Insulin and insulin-mimetic substances are prohibited at all times under the 2026 World Anti-Doping Agency (WADA) Prohibited List, effective 1 January 2026. A suspected case is established through official sample collection, specialist laboratory analysis and the relevant anti-doping organization’s results process—not by a coach, rival, journalist or consumer glucose monitor.
What counts as a prohibited substance?
WADA lists insulins and insulin-mimetics under S4, the category for hormone and metabolic modulators. The 2026 List took effect on 1 January 2026, and WADA updates the list annually. The Athletics Integrity Unit explains that WADA produces the list of substances and methods banned in sport. The applicable rules and reporting process depend on the athlete’s sport, country and governing anti-doping body.
How anti-doping testing can detect insulin-related substances
Official sample collection
Testing begins with a sample collected through the anti-doping system. Depending on the method and substance being sought, published research describes analysis of urine, plasma or other blood-derived samples. USADA describes samples being collected and analyzed by WADA-accredited laboratories.

Published approaches use liquid chromatography coupled with high-resolution tandem mass spectrometry (LC-HRMS/MS). In broad terms, this specialist instrument-based analysis separates compounds in a sample and examines their measured characteristics. Anti-doping laboratories use analytical methods designed for the substances and sample types under investigation.

Rule-based interpretation and results management
A laboratory result is considered within the applicable anti-doping rules, then handled by the competent anti-doping organization through its results-management process. A published analytical method or a suspicious observation alone does not establish that a particular athlete committed a violation.
What published studies report—and what the numbers mean
The figures below describe particular studies and analytical methods, not a universal guarantee that every exposure, insulin form or sample will be detected.

An MRPL is a laboratory performance requirement as reported in the 2021 paper; it is not an individual athlete’s probability of being detected, nor does it mean every test uses the same method or has the same limit. The 10–25 pg/mL range applies only to the synthetic analogues tested in that urine-method study. The 0.5 ng/mL figure applies to the 2023 plasma assay for S597 and S519. These values should not be compared as if they came from identical sample matrices or tests.
Thomas and co-authors reported in 2023 that LC-HRMS/MS assays for insulin-related substances in blood-derived samples and urine had been published and were routinely employed by anti-doping laboratories. That broad statement does not establish that every laboratory uses every published assay or that all substances can be detected under all circumstances.

Why a glucose monitor or visual clues cannot identify doping
A consumer glucose monitor measures glucose; it does not identify insulin or insulin-mimetic substances in an anti-doping sample. No cited anti-doping method supports using a consumer device as a doping detector. Appearance, performance changes or other observations likewise cannot establish what substance an athlete used. Treating them as proof risks a false accusation and bypasses the testing and rules process.

- Find the anti-doping organization responsible for the athlete’s sport and jurisdiction. The relevant body and reporting route vary by sport and country.
- Use that organization’s official reporting channel and provide factual information you can substantiate. Do not present speculation, performance changes or a glucose reading as proof.
- Leave testing and case interpretation to authorized anti-doping officials and laboratories. Avoid public accusations based on inference.
Questions about therapeutic use or a therapeutic use exemption (TUE) also need to be handled under the applicable sport’s rules by the competent anti-doping organization; requirements and procedures are not uniform across every governing body.
