Most persistent low back pain is not caused by a dangerous disease, and routine scans are usually not helpful when there are no warning signs. The decision changes if you have new bladder, bowel or sexual-function problems, numbness around the genitals or anus, rapidly worsening weakness, cancer or infection risk, major trauma, fracture risk, or other findings that could change treatment.
This guide explains which symptoms need emergency assessment, when to arrange a routine or urgent appointment, what MRI, CT and X-rays can and cannot show, and why a scan should be ordered only when its result is likely to change management.
Is chronic back pain serious?
“Chronic” describes pain that continues or returns over time; it does not, by itself, identify the cause. Many people have nonspecific low back pain that improves gradually or can be managed without a procedure. National Institute for Health and Care Excellence (NICE) guidance says not to routinely offer imaging in a non-specialist setting for low back pain with or without sciatica when there is no indication that a result will alter care.
Serious causes are uncommon, but missing one can have major consequences. Clinicians look for evidence of cancer, infection, fracture, inflammatory disease, cauda equina syndrome, severe or progressive neurological deficit, or another condition requiring specific treatment. A symptom called a “red flag” is a reason for assessment, not proof that you have a particular diagnosis.

The population burden is large: an American College of Radiology (ACR) 2021 update describes acute low back pain, with or without radiculopathy, as the leading cause of years lived with disability and the third-ranking cause of disability-adjusted life-years in the United States. That statistic describes societal impact, not your personal probability of a dangerous condition.
When back pain is an emergency
Seek immediate medical assessment through your local emergency pathway if back pain is accompanied by symptoms suggesting compression of the nerves at the bottom of the spinal canal or rapidly worsening neurological function.
Possible cauda equina syndrome
NICE recommends immediate referral for severe low back pain radiating into a leg when there is new-onset disturbance of bladder, bowel or sexual function, or new numbness in the perineum (the area around the genitals and anus). These symptoms require assessment for cauda equina syndrome.

- New difficulty starting urination, inability to urinate, or loss of bladder control.
- New loss of bowel control or inability to sense the need to open your bowels.
- New sexual-function disturbance associated with the back and leg symptoms.
- New numbness or altered sensation around the genitals, anus, buttocks or inner thighs (“saddle” or perineal numbness).
Do not wait for a routine appointment, physiotherapy session or an outpatient scan if these symptoms are new. Tell the emergency service exactly when they began and whether they are getting worse.
Rapidly progressive weakness
Weakness that is quickly worsening, such as a foot that repeatedly slaps the ground, a knee that gives way, or an inability to lift the foot, needs immediate neurological assessment. Severe or progressive neurological deficits are one of the situations in which the American College of Physicians/American Pain Society guideline recommends diagnostic imaging and testing.

Warning signs that need urgent clinical review
Arrange an urgent medical assessment when your history or examination raises concern for a serious disease, even if you do not have the emergency neurological symptoms above.
Cancer or possible spinal spread
A current or previous cancer diagnosis changes the threshold for investigation, particularly when pain is new, persistent, progressively worse or associated with neurological symptoms. NICE guidance on spinal metastases and metastatic spinal cord compression (MSCC) lists night-time back pain that disturbs sleep among concerning features. If MSCC is suspected, NICE recommends an MRI as soon as possible and always within 24 hours.
Infection or systemic illness
Back pain with fever, chills, unexplained weight loss, marked fatigue or feeling acutely unwell can indicate infection or another systemic disease. Risk is also higher with immunosuppression or other conditions that reduce the body’s ability to fight infection. These findings warrant prompt clinician review rather than self-treatment alone.
Trauma or fracture risk
Major trauma can injure the spine even when the skin looks normal. A lower-impact event can also cause a fracture when bone is fragile or fracture risk is otherwise high. The ACR identifies CT as the preferred modality in fracture-focused scenarios, but the history and examination determine whether that pathway applies. Tell the clinician about the mechanism of injury, osteoporosis or other bone-weakening conditions, and medicines that affect bone strength.
Inflammatory or other specific disease
Persistent morning stiffness, pain that is unusual for your previous episodes, or symptoms affecting several body systems may prompt assessment for inflammatory disease or another specific cause. Do not attempt to confirm or exclude these conditions from a symptom list alone; the clinician combines your history, examination and, when needed, targeted tests.

When to book a routine appointment
Arrange a clinical review when pain persists, limits work, sleep or daily activities, repeatedly returns, or fails to improve with sensible conservative care. Book sooner if pain travels into a leg or is accompanied by numbness, tingling, weakness or systemic symptoms.

