Short answer: cuffless blood-pressure technology is closer to a consistent validation pathway, but it is not yet a dependable replacement for a conventional upper-arm cuff when diagnosing or treating hypertension. Newer FDA guidance, IEEE 1708-2025 and European recommendations clarify what manufacturers must test; they do not establish that every cleared smartwatch or wearable is clinically accurate.

What changed in 2026?

The U.S. Food and Drug Administration’s January 2026 draft guidance is the clearest recent signal that cuffless devices are being judged against more explicit clinical-performance requirements. The document is draft, non-binding guidance, not a final approval rule. Where applicable, it recommends submitting data from Static, Stability and Change tests, with clinical investigations using methods from ISO 81060-2.

The guidance also ties accuracy to the device’s intended use and to users measured in a seated, resting position. A device intended to report an occasional blood-pressure estimate is not making the same claim as one that reports continuous, beat-to-beat changes during sleep or exercise.

IEEE 1708-2025 adds a normative definition and evaluation framework for wearable cuffless blood-pressure devices. It covers intermittent readings, continuous monitoring, beat-to-beat estimates and blood-pressure-variability outputs. The complete standard may require purchase to access, but its existence gives developers and evaluators a common technical reference.

These developments make validation more specific. They do not amount to proof that cuffless readings are ready for routine diagnosis or medication management.

Are cuffless blood-pressure monitors accurate yet?

Accuracy is device- and claim-specific, not a category-wide yes or no. The American Heart Association (AHA) has warned that “FDA clearance of cuffless devices does not mean they will prove accurate enough for clinical use.” Clearance indicates that a device met the requirements for its regulatory pathway and stated intended use; it is not a guarantee that readings will be reliable enough for every patient, posture, activity or treatment decision.

Cuffless systems infer pressure from signals such as optical or electrical measurements and a model that converts those signals into a blood-pressure estimate. Because the estimate can change with body position, movement, sleep state, exercise, vascular characteristics and time since calibration, a result that looks plausible in a resting test may not perform the same way in daily life.

  • Adjustable Arm Cuff: The arm cuff range from 8.7 - 15.7 inches (22-40 cm) fits your size, ensuring a comfortable and secure fit for most adults

The AHA also identifies a larger evidence gap: the field does not yet show that the enormous volume of cuffless readings leads to better outcomes or more actionable treatment decisions. More measurements are not automatically better measurements.

What validation does a cuffless device need?

A credible claim should identify the operating mode, the output being claimed and the conditions under which performance was measured. Use these questions when reading a product’s validation summary.

Validation dimension What to check Why it matters
Measurement mode Intermittent spot estimate, continuous trend, beat-to-beat output or variability metric Each mode has different error patterns and reference requirements.
Intended output Absolute systolic/diastolic values or a change from baseline A device may support trend detection without proving accurate absolute pressure.
Calibration Whether a cuff reading is required, how often recalibration is needed and how drift is tested An initially good estimate can become unreliable as calibration drifts.
Reference method Use of an accepted cuff or invasive reference method under a defined protocol Without a suitable comparator, an error number is difficult to interpret.
Posture and activity Seated rest, standing, exercise, sleep and other tested states Position and activity can materially change performance.
Population Age range, sex, skin characteristics, health conditions and blood-pressure range represented Results from a narrow study group may not generalize to every user.
Motion robustness Whether movement and signal-quality failures were tested Daily wear rarely matches a motionless laboratory measurement.
Evidence quality Peer-reviewed results and a protocol consistent with the claimed use A marketing accuracy claim is not equivalent to independently reviewed clinical evidence.

How the main standards and recommendations test performance

FDA draft guidance: Static, Stability and Change

The January 2026 FDA draft guidance recommends three types of clinical-performance testing when they apply to the intended claim:

  • Static: whether readings agree with a reference measurement under the specified resting conditions.
  • Stability: whether performance remains consistent over time rather than degrading as calibration ages.
  • Change: whether the device detects a meaningful increase or decrease when blood pressure changes.

The FDA recommends clinical-performance data “from Static, Stability, and Change tests, when applicable,” and points sponsors to ISO 81060-2 methods for clinical investigations. Because the document is draft guidance, manufacturers and regulators may refine their expectations before a final version.

ESH 2023: six tests for intermittent devices

The European Society of Hypertension’s 2023 recommendations describe six tests for intermittent cuffless devices:

  1. Static accuracy against a reference measurement.
  2. Effect of device position.
  3. Response to a treatment-induced decrease in blood pressure.
  4. Differences between awake and asleep states.
  5. Increase during exercise.
  6. Stability after recalibration.

This list is important because it moves beyond a single resting snapshot. A product that passes a static test but has not been assessed during movement, sleep or recalibration drift has not demonstrated performance in those situations.

IEEE 1708-2025: a framework for wearable modes

IEEE 1708-2025 defines an objective evaluation framework spanning intermittent, continuous, beat-to-beat and blood-pressure-variability devices. When comparing two claims, check that the testing mode matches the mode the product actually advertises. Evidence for occasional spot estimates should not be presented as proof of continuous beat-to-beat accuracy.

Can a smartwatch replace a blood-pressure cuff?

Not for diagnosing hypertension or changing medication on the strength of a cuffless reading alone. A smartwatch or other wearable may provide useful trends, alerts or research data, but the evidence described by the AHA does not yet establish that its readings improve clinical decisions.

For home checking, a validated automated upper-arm blood pressure monitor remains the practical comparator. Use it according to the manufacturer’s instructions and record the cuff result alongside any cuffless estimate. If the values disagree, do not assume the wearable is correct because it reports more frequently or looks more convenient; discuss persistent differences with a clinician.

Do not treat a cuffless alert as proof of an emergency, and do not delay urgent care because a wearable reports a reassuring number. Symptoms and clinical advice take priority over an unconfirmed consumer estimate.

How to evaluate a product’s validation claim

  1. Identify the exact model and software version. Validation for one device does not automatically apply to another model or a later algorithm.
  2. Read the intended-use statement. Note whether the claim is an intermittent estimate, a trend, a change from baseline or a continuous value.
  3. Look for the reference and protocol. Check whether ISO 81060-2 methods or another named protocol was used, and whether the study was peer reviewed.
  4. Check calibration and drift. Record whether the device needs a cuff reading, how often it must be recalibrated and whether stability was tested after recalibration.
  5. Check tested conditions. Look for seated rest, device position, exercise, sleep/wake status and motion—not just a single resting condition.
  6. Check who was studied. A study should describe its participants and blood-pressure range clearly enough to judge whether its results apply to you.
  7. Use a conventional comparator at home. Take readings with an upper-arm monitor when advised by a healthcare professional, and bring both sets of records to an appointment rather than self-adjusting treatment.

What remains unresolved?

Three limitations still prevent a broad “cuff replacement” conclusion. First, validation depends on the exact intended use and operating mode. Second, real-world factors—including posture, exercise, sleep, motion and recalibration drift—can change performance. Third, outcome evidence is missing: the field has not yet demonstrated that high-frequency cuffless data lead to better diagnosis, safer treatment changes or improved health outcomes.

The new guidance and standards are therefore a step toward comparable evidence, not a declaration that the technology has finished validation. Until model-specific, protocol-compliant evidence covers the conditions and population in which you plan to use a device, keep a conventional upper-arm monitor as your reference.
