Short answer: Current evidence does not establish that Ozempic or other GLP-1 medicines cause nail problems. A 2025 analysis found a small number of spontaneous reports coded as nail changes, but those reports cannot show that the medicine caused the change or indicate how often nail problems occur among everyone taking treatment.
Ozempic’s current prescribing information lists postmarketing reports of alopecia (hair loss), not nail changes, and warns that voluntary reports come from a population of unknown size. If your nails change after starting or changing a GLP-1 medicine, tell the prescribing clinician and consider an evaluation by a board-certified dermatologist rather than assuming the drug is responsible.
What is actually known about GLP-1 drugs and nails?
GLP-1 medicines include semaglutide products such as Ozempic and other drugs in the same therapeutic class. They are prescribed for conditions including type 2 diabetes and, depending on the product and local authorization, weight management. The question about nails is reasonable because people sometimes notice changes after beginning a medicine, losing weight, becoming ill, or changing their diet. Timing alone, however, cannot identify the cause.

The best-supported conclusion is limited: nail changes have appeared in safety-reporting data, but a causal adverse effect has not been demonstrated. There is also no established incidence rate for nail problems among GLP-1 users and no proven biological mechanism showing that these medicines directly disrupt human nail growth.

What the official Ozempic label says
Alopecia is listed; nail changes are not in the cited passage
The current Ozempic prescribing information on DailyMed includes alopecia under postmarketing skin and subcutaneous-tissue reactions. Alopecia means hair loss, which is different from a nail disorder. The postmarketing passage cited for this question does not list nail changes.
Why a postmarketing listing does not prove causality
DailyMed explains that postmarketing reports are voluntary. The number of people exposed to the drug is uncertain, so the frequency of a reported reaction cannot be estimated reliably. A report can also involve other medicines, illnesses, nutritional changes, or unrelated events. The label therefore cautions that a voluntary report does not establish that semaglutide caused the event.
What the 2025 nail-report analysis found
The number and denominator
An April 2025 analysis in the Journal of Drugs in Dermatology tabulated 71 reports coded as nail changes across the included GLP-1 agonists. Those reports represented 0.61% of all reactions analyzed in that dataset. The result should be written precisely as 71 reports, or 0.61% of analyzed reactions—not as 0.61% of patients, users, prescriptions, or people taking GLP-1 drugs. The analysis is available as a PDF from the Journal of Drugs in Dermatology.

What spontaneous reports can and cannot show
| Question | What the analysis can show | What it cannot show |
|---|---|---|
| Did someone report a nail-related event? | Yes. Seventy-one reports were coded as nail changes among the reactions examined. | It cannot confirm that every report represented the same clinical nail condition. |
| How common are nail problems in GLP-1 users? | It identifies a safety signal worth studying. | It cannot calculate the percentage of treated patients because there is no reliable exposed-patient denominator. |
| Did a GLP-1 medicine cause the change? | It supports further investigation of a possible association. | It cannot establish causality or exclude other explanations. |
| Is one GLP-1 drug riskier than another? | It records reports associated with the included medicines. | Raw report counts cannot rank products without comparable exposure and follow-up data. |
Spontaneous-report systems are useful for detecting unusual or unexpected patterns after a medicine reaches broad use. They are not controlled clinical trials. People are more likely to report a striking symptom, reports may be incomplete, and the same person or event can be represented in different ways. Without knowing how many people used each medicine and for how long, a count cannot be converted into an individual risk.
Is there a known mechanism involving nail biology?
Not at present. A 2025 dermatology review states that no research has specifically investigated whether GLP-1 receptors are present in, or functionally relevant to, human nail biology. The review, Dermatologic Implications of Glucagon-Like Peptide-1 Receptor Agonist Medications, is available from Karger.
That evidence gap matters. A plausible mechanism can help researchers design studies, but the absence of a studied mechanism does not prove that a symptom is impossible. Conversely, a symptom occurring after treatment does not prove a direct drug effect. At this point, neither a direct nail toxicity mechanism nor a class-wide nail adverse effect has been established.

