Yes, many people with knee pain can remain active—but the right answer depends on the cause, the activity and how your symptoms respond. For diagnosed knee osteoarthritis, clinical guidance supports strengthening and low-impact aerobic exercise. A painful knee after a twist, fall or other injury needs a different level of caution, and running while the knee hurts should be paused rather than forced.
This is general information, not a diagnosis or an individual rehabilitation plan. U.S. osteoarthritis guidance and U.K. NHS advice use different health systems and sometimes different thresholds, so use the recommendations below as a safety framework and arrange an assessment if symptoms are severe, worsening or persistent.
Can you exercise with knee pain?
Often, yes—by changing the activity, duration or intensity. Exercise is not automatically harmful to a painful knee, but “push through it” is not a safe universal rule. Knee pain can come from osteoarthritis, an overuse problem, a sprain or other injury, and conditions that require medical treatment. General exercise advice cannot identify the cause or clear you to train after an injury.

For symptomatic knee osteoarthritis, the American Academy of Orthopaedic Surgeons (AAOS) strongly recommends self-management, strengthening, low-impact aerobic exercise, neuromuscular education and physical activity consistent with national guidelines. Its guideline states: “We recommend that patients with symptomatic osteoarthritis of the knee participate in self-management programs, strengthening, low-impact aerobic exercises, and neuromuscular education; and engage in physical activity consistent with national guidelines.” Read the AAOS knee osteoarthritis guideline.

That recommendation is for people with a diagnosed condition, not a blanket instruction to exercise through every painful knee. A tolerable alternative may be appropriate when a particular movement aggravates symptoms; substantial or escalating pain is a signal to reduce, change or stop that activity.
When should knee pain stop you from exercising?
Get urgent medical advice for red flags
Use urgent services appropriate to your location for a very painful knee, inability to move it or bear weight, major swelling or an obvious deformity. A hot, red knee accompanied by a fever or feeling unwell also needs prompt assessment. These warning signs are listed in NHS guidance on knee pain and joint pain.
Seek an assessment after a significant injury
If pain began with a twist, fall, collision or other notable injury, do not rely on a generic exercise article to decide whether to continue. A clinician can check for injuries that need protection, imaging or a specific rehabilitation plan.

Arrange routine review for persistent symptoms
Book a GP, doctor or physiotherapist appointment when pain is worsening or has not improved within a few weeks. The cause, range of motion, swelling, strength and ability to bear weight all influence which activities are sensible.
What exercise is recommended for knee osteoarthritis?
AAOS’s third-edition non-arthroplasty guideline says: “Supervised exercise, unsupervised exercise, and/or aquatic exercise are recommended over no exercise to improve pain and function for treatment of knee osteoarthritis.” Exercise can therefore be part of treatment rather than something to avoid altogether.
The U.S. Physical Activity Guidelines for Americans, second edition (published in 2018), say most people with osteoarthritis can usually tolerate 150 minutes or more of moderate-intensity activity per week. The same guidance stresses that the type and amount must match a person’s ability and the severity of their condition. Treat 150 minutes as broad national-guideline context, not an immediate target for someone starting with pain.

