No. MRI and PET scans cannot currently confirm or rule out depression in an individual. Depression is diagnosed through a clinician’s assessment of symptoms, their duration and course, severity, effect on daily life, personal and medical history, and possible alternative or coexisting conditions.
Scans are valuable in research and may be ordered for a separate neurological concern, but a research finding or a clinical scan result is not, by itself, a depression diagnosis.
Why a brain scan cannot diagnose depression on its own
Depression does not have one visible lesion, chemical level, or pattern of brain activity that reliably separates every person with depression from every person without it. MRI shows anatomy and, with specialized methods, aspects of brain function. PET measures the distribution of a radioactive tracer to study metabolism or particular biological targets. Those measurements can be scientifically informative without being accurate enough for routine diagnosis of one patient.
The National Institute of Mental Health (NIMH) describes depression diagnosis as a provider’s evaluation of persistent symptoms over time. Its Research Domain Criteria FAQ states: “Currently, the diagnosis of mental disorders is based on clinical observation—identifying symptoms that tend to cluster together.” That is different from diagnosing a broken bone or a tumor, where an image may directly demonstrate the condition.

A scan may look different in a person who has depression, but a difference observed in a group of research participants does not automatically identify the disorder in an individual. Brain structure and activity vary naturally, and findings can be influenced by age, medication, sleep, other illnesses, the type of depression being studied, and the scanner or analysis method. A normal scan therefore does not disprove depression, and an unusual finding does not prove it.
What a depression assessment actually examines
Guidance from the National Institute for Health and Care Excellence (NICE) says clinicians should not rely simply on counting symptoms. They should build a comprehensive picture and confirm that the diagnosis fits the person’s presentation.
Symptoms and their pattern
The clinician asks which emotional, cognitive, physical, and behavioral changes are present; when they began; how often they occur; and whether they are improving, worsening, or recurring. The duration and course matter because a short-lived reaction, a persistent depressive episode, and a fluctuating pattern can require different interpretations and treatment plans.
Severity and day-to-day function
Assessment includes the intensity of symptoms and their effect on work, school, relationships, self-care, sleep, appetite, concentration, and other activities. Two people with a similar symptom count can have very different levels of impairment, risk, and support needs.
Previous episodes and treatment history
Past depressive episodes, hospitalizations, psychotherapy, medication trials, responses, side effects, and periods of recovery help the clinician understand the current episode and choose next steps. Family history can also add context, although it cannot establish a diagnosis by itself.

Coexisting conditions and alternative explanations
NICE recommends considering coexisting mental and physical conditions. Anxiety, trauma-related symptoms, substance use, sleep disorders, chronic pain, and other conditions can occur alongside depression or resemble parts of it. A clinician also considers whether medication effects or a medical illness could be contributing.
Past mood elevation and possible bipolar disorder
Questions about past periods of unusually elevated or irritable mood, increased energy, reduced need for sleep, impulsive behavior, or unusually rapid activity are important. A history suggesting bipolar disorder changes the diagnostic approach and can alter which treatments are safe. Depression should not be labeled in isolation from the person’s broader mood history.

Safety and immediate needs
A responsible assessment includes questions about hopelessness, self-harm, suicidal thoughts, access to means, psychosis, severe inability to care for oneself, and personal safety. These questions are part of clinical care, not something a scan can answer.
How current tests differ in purpose
These approaches are complementary, not interchangeable. Only the first involves the comprehensive diagnostic judgment; the others support assessment, investigate alternatives, or answer research and medical questions.

What questionnaires can—and cannot—tell you
Clinicians may use a validated symptom scale to organize questions, estimate current severity, and compare scores over time. NICE says a validated measure may help inform assessment and evaluate treatment. A score can identify that someone is reporting substantial symptoms or that symptoms have changed, but it does not establish the cause.

A questionnaire cannot determine whether symptoms arise from depression, bipolar disorder, grief, a medication effect, thyroid disease, another medical condition, or several conditions at once. It also cannot assess every element of risk, functioning, history, or context. Completing a form online can be a useful prompt for seeking care, but it is not a diagnosis and should not be used to decide independently whether treatment is needed.

