Usually, yes. A mild cold or other minor illness normally is not a reason to postpone an influenza vaccine. Pregnancy is a reason to receive an age-appropriate flu vaccine, not a reason to avoid it. A flu shot can generally be given at the same visit as Tdap, RSV, or the current COVID-19 vaccine, including during pregnancy. The important exceptions are moderate or severe acute illness, a previous severe allergic reaction to a flu vaccine or one of its components, Guillain–Barré syndrome within six weeks of a previous flu vaccination, and recent use of certain influenza antivirals if a live nasal-spray vaccine is being considered.

This guidance reflects U.S. CDC/ACIP recommendations for the 2025–26 influenza season and current American College of Obstetricians and Gynecologists (ACOG) pregnancy guidance. Vaccine products and seasonal recommendations can change, so tell the vaccinator about your symptoms, pregnancy, medications, allergies, and prior vaccine reactions before the dose is given.

Can you get a flu shot if you have a cold?

A mild upper-respiratory infection, mild diarrhea, or another low-grade illness is generally compatible with influenza vaccination. CDC general immunization principles do not treat these minor illnesses as contraindications or precautions. A runny nose, mild sore throat, modest cough, or feeling slightly off does not automatically make the vaccine less appropriate or less effective.

When illness should delay the decision

CDC/ACIP lists “moderate or severe acute illness with or without fever” as a precaution for influenza vaccination. That wording matters: a person can be significantly ill without having a high temperature, and a fever by itself does not define every situation. The vaccination provider should assess whether the illness is serious enough to wait and whether symptoms could represent an infection that needs medical attention.

Postponing during a substantial acute illness is usually about making a clear assessment and avoiding confusion between illness symptoms and vaccine side effects. It is not evidence that the vaccine is permanently unsafe for you. Once the acute illness has resolved, arrange vaccination promptly if you still need protection.

Can you get a flu vaccine while pregnant?

Yes. CDC/ACIP recommends an age-appropriate influenza vaccine during pregnancy. Influenza infection can be more serious during pregnancy, and CDC reports that vaccination is associated with a lower risk of respiratory illness and influenza in pregnant or postpartum women and in their infants during the first months of life.

Which influenza product is used?

During pregnancy, use an inactivated influenza vaccine (the injectable flu shot) or a recombinant influenza vaccine that is appropriate for your age. Do not use LAIV3, the live attenuated influenza vaccine delivered as a nasal spray, during pregnancy.

Pregnancy itself is therefore not a reason to wait for a later trimester. Tell the prenatal-care team and the vaccination provider that you are pregnant, how far along you are, and whether you have had a previous reaction to influenza vaccine. They can select the appropriate formulation and review any additional precautions.

Why vaccination during pregnancy also protects a newborn

Antibodies produced after vaccination can help protect the pregnant person and provide passive protection to the infant during the early months after birth, when the infant is too young for routine influenza vaccination. This is one reason CDC recommends vaccination during pregnancy rather than waiting until after delivery.

Can a flu vaccine be given with Tdap, RSV, or COVID-19 vaccine?

Yes. ACOG states: “The influenza vaccine can safely be administered at the same time as other vaccines recommended during pregnancy.” ACOG specifically names Tdap, RSV, and the current COVID-19 vaccine and says that coadministered vaccine efficacy is not impacted.

Vaccine given with influenza vaccine Pregnancy guidance What to tell the provider
Tdap ACOG supports same-visit administration when Tdap is recommended. Share your pregnancy week and any prior vaccine reaction.
RSV vaccine ACOG specifically includes RSV vaccine among vaccines that can be administered at the same visit when recommended. Confirm which RSV product and timing are appropriate for your pregnancy and local guidance.
Current COVID-19 vaccine ACOG supports coadministration and states that efficacy is not reduced. Provide your recent COVID-19 vaccination dates and infection history if asked.

Outside pregnancy, other vaccines generally do not require spacing from an injectable influenza vaccine. A same-visit appointment can reduce missed opportunities, but the clinician may still recommend separate timing based on your illness, prior reactions, available products, or a condition that needs individual assessment.

What if you are taking Tamiflu or another influenza antiviral?

Recent influenza antiviral use matters particularly when the planned product is LAIV3, the live nasal-spray vaccine. Antiviral medicines can interfere with replication of the live vaccine virus, so CDC’s 2025–26 timing table specifies minimum intervals after particular drugs:

Influenza antiviral CDC interval before LAIV3
Oseltamivir (Tamiflu) or zanamivir Wait 48 hours after the last dose.
Peramivir Wait 5 days after treatment.
Baloxavir Wait 17 days after treatment.

