AI-generated code is an untrusted draft, even when it compiles and looks polished. The most dangerous defects are usually semantic: a valid API used with the wrong lifecycle, a boundary condition that returns the wrong result, an authorization check applied too late, or a dependency that never existed. Catch them with layered review: define the intended behavior, verify APIs and packages against authoritative documentation, build and execute the exact project, test negative and edge cases, trace untrusted data, run static and dependency analysis, and require qualified human approval for security-sensitive paths.

Why plausible AI code fails review

Large language models optimize for likely text, not for proving that a program satisfies your system’s contract. They can produce syntactically convincing code that assumes the wrong library version, invents a package, omits cleanup, mishandles a timeout, or silently changes behavior outside the happy path. Visual similarity to code written by an experienced developer is therefore weak evidence.

Quantified studies show why reviewers should start skeptical:

Evidence What was observed What it means for review
RobustAPI study, AAAI 2024 62% of GPT-4-generated code contained API misuses across 1,208 Stack Overflow questions involving 18 Java APIs. A real method name and a successful compile do not prove correct API usage, parameter choices, or lifecycle handling.
Monash University/ACM study, 2024 Among 4,066 ChatGPT programs, 2,756 were classified as correct, 1,082 produced wrong outputs, and 177 had compilation or runtime errors. Static analysis found maintainability issues in 1,930 snippets. Correctness, execution reliability, and maintainability need separate checks. The figures are study-specific classifications, not a universal defect rate.
CodeQL comparison, IEEE Security and Privacy Workshops 2024 ChatGPT snippets contained 248 vulnerabilities across 19 CWE types; Stack Overflow snippets contained 302 across 22 CWE types. Security risk varies with task and source. A lower count in one benchmark does not make generated code safe.
GitHub-project study by Fu et al. (2023, revised 2025) Copilot Chat fixed up to 55.5% of reported security issues when given static-analysis warnings. Model-assisted repair can be useful after detection, but partial fixes are not proof that the remaining code is secure.
Sandoval et al., USENIX Security 2023 In a low-level C user study, AI-assisted users produced critical security bugs at a rate no greater than 10% above the control group. This result is limited to the language, tasks, and study design; it cannot be generalized to every project.

These studies use different models, prompts, languages, datasets, and definitions. Treat them as evidence for a disciplined workflow, not as a production probability that applies to your repository.

The bug patterns reviewers miss most often

1. A real API used in the wrong way

AI often selects a method that exists but violates the API’s contract. Typical failures include passing units in the wrong order, confusing inclusive and exclusive limits, using a default that is unsafe for your case, calling an asynchronous method without awaiting it, or closing an object before a deferred operation completes. A generated database client may open a connection correctly yet fail to release it on exceptions. A cryptographic library call may compile while using a deprecated mode or a nonce incorrectly.

Review the exact library and version used by the project. Confirm the method signature, default values, error behavior, thread-safety guarantees, required initialization and cleanup, and deprecation notices. Then write a test that exercises both success and failure paths. The RobustAPI result—62% API misuse in its GPT-4 test set—makes this a first-class review task, not a style concern.

2. Code that compiles but returns the wrong answer

Compilation proves that a particular set of types and symbols fit together. It does not prove that comparisons, units, state transitions, or business rules are correct. AI-generated code commonly fails at boundaries: an empty collection causes an index error, a maximum value overflows, a pagination loop skips the final page, a date is interpreted in local time, or a filter reverses an inequality.

Turn the prose requirement into explicit examples before judging the implementation. Include empty, minimum, maximum, duplicate, malformed, unauthorized, timeout, and partial-failure inputs. For transformations, test known input/output pairs and properties such as idempotence or conservation. For stateful code, test every permitted transition and at least one forbidden transition. The Monash study’s 1,082 wrong-output programs demonstrate why a green build is not a correctness verdict.

3. Compilation and runtime failures hidden by incomplete review

Snippets copied from a chat may depend on imports, generated files, environment variables, or package versions that are absent from the real repository. A symbol can exist in one release but not another. A type can compile under one platform and fail under another. Runtime failures appear when a null value, malformed response, permission error, network interruption, or unexpected encoding reaches an untested branch.

Build the exact checkout in a clean environment, not a developer’s already-configured machine. Run unit, integration, and regression tests, and execute the error paths introduced by the patch. The Monash study recorded 177 programs with compilation or runtime errors, showing that apparent completeness often reflects an incomplete execution check.