MRI research
MRI can provide detailed structural images without ionizing radiation. Research protocols may also examine patterns of connectivity or activity. Investigators compare groups, look for biological subtypes, and test whether measurements might eventually help predict treatment response. A research scan is designed to answer a scientific question across participants, not to issue a clinical label to the person in the scanner.
PET research
PET uses a radioactive tracer to measure selected biological processes. Depending on the tracer and protocol, researchers can study metabolism or specific molecular targets. PET is more specialized than routine structural imaging and is not a standard diagnostic test for depression.
The NIMH position on current studies
NIMH’s “Brain Imaging and Depression Studies” page describes MRI and PET as tools for learning more about the brains of people with depression and for exploring whether doctors might match treatments to people in the future. The study description does not present either scan as a test offered to determine whether an individual has depression.
What the 2018 APA review concluded
The American Psychiatric Association’s 2018 “Resource Document on Neuroimaging” reported that, in its review, no brain-imaging biomarkers were clinically useful for psychiatric diagnostic categories. It also concluded that imaging had not significantly changed diagnosis or treatment for individual patients at that time. This is dated review context, not a claim that neuroscience has stopped or that imaging lacks value for other medical conditions.

When a clinician might order brain imaging anyway
A person with depressive symptoms may receive an MRI or another scan because the presentation raises a separate neurological or medical question. Examples include new focal neurological findings, seizures, a significant change in cognition, an unusual headache pattern, head injury, or other signs that require investigation. The exact decision depends on the examination, history, age, risk factors, and local clinical guidance.
In that situation, the scan is looking for the suspected neurological problem. It is not being used as a depression detector. If the scan is normal, depression can still be present. If it reveals an abnormality, that finding must be interpreted in its own clinical context and does not automatically explain every mood symptom.
How medical causes are checked
NIMH notes that some medications and medical conditions, including thyroid disorders, can cause symptoms that resemble depression. A clinician may therefore combine an interview with a physical examination and laboratory tests when the history or examination makes that appropriate. This is targeted evaluation, not a universal requirement and not a blood test that diagnoses depression.
Tell the clinician about prescription drugs, over-the-counter products, supplements, alcohol or other substances, major sleep changes, chronic illnesses, hormonal or neurological symptoms, and recent life events. Those details help determine whether additional medical testing is useful and whether more than one problem may be present.

What to expect at an appointment
- Describe the change that brought you in. Note when symptoms started, whether they are constant or episodic, and what has changed in sleep, appetite, energy, concentration, enjoyment, movement, or daily responsibilities.
- Review your history. Include prior mood episodes, treatments, family history, medical conditions, medications, substances, and any periods of elevated or unusually activated mood.
- Complete structured questions if offered. A validated scale can provide a baseline and make follow-up more consistent, but discuss the answers rather than treating the score as a verdict.
- Discuss safety directly. Tell the clinician about suicidal thoughts, self-harm urges, psychotic experiences, or inability to stay safe. If there is immediate danger, contact local emergency services or a crisis service rather than waiting for a scan or routine appointment.
- Agree on next steps. The plan may involve monitoring, psychotherapy, medication, lifestyle and support measures, targeted medical tests, referral, or urgent care depending on the assessment.
How to interpret a scan result reported during a depression work-up
If the report is normal
A normal MRI or PET does not rule out depression. Most of the diagnostic information comes from the clinical assessment, not from the absence of a visible brain abnormality.

If the report mentions a brain difference
Ask what the finding means for the specific reason the scan was ordered, whether it is common and incidental, and what follow-up is recommended. A group-level research association or an incidental clinical finding is not proof that depression caused it or that it explains every symptom.
If someone offers a “brain-based depression test”
Ask whether it has been validated for routine use in individuals, whether an independent clinical guideline recommends it, and whether its result changes an established treatment decision. Claims that a consumer device or a single scan can definitively diagnose depression go beyond the current evidence described by NIMH, NICE, and the APA review.
Why promising research has not become a routine diagnostic scan
To work as a clinical test, a biomarker must perform reliably in new people, distinguish depression from similar conditions, account for meaningful differences among patients, and improve decisions compared with a careful assessment alone. A statistically different average between research groups is only an early step. Studies may use different scanners, tracers, image-processing methods, participant definitions, medications, and comparison groups, making results difficult to apply as one universal test.
Researchers are investigating whether combinations of brain measures, symptoms, genetics, sleep, and other data could eventually support treatment matching. “Could support” is important: a future tool would still need rigorous validation, regulatory and clinical acceptance, and evidence that it benefits patients. That possibility does not make today’s research images diagnostic for an individual.
When to seek help
If low mood, loss of interest, sleep or appetite changes, fatigue, concentration problems, guilt, hopelessness, or other concerning symptoms persist or interfere with life, arrange an assessment with a qualified health professional. You do not need to obtain a scan first. Bring a symptom timeline and medication list, and mention any medical or neurological symptoms.
If you may hurt yourself or cannot stay safe, seek immediate help through local emergency services, an emergency department, or a crisis line available in your country. A brain scan cannot provide the urgent safety assessment or support you need.
