Caregiver stress is the emotional, physical and behavioral strain that can build while you provide unpaid care for an older adult or a person with illness or disability. Common signals include exhaustion, anxiety, irritability, isolation, sleep problems, low mood, headaches and skipped personal care. Treat persistent or worsening changes as a health issue: tell a clinician that you are a caregiver, arrange a specific break, and ask a trusted person or local service to share the work.
What is caregiver stress?
Caregiving can involve medication reminders, bathing, meals, transportation, supervision, paperwork and constant decisions. Stress is the response that develops when those demands exceed the time, energy, money, skills or support available. It can be temporary after a difficult week or become chronic when there is no reliable relief.
Stress is not proof that you do not love the person you support, and needing help is not a personal failure. It is a reason to protect both people’s health and make the care plan more sustainable. The National Institute on Aging (NIA) advises caregivers to seek help before they are completely overwhelmed.
What are the signs of caregiver stress?
You may notice only a few signs, and they may change over time. NIA’s caregiver guidance identifies these warning patterns.

Emotional and thinking changes
- Feeling exhausted, overwhelmed or persistently anxious.
- Becoming unusually angry, impatient or short-tempered.
- Feeling lonely, disconnected or unable to talk honestly about caregiving.
- Feeling sad or hopeless, or losing interest in activities you normally enjoy.
Sleep and physical changes
- Difficulty falling asleep, staying asleep or getting restorative sleep.
- Frequent headaches, pain or other physical problems that are new or worsening.
- Having no time for exercise or regular, nutritious meals.
- Skipping your own bathing, dental care, medical appointments or preventive care.
Behavior and coping changes
- Using alcohol, drugs or prescription medicines in a way that is unsafe or different from how they were prescribed.
- Withdrawing from family, friends or community activities.
- Missing your own appointments or putting off treatment because the care recipient’s needs always seem more urgent.
A symptom that lasts, interferes with daily life or is getting worse deserves attention even if you cannot identify every sign on the list. Tell a health professional what has changed and how long it has been happening.
How can caregiver stress affect mental and physical health?
NIA says caregivers may neglect preventive care and self-care and describes higher risks of physical and mental health problems, sleep problems, chronic conditions such as high blood pressure, and premature death. These are population-level risk statements, not a prediction that any one caregiver will develop a particular illness. The available NIA guidance does not provide a numeric risk estimate for an individual.
Stress can also make existing conditions harder to manage. Poor sleep may reduce concentration; skipped meals can leave you without energy; and constant vigilance can make it difficult to notice your own symptoms. The Centers for Disease Control and Prevention (CDC) advises caregivers not to ignore signs of illness and to attend to their mental and emotional health.

Make routine care part of the caregiving plan: keep primary-care and dental appointments, refill your medicines, and tell clinicians that you provide care. A clinician can screen for depression, anxiety, sleep problems or substance misuse and discuss treatment or counseling. Self-care steps help, but they do not replace professional care when symptoms are significant.
What should I do if I feel overwhelmed?
Choose one concrete action rather than trying to solve every problem at once. The following sequence is designed for the next day or week.
- Name the situation to a professional. At your next appointment, say, “I am an unpaid caregiver and I am exhausted,” then describe your sleep, mood, physical symptoms and the hours of care you provide. Ask what support or screening is appropriate.
- Request one defined task or time block. Ask a relative, friend or neighbor to cover a specific duty, such as Tuesday dinner, a Saturday morning shift or one weekly pharmacy run. A precise request is easier to accept than “Let me know if you can help.”
- Put the agreement on a shared calendar. Record who is responsible, the start and end time, transportation details and a backup contact. Update it when appointments or symptoms change.
- Protect a recurring break. CDC guidance recommends consistent breaks; even a few hours of respite each week can improve well-being. Schedule the break before you reach a crisis point.
- Use emotional support. A counselor, support group, faith leader or trusted friend can provide a place to discuss grief, anger, guilt and uncertainty. NIA points caregivers toward both online and in-person groups.
- Reassess the care plan. If needs have increased, ask the clinician or social worker whether home-care services, adult day services, equipment, transportation or a higher level of care should be considered.
If you feel unable to stay safe, have thoughts of suicide, or believe the person you care for is in immediate danger, call emergency services or a crisis service in your area now. Do not leave an unsafe situation unaddressed while waiting for a routine appointment.

What is respite care?
Respite care is temporary care that gives the regular caregiver time away. Depending on local availability and the person’s needs, it may take place in the home, at an adult day center or in a health care facility. Arrangements can last a few hours, a full day, overnight or several weeks. NIA notes that providers may charge by the hour, day or week; availability, eligibility and payment rules vary.

- Family or friends: Useful for a predictable shift, errands or companionship, provided the helper understands the person’s needs.
- In-home providers: A worker comes to the home for supervision or personal-care tasks allowed by the service. Confirm training, hours, supervision and cancellation policies.
- Adult day services: The person attends a program during set daytime hours, giving you a regular block for work, appointments or rest. Ask about transportation, meals, activities and clinical capabilities.
- Short-term facility stays: Some facilities offer temporary residential care. Ask about assessment requirements, medication management, length of stay and total charges.
- Community or volunteer programs: Local aging agencies, faith communities and nonprofit groups may offer visits, meals or transportation, subject to their schedules and eligibility rules.

Where can I find caregiver support?
Start with people who already know the care recipient or your community. NIA lists two national navigation services:
- Eldercare Locator: 800-677-1116, for connections to local aging services.
- Family Caregiver Alliance navigator: 800-445-8106, for caregiver information and service navigation.
When you call, have the care recipient’s location, age, main needs, your preferred schedule and any language or transportation requirements ready. Ask whether a service has a waitlist, what an assessment involves, and which costs are yours.
Support groups and counseling
Support groups can reduce isolation and provide practical ideas from people facing similar responsibilities. Online groups may fit an unpredictable schedule; in-person groups may provide stronger local connections. A counselor can help with anxiety, depression, grief, family conflict, boundaries and decision-making. If you are feeling anxious, frustrated or depressed, do not wait for the situation to become unmanageable before contacting one.

Some states offer payment or other assistance through state-specific Medicaid programs, but eligibility, covered tasks, enrollment steps and funding differ by state. Payment is not automatic and is not guaranteed for every family caregiver. Contact your state Medicaid agency or local aging office and ask about consumer-directed services, caregiver-support programs and required assessments. How can I use official caregiver tools?
NIA’s caregiver toolkit includes free worksheets and a handbook. Use them to list daily tasks, medications, appointments, emergency contacts, questions for clinicians and people who can provide backup. A written care summary reduces the chance that essential information is lost when another person takes a shift.
CDC guidance published or reviewed in 2024 and 2025 emphasizes reasonable expectations, delegating tasks, taking breaks and paying attention to your own health. Apply that advice operationally: define what must happen today, what can wait, who else can do it and what would signal that the current plan is no longer safe.
When is caregiver stress an urgent health problem?
Seek prompt medical help for chest pain, severe shortness of breath, fainting, sudden confusion, signs of stroke, serious injury or symptoms that could indicate a medical emergency. Contact a clinician soon for persistent hopelessness, inability to sleep, escalating substance use, panic, uncontrolled anger or physical symptoms that interfere with functioning. If there is immediate danger to you or the person receiving care, call emergency services and arrange a safe handoff.
