Neither ChatGPT nor Claude is a therapist, diagnostician, or treatment provider. Both can offer general information, help you organize thoughts, and suggest human support, but neither should be used to diagnose depression or anxiety, choose treatment, or manage an immediate safety crisis. The providers’ published safeguards do not establish that one service is safer or more effective than the other for an individual.
Use a chatbot only for low-stakes reflection or preparation for a conversation with a qualified professional. If you may hurt yourself or someone else, cannot stay safe, or need urgent medical attention, stop the chat and contact local emergency services or a crisis helpline, and involve a trusted person.
What ChatGPT and Claude can—and cannot—do for mental-health questions
A chatbot can generate a calm, conversational response in seconds. That can make it useful for turning a confusing experience into a list of questions, explaining general mental-health terminology, or rehearsing how to ask a doctor for help. It cannot examine you, verify your history, observe changes in your behavior, or assume responsibility for your safety. A fluent answer is not evidence that the model understands your condition.

What ChatGPT’s mental-health features mean in practice
ChatGPT Health is support, not clinical care
OpenAI describes ChatGPT Health as a place where eligible users can connect health information with permission and receive support. At the time described by OpenAI, access was for eligible logged-in adults in the United States, with a gradual rollout and supported web and iOS access. Eligibility, integrations, and platform support can change, so check the current product notice before assuming that a feature is available to you.
Connecting information can make a conversation more relevant, but it does not turn the model into a clinician. The stated purpose remains support rather than diagnosis or treatment. Do not infer that a response is medically reviewed merely because it appears in a health-focused interface.
Trusted Contact has narrow, stated limits
OpenAI describes Trusted Contact as an optional feature. If automated systems and trained reviewers identify a serious concern, the feature may trigger a limited notification to the person you selected. OpenAI says that notification does not share chat details or a transcript. It also cautions that the system may not interpret every conversation perfectly.
A Trusted Contact is therefore an additional possible connection to a human, not a crisis-response service, emergency dispatch, or substitute for calling for help. If you are in immediate danger, do not wait for an automated review or notification. Contact local emergency services or a crisis helpline directly and tell someone who can be physically present.

How to interpret OpenAI’s safety figures
In an October 27, 2025 update, OpenAI said it had worked with more than 170 mental-health experts on sensitive-conversation improvements. It estimated a 65% reduction in responses that fell short of desired behavior under its own taxonomies. That is an OpenAI-reported evaluation result. It is not an independent comparison with Claude, and it does not mean that 65% of users receive a particular level of protection or that harmful responses are impossible.
What Claude’s policies and usage figures mean in practice
Claude’s stated boundary
Anthropic’s policy says Claude should respond to signs of distress with care, be honest about its limitations as an AI, and point people toward human support. Its December 2025 protocol for expressions of suicidal ideation, suicide, and self-harm risk repeats that Claude is not intended to provide diagnoses, therapy, or treatment recommendations.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesThe protocol describes automated detection in the consumer-facing application and a layered approach involving training and response measures. Anthropic also describes classifiers and real-time support resources. Those descriptions explain an intended safety process; they are not a promise that every distress signal will be recognized, interpreted correctly, or answered perfectly.
Why the reported 2.9% and under-0.5% figures do not show clinical value
Anthropic says 2.9% of Claude.ai interactions were classified as affective conversations. It reports that companionship and roleplay together accounted for less than 0.5% of conversations. These figures describe categories of use as Anthropic defined them. They do not tell you whether conversations improved symptoms, prevented harm, replaced professional care, or produced accurate guidance. Anthropic notes that platforms built specifically for therapeutic or medical use may show different patterns.
What to do when the situation is urgent
If you may not be able to stay safe
- Stop using the chatbot as your source of help for the moment.
- Contact your local emergency service or a crisis helpline. State plainly that you may be in immediate danger or cannot keep yourself safe.
- Tell a trusted person what is happening and ask them to stay with you or help you reach urgent care.
- Follow the instructions of emergency or crisis professionals. If medical attention is recommended, go to the nearest appropriate service.
Do not wait for a chatbot to escalate a conversation, for a Trusted Contact notification, or for a model to decide that your situation is serious enough. Automated systems can miss context, and a conversational reply cannot provide physical supervision.

