COX inhibitors are not yet in a proven clinical comeback. Selective COX-2 drugs remain clinically relevant and continue to be investigated, but the available evidence does not identify a newly approved medicine, a named returning candidate, or a recent milestone that would justify saying the class has definitively returned.
The reason for renewed interest is a familiar trade-off: targeting COX-2 can reduce some gastrointestinal injury associated with nonselective NSAIDs, yet cardiovascular and kidney risks remain dependent on the drug, dose, duration, patient and comparator. The best-supported modern comparison, the PRECISION arthritis trial, found similar rates of its primary cardiovascular outcome for celecoxib, naproxen and ibuprofen under the study’s specific conditions—not zero risk.
What “COX inhibitor” means
COX stands for cyclooxygenase, an enzyme system involved in making prostaglandins. Nonsteroidal anti-inflammatory drugs (NSAIDs) inhibit cyclooxygenase to reduce pain and inflammation and to lower fever. “COX inhibitor” is therefore a broad pharmacologic category, not the name of one medicine.
Two practical groupings matter:
| Group | Examples in the available evidence | Pharmacologic aim | Main trade-off |
|---|---|---|---|
| Nonselective NSAIDs | Ibuprofen, naproxen | Inhibit both COX-1 and COX-2 to varying degrees | Potential gastrointestinal injury alongside cardiovascular and renal concerns |
| More COX-2-selective inhibitors | Celecoxib | Emphasize COX-2 inhibition in an attempt to improve gastrointestinal tolerability | COX-2 selectivity does not remove cardiovascular or renal risk |
Selectivity is a relative property, not an on/off safety switch. A drug can favor COX-2 at one dose and become less selective at another, while the patient’s baseline risk and treatment duration also change the balance of benefits and harms.

Why researchers keep revisiting COX-2 selectivity
The central rationale is gastrointestinal tolerability. Some nonselective NSAID effects on COX-1 are associated with loss of protective prostaglandin activity in the gastrointestinal tract. Developing a medicine that inhibits COX-2 more than COX-1 was intended to preserve more of that protection while retaining anti-inflammatory action.
That strategy created a second safety question. COX-2-related prostaglandin pathways also participate in vascular and kidney physiology. A medicine that looks attractive from a stomach-outcome perspective can still produce clinically important blood-pressure, fluid-retention, cardiovascular or renal problems in susceptible people. The resulting question is not “Are COX-2 inhibitors safe?” but “For this patient, at this exposure, compared with which alternative, what outcome changes?”
A 2024 review, NSAIDs between past and present; a long journey towards an ideal COX-2 inhibitor lead in RSC Advances, describes continuing work on selective COX-2 compounds. It is a review of the field, not evidence that an unnamed candidate has returned to routine practice or obtained a new approval.

The most useful modern head-to-head evidence in the available material comes from the randomized PRECISION trial, reported in the New England Journal of Medicine in 2016. It enrolled patients with arthritis and compared celecoxib, naproxen and ibuprofen. In the intention-to-treat analysis, the trial’s primary cardiovascular composite occurred at the following proportions:

| Assigned treatment | Primary cardiovascular composite | What the figure represents |
|---|---|---|
| Celecoxib | 2.3% | Participants assigned celecoxib who experienced the prespecified composite during the trial |
| Naproxen | 2.5% | Participants assigned naproxen who experienced the same composite |
| Ibuprofen | 2.7% | Participants assigned ibuprofen who experienced the same composite |
Celecoxib met the trial’s noninferiority comparisons with naproxen and ibuprofen for that primary outcome. Noninferiority means the study ruled out a prespecified amount of excess risk under its design; it does not prove that celecoxib is superior, universally safer or risk-free.
How to read those percentages
- They are trial outcome proportions, not an individualized prediction for a particular reader.
- They apply to the trial’s arthritis population, treatment regimens, follow-up, endpoint definition and intention-to-treat analysis.
- They do not establish that the same relationship holds at every dose, for every treatment duration, in people with different cardiovascular or kidney risk, or for every other NSAID.
- The small numerical differences should not be treated as a ranking that ignores confidence intervals, adherence, discontinuation and the study’s noninferiority design.
The result is important because it challenges a simplistic assumption that a COX-2-selective option must necessarily have worse cardiovascular outcomes than the compared nonselective drugs. It does not erase the reason cardiovascular monitoring remains part of NSAID prescribing.
The safety trade-off: gastrointestinal, cardiovascular and renal outcomes
Gastrointestinal outcomes
COX-2 selectivity was pursued partly to reduce gastrointestinal injury associated with some nonselective NSAID exposure. That is a potential tolerability advantage, not a guarantee. Gastrointestinal risk still depends on dose, duration, previous ulcer or bleeding history, age, other medicines and the specific endpoint being measured.

