Start by asking your oncology team about fertility and treatment-related hormone or sexual-function changes before therapy begins, when medically feasible. Ask for an urgent reproductive-specialist referral if you are interested, unsure, or simply want to understand your choices. At the same time, request a social worker or financial navigator and psychosocial support. These steps can protect options that become harder to recover after treatment starts.

This guide explains what to ask, how preservation options differ, how timing and insurance affect decisions, and where young adults can obtain practical and emotional help. It is general education, not an individualized treatment plan; your diagnosis, anatomy, age, treatment protocol, location, and insurance determine what applies.

Your first conversations after diagnosis

A cancer diagnosis can make every decision feel urgent. Use your first oncology visits to establish three parallel tracks: cancer treatment, future fertility and reproductive goals, and practical and emotional support. You do not have to decide whether you want children now. You do need enough information to keep choices open where possible.

Ask about risks before treatment starts

Ask this plainly: “Could my planned surgery, chemotherapy, radiation, immunotherapy, or endocrine treatment affect fertility, hormones, sexual function, or a future pregnancy?” Ask the clinician to document the answer in your treatment plan. Risk depends on the drugs, doses, radiation field, surgery, age, reproductive organs, and whether treatment is combined.

The American Society of Clinical Oncology (ASCO) strongly recommends discussing possible infertility as early as possible, before treatment starts, to preserve the full range of options. The recommendation applies even when you are ambivalent. Ask for a reproductive endocrinologist, fertility specialist, or urologist referral promptly rather than waiting for treatment to begin.

Do not delay cancer treatment on your own

Some preservation procedures require appointments, medication, monitoring, anesthesia, or surgery. The National Cancer Institute (NCI) notes that success, cost, and availability vary, and your oncology team must advise whether a procedure could delay cancer therapy. Ask how many days are medically acceptable, whether treatment can start while testing is completed, and which steps are optional if time is extremely limited. Never postpone chemotherapy, radiation, surgery, or another prescribed treatment without agreement from the oncology team.

Fertility-preservation options

Your options depend on the reproductive organs you have, the cancer and its stage, treatment urgency, age, hormone sensitivity, and your family-building goals. A reproductive specialist can explain likely outcomes, alternatives, and whether a method is established or still emerging.

Option What it involves Timing and trade-offs to discuss
Sperm cryopreservation Collecting and freezing sperm samples for later use. Often requires one or more collections before treatment. Ask about collection difficulty, alternatives if ejaculation is not possible, storage duration, and future use with assisted reproduction.
Embryo cryopreservation Ovarian stimulation, egg retrieval, fertilization with sperm, and freezing embryos. Usually requires stimulation and monitoring and may take time before cancer therapy. It requires a sperm source and raises decisions about storage, disposition, and future use.
Oocyte (egg) cryopreservation Ovarian stimulation, retrieval, and freezing unfertilized eggs. Does not require a current sperm source, but still involves medication, monitoring, and retrieval. Ask about the number of cycles that may be feasible and storage fees.
Ovarian-tissue cryopreservation Surgically removing and freezing ovarian tissue for possible later reimplantation or other use. May be considered when stimulation cannot be completed or time is short. It requires surgery and a specialist assessment; availability and eligibility vary.
Ovarian transposition Surgically moving ovaries away from a planned pelvic radiation field. Must be coordinated with the radiation plan. Protection is not guaranteed, and the ovaries may move back or still receive scatter radiation.
Selected conservative surgery Choosing an organ-sparing operation when medically appropriate for a particular diagnosis. Only some cancers and stages qualify. Cancer control comes first; discuss recurrence risk, follow-up, and whether later pregnancy is medically safe.

ASCO considers sperm, embryo, oocyte, and ovarian-tissue cryopreservation, ovarian transposition, and selected conservative gynecologic surgery established options for appropriate patients. In-vitro maturation is described as an emerging approach, not a universal substitute for established methods. Gonadotropin-releasing hormone agonists should not replace established preservation methods; ask whether they have a role as an adjunct in your specific treatment.

Questions that make a fertility consultation useful

  • Which part of my treatment creates the greatest fertility risk, and is the risk temporary or permanent?
  • How much time can safely be spent on evaluation or preservation?
  • What success information is available for people my age and diagnosis, and what uncertainty remains?
  • Will I need anesthesia, surgery, hormone stimulation, or multiple visits?
  • How long are specimens stored, who controls future use, and what recurring storage fees apply?
  • What alternatives exist if I cannot produce a sample, tolerate stimulation, or undergo surgery?
  • Could treatment affect pregnancy safety, sexual function, menstrual cycles, testosterone, or menopause even if fertility is preserved?

Coordinate timing with the cancer team

Ask the oncologist and fertility specialist to communicate directly. Bring the proposed cancer-treatment start date, pathology information, imaging, medication list, and any concerns about hormones or pregnancy. Ask for a written timeline showing referrals, testing, medication, monitoring, retrieval or surgery, storage, and the date cancer treatment is expected to begin.