4. Security flaws in ordinary glue code

Security defects are often hidden in code that looks like routine plumbing. Inspect:

  • Authentication and authorization: Is identity established before the operation, and is every object-level permission checked?
  • Input validation: Are type, length, range, format, and allowed values enforced server-side?
  • Output encoding: Is data encoded for its actual sink—HTML, JavaScript, SQL, a shell, a log, or a URL?
  • Queries and commands: Are values bound through safe parameters rather than concatenated strings?
  • Files and deserialization: Can an attacker select a path, type, archive entry, or object class?
  • Secrets and logs: Are credentials excluded from source, telemetry, exceptions, and crash dumps?
  • Error responses: Do failures reveal stack traces, tokens, internal paths, or authorization details?

Run a static application-security scan, such as CodeQL or an equivalent SAST system, and manually follow each finding from source to sink. A 2024 literature review covering 15 primary studies identified seven vulnerability kinds and six mitigation strategies or practices; the breadth of findings is a reminder that no single rule set covers every language or threat.

5. Hallucinated packages and unsafe dependencies

A plausible import name is not evidence that a package exists, is maintained, or is trustworthy. Generated code may select a similarly named library, an abandoned version, or a package with an unexpected transitive dependency. It may also add a dependency for functionality already available in the standard library.

Resolve every import in the project’s real registry and lockfile. Check the exact version, publisher or maintainer, release history, license, known vulnerabilities, and transitive dependency tree. Prefer the project’s existing dependency patterns. Pin versions through the normal lockfile process, review the resulting diff, and reject unexplained additions. A clean install in an isolated environment is a practical hallucination test: missing or non-reproducible packages fail before merge.

6. Maintainability defects that become future bugs

Generated code can duplicate logic, introduce unnecessary abstraction, swallow exceptions, use inconsistent naming, omit tests, or create a function too complex for a reviewer to reason about. These defects increase the chance that the next change will be wrong, even if today’s tests pass. Static analysis in the Monash study identified maintainability issues in 1,930 snippets.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review whether the patch has a clear owner, observable behavior, coherent error handling, and tests that explain the contract. Remove duplicate branches, narrow broad exception handlers, document non-obvious invariants, and simplify code whose only purpose is to imitate a pattern from the prompt. Treat readability and testability as defect prevention, not cosmetic cleanup.

A layered review sequence before merging AI-generated code

Step 1: Define behavior and threat boundaries

Write down what the change must do before reading the implementation. Specify inputs and outputs, allowed callers, data sensitivity, failure behavior, performance limits, and side effects. Identify trust boundaries: browser to server, service to service, file to parser, user input to interpreter, and model output to production configuration. Without this contract, a reviewer can only compare code with intuition.

Step 2: Verify every external assumption

  1. List every API, framework feature, package, protocol, environment variable, and configuration key introduced or changed.
  2. Open the official documentation for the exact version present in the repository.
  3. Confirm signatures, units, defaults, error behavior, lifecycle obligations, thread-safety, and deprecations.
  4. Compare the generated call with a documented example and with the project’s established usage.
  5. Record any assumption that remains uncertain and resolve it before approval.

Do not let an AI explanation substitute for documentation. Ask the model to state assumptions and failure modes if that helps expose gaps, then verify each statement independently.

Step 3: Build and execute the exact repository

  1. Start from a clean checkout or isolated build environment.
  2. Install only the locked dependencies and use the project’s declared runtime and compiler versions.
  3. Compile all affected targets, including optional or platform-specific targets when the patch touches them.
  4. Run unit, integration, regression, type, format, and contract tests required by the repository.
  5. Capture failures with the input and environment that triggered them; do not dismiss an error because the happy path works.

Step 4: Add adversarial and boundary tests

Generated code is especially likely to omit cases that were not prominent in the prompt. Add tests for:

  • Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
  • Empty, null, malformed, oversized, duplicate, and maximum-value inputs.
  • Expired credentials, missing permissions, replayed requests, and cross-tenant identifiers.
  • Timeouts, retries, partial responses, unavailable services, and interrupted writes.
  • Timezone changes, Unicode and encoding differences, daylight-saving transitions, and locale-specific formatting.
  • Concurrent calls, cancellation, process restarts, and repeated execution.

Use property-based or fuzz testing where input space is large. A test that merely repeats the example from the prompt provides little independent evidence.

Step 5: Trace untrusted data end to end

Mark data from users, network responses, files, environment variables, and model-generated strings. Follow each value into interpreters, queries, commands, templates, redirects, deserializers, logs, and permissions. At every sink, identify the required validation or encoding. Check that authorization is performed on the canonical resource identifier, not on a user-controlled alias, and that validation is not discarded during a later conversion.

