To create a fillable PDF in Adobe Acrobat, open All tools > Prepare a form, choose a blank page, an existing document, or a scan, then review the fields Acrobat detects, add any missing fields, preview, and save. An online web form uses a separate Acrobat web workflow and requires Acrobat Pro or Acrobat Pro for teams.
Choose the right kind of form
A fillable PDF is a document with interactive fields that recipients can complete in a PDF viewer. Start in Acrobat’s Prepare a form tool if that is what you need. If people should complete the form as an online web experience, use Acrobat on the web’s separate web-form feature instead.
| Starting point or result | When to use it | What to expect |
|---|---|---|
| Existing digital document | You already have a document whose layout and labels should be retained. | Acrobat detects likely fields; inspect and correct them before saving. Adobe’s document-conversion instructions describe this workflow. |
| Paper form scan | You have a paper layout and want to make it fillable. | Select the scan in Prepare a form, then review the detected fields and add or adjust controls as needed. Adobe documents creating fields from a document or scan. |
| Blank PDF | You want to build the layout and fields from the beginning. | Add labels and other static text, then insert the interactive fields. See Adobe’s blank-form instructions. |
| Online web form | You want respondents to use a web-based form rather than fill out a PDF. | In Acrobat on the web, use E-sign > Create a web form. Adobe says access requires a valid Acrobat Pro or Acrobat Pro for teams subscription. See Adobe’s web-form guide. |
Create a fillable PDF from an existing document or scan
- Open Acrobat and select All tools > Prepare a form.
- Select the digital file or scan you want to use, then choose Create form.
- Review the fields Acrobat identifies. Check that each field is aligned with the right label and answer area; move, resize, rearrange, or delete fields that are misplaced or unnecessary.
- Add any fields Acrobat missed, then preview the form and test that its controls make sense to complete.
- Save the finished PDF. For more detail on the conversion flow, see Adobe’s instructions for converting documents to forms.
Automatic field recognition is a starting point, not a quality check. Inspect every detected field before sharing the form.

- Start with a blank PDF and open the form-creation workflow. Adobe’s current desktop instructions use Menu on Windows or File on macOS, then Prepare a form.
- Choose Start from blank page and create the form.
- Add static text such as headings, instructions, and field labels, then insert the interactive controls respondents need.
- Preview the form, check the layout and fields, and save it. See Adobe’s step-by-step guide to creating forms from scratch.
Choose fields that fit the answers
Acrobat supports text fields, drop-down lists, radio buttons, checkboxes, list boxes, and other form controls. Match the control to the response: use checkboxes when respondents can select multiple options, and radio buttons when they must choose one option from a set. Use a text field for a written answer and a drop-down when a respondent should select from a defined list.

Field properties can mark a field as required or read-only and can configure behaviors such as dates, calculations, and tooltips. Set only the properties that fit the purpose of the form. Adobe’s PDF forms overview covers field creation, distribution, and response collection.
Make fields more accessible
Give each field a meaningful description in its Tooltip box. Screen readers read tooltip text aloud, helping users understand what information belongs in a field. Also consider accessibility in the source document and prepare fields and navigation so the form can be used in a logical order. See Adobe’s accessibility guidance for PDFs.

Distribute the PDF or create a web form
Acrobat’s PDF-form workflow supports distributing a form and collecting responses through email or a server. Adobe also documents tracking and managing form response data in its PDF forms overview. A fillable PDF and a web form are different outputs: to make a web form, go to Acrobat on the web and select E-sign > Create a web form. That feature requires a valid Acrobat Pro or Acrobat Pro for teams subscription, according to Adobe’s web-form documentation.