These intervals address the live nasal vaccine, not a blanket prohibition on every influenza vaccine after antiviral treatment. Tell the provider the exact drug, the date and time of your last dose, and whether you are still ill. The provider can choose an inactivated or recombinant product when appropriate or schedule LAIV3 after the applicable interval. Do not stop a prescribed antiviral merely to obtain a vaccine without medical advice.

Who should not receive a flu vaccine without a product-specific review?

Previous severe allergic reaction

A history of anaphylaxis after a previous influenza vaccine or to a component requires a formulation-specific review. Depending on which product caused the reaction and which product is being considered, the history may be a contraindication or a precaution. Bring the name of the vaccine, the approximate date, and a description of the reaction if you have those records. “I felt unwell” is different from anaphylaxis; the provider needs the details to classify the event safely.

Guillain–Barré syndrome after a previous flu vaccine

Guillain–Barré syndrome (GBS) within six weeks after a previous influenza vaccination is a precaution for influenza vaccination. It does not automatically answer every future vaccination decision. A clinician should weigh the timing and certainty of the diagnosis, your risk from influenza, and your risk factors before recommending another dose.

Egg allergy

Egg allergy alone is not a reason to skip influenza vaccination. CDC recommends an otherwise appropriate influenza vaccine for people with egg allergy. Tell the vaccinator about the allergy so routine screening and observation procedures can be followed, especially if you have had a severe allergic reaction to any vaccine in the past.

A practical decision guide for the appointment

  1. Describe your current health. Say whether symptoms are minor and improving or whether you have moderate or severe illness, breathing difficulty, dehydration, rapidly worsening symptoms, fever, or a recent hospitalization.
  2. State whether you are pregnant. The provider should select an inactivated or recombinant product rather than LAIV3 during pregnancy.
  3. List vaccines you want or recently received. Mention Tdap, RSV, COVID-19, or any other vaccine. Same-visit administration is generally possible, and ACOG supports it for the named pregnancy vaccines.
  4. List medicines and the last dose time. Specifically report oseltamivir, zanamivir, peramivir, or baloxavir. The exact interval matters only when LAIV3 is under consideration, but the medication still needs to be documented.
  5. Review previous reactions. Identify any anaphylaxis after an influenza vaccine or component and any GBS diagnosis that began within six weeks of a prior flu vaccine.
  6. Follow the provider’s timing recommendation. If a precaution applies, ask whether to vaccinate with a different formulation, wait until recovery or the antiviral interval has passed, or obtain an individualized consultation.

How the common situations compare

Situation Typical implication Key action
Mild cold, mild diarrhea, or other minor illness Usually compatible with vaccination. Tell the provider; proceed if the assessment confirms the illness is minor.
Moderate or severe acute illness, with or without fever CDC/ACIP precaution. Have the vaccinator assess whether to defer until recovery.
Pregnancy Influenza vaccination is recommended. Use inactivated or recombinant vaccine; do not use LAIV3.
Same visit as Tdap, RSV, or current COVID-19 vaccine during pregnancy ACOG supports coadministration and says efficacy is not impacted. Tell the provider all vaccines due or recently received.
Oseltamivir or zanamivir recently taken LAIV3 timing issue. Wait 48 hours after the last dose before LAIV3.
Peramivir recently taken LAIV3 timing issue. Wait 5 days before LAIV3.
Baloxavir recently taken LAIV3 timing issue. Wait 17 days before LAIV3.
Anaphylaxis to a prior influenza vaccine or component Product-specific contraindication or precaution. Have the exact product and reaction reviewed before vaccination.
GBS within six weeks of a prior influenza vaccine Precaution. Discuss individual benefits and risks with a clinician.
Egg allergy alone Not a reason to skip an otherwise appropriate vaccine. Tell the provider about the allergy and any prior vaccine reactions.

Questions to ask if the provider recommends waiting

  • Is the concern the severity of my current illness, a specific vaccine formulation, a medication interaction, or a previous reaction?
  • What symptom improvement or recovery point should I reach before returning?
  • Would an inactivated or recombinant vaccine avoid the issue with a live nasal vaccine?
  • When exactly should I reschedule, and which vaccines can be given at that visit?

Seek prompt medical care for severe breathing difficulty, inability to keep fluids down, signs of dehydration, confusion, or rapidly worsening illness rather than treating the vaccination appointment as the main issue. For a history of anaphylaxis, GBS, immunocompromise, pregnancy complications, or uncertainty about antiviral timing, use individualized advice from a clinician or qualified vaccination provider.