Step 6: Run static, dependency, and secret analysis

  • Run CodeQL or another SAST tool against the language and framework versions in use.
  • Run dependency vulnerability and license checks against the lockfile and transitive tree.
  • Scan for hard-coded keys, tokens, certificates, passwords, and accidentally committed test credentials.
  • Run type checking, formatting, and compiler warnings at the repository’s normal strictness level.

Investigate findings rather than counting them. Tools have coverage gaps and false positives; a clean report is limited evidence, not a security certificate. Conversely, suppressions should include a reason, scope, and owner so that a generated flaw cannot disappear under a blanket ignore rule.

Step 7: Review the diff for intent

Read the patch line by line against the behavior contract. Look for silent changes to defaults, authorization order, exception handling, retries, timeouts, resource ownership, logging, and concurrency. Check whether generated code copied a pattern without the surrounding invariant—for example, a cache write without invalidation or a lock without a matching release on every path. Confirm that deleted tests or broadened permissions are intentional.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 8: Require human sign-off where the impact is high

A qualified reviewer who understands the system and threat model should approve changes involving authentication, authorization, cryptography, payments, personal data, deserialization, infrastructure permissions, or deployment configuration. The reviewer should be able to explain what happens on malformed input, partial failure, replay, timeout, and denial of access. If that explanation is not possible, the patch is not ready, regardless of how confidently the model describes it.

How to evaluate AI-review and scanning workflows

Teams often compare an AI reviewer, a conventional scanner, and manual review as if one must replace the others. A more useful comparison evaluates the complete workflow on these axes:

Axis Questions to ask
Language and framework coverage Does the tool understand the versions, build system, generated code, and deployment configuration in this repository?
API and dependency awareness Can it detect wrong signatures, deprecated behavior, missing cleanup, hallucinated packages, and risky transitive dependencies?
Data-flow and security detection Can it follow tainted data across files and services to queries, commands, templates, redirects, logs, and authorization checks?
Test generation and execution Does it produce independent edge-case tests, and are those tests actually run in the project’s environment?
False-positive and false-negative handling Can reviewers see evidence, reproduce a finding, and record why a warning was dismissed?
Pull-request and CI integration Does it run consistently before merge, enforce required checks, and preserve results for audit?
Explainability Does the output identify the violated contract and data flow, rather than merely assigning a risk score?
Human validation Which findings require a reviewer with system and threat-model knowledge, and is that gate enforced?

The Copilot Chat study’s result—up to 55.5% of security issues fixed when warnings were supplied—supports using models as repair assistants after detection. It does not establish that a model will find every issue, that a proposed fix preserves behavior, or that the unfixed remainder is harmless. Re-run the same checks after every AI-generated repair.

Practical checks for common failure symptoms

The code uses an API that “looks right” but behaves strangely

  • Pin the dependency version and open its official reference for that version.
  • Write a minimal test that asserts the documented behavior, including cleanup and failure.
  • Compare units, encoding, indexing, nullability, and asynchronous control flow.
  • Search the repository for an established, reviewed usage pattern.

The build passes, but production output is wrong

  • Reproduce with boundary and malformed inputs rather than another happy-path example.
  • Check timezone, locale, precision, rounding, pagination, ordering, and duplicate handling.
  • Compare the implementation with an executable specification or known-good fixture.
  • Instrument intermediate state at trust boundaries and verify assumptions about external responses.

A scanner reports a vulnerability

  • Trace the reported source-to-sink path and identify whether input is actually attacker-controlled.
  • Confirm the sink’s required defense: parameterization, encoding, allowlisting, isolation, or authorization.
  • Fix the root cause, add a regression test, and rerun the scanner.
  • Document narrowly scoped false positives; never silence an entire rule family to remove noise.

A generated dependency cannot be installed or seems suspicious

  • Check the package in the language’s official registry and compare its name and publisher.
  • Remove it if the package is absent, unmaintained, or unnecessary.
  • Use an existing, vetted dependency or standard-library feature where possible.
  • Regenerate the lockfile in an isolated environment and review all transitive additions.

What “trusted enough to merge” should mean

Trust is an evidence threshold, not a property of the model or the code’s appearance. For ordinary changes, that threshold should include a clear behavior contract, verified APIs and dependencies, a clean build, passing unit and integration tests, adversarial cases, static and dependency analysis, and a diff review that explains each side effect. For security-sensitive changes, add a reviewer who can reason about the threat model and require explicit sign-off.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The strongest general conclusion from the available studies is that executable code is not equivalent to reliable and robust code. Another comparison study summarized the engineering obligation plainly: code copied and pasted, whether created by AI or humans, cannot be trusted blindly. Layered checks make that principle operational: detect, reproduce, fix, retest, and document before merge.