Urgency can change. A treatment plan may be revised after staging, surgery, pathology, or a response assessment. ASCO recommends revisiting fertility and family-building goals during survivorship and whenever treatment plans change. A decision not to preserve now can be revisited later, but options may narrow after exposure to gonadotoxic treatment.

Paying for preservation and treatment

Coverage in the United States is uneven. ASCO reported that in 2024, 17 states and the District of Columbia had mandates applying to some state-regulated plans for medically indicated fertility preservation. A mandate does not cover every plan: self-funded employer plans are generally outside state mandates, and Medicaid coverage and rules differ by state. Even when a service is covered, deductibles, coinsurance, copayments, medication exclusions, monitoring limits, and storage fees can leave substantial costs.

Build a written payment plan

  1. Call the insurer. Ask whether your plan covers fertility preservation because of a cancer diagnosis, which procedures and medications qualify, whether storage is covered, and whether a state mandate applies.
  2. Ask about authorization. Confirm referral requirements, in-network rules, prior authorization, documentation, benefit limits, exclusions, and the deadline and method for an appeal.
  3. Request itemized estimates. Ask the fertility clinic for separate estimates for consultation, laboratory tests, medication, monitoring, anesthesia, surgery or retrieval, specimen processing, and first-year and ongoing storage.
  4. Use cancer-center resources. Request a social worker, financial counselor, or patient navigator. They may identify grants, payment assistance, transportation help, charitable programs, and lower-cost clinics.
  5. Keep records. Save names, dates, reference numbers, estimates, bills, denial letters, and appeal submissions. Ask for written explanations rather than relying only on phone calls.

Eligibility rules and nonprofit programs change. The NCI identifies LIVESTRONG Fertility for financial support and clinic finding, the Alliance for Fertility Preservation for insurance-law information, and the Oncofertility Consortium for navigation and community resources. Ask a social worker to check current eligibility in your state and for your plan.

Support for emotional, relationship, and practical needs

Cancer during the years when people often date, study, work, form families, or establish housing can disrupt identity and plans. Fertility decisions may affect a partner, a future partner, or your sense of gender and sexuality. You can ask for support without having a mental-health crisis.

Request psychosocial care early

Ask for a mental-health professional familiar with cancer and reproductive decision-making. ASCO recommends psychosocial support within fertility-preservation care. Counseling can help with uncertainty, grief, body changes, sexual concerns, relationship conversations, and decisions made under time pressure. NCI advises discussing how you feel emotionally so you receive the psychosocial support you need and deserve.

Find age-appropriate and practical help

Many cancer centers offer some combination of young-adult peer groups, individual counseling, fertility counseling, art or music therapy, and adventure-based programs. Ask a navigator about appointment coordination, transportation, work or school documentation, housing, legal questions, childcare, and bills. If a program is not available locally, ask whether telehealth or a community referral is possible.

Tell clinicians the kind of support you want. You might need someone to attend appointments, help explain options to a partner, interpret medical language, arrange transportation, or provide private counseling. Support should respect your culture, sexual orientation, gender identity, reproductive goals, and decision about whether to share information.

A checklist for your next appointment

  • Write down the exact cancer diagnosis, planned treatments, and proposed start dates.
  • Bring a list of medicines, supplements, prior surgeries, menstrual or reproductive history, and questions about sexual function.
  • Ask the oncology team to document fertility and hormone risks.
  • Request an urgent reproductive-specialist referral if interested, uncertain, or ambivalent.
  • Ask which established preservation options fit your anatomy and diagnosis and which are emerging or unavailable.
  • Ask whether preservation could delay treatment and what maximum delay is acceptable.
  • Request a social worker, financial counselor, patient navigator, and psychosocial professional.
  • Call the insurer and clinic for written coverage and itemized cost information.
  • Ask how storage, future use, and follow-up will be managed.

What survivorship planning should include

After active treatment, ask for a survivorship plan that records what treatment you received, the potential effects on fertility, hormones, sexual function, and pregnancy, and which specialists should follow you. Revisit family-building goals even if preservation was not possible or you were not ready to decide initially. Ask when fertility testing, hormone evaluation, contraception counseling, or pregnancy planning is appropriate for your diagnosis.

Approximately 70,000 people ages 15 to 39 in the United States were diagnosed with cancer in 2020, according to ASCO. Young-adult cancer care therefore includes more than tumor treatment: it also means planning for work, relationships, mental health, financial stability, and the possibility of future parenthood.

Reliable starting points

The NCI Cancer Information Service can be reached at 1-800-4-CANCER for cancer information and referrals. NCI also identifies the Oncofertility Consortium, the Alliance for Fertility Preservation, and LIVESTRONG Fertility as resources for fertility navigation, insurance information, financial assistance, and clinic finding. Use these organizations alongside—not instead of—your oncology and reproductive-care teams.
