Usually, hydrocephalus cannot be permanently corrected without some form of cerebrospinal-fluid (CSF) diversion. The realistic option depends on the subtype and cause. Medicines and CSF drainage can sometimes lower pressure or predict whether a person will benefit from a shunt, but they generally do not fix the underlying flow problem. Endoscopic third ventriculostomy (ETV) is an operation that may avoid an implanted shunt in selected obstructive cases; it is not a routine nonsurgical treatment and has very limited evidence for idiopathic normal-pressure hydrocephalus (iNPH).
Anyone with suspected or known hydrocephalus needs assessment by a clinician familiar with the condition. Rapidly worsening headache, repeated vomiting, increasing drowsiness or confusion, new visual symptoms, a sudden gait decline, or another fast neurological change requires urgent medical evaluation.
What “without surgery” can mean
People use “without surgery” to describe several different goals, and separating them prevents false expectations:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Temporary pressure reduction: removing CSF through a lumbar puncture or drain can ease symptoms for a limited time.
- Diagnostic prediction: a tap test or drainage trial can show whether symptoms improve enough to make shunt benefit more likely.
- Symptom support: physical, occupational, and continence services can improve safety and daily function without changing CSF flow.
- Medication: drugs such as acetazolamide may be considered when a person is not a surgical candidate, but they are not established as a dependable long-term replacement for diversion.
- Alternative operation: ETV creates an internal bypass around a blockage. It is still brain surgery, even though it avoids an implanted shunt.
The reviewed clinical guidance does not support supplements, special diets, consumer devices, or exercises as treatments that reverse hydrocephalus. Such measures should not delay specialist evaluation.

Why the hydrocephalus subtype changes the answer
Hydrocephalus is a CSF-flow and pressure disorder, not one single disease. The key first distinction is whether fluid is blocked within the ventricular system (obstructive hydrocephalus) or whether the problem is impaired absorption after CSF has circulated (communicating hydrocephalus, including iNPH). Imaging, the symptom pattern, medical history, and sometimes a drainage test are used together; no symptom or scan result alone establishes the best treatment.
| Subtype or situation | What a nonsurgical approach can realistically do | What usually provides durable CSF diversion |
|---|---|---|
| Obstructive hydrocephalus | Medication or temporary drainage may lower pressure while the cause is evaluated. ETV can bypass a suitable blockage in selected patients. | ETV or a shunt, chosen according to the location and cause of obstruction and the patient’s risks. |
| Communicating hydrocephalus | Lumbar puncture or external drainage can provide temporary relief and help predict shunt response; supportive therapies address function. | Often a shunt when symptoms, imaging, and testing support benefit. |
| Idiopathic normal-pressure hydrocephalus (iNPH) | Tap tests, repeated lumbar punctures, or external lumbar drainage are mainly assessment tools. Medication is not a proven cure. | CSF shunting remains the best-supported disease-directed treatment, with risks that must be weighed individually. |
Medication: a limited option, not a cure
Acetazolamide and related drugs
Carbonic-anhydrase inhibitors such as acetazolamide reduce CSF production. A clinical review cited for this topic places these medicines in a narrow role for people who are not surgical candidates. That wording matters: it does not establish medication as a permanent fix, nor as a reliable substitute for CSF diversion when hydrocephalus is causing progressive problems.
A clinician must decide whether the potential pressure-lowering effect is appropriate, taking account of the hydrocephalus cause, kidney function, electrolyte risk, other medicines, and the possibility that symptoms reflect a different or worsening neurological problem. Do not start, stop, or change acetazolamide without a prescriber. If symptoms progress while taking it, the response should be treated as a reason for reassessment, not as evidence that a higher dose will solve the underlying obstruction or absorption failure.

Why medication often cannot address the mechanism
Hydrocephalus can persist because CSF has nowhere to flow or cannot be absorbed adequately. Lowering production may reduce pressure temporarily, but it does not open a blocked channel or restore absorption. For that reason, medication is generally a bridge, a fallback for selected nonsurgical candidates, or part of care for a specific underlying condition rather than definitive hydrocephalus treatment.
Lumbar puncture and lumbar drainage
One-time lumbar puncture (“tap test”)
A lumbar puncture removes a measured amount of CSF through the lower back. In suspected iNPH, clinicians may assess walking, balance, cognition, bladder symptoms, or other functions before and after the procedure. A clear, reproducible improvement can increase the likelihood that a shunt will help.
A tap test is not a cure. Its effect, when present, is usually temporary because CSF continues to be produced. A negative result also does not reliably exclude shunt benefit; the American Academy of Neurology (AAN) guideline notes that a person may still respond despite a negative tap test. Testing conditions, the amount removed, timing of reassessment, baseline disability, and coexisting illnesses can all affect the result.

External lumbar drainage
An external drain removes CSF repeatedly over a monitored period, usually in a hospital or specialist setting. It can reveal improvement that a single tap test misses and is sometimes used when the initial test is equivocal. Because a catheter remains in place, it is an invasive procedure with infection, bleeding, over-drainage, and other procedural risks; it is not an at-home alternative to a shunt.

