ACIP recommends; the CDC Director adopts; CDC publishes and implements the policy. The Advisory Committee on Immunization Practices (ACIP) is an external panel of medical and public-health experts. It reviews evidence and votes on how vaccines should be used in the U.S. civilian population. That vote is advisory until the CDC Director adopts it. CDC then issues the official recommendation, commonly through the Morbidity and Mortality Weekly Report (MMWR) and corresponding CDC guidance pages.

The short answer: ACIP advises, the CDC Director makes it official

People often ask whether “CDC” or “ACIP” decides the U.S. vaccine schedule. The two names refer to different steps in one policy process:

  • ACIP evaluates evidence, considers policy trade-offs and votes on recommendations for vaccines and related agents.
  • The CDC Director decides whether to adopt an ACIP recommendation as official CDC policy.
  • CDC publishes the adopted recommendation and incorporates it into schedules, clinical guidance and implementation materials.

ACIP is chartered to provide expert external advice and guidance to the CDC Director and the Secretary of the U.S. Department of Health and Human Services (HHS) on controlling vaccine-preventable disease in the civilian population. The operational adoption step for CDC policy is the CDC Director’s action. An ACIP vote by itself is not yet an official CDC recommendation.

CDC and ACIP compared

Aspect ACIP CDC and its Director
Role External advisory committee of medical and public-health experts Federal public-health agency; the Director is the official adopting authority for CDC policy
Decision point Members deliberate and vote on a recommendation The CDC Director adopts, modifies through the formal process, or does not adopt the recommendation
Output Advice on vaccine use for the U.S. civilian population Official CDC policy and published recommendations
Evidence work Work groups conduct systematic reviews, assess certainty with GRADE and complete the Evidence to Recommendations (EtR) framework CDC staff support the process, publish the adopted recommendation and provide implementation guidance
Transparency artifacts Evidence profiles, GRADE assessments, EtR judgments and public meeting materials Adoption decision, MMWR recommendation and linked CDC schedule or vaccine pages

HHS has a role because the ACIP charter includes advice to the HHS Secretary. That chartered relationship does not change the practical distinction readers need: ACIP supplies the expert recommendation, while the CDC Director’s adoption makes it CDC policy.

What ACIP is and what it covers

ACIP is not a product regulator and it does not approve vaccines for sale. Its subject is use: which people should receive a vaccine, which product or formulation is appropriate, what interval or dose is advised, and how vaccination should be used to control disease. Its remit covers the U.S. civilian population, so an ACIP recommendation is not automatically a rule for every military, occupational or institutional setting.

The committee works through topic-specific work groups. A work group may include ACIP members and technical experts who examine clinical trials, observational studies, surveillance and other relevant evidence. The full committee then considers the work group’s analysis in an open policy process before voting.

How a vaccine question becomes an ACIP recommendation

ACIP’s handbook describes a sequence designed to prevent a policy decision from resting on a convenient selection of studies or on a single efficacy result. The exact meeting schedule varies by topic, but the analytical stages are consistent.

1. Form a guideline-development panel or work group

CDC and ACIP organize a work group for the vaccine or policy question. The group defines the issue it must answer, identifies the populations and outcomes that matter, and plans how evidence and stakeholder considerations will be handled. This division of labor lets specialists examine the technical record before the full committee deliberates.

2. Define a PICO question and the important outcomes

The work group turns the policy issue into a PICO question:

  • Population: the people under consideration, such as an age group, pregnancy status or risk category.
  • Intervention: the vaccine, formulation, dose or schedule being evaluated.
  • Comparator: no vaccination, a previous formulation, another schedule or another relevant alternative.
  • Outcomes: benefits and harms, separated into critical and important outcomes.

Defining outcomes before judging the results helps the group avoid changing the question after seeing which findings are statistically attractive. A recommendation can depend on more than prevention of infection; severe disease, hospitalization, death, adverse events and other outcomes may have different levels of importance.

3. Find or conduct a systematic review

The handbook requires evidence to be identified and retrieved systematically before GRADE is applied. The group may use a high-quality published systematic review or conduct a new review when an existing one does not answer the question adequately.

A systematic search is intended to capture the full body of relevant evidence, not only the most favorable studies. Reviewers specify search methods and eligibility criteria, assess the included evidence and document how studies map to each outcome. This foundation matters because a certainty judgment is only as complete as the evidence that was systematically assembled for the question.

4. Synthesize the results quantitatively and narratively

Where studies are sufficiently comparable, the work group may combine estimates quantitatively. It also prepares a narrative summary explaining differences in populations, products, follow-up periods, outcome definitions and study quality. A pooled estimate can be useful, but it does not erase important differences among studies.