Which activities are low impact for bad knees?
The federal guidelines identify walking, swimming, tai chi and many strengthening exercises as low-impact, low-risk examples for people with osteoarthritis. “Low impact” does not mean symptom-free for everyone; monitor your response and adjust the dose.
| Activity | Why it may help | Ways to modify it | When to stop or switch |
|---|---|---|---|
| Walking | Weight-bearing aerobic exercise that can be performed without special equipment. | Use a shorter route, slower pace, flatter surface or several brief walks. | Reduce or replace it if each walk causes substantial or progressively worse pain. |
| Swimming or aquatic exercise | Water reduces loading while allowing aerobic movement; aquatic exercise is included in AAOS recommendations for osteoarthritis. | Choose an easy stroke, shorter session or supported pool exercises. | Stop if knee pain escalates, or if another medical issue makes pool activity unsafe. |
| Tai chi | Gentle, controlled movement is listed as a low-impact option in the U.S. guidelines. | Use a chair or shallower range of motion for balance and comfort. | Pause movements that produce marked pain, instability or loss of balance. |
| Strengthening | AAOS recommends strengthening to improve function in knee osteoarthritis. | Start with a clinician-approved range, light resistance and fewer repetitions; progress gradually. | Do not ignore pain during an exercise; ask a doctor or physical therapist how to alter it. |
| Stationary bicycle | AAOS names a stationary bicycle as one possible low-impact warm-up. | Use very light resistance and a comfortable seat height if a clinician says cycling is suitable. | A bicycle is optional, not required, and does not treat the underlying cause by itself. |
How should you start exercising with a painful knee?
- Clarify the situation. Note when the pain began, whether there was an injury, where it hurts, and whether swelling, locking, giving way or difficulty bearing weight occurs. Arrange clinical advice first when those features are significant.
- Choose a tolerable, lower-impact option. Walking, swimming, tai chi or a clinician-selected strengthening exercise may be more suitable than the activity that triggered symptoms.
- Warm up for 5–10 minutes. The AAOS Knee Conditioning Program suggests low-impact activity such as walking or a stationary bicycle. AAOS says the program is intended to be done under a doctor’s supervision; ask a doctor or physical therapist which exercises fit your goals.
- Begin below your usual dose. Shorten the session, lower the speed or resistance, and use breaks. The first objective is a repeatable level that your knee tolerates, not a full workout.
- Check symptoms during and after. Mild, temporary muscle soreness or a short-lived increase in osteoarthritis pain can occur in an exercise program. Substantial, worsening or persistent joint pain means the activity needs modification or a pause.
- Progress one variable at a time. Add a little duration, frequency or resistance only when the current level remains tolerable. If symptoms flare, return to the last comfortable level and seek professional guidance if the flare does not settle.
- Build toward general activity guidance only as able. The 150-minute weekly figure is a national guideline for people who can tolerate it, not a requirement to meet while the knee is acutely painful.
Should you stop exercising if your knee hurts?
Stop or change the specific movement when pain is substantial, becomes sharper or increases as you continue. AAOS’s conditioning material uses the heading “Do not ignore pain” and says, “Talk to your doctor or physical therapist if you have any pain while exercising.” The advice does not require stopping all movement forever; it means finding out whether the exercise, dose or diagnosis needs to change.
The Arthritis Foundation’s consumer guidance advises stopping for moderate to severe joint pain. That is different from ordinary muscle fatigue or mild soreness after unfamiliar strengthening. Pain that lingers, worsens after each session or is accompanied by swelling, heat, instability or reduced function deserves assessment rather than repeated testing.

Can you run with knee pain?
Running is a separate case from low-impact exercise. NHS guidance advises not running while the knee hurts. If the pain remains after a week of rest, it advises consulting a GP or physiotherapist. The appropriate return-to-running point depends on the cause and severity, so there is no universal countdown that applies to every runner.
While waiting for advice, choose an activity that does not reproduce the running pain and can be shortened or made gentler. A diagnosis of osteoarthritis does not automatically ban running, but NHS running-injury advice still means that symptomatic running should be paused and assessed rather than pushed through.

Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →How do you decide between exercise options?
Compare each option using four questions:
- Impact and response: Does the activity load the knee heavily, and what happens during the session and later that day?
- Cause and timing: Is this a known, clinically assessed chronic condition, or did symptoms follow a sudden injury?
- Adjustability: Can you shorten it, slow it, reduce resistance or substitute a lower-impact version?
- Function and warning signs: Can you move and bear weight normally, without major swelling, deformity, heat or systemic illness?
For diagnosed osteoarthritis, low-impact aerobic exercise and strengthening have the clearest support from AAOS and federal guidance. For unexplained or injury-related pain, the answer depends on examination findings rather than the activity’s reputation.

What can a doctor or physiotherapist add?
A clinician can distinguish osteoarthritis from an acute injury or another cause, identify movements to protect, and tailor strengthening, balance and aerobic work to your range of motion and function. Bring a short record of triggers, pain severity, swelling, walking tolerance and what happens overnight or the next day. Ask which activities are acceptable now, what pain response is expected, how to progress, and which symptoms should trigger an urgent call.
AAOS describes its conditioning program as supervised medical guidance rather than individualized medical advice. A stationary bicycle may be one warm-up option, but purchasing equipment is not necessary to follow the general recommendations.
