An AI coding assistant can give an outdated answer even when the model itself is capable: it may be looking at an older repository snapshot, failing to retrieve the changed files, losing relevant details in a crowded session, or following project instructions that no longer match the code. Diagnose those layers in order instead of assuming there is one universal refresh button.
Why an AI coding assistant can miss recent code
“Context” is not just the codebase. Depending on the product, it can include the working tree or a remote repository, files found by search or indexing, earlier messages and tool results, and persistent project instructions. Any of those inputs can be absent, stale, noisy, or too crowded to be useful.
That is different from proving that a model was trained on old code. The documented mechanisms here concern what a tool can see and retrieve for a particular task; they do not establish a single cause or a measured ranking of causes across coding assistants.

Use this diagnostic sequence

Confirm which copy of the code the assistant can see
Check whether the assistant is operating on your local working tree, a remote repository, or a committed snapshot. If it relies on a remote source, unpushed local edits may not be available to it. Product implementations differ, so verify the source shown in the tool rather than assuming it is reading the editor’s current buffer.

Check whether indexing or retrieval is current
For GitHub Copilot repository context, GitHub says answers are optimized when the repository’s semantic code search index is up to date. Its documentation says initial indexing of a large repository can take up to 60 seconds; after first indexing, re-indexing is typically much quicker and the index is usually updated within seconds of starting a new conversation. Those are GitHub-documented expectations, not guarantees for other products or every repository. See GitHub’s repository-indexing documentation.
In VS Code, inspect the Copilot Status Bar for indexing status. If needed, use the Command Palette command Build Codebase semantic index to build or rebuild the index; the exact availability and behavior depend on repository type, account, and organizational policy. VS Code documents automatic index maintenance as well. See VS Code’s workspace context documentation.

Find out what the assistant actually retrieved
Do not treat a successful search as proof that the right implementation was inspected. VS Code’s Copilot agents choose search tools and may search again; text-search matches can enter conversation context even if the agent does not open each matching file. Generated files, build output, logs, and large data files can add irrelevant matches and crowd out useful evidence.

In VS Code, exclusions and indexability can be affected by workspace settings and policy; consult the workspace context reference for the controls available in your setup.

Check whether the session is overloaded
Conversation history, instructions, tool calls, and tool results can all consume context. GitHub Copilot CLI specifically documents that prompts, responses, tool outputs, and system instructions occupy a model-dependent context window; its
/contextcommand shows current usage. If a long session is crowded, start a fresh session with a concise summary and the relevant file references. These details apply to Copilot CLI, not necessarily to other assistants. See GitHub’s Copilot CLI context guide. -
Audit saved project instructions
Persistent guidance can describe conventions that have since changed. Anthropic’s Claude Code guide, dated April 15, 2026, says CLAUDE.md files at or above the working directory are read at session start and are not reread from disk every turn; files in subdirectories are loaded on demand when Claude reads there. These loading rules are specific to Claude Code.

Review guidance when conventions change or recurring errors reveal a missing rule. Keep it focused, review generated drafts, and remove obsolete instructions; Anthropic’s guide says outdated instructions can be worse than none. Do not assume another tool loads instruction files on the same schedule. Read Anthropic’s CLAUDE.md and prompting guide.

Retest against the implementation
Ask for a specific outcome, identify the subsystem and current behavior, and name the relevant files or symbols. Ask the assistant to inspect the current implementation and cite the code path or lines supporting its answer. If the response still describes the old behavior, check whether it opened the expected files and whether those files are actually in the source tree it can access. A precise prompt helps test retrieval; by itself, it does not prove or repair stale context.

What the evidence can—and cannot—tell you
A 2026 preprint, “Context Rot in AI-Assisted Software Development,” reports that applying an existing README/wiki consistency checker to a statistically representative sample of 356 repositories identified stale code-element references in 23.0% of the repositories. This is evidence about repository context artifacts, not a rate of incorrect AI answers, a causal estimate, or proof that a stale reference caused any particular answer. Read the preprint.
A separate exploratory study, “Configuring Agentic AI Coding Tools: An Exploratory Study,” describes configuration mechanisms across coding tools and repositories. Its abstract reports that context files dominate and that AGENTS.md is emerging as an interoperable format. The study does not establish that a particular file format makes answers more accurate. Read the study record.
Vendor documentation describes intended feature behavior; it is not an independent benchmark of freshness or correctness. Indexing policies also matter: GitHub says some repository-context indexing features upload data to GitHub, are limited to GitHub.com, and are policy-controlled and disabled by default for Business and Enterprise organizations unless an owner enables them. Check your organization’s settings before enabling a feature that indexes code. Details are in GitHub’s indexing documentation.