The result is an evidence record that shows both the numerical findings and the context needed to interpret them. Benefits and harms are examined together rather than treating safety as an afterthought.

5. Assess certainty with GRADE and build evidence profiles

GRADE—the Grading of Recommendations, Assessment, Development and Evaluation approach—assesses how certain the committee can be that the observed effects reflect the true effects. ACIP commonly presents certainty separately for benefits and harms, because confidence may be high for one and lower for the other.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Certainty is about confidence in the evidence, not a guarantee that a vaccine will produce the same result for every person. Randomized and observational evidence can contribute, and the assessment considers limitations such as risk of bias, inconsistency, indirectness, imprecision and publication bias. The work group records its judgments in evidence profiles so readers can see which outcomes support a conclusion and where uncertainty remains.

A high-certainty estimate of a benefit does not by itself dictate policy. A small but certain benefit may be weighed differently from a larger estimate with substantial uncertainty, especially when harms, feasibility or equity differ between options.

6. Complete the Evidence to Recommendations framework

The Evidence to Recommendations (EtR) framework documents the policy judgments that connect evidence to a recommendation. It prevents the committee from presenting a value-laden decision as if it were a direct quotation from a clinical trial.

For the population and intervention under review, EtR considers:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • How important the disease is as a public-health problem.
  • The balance between expected benefits and harms.
  • The type and certainty of the evidence.
  • Patients’ or affected people’s values and preferences.
  • Health-economic effects and other resource use.
  • Acceptability to patients, clinicians, programs and other stakeholders.
  • Feasibility of delivering the recommendation in real settings.
  • Equity, including whether the policy could reduce or worsen differences in access or outcomes.

These factors explain why two options with similar efficacy estimates might receive different policy judgments. A schedule that is difficult to deliver, poorly accepted or likely to widen disparities may be less suitable than an option with comparable clinical effects but better feasibility and equity.

7. Formulate and vote on the recommendation

After reviewing evidence profiles and the EtR judgments, the work group formulates a proposed recommendation for the full ACIP. Members discuss the wording, population, timing, exceptions and implementation details, then vote at a public meeting. The vote is the committee’s formal advice; it is not yet official CDC policy.

What happens after the ACIP vote

The CDC Director must adopt the recommendation before it becomes official CDC policy. CDC then publishes the adopted recommendation, generally in MMWR with supporting material on CDC webpages. Publication gives clinicians, health departments and the public an authoritative statement of the population, schedule, intervals, precautions and other conditions that apply.

This sequence explains why a news report about an ACIP vote and a later CDC schedule may appear on different dates. The vote is the advisory milestone; adoption and publication are the policy milestones. Until adoption, the proposal should not be described as an official CDC recommendation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Adoption also does not mean that every delivery setting changes instantly. Health departments, electronic health-record vendors, insurers, clinics and professional organizations may need time to update protocols and operational materials. Those implementation steps follow the published policy and do not replace the Director’s adoption decision.

Worked example: the 2024–2025 COVID-19 vaccine recommendation

The 2024–2025 COVID-19 recommendation for people aged 6 months and older illustrates how the method operates. The ACIP COVID-19 Vaccines Work Group assessed benefits and harms with GRADE and used EtR to consider the policy context.

The MMWR methods description states that the work group assessed the certainty of evidence for benefits and harms associated with an updated bivalent or 2023–2024 COVID-19 vaccine administered in the United States during September 2022 through May 2024. That date range is important: the assessment reflects the evidence available for that review period, not every subsequent study.

For EtR, the work group reviewed the importance of COVID-19 as a public-health problem, resource use, benefits and harms, patient values, acceptability, feasibility and equity related to COVID-19 vaccination. The example shows why an ACIP recommendation cannot be reduced to one efficacy percentage. The committee had to judge how the evidence and these contextual factors supported a population-level policy for everyone aged 6 months and older.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

After the committee’s deliberation and vote, the recommendation still required CDC Director adoption to become official CDC policy. The final MMWR recommendation and CDC guidance are the documents to use when determining the adopted policy rather than relying on a meeting presentation or preliminary report.

How to trace a recommendation from studies to policy

Readers who want to inspect a vaccine decision can follow the documentary chain instead of treating a schedule table as a black box.