If the concern is serious but not immediately dangerous
Use the chatbot only to prepare for human care: write down when symptoms began, what makes them better or worse, sleep and substance changes, current medicines, and questions for a clinician. Then arrange an appointment with a qualified mental-health professional or primary-care provider. If symptoms worsen or safety changes, use urgent human support rather than continuing to troubleshoot with the model.
A safer workflow for low-stakes use
1. Define a limited purpose
Choose a task that does not require a diagnosis or treatment decision. Examples include turning free-form notes into questions for an appointment, explaining a term in plain language, or suggesting a neutral journaling structure.
2. Minimize identifying information
Remove names, addresses, employer details, account numbers, and information about other people that is not necessary for the task. Use approximate ages or dates when precision is not needed. Avoid pasting an entire medical record merely because the interface accepts it.
3. State the boundary in your prompt
A prompt such as Explain this term in general language, list questions I could ask a licensed clinician, and identify what you cannot determine from this information. encourages a limited answer. It does not make the response medically authoritative, but it makes the purpose explicit.
4. Ask for uncertainty and alternatives
Request several possible explanations at a general level, what information would distinguish them, and which questions require professional assessment. Be cautious when the model gives one confident explanation without discussing uncertainty.

5. Verify anything consequential
Check medication, diagnosis, legal, workplace, insurance, and urgent-care implications with the appropriate professional. A chatbot can help you formulate the question; it should not be the final decision-maker.
6. Set a stop condition
End the chat if you feel more distressed, the model becomes repetitive or judgmental, it encourages secrecy or dependence, or the issue moves from reflection to immediate safety. Switch to a human contact or professional service.
7. Bring a concise summary to a professional
Save your own observations rather than treating the chatbot transcript as a clinical record. Tell the professional which parts are your experience and which parts were generated by the model.

Confident but incorrect explanations
Language models can produce plausible descriptions that do not fit your history. Reduce the risk by asking for general education, requesting uncertainty, and verifying with a clinician. Never use a confident label as proof of a disorder.

Missed or misunderstood crisis signals
People describe danger indirectly, use slang, change topics, or write in a language or context a system handles poorly. Provider protocols and classifiers are safeguards, not guarantees. Use direct human crisis support when safety is uncertain.
False reassurance
A sympathetic sentence can feel like confirmation that everything is manageable. Treat empathy as conversational style, not a clinical assessment. If functioning, sleep, relationships, or safety are deteriorating, seek a professional evaluation even when the chatbot sounds reassuring.
Privacy assumptions
One documented privacy detail does not describe every way a service handles data. OpenAI’s statement that a Trusted Contact notification excludes chat details and transcripts applies to that notification flow. It should not be read as a blanket promise about all conversations, connected health information, or referrals.

Frequent conversations can feel easier than contacting a person, especially when the model is always available. Keep human relationships and professional care in the loop. A system that encourages secrecy, exclusivity, or replacing all human support is a reason to stop using it for that purpose.
How to choose between ChatGPT and Claude without declaring a winner
There is no recent independent, head-to-head evaluation in the cited material that tests current ChatGPT and Claude versions for mental-health safety or usefulness. Choose based on the narrow task and the human support you have—not on a company metric that was not designed as a cross-service benchmark.
| If your priority is… | What you can reasonably consider | What you should not infer |
|---|---|---|
| A health-oriented ChatGPT interface | Check whether ChatGPT Health is available to your eligible U.S. account and whether its connected-information controls fit your needs. | Availability does not imply diagnosis, treatment, or clinician review. |
| Understanding Claude’s stated distress policy | Review Anthropic’s current wellbeing policy and protocol, then use Claude only for bounded, nonclinical tasks. | A published protocol does not guarantee correct detection in your conversation. |
| A human escalation option in ChatGPT | Consider the optional Trusted Contact feature if it is offered to you and you understand its limited notification behavior. | It is not emergency monitoring or a replacement for calling for help. |
| Evidence that one service is safer | Look for a genuinely independent comparison of the current versions, with transparent methods and comparable tests. | OpenAI’s 65% figure and Anthropic’s usage percentages cannot be ranked against each other. |
Questions to ask before you type
- Is this a request for general information, or am I asking the model to make a clinical decision?
- Could an incorrect answer cause harm if I acted on it?
- Have I removed information that identifies me or someone else?
- Who will I contact if the conversation increases my distress?
- Would a doctor, therapist, pharmacist, or crisis professional be the right person for this question?
If the answer to the second question is yes, use the chatbot to prepare questions, not to decide what to do. If the answer to the fourth question is “no” and you feel unsafe, contact a crisis service or emergency service before continuing.
What the current evidence can and cannot establish
OpenAI and Anthropic have published boundaries, safety processes, and selected measurements. Those documents support a cautious conclusion: both services may be useful for limited reflection and directing people toward human support, while neither is clinical care. The materials do not establish comparative effectiveness, diagnostic accuracy, treatment benefit, or a universal safety ranking. Model versions, policies, eligibility, and features can change, so reassess the current product information when a decision depends on a specific capability.
