Yes—sometimes. In selected childhood leukemias, scientists have found the leukemia-associated DNA sequence in blood spots collected at birth, years before the child was diagnosed. That finding shows a cancer-associated cell clone was already present before birth; it does not mean a fully developed, symptomatic cancer or tumor was present in the newborn.
What “started before birth” actually means
For cancer biology, “started” can refer to several different milestones. A genetic change may arise in one cell before birth, creating a small preleukemic clone. That clone can remain clinically silent while it acquires additional changes. Only later might enough changes accumulate for leukemia to become detectable by symptoms, blood counts, or diagnostic testing.
Therefore, finding a leukemia-associated sequence in a newborn sample is evidence of prenatal initiation—the lineage carrying the initiating change existed by birth. It is not proof that the child had an established mass of cancer cells, abnormal blood counts, or a diagnosis at that time. The distinction matters because initiation and diagnosis can be separated by months or years.
How researchers detect a prenatal cancer-associated clone
Archived newborn blood spots
Many newborns have a few drops of blood collected on filter paper shortly after birth for screening. These cards, often called Guthrie cards, may be archived. After a child is diagnosed with a particular leukemia, researchers can identify a molecular marker unique to that leukemia and look for the same marker in the stored birth sample.

Patient-specific molecular testing
In the reported studies, polymerase chain reaction (PCR) testing searched for a fusion sequence created by a leukemia-associated chromosome rearrangement. A matching sequence in the birth-card DNA means that at least one cell carrying that molecular change was present by birth. The approach is retrospective: the later leukemia diagnosis reveals which sequence to seek in the earlier specimen.
This is different from testing every newborn for every possible cancer marker. The method depends on an archived sample, a later diagnosis, and a marker that can be measured reliably in that individual’s disease.
What the leukemia studies found
1997: MLL-AF4 sequences in children with acute lymphoblastic leukemia
A 1997 primary study examined children who later developed acute lymphoblastic leukemia (ALL) with an MLL-AF4 fusion. The children were diagnosed between 5 months and 2 years of age. Researchers detected patient-specific MLL-AF4 sequences in their archived newborn blood spots, providing direct evidence that the leukemia-associated clone existed before birth in those cases.

The result supports prenatal initiation of a leukemia lineage. It does not establish that every child with ALL has a prenatal origin, nor that the entire leukemia population was already formed at birth.

2002: t(12;21) in B-precursor ALL
A 2002 study looked at nine children with B-precursor ALL carrying the t(12;21) chromosome translocation. Clone-specific fusion sequences were found in the birth-card DNA of three children. Their diagnoses occurred at 2 years 11 months, 3 years 4 months, and 5 years 8 months.
These results show that a preleukemic clone can persist for several years before clinical leukemia is diagnosed. Because the study involved only nine children and three positive birth samples, it supports prenatal initiation in those cases rather than a universal rule for B-precursor ALL or childhood cancer generally.
What each type of evidence can—and cannot—show

Why a prenatal clone may not cause leukemia until years later
Reviews of childhood ALL describe a multistep process. An initial chromosome rearrangement can create a preleukemic cell population during fetal development. That population may be too small or biologically incomplete to produce disease. Additional genetic alterations, changes in cell regulation, or other influences may be needed before leukemia becomes clinically apparent.

This model explains why a child can have a detectable leukemia-associated sequence at birth and still be diagnosed at age 2, 3, 5, or later. The birth sample records the presence of the initiating lineage, not the future pace of its evolution. A positive retrospective result also cannot by itself predict whether a clone would ever have progressed to leukemia.

Embryonic mosaicism is related, but it is not the same claim
Mutations can arise very early in embryonic development and therefore occur in only a subset of the body’s cells. This pattern is called mosaicism. If the affected cells include tissues involved in cancer susceptibility, the mutation may raise the person’s risk of developing certain cancers later.
The National Cancer Institute described a 2022 study of more than 35,000 people with cancer in which 36 had mosaic mutations in cancer-susceptibility genes. The investigators estimated a frequency of about 1 in 1,000 in that cohort, while noting that the true frequency could be higher. That figure applies to the selected clinical cohort and genes studied; it is not the proportion of all cancers that began before birth.
Mosaicism can be important when clinicians interpret genetic results and decide how to monitor or treat a patient. However, a susceptibility mutation is an inherited-at-the-cell-lineage level risk factor, not proof that the person’s eventual tumor was already present at birth. Establishing tumor origin requires disease-specific evidence, such as matching a tumor-associated molecular sequence in an earlier sample.

