A colorectal cancer pathology report can place histology, grade, stage, and a CDX2 result close together, but they are not interchangeable. Histology identifies the tumor type recorded by pathology; grade describes how differentiated the tumor looks; stage records how far the cancer has spread under staging criteria; and CDX2 is a separate marker result whose meaning must be interpreted in the context of the full report and clinical situation.
What each term tells you
Start by separating the questions the report is answering. The College of American Pathologists (CAP) colorectal protocol treats histology, grade, margins, lymph node status, and staging as distinct structured reporting elements. Their appearance together does not mean that one field determines all the others.
| Report item | Question it answers | What it describes |
|---|---|---|
| Histology | What type of tumor did pathology identify? | The tumor type recorded by the pathologist. |
| Grade | How differentiated does the tumor look? | A differentiation category, based on the tumor’s appearance under the grading system being used. |
| Stage | How far has the cancer spread according to staging criteria? | An assessment documented using staging details from pathology, imaging, and other sources as applicable. |
| CDX2 | What does this marker result contribute in this particular case? | A separate marker result; the sources cited here do not establish what a particular CDX2 staining result means diagnostically or prognostically. |
These fields answer different questions. A grade is not a stage, and neither one is a substitute for the histology. A marker result is another piece of information, not a stage or grade label.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsOne practical way to keep them straight is to ask what kind of information each field supplies: histology is a tumor description, grade is a differentiation category, stage is a spread assessment, and CDX2 is a marker result. Other report entries have their own roles, too. Margins and lymph node status may be important findings, but they are not alternate names for histology, grade, stage, or CDX2.

What does histology mean on a pathology report?
Histology is the tumor type recorded by pathology. CAP’s colorectal reporting protocol includes histology as a structured element alongside site, procedure, tumor size, grade, margins, lymph node status, staging, and other site-specific items. It is therefore normal for a report to contain several kinds of descriptions rather than one all-purpose diagnosis field.
When reading the histology entry, use the exact wording in the report rather than trying to infer a tumor type from another field. Grade describes differentiation, while histology names the type recorded by the pathologist. The available sources establish that these are separate report concepts; they do not provide a glossary for every histology term a report might use. If the wording is unfamiliar, ask the treating clinician or pathologist to explain what the specific term means in the case.
Histology is also not a substitute for the site or procedure information. A structured report can include the tumor site and the procedure used to obtain the specimen as separate items. Those details help identify the context of the report, while the histology entry answers what tumor type pathology recorded. Keeping the entries distinct can make a dense report easier to discuss without trying to derive one answer from another.

Grade describes differentiation category. SEER Training lists the colorectal registry categories as G1, well differentiated; G2, moderately differentiated; G3, poorly differentiated; G4, undifferentiated or anaplastic; and unknown. These are registry training categories, not a personalized prognosis or a stand-alone prediction of what will happen.
The categories are labels within a grading system. They do not tell you by themselves how far cancer has spread, what the histology wording means, or what a CDX2 result contributes. If a report includes a grade, read the label alongside the specimen context and the rest of the report rather than treating it as a complete summary of the cancer.
Why the specimen context matters
SEER distinguishes clinical grade, based on biopsy information, from pathological grade, based on surgical resection information. A biopsy samples tissue, while a resection is a different specimen context; the report should be read with attention to which specimen supplied the grade. If a report contains more than one grade entry, or a grade is marked unknown, do not assume the entries conflict or that a missing category can be filled in from stage. Ask which specimen and assessment each entry refers to.

This distinction is useful when comparing entries from different points in care. The words “clinical grade” and “pathological grade” refer to the information source described in SEER Training: biopsy information for clinical grade and surgical resection information for pathological grade. They should not be read as two stage groups or as a scale in which one automatically replaces the other. If both appear, the clinician or pathologist can clarify which specimen each entry describes and how the entries are being used.

What the grade categories do—and do not—say
The category identifies the differentiation label recorded under the grading system. It does not tell you the cancer’s anatomic extent, which is the separate purpose of staging. The category list alone also does not establish an individual outlook or treatment plan. Those conclusions require interpretation of the complete clinical and pathology information by the care team.
“Unknown” is included as a registry category in the SEER Training list. It is a recorded category, not an invitation to infer a value from a different report field. If the report says grade is unknown, ask whether that reflects the specimen or information available for that assessment and whether any further explanation is needed for the clinical discussion.
What does pathological stage mean?
Stage is an assessment of how far cancer has spread under staging criteria. “Pathological” indicates that pathology findings contribute to the documented assessment; it does not mean the stage can always be read from one line or one tissue finding. SEER’s colorectal staging guidance says staging details should be documented from pathology, imaging, or other sources, and points to current site-specific extent-of-disease resources.
That distinction matters when a report shows several staging-related fields. A pathology report can supply important findings, but the full stage may draw on other available information as well. Read the stage in the context of the clinician’s overall staging documentation, rather than treating histology, grade, a margin entry, or CDX2 as a substitute for it.

The material available for this article does not set out a current colorectal stage-group table or the criteria for assigning each group. This avoids applying a staging rule that may not match the relevant site-specific system or clinical context.