How nail changes differ from hair and skin effects
Patients may use the word skin broadly, but nails, hair, and skin are different tissues with different growth processes. The Ozempic label’s postmarketing alopecia entry concerns hair loss, not brittle nails, ridges, discoloration, separation, or changes in nail thickness. Do not treat the presence of hair-loss information as confirmation of a nail risk.
The American Academy of Dermatology (AAD) discusses possible skin, hair, and nail concerns with GLP-1 medicines and recommends involving the prescribing medical team and a board-certified dermatologist when these symptoms occur. Dermatologist Steven Daveluy, MD, FAAD, told the AAD: “We now know that GLP-1 medications can have an impact on the skin and hair. There are some skin conditions that may improve after starting a GLP-1, like psoriasis and hidradenitis suppurativa. There are other conditions, such as hair loss or facial appearance, which may get worse. Understanding these risks and benefits can help you make a more informed choice when thinking about starting a GLP-1.” This statement addresses skin and hair broadly; it is not proof of a nail-specific effect. The AAD guidance is at aad.org.
What to do if your nails change after starting treatment
1. Record the timeline and appearance
Write down the date you first noticed the change, when the GLP-1 medicine was started or adjusted, and whether the change affects one nail or several. Photographing the nails in consistent lighting can help a clinician compare progression. Note recent illnesses, injuries, manicures, occupational exposures, new medicines, and major diet or weight changes. This information does not diagnose the cause, but it prevents the appointment from relying on memory alone.

2. Contact the prescribing clinician
Tell the clinician who manages your GLP-1 prescription. Ask whether the symptom could reflect a known condition, another medicine, an underlying illness, or a local nail problem. Do not stop, restart, or change the dose on your own; the benefits and risks of altering treatment depend on why it was prescribed, your other medicines, and your health history.
3. Ask whether dermatology evaluation is appropriate
The AAD recommends partnering with a board-certified dermatologist for skin, hair, or nail side effects. A dermatologist can examine the nail plate, surrounding skin, and other nails, then decide whether testing or monitoring is needed. A nail change may have a local cause, such as trauma or infection, that is unrelated to a systemic medicine; examination is needed to distinguish possibilities.

4. Bring the complete medication and supplement list
Include prescription drugs, over-the-counter products, vitamins, and supplements, with start dates if possible. Also mention diabetes control, recent infections, inflammatory skin disease, and dietary changes. These details help clinicians assess the whole clinical picture instead of attributing a symptom to the most recently started drug.

Can you compare Ozempic with other GLP-1 drugs for nail risk?
No reliable ranking can be made from the available nail-report data. The 2025 analysis covers reports across included GLP-1 agonists, but spontaneous-report counts do not use comparable exposure denominators. A medicine with more reports could simply have been used by more people, for a longer period, or reported more often. A lower count does not establish a lower risk.
For a meaningful comparison, researchers would need consistent definitions of nail outcomes, verified treatment exposure, follow-up time, relevant medical history, and an appropriate comparison group. The cited evidence does not provide those data, so it cannot support saying that Ozempic is safer or riskier for nails than another GLP-1 medicine.
When a nail symptom deserves prompt medical attention
Arrange medical advice promptly if a nail change is painful, rapidly worsening, associated with swelling or drainage, follows a significant injury, or occurs with broader symptoms such as feeling acutely unwell. Those features do not identify a medication reaction, but they can indicate a condition that should not wait for a routine medication review. A clinician can advise how urgently you need an examination.
For a stable, painless change, document it and discuss it at the next available appointment, especially if several nails are involved or the change persists as the nail grows. Nail plates grow slowly, so an abnormal segment can remain visible for months even after the triggering event has ended.
Questions to ask at your appointment
- What nail finding do you see, and what conditions could cause it?
- Could another medicine, illness, injury, infection, or dietary change explain the timing?
- Should my GLP-1 dose be continued while this is evaluated?
- Would a dermatologist or laboratory testing help?
- What changes should prompt an earlier call?
Bottom line on Ozempic and nail problems
Reports of nail changes exist, but they are signals for investigation rather than proof that Ozempic or another GLP-1 drug caused a nail condition. The 2025 figure—71 reports and 0.61% of analyzed reactions—is not a patient incidence rate. Ozempic labeling lists postmarketing alopecia, not nail changes in the cited section, and research has not established a GLP-1 receptor role in human nail biology. Report new or persistent nail changes to your prescribing clinician and seek a board-certified dermatologist’s assessment when appropriate.