The strongest direct examples in this evidence base involve particular subtypes of childhood ALL, including cases with MLL-AF4 or t(12;21)-related fusion sequences. Reviews discuss developmental origins across childhood cancers, but archived birth-sample proof is not equally available for every cancer.

You should not generalize these leukemia findings to all pediatric cancers, adult cancers, or cancer as a whole. Different cancers arise through different genetic and environmental pathways, and many do not have a distinctive marker that can be retrospectively sought in a stored newborn card.
Can a person be tested years later?
In principle, a later diagnosis can enable retrospective testing if an archived newborn blood spot still exists and the cancer has a measurable, patient-specific marker. The laboratory would compare DNA from the birth card with the molecular abnormality identified in the leukemia.
Several practical limits apply:
- Not every newborn blood spot is retained for the same length of time, and access rules vary by jurisdiction and health system.
- The test is useful only when the later cancer has a marker that the assay can detect in the small amount of DNA available.
- A negative result may reflect sample quality, limited DNA, or an assay that does not capture every cell lineage; the cited studies do not establish general sensitivity or specificity.
- Interpretation requires the treating hematologist, a qualified molecular laboratory, and, when hereditary or mosaic risk is suspected, clinical genetics expertise.
The published work does not validate a routine screening test for healthy newborns, healthy adults, or consumers who want to determine whether a cancer began prenatally. It demonstrates how origin can be investigated after a disease has been diagnosed.
How to interpret a result in clinical care
If a birth sample contains the matching sequence
A matching sequence supports the conclusion that the leukemia-associated clone was present by birth. Clinicians may use that information to understand disease biology and, where relevant, to distinguish an early initiating event from later changes. It does not indicate that the child should have been diagnosed at birth or that the disease was clinically preventable then.

If no matching sequence is found
A negative result does not prove that the cancer began after birth. The marker may not have been present in the sampled cells, the DNA may have degraded, or the assay may not have been sensitive enough. A negative finding must be interpreted with the laboratory method, specimen quality, and the specific leukemia subtype in mind.
If a mosaic susceptibility mutation is identified
That result can prompt a genetics consultation to clarify cancer risks, family implications, and appropriate monitoring. It should be discussed as a predisposition finding unless separate evidence demonstrates that the person’s tumor clone was already present in a prenatal or newborn specimen.
Common misunderstandings
- “A prenatal origin means cancer was visible at birth.” No. The evidence can show a molecularly defined clone without symptoms, a mass, or a clinical diagnosis.
- “If one leukemia subtype can begin before birth, all cancers do.” No. The direct evidence is concentrated in selected childhood leukemias.
- “A mutation found in some embryonic cells proves the tumor was prenatal.” No. Mosaic susceptibility and direct proof of a tumor-associated clone are different findings.
- “A newborn blood spot can routinely predict future cancer.” No. The studies used retrospective, disease-specific testing after diagnosis and did not establish population-screening performance.
- “A positive birth-card result means the cancer was inevitable.” No. A preleukemic clone may require additional events before disease develops, and the cited evidence does not show that every such clone progresses.
Questions to ask a clinician or genetics team
If you or a family member has a leukemia with a characteristic fusion or an unusual mosaic genetic finding, useful questions include:

The practical answer
Some childhood leukemias can be traced molecularly to before birth by finding the same leukemia-associated sequence in an archived newborn blood spot. That is evidence of prenatal initiation of a cell clone, not evidence that a fully developed cancer was present or diagnosable at birth. The conclusion is subtype-specific, based on relatively small retrospective studies, and should not be extended to all cancers. Embryonic mosaic mutations add a separate way that early development can influence later cancer risk, but they do not by themselves establish prenatal origin of an individual tumor.