They help a team decide whether the expected benefit of durable diversion justifies the risks of an implanted shunt. They do not permanently correct hydrocephalus or guarantee a particular outcome.
Endoscopic third ventriculostomy: an alternative to a shunt, not to surgery
ETV uses an endoscope to create an opening in the floor of the third ventricle, allowing CSF to flow around an internal blockage. NHS guidance says ETV may be an option when CSF buildup is caused by a blockage, but it is not suitable for everyone. Suitability depends on the obstruction, anatomy, prior procedures, age and clinical condition, and the surgeon’s assessment.

ETV can avoid implanted hardware and the lifelong shunt pathway, but it still carries operative risks and can fail if the opening closes or does not provide enough flow. A person may need urgent reassessment if symptoms recur after ETV. The procedure should therefore be discussed as a different form of CSF diversion, not as a nonsurgical treatment.
ETV in idiopathic normal-pressure hydrocephalus
Classic iNPH is not usually caused by a single blockage that ETV can bypass. It is therefore not routine therapy for iNPH. A Cochrane review found very-low-quality evidence for ETV in iNPH and concluded that patients and doctors should recognize the limitations of the evidence on its effectiveness and safety. Shunting has substantially stronger guideline support for appropriately selected iNPH patients.

What the evidence says about shunting in iNPH
For iNPH, the AAN guideline (2015) reports a 96% chance of subjective improvement and an 83% chance of improvement on a timed walk test at six months in the reviewed study populations. It also reports an 11% risk of serious adverse events. These figures are evidence estimates from selected studies, not guarantees for an individual patient; outcomes can differ with age, comorbidities, symptom duration, frailty, imaging findings, and baseline function.

The AAN recommendation is that clinicians may offer shunting for subjective iNPH symptoms and gait while carefully weighing the significant adverse-event risk. A positive response to external lumbar drainage or repeated lumbar punctures can increase the chance of benefit, but a negative test should not automatically rule out treatment.
The 2021 Japanese iNPH guideline states that CSF shunt intervention is effective and recommends a programmable-pressure valve. It says an anti-siphon or gravity mechanism can be added when intracranial hypotension is detected. Valve choice and settings require follow-up; they are not self-management decisions.

- Physical therapy: gait, balance, strength, and transfer practice can reduce falls and preserve mobility.
- Occupational therapy: home-safety changes, adaptive techniques, and help with daily activities can reduce injury risk.
- Continence support: scheduled toileting, skin care, and appropriate supplies can protect comfort and independence.
- Cognitive and caregiver support: routines, medication supervision, and monitoring for functional change can help families respond early.
These services should be presented honestly: they may improve function and safety but do not remove excess CSF, reopen an obstruction, or replace specialist treatment.
How specialists decide whether an intervention is appropriate
- Confirm the clinical problem. The team reviews symptom timing and progression, neurological examination, prior illnesses and procedures, medications, and conditions that can mimic gait, cognitive, bladder, headache, or visual symptoms.
- Characterize the hydrocephalus. Brain imaging is interpreted for ventricular enlargement, a possible obstructing lesion, and features that support or weaken an iNPH diagnosis. The subtype determines whether ETV is even anatomically plausible.
- Estimate reversible benefit. For suspected iNPH, baseline and post-drainage measures may include a timed walk and observed functional change. A positive response supports—but does not prove—future shunt benefit.
- Compare procedures and risks. The discussion should cover ETV versus shunt when obstruction is present, programmable valve options, the possibility of over-drainage or under-drainage, infection, bleeding, obstruction, revision, and the person’s surgical and anesthesia risk.
- Plan monitoring. Any diversion strategy needs follow-up for symptom response, valve adjustment when applicable, recurrence, and complications. A plan for urgent review should be explicit before leaving care.
Common misconceptions and safer interpretations
- “A lumbar puncture cured it.” Improvement after CSF removal is usually temporary and is primarily useful for prediction.
- “No improvement after the tap test means no shunt.” A negative test does not reliably exclude benefit, according to the AAN guidance.
- “ETV means no operation.” ETV is an operation; its advantage is avoiding an implanted shunt in selected obstructive cases.
- “Normal-pressure means normal risk.” The name refers to pressure patterns in iNPH, not an absence of disease or procedural risk.
- “Supplements or exercise can reverse hydrocephalus.” The reviewed evidence does not support consumer products, supplements, diet plans, or exercises as treatments that correct CSF circulation.
When symptoms need urgent assessment
Seek urgent medical care for rapidly worsening headache, repeated vomiting, increasing sleepiness, confusion, a new or quickly worsening visual problem, sudden deterioration in walking, weakness, seizures, or another abrupt neurological change. These signs can have many causes, and they cannot diagnose hydrocephalus by themselves, but delaying assessment can be dangerous in someone with known or suspected CSF obstruction or pressure problems.
The practical answer is therefore conditional: medication and drainage have narrow or temporary roles; ETV may replace a shunt only for selected obstructive hydrocephalus and remains surgery; and for appropriately evaluated iNPH, shunting has the strongest disease-directed evidence despite meaningful risks. A neurosurgical or neurological team should match the intervention to the subtype, imaging, test response, comorbidities, and the person’s goals.