  1. Start with the final MMWR recommendation. Confirm the vaccine, population, dates and wording that CDC adopted.
  2. Check the CDC vaccine recommendation page. It provides the current operational guidance and identifies that recommendations become official CDC policy after adoption by the CDC Director.
  3. Read the ACIP meeting materials. Look for the work-group presentation, proposed recommendation and recorded vote.
  4. Open the GRADE evidence profiles. Examine which outcomes were rated, the direction of effects and the certainty assigned separately to benefits and harms.
  5. Review the EtR framework. This is where values, resource use, acceptability, feasibility and equity are recorded alongside the clinical evidence.
  6. Compare dates and versions. A later formulation, variant, surveillance period or safety signal can lead to a new review and a revised recommendation.

Reading these materials in order distinguishes three different questions: What did studies find? How certain is that finding? Why did the committee judge that a particular policy was appropriate?

Common misunderstandings about CDC and ACIP

“ACIP sets the schedule.”

ACIP recommends a schedule or other use policy. The CDC Director’s adoption makes it official CDC policy, and CDC publishes the adopted schedule. Saying that ACIP “sets” the schedule skips the required adoption step.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“The CDC Director makes the recommendation from scratch.”

The Director’s role is adoption, not replacement of the committee’s evidence review. ACIP work groups perform the systematic-review, GRADE and EtR work, and the committee votes on the resulting advice. The Director’s action is the formal step that gives the recommendation CDC-policy status.

“A recommendation is based only on vaccine efficacy.”

Clinical effects are central, but EtR also records harms, certainty, values and preferences, resource use, acceptability, feasibility and equity. Those considerations can affect the policy even when two choices have similar efficacy estimates.

“Low-certainty evidence means ACIP has no basis for acting.”

Low certainty means the true effect could differ materially from the estimate. It does not mean that no evidence exists. The committee can still make a recommendation when the public-health problem is serious and the expected benefits, harms and practical considerations support action; the uncertainty should remain visible in the evidence profile and the wording of the policy.

“The vote and the published policy are the same document.”

A meeting vote is an advisory action. The adopted recommendation appears later in CDC’s official publication and guidance. For clinical or program decisions, use the adopted wording and its effective dates.

What the process means for different readers

Patients and families

When a schedule changes, ask which population and formulation the recommendation covers, when it was adopted, and whether it includes exceptions or catch-up provisions. The evidence profile can show how certain the benefits and harms are, while the EtR record explains broader policy considerations. A recommendation is population guidance; an individual clinician still considers medical history, contraindications and preferences.

Clinicians and immunization programs

Use the final MMWR and current CDC implementation pages rather than an unadopted proposal or a slide from an earlier meeting. Check product-specific intervals, age boundaries, precautions and transition language. Operational systems may lag behind publication, so document the version and effective date used in a protocol.

Journalists, educators and policymakers

Describe the institutional roles precisely. “ACIP recommended” refers to the committee vote; “CDC adopted” refers to the Director’s action; “CDC recommends” should be reserved for the adopted policy. When explaining disagreement or uncertainty, separate the evidence estimate from the EtR judgment about values, feasibility, resources and equity.

Why this evidence architecture matters

Vaccination policy affects millions of people, so an expert panel needs more than an informal consensus or a single favorable study. Systematic reviews reduce the chance that relevant evidence is missed. GRADE makes confidence in each important outcome explicit. EtR records the judgments that cannot be answered by a clinical trial alone, such as whether a program is feasible or equitable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The architecture also makes revision possible. If new safety data, a different formulation, changing disease burden or improved delivery options alter the balance, a work group can update the evidence and EtR assessment. The resulting recommendation can then move through the same vote, adoption and publication pathway.

A practical decision map

Question Where to look What it tells you
What does the evidence show? Systematic review, quantitative synthesis and narrative summary Effects observed across relevant studies and important differences among them
How confident is ACIP? GRADE evidence profile Certainty of evidence for each critical or important benefit and harm
Why this policy rather than another? EtR framework and meeting discussion How evidence was weighed with values, resources, acceptability, feasibility and equity
Did the committee advise it? ACIP agenda, vote and recommendation wording The advisory decision and its proposed population or schedule
Is it official CDC policy? CDC Director adoption and final MMWR or CDC guidance The adopted, publishable policy used for implementation

Bottom line

ACIP is the evidence-review and recommendation body; the CDC Director is the adoption authority. ACIP work groups systematically gather evidence, assess certainty with GRADE and use EtR to make policy trade-offs visible. After the committee votes, the Director’s adoption and CDC’s publication turn that advice into official U.S. civilian vaccine policy. Following the systematic review, GRADE profile, EtR record, vote and final MMWR lets readers see the complete path from studies to the schedule used in practice.