- Ask when the pain started, how it behaves, what makes it better or worse, and whether it follows an injury.
- Review leg symptoms, walking ability, bladder and bowel function, sexual function, fever, weight change, cancer history, infection risks and medicines.
- Examine movement, strength, sensation and reflexes, and look for signs that point to a non-spinal cause.
- Decide whether self-management, physical therapy, blood tests, imaging, specialist referral or urgent escalation is appropriate.
The ACP/APS guideline recommends imaging and testing when severe or progressive neurological deficits are present or when a serious underlying condition is suspected from the history and physical examination. Otherwise, the initial plan commonly focuses on function and symptom control while monitoring for change.
Do you need an MRI, CT or X-ray?
The key question is not “Which scan shows the most detail?” but “What management-changing question are we trying to answer?” NICE advises against routine imaging in a non-specialist setting for low back pain with or without sciatica. Specialist imaging should be considered only when the result is likely to change treatment.

The table describes decision pathways, not a personal prescription. Local services may use different referral routes, and the clinician may choose tests other than imaging when the examination suggests a non-spinal problem.

What each type of scan can show
MRI
MRI produces detailed images of discs, nerves, the spinal cord and other soft tissues without ionising radiation. It is the preferred test in the ACP/APS pathway when persistent radiculopathy or spinal stenosis could lead to surgery or an epidural injection. MRI is also the urgent test recommended by NICE when MSCC is suspected.
CT
CT is particularly useful for evaluating bone and is the preferred modality in the ACR’s fracture-focused scenarios. ACP/APS lists CT as an alternative to MRI for selected persistent nerve symptoms. CT uses ionising radiation, so the expected benefit is weighed against exposure and the clinical question.
Plain X-ray
An X-ray may be considered in selected situations, but the guidance summarized here does not support routine radiographs for uncomplicated low back pain. An X-ray cannot answer every question about discs, nerves or infection. Whether it is useful depends on the suspected cause, injury history and examination findings.

Spinal imaging often shows age-related findings in people who have little or no pain. Disc bulges, degeneration or narrowing can be incidental, while a painful problem may not appear as a single obvious abnormality. A radiology report therefore cannot be interpreted in isolation.

A finding that matches a nerve pattern and examination may be important; the same finding in someone without corresponding symptoms may not require treatment. This is why ordering a scan “just to see what is there” can lead to anxiety, unnecessary follow-up or procedures without improving care.

What to do while waiting for an appointment
When no emergency symptom is present, NICE recommends tailored self-management information and encouragement to continue normal activities as tolerated. Avoiding every movement can increase disability; pace tasks, change position regularly and stop activities that cause a clear deterioration.
The AAFP’s 2024 review describes exercise, physical therapy, behavioral approaches and other nonpharmacologic options as first-line care when there is no condition requiring specific medical or surgical treatment. A clinician or physical therapist can adapt an exercise plan to your strength, mobility, work demands and other health conditions.
- Record the pain’s location, severity, timing, triggers and effect on sleep or walking.
- Note any leg pain, numbness, tingling or weakness and whether it is progressing.
- Write down fever, weight change, recent infection, injury, cancer history, immune-suppressing medicines and fracture risks.
- List medicines and treatments you have tried, including the dose, duration and benefit or side effects.
Braces, electrical stimulation devices, heat products, posture gadgets and similar items should not substitute for examination when red flags are present or for indicated imaging when a clinician believes it will change management.

- Check for emergency neurological symptoms. New bladder, bowel or sexual-function disturbance, perineal numbness or rapidly progressive weakness means immediate assessment.
- Check for serious-disease clues. Cancer history, suspected infection, major trauma, fracture risk, immunosuppression, severe or progressive neurological deficit, systemic illness or sleep-disturbing night pain warrants urgent clinical review.
- If neither applies, assess impact and duration. Persistent, recurrent or function-limiting pain deserves a planned appointment, especially when it is not improving.
- Let the examination define the question. Imaging is most useful when the result could change treatment, such as planning surgery, an epidural injection, fracture care or urgent treatment for cord compression.
- Reassess if the pattern changes. New weakness, numbness, fever, bladder or bowel symptoms, or rapidly increasing pain changes the urgency even if an earlier assessment was reassuring.
Questions to ask your clinician
- What diagnosis or complication are you considering?
- What findings on my history or examination make imaging useful now?
- Would the result change medication, physical therapy, referral, injection or surgery?
- Why is MRI, CT or an X-ray the best test for that question?
- What symptoms should make me seek emergency care, and how quickly should I follow up if I do not improve?
These questions keep the focus on safety and on whether a test will change the next decision, rather than on obtaining a scan for reassurance alone.