A useful follow-up is to ask where the stage is documented and which findings contributed to it. SEER Training describes staging details as coming from pathology, imaging, or other sources; that means a single report field may not display the whole assessment. If you see staging-related details in the pathology report without an overall stage, do not assume a stage is missing or try to calculate one from those details alone. The treating team can explain how the documented assessment was assembled.
What does CDX2 mean in colorectal cancer?
CDX2 is a marker result that should be read separately from histology, grade, and stage. The authoritative material available for this article does not establish the diagnostic application of CDX2 staining in colorectal cancer, the meaning of a positive or negative result, or a CDX2-specific prognosis. It would not be accurate to infer any of those conclusions from the marker name alone.
Interpretation depends on the complete pathology report and the clinical situation. If CDX2 appears in the report, ask the clinician or pathologist what question the test was intended to address, how the reported result fits with the other findings, and whether it changes any diagnostic or care decisions in this specific case. The result should not be treated as a stage, grade, or histology label.

For readers looking for a simple translation of “CDX2 positive” or “CDX2 negative,” the safest answer here is that the wording alone is not enough to assign a diagnosis, stage, or outlook. The result needs case-specific interpretation alongside the rest of the report and clinical information. Avoid applying a general explanation from another marker or another context to this entry without asking the care team.
How biomarker information fits in
Biomarker tests can address questions relevant to treatment. CAP’s colorectal biomarker guidance discusses molecular-marker testing in relation to targeted-therapy decisions and personalized care, and highlights MSI testing as a marker of deficient mismatch repair relevant to immunotherapy selection. That establishes a role for some biomarker testing; it does not establish CDX2’s purpose or implications. Do not transfer what a report says about one marker to another.
A 2017 ASCP/CAP/AMP/ASCO biomarker guideline included 21 statements and recommendations, but its publisher marks it archived and says it may no longer be clinically relevant. It should not be treated as current guidance for interpreting an individual CDX2 result.

In other words, the fact that biomarker testing can inform care does not mean every marker answers the same question. CAP’s example concerns MSI testing and deficient mismatch repair in relation to immunotherapy selection. That example should not be used to infer why CDX2 was ordered or what its result means. The report or care team is the appropriate place to establish the purpose and interpretation of the particular test in this case.

- Find the exact diagnosis and histology wording. This identifies the tumor type recorded by pathology. Note the specimen and the site information shown in the report.
- Locate the grade and specimen context. Check whether the entry is based on a biopsy or surgical resection information, and note the category or whether it is recorded as unknown.
- Find the staging documentation. Look for the stage assessment and related staging details. Remember that staging may draw on pathology, imaging, and other sources.
- Read CDX2 as a separate result. Record the report’s exact wording, but do not translate the result into a stage, grade, tumor type, or prognosis without the care team’s interpretation.
- Keep other structured fields in their own roles. CAP’s protocol also includes items such as margins and lymph node status. They add different information to the report and should not be used as substitutes for the fields above.
This order is a way to organize questions, not a method for independently diagnosing or staging cancer. The report’s structured fields describe different aspects of the case; the treating team integrates them with the rest of the clinical information.
When reviewing a report, it may help to write down the exact wording of each entry rather than paraphrasing it from memory. A practical comparison list is: tumor type (histology), differentiation category and specimen context (grade), documented spread assessment and its information sources (stage), and the exact marker result plus the question it was meant to address (CDX2). This keeps the discussion focused on what each field actually records.
Questions to take to the clinician or pathologist
- What does the histology wording in this report mean?
- Is the listed grade based on a biopsy or on a surgical resection, and what does the recorded category describe?
- Where is the stage documented, and which pathology, imaging, or other findings contributed to it?
- What question was CDX2 testing intended to address in this case, and how should its reported result be understood alongside the other findings?
- Are margins, lymph node status, or other structured report items relevant to the explanation of this diagnosis or staging assessment?
- If there is more than one grade or staging-related entry, what specimen or source does each one refer to?
- Which parts of the report are descriptions, and which parts are being used in the overall clinical assessment?
These questions seek a case-specific explanation rather than a general conclusion from one field. If the report and the clinician’s explanation seem to use a term differently, ask which entry or staging assessment is the one being used for care decisions.

What does CDX2 mean in colorectal cancer?
CDX2 is a separate marker result, not a synonym for histology, grade, or stage. The material summarized here does not establish the diagnostic or prognostic meaning of a particular positive or negative staining result. Ask the clinician or pathologist how it fits with the complete report and clinical situation.
What does pathological stage mean?
It is a staging assessment to which pathology findings contribute. SEER Training notes that staging details may be documented from pathology, imaging, or other sources, so the full assessment should be reviewed with the treating team rather than inferred from one pathology field.

What is the difference between cancer grade and stage?
Grade is a differentiation category; stage is an assessment of how far cancer has spread according to staging criteria. SEER Training lists colorectal grade categories from well differentiated through undifferentiated/anaplastic, as well as unknown. Grade does not replace stage, and stage does not supply a missing grade.
What does histology mean on a pathology report?
Histology is the tumor type recorded by pathology. CAP includes it as a distinct structured reporting element alongside other information such as grade, margins, lymph node status, and staging. Ask the care team to explain unfamiliar histology wording rather than inferring it from a marker or grade.
