in that study, with a smaller relative difference than the 2011 report.
Weizman et al., American Academy of Pediatrics report (2005) 201 healthy term infants assigned to formulas containing BB-12, Lactobacillus reuteri or control formula. No significant difference in respiratory-illness rate among the three groups. Shows that a formula trial using BB-12 did not reproduce a respiratory benefit.
Randomized BB-12 formula trial 172 formula-fed infants; the primary focus was immune markers rather than respiratory infections. BB-12 increased fecal secretory IgA and some vaccine-specific IgA measures. An immune-biomarker change is not evidence that colds or other respiratory infections were prevented.
2026 systematic review Nine randomized trials in children aged 24 months or younger; strains, doses, formats and populations varied. Upper-respiratory infection OR 0.95 (95% CI 0.47–1.95); any respiratory infection OR 0.66 (95% CI 0.35–1.25). The pooled estimate is inconclusive or negative for routine prevention, with substantial heterogeneity.

Is BB-12 worth choosing for a formula-fed baby?

BB-12 is the strain with the clearest positive respiratory findings in the infant evidence summarized here, but those findings are inconsistent. The 2005 study found no respiratory difference, and the pooled 2026 analysis did not show a significant overall benefit. Therefore, BB-12 can be described as an optional strain with mixed evidence—not as a dependable way to stop a baby catching colds.

Check the full strain designation on any product. “Bifidobacterium” or “probiotic” alone does not establish that it contains BB-12, the same dose used in a trial, or the same delivery format. A commercial powder or formula may also have different storage requirements, viability at the time of use and additional ingredients.

Does probiotic formula work better than probiotic drops?

The evidence summarized here does not establish that drops are safer or more effective than probiotic formula, or vice versa. Delivery format is one of the factors that varied between trials, and the studies do not provide a head-to-head answer for respiratory-infection prevention. Compare the strain, dose, infant age range and clinical outcome rather than treating the format as a guarantee of benefit.

What safety guidance says

ESPGHAN guidance on biotics in infant formula reports no safety concerns regarding growth, tolerance or adverse effects for the evaluated probiotics, prebiotics, synbiotics and postbiotics in healthy infants. This safety conclusion does not demonstrate infection-prevention efficacy, and it does not mean that every product has been tested in every infant population. The guidance also notes that heterogeneous and limited randomized evidence may explain inconsistent efficacy results.

How parents can make a careful decision

  1. Identify the exact product. Record the organism, strain designation (for example, BB-12), stated daily dose and whether it is mixed into formula or supplied separately.
  2. Check whether the evidence matches your baby. Consider age, term status, feeding pattern and the outcome studied. A trial in healthy term infants or a formula containing one strain may not apply to a different infant or product.
  3. Separate biomarkers from outcomes. Higher secretory IgA is an immune measurement; it is not the same as fewer diagnosed respiratory infections.
  4. Discuss the choice with the baby’s clinician. Ask whether the product is appropriate for your infant and whether its formulation resembles the one studied. Treat supplementation as optional rather than necessary for routine cold prevention.
  5. Get medical advice for illness. A probiotic should never replace professional assessment when an infant has respiratory symptoms or is otherwise unwell.

Bottom line for the headline claim

Some BB-12 trials reported fewer respiratory infections, but another BB-12 formula trial did not, and the 2026 pooled evidence found no significant preventive effect. For formula-fed babies, a probiotic label is not enough to predict benefit: strain, dose, format and population all matter. Current evidence does not support routinely using probiotic or synbiotic formula solely to prevent respiratory infections.