Cardiovascular risk can include several different outcomes rather than one universal measure. A trial’s composite might combine events such as cardiovascular death, nonfatal myocardial infarction or nonfatal stroke, while another study may use a different definition. Comparing percentages without checking the endpoint can create a false impression of precision.

The PRECISION figures show that, in its selected population and regimen, celecoxib was noninferior to naproxen and ibuprofen for the primary cardiovascular composite. They do not show that a person with established cardiovascular disease faces no added risk, nor do they justify changing treatment without clinical advice.
Renal outcomes
Kidney effects are a separate axis from gastrointestinal and cardiovascular outcomes. COX inhibition can alter prostaglandin-dependent kidney blood flow and fluid handling, so renal risk may become more important with dehydration, pre-existing kidney impairment, heart failure, older age or interacting medicines. A gastrointestinal benefit cannot be counted as a complete safety assessment when renal outcomes were not the endpoint under discussion.
Why dose and comparison design change the answer
The 2022 review Lessons from 20 years with COX-2 inhibitors: Importance of dose-response considerations and fair play in comparative trials, by Carl-Olav Stiller and Paul Hjemdahl of Karolinska Institutet and Karolinska University Hospital, emphasizes two interpretive rules.

Dose-response matters
Risk is not a fixed label attached to a molecule. Higher exposure can change selectivity, prostaglandin suppression and adverse-event rates. A result obtained with a moderate regimen cannot automatically be applied to a higher dose, prolonged use or a different formulation. Any credible “comeback” claim should state the dose tested and whether the dose is proposed for routine use.

Fair comparisons matter
A fair comparison matches the clinical question. If one study compares a selective drug with a nonselective NSAID in arthritis, its findings do not automatically answer whether the selective drug is preferable to acetaminophen, topical therapy, a non-drug intervention or no treatment. The comparator, background therapy, follow-up and outcome definition all affect the conclusion.
How to test a claim that COX inhibitors are “back”
When a headline uses comeback language, use this checklist before accepting it:
- Name the candidate. A real clinical development should identify the molecule, sponsor or research program. A general review of selective COX-2 chemistry is not a new approval.
- Check the regulatory event. Look for a dated authorization, expanded indication or label change and note the country or region. The available evidence does not establish such an event for a newly returning candidate.
- Separate laboratory progress from patient evidence. Enzyme assays, animal studies and medicinal-chemistry leads can show selectivity without demonstrating clinical benefit or safety.
- Read the endpoint. “Fewer stomach events” and “no excess cardiovascular risk” are different claims. Identify whether the outcome was gastrointestinal, cardiovascular, renal, pain-related or a composite.
- Carry the dose and duration with the result. Do not quote a percentage without the regimen and follow-up that produced it.
- Match the population. Age, arthritis type, baseline cardiovascular disease, kidney function and concurrent medicines can change absolute risk.
What is and is not established about a present-day comeback
The current evidence supports a narrower conclusion than the headline suggests:
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minute- COX inhibitors remain an active clinical and research subject.
- Selective COX-2 development continues, according to the 2024 review.
- The PRECISION trial provides reassuring comparative cardiovascular evidence for celecoxib under its study conditions, with primary-composite rates of 2.3% for celecoxib, 2.5% for naproxen and 2.7% for ibuprofen.
- No specific new COX-2 candidate, recent approval or clearly documented return to clinical use is identified in the available material.
- Current regulatory status and geographic availability for any newly returning candidate therefore remain unresolved.
That distinction matters for readers because “research interest,” “late-stage development,” “regulatory approval” and “routine prescribing” are different milestones. Treating them as interchangeable turns a cautious scientific update into an unsupported comeback story.

Questions to take to a clinician
If you are offered an NSAID or are reading about a selective COX-2 option, ask which outcome the treatment is intended to improve and which risks are most relevant to you. Useful questions include:
- Is the proposed drug nonselective or relatively COX-2 selective?
- What dose and duration are planned, and what is the reassessment point?
- How do my cardiovascular, gastrointestinal and kidney risks affect the choice?
- Which medicines, supplements or illnesses could increase adverse effects?
- What symptoms or test results should prompt a call or urgent care?
- What alternatives exist if the expected benefit does not occur?
Do not use the PRECISION percentages as a personal risk calculator, and do not start, stop or switch an NSAID solely because a headline describes a comeback.
Bottom line
COX inhibitors have not demonstrated a single, identifiable comeback event. They have demonstrated something more nuanced: selective COX-2 inhibition remains a legitimate design strategy, and comparative evidence does not support assuming that celecoxib is automatically more dangerous cardiovascularly than naproxen or ibuprofen in every setting. The unresolved work is to define, for each drug and patient, whether any gastrointestinal advantage outweighs cardiovascular and renal risks at the dose and duration actually used.
