CareRelay Local is a software project that turns rough caregiving notes into structured handovers for the next caregiver or a family member. Its builder, Dinuka Ekanayake, describes a workflow that runs notes through a local ASP.NET backend to Ollama and Gemma 3 4B. The project is intended to organize information—not provide medical advice—and its local and offline behavior is reported by the builder, not independently audited.
What CareRelay Local does
Ekanayake says he built the tool for a friend who helps care for an older family member. The user enters ordinary notes, such as what the person ate, medication mentioned, activities, knee pain, or an upcoming appointment. CareRelay Local turns them into a report organized under these headings:
- Summary
- Meals & Hydration
- Medication Mentioned
- Activities & Mobility
- Observations
- Rest
- Appointments & Reminders
- Information for the Next Caregiver
The purpose is to make already-recorded information easier to pass along. It does not replace the caregiver’s notes, judgment, or communication with a health professional.
Two handover modes serve different readers

How the local AI workflow is described
The project write-up names ASP.NET Core/.NET 10, HTML, CSS, vanilla JavaScript, Ollama, and Gemma 3 4B. In the described request path, the browser sends notes to the local ASP.NET backend at /api/handover; the backend sends them to Ollama on localhost, where the model identifier is gemma3:4b. Ollama returns the generated handover to the application.
Ekanayake reports that, after the model is installed, the workflow can generate reports while the laptop is disconnected from the internet. This is the builder’s account of the project’s design and operation, not an independent network-traffic or security audit. The write-up does not establish hardware requirements, supported operating systems, licensing or distribution terms, or a privacy policy.

What the prompt is meant to prevent—and what it cannot guarantee
The builder says the prompt directs the model to use only details in the submitted notes, preserve uncertainty, avoid diagnoses and treatment recommendations, refrain from inventing medication names or dosages, and mark missing details as “Not mentioned.” These are intended safeguards, not proof that every generated report will be complete or correct. A language model can still misread, omit, or rearrange information.

- Check medication references, timings, appointments, and observations against the original notes.
- Keep the original notes available so the handover can be verified.
- Do not treat a generated summary as a diagnosis, treatment recommendation, or substitute for professional advice.
Privacy claims and evidence limits
The project’s local-processing design is meant to avoid sending care notes to an external AI provider for inference, and the builder reports offline use after model installation. That may matter when notes contain information a family does not want sent to a cloud AI service. However, the project write-up does not report an independent review of network traffic, security, data retention, or all application dependencies, so it does not establish that the entire setup has been independently verified as private or secure.

The builder says he tried cases involving empty notes, missing information, ambiguous medication references, an unavailable Ollama service, and potentially medical observations. He also reports qualitative feedback from one friend. This is not a formal usability study, measured accuracy evaluation, or clinical validation, and the write-up supplies no quantified outcome data.
