AI-assisted breast screening can help find more cancers in some clinical workflows, but it has not been shown to carry a patient from a mammogram to a better, individualized treatment plan—or to close mortality and access gaps. The strongest evidence so far concerns specific screening tasks, with trade-offs and limits that matter to patients.

Can AI detect breast cancer on a mammogram?

Yes. AI can analyze mammogram images and support clinicians in identifying suspicious findings. Evidence from several countries suggests that AI-supported workflows can raise cancer detection in particular screening settings. That does not mean every AI system works the same way, or that an AI result is a diagnosis: the role of the software depends on its intended use and how it is incorporated into clinical care.

United States: more cancers detected, with a small rise in recalls

The 2025 US ASSURE study evaluated an AI-supported workflow for digital breast tomosynthesis across five practices and 109 sites. It compared 208,891 exams in the AI workflow with 370,692 earlier exams under standard care. The workflow used AI detection and diagnosis, with a breast imaging radiologist providing safeguard review for selected cases. The investigators reported 5.6 cancers detected per 1,000 exams in the AI workflow, compared with 4.6 per 1,000 under prior care—a 21.6% relative increase. The comparison was a prospective deployment against a historical standard-care period, not a randomized trial, so the difference cannot be attributed to AI alone. ASSURE study, Nature Health.

The additional detections came with more follow-up: the recall rate was 11.1% in the AI workflow versus 10.6% previously. Among recalled exams, positive predictive value—the proportion that were cancer—was 5.0% versus 4.4%. These figures describe the study’s workflow and population; they do not establish what another system or screening program would achieve.

Germany: an observational double-reading study

In the 2025 PRAIM study, organized screening for women ages 50–69 used radiologist double reading, with AI-supported reading voluntarily adopted at some sites. Cancer detection was 6.7 per 1,000 women screened in the AI-supported group and 5.7 per 1,000 in the control group, a 17.6% higher rate in the AI group. Because the implementation was observational and AI use was voluntary, the groups may have differed in ways that influenced results. The German double-reading setting also differs from US programs that commonly use single reading. PRAIM study, Nature Medicine.

United Kingdom: retrospective accuracy and feasibility evidence

A 2026 evaluation assessed Google mammography AI version 1.2 retrospectively on 115,973 mammograms and also conducted a noninterventional feasibility deployment. In the retrospective comparison with first-reader results, sensitivity was 0.541 for AI versus 0.437 for the first reader, while specificity was 0.943 versus 0.952. The authors reported higher sensitivity and noninferior specificity, and the study also found some interval cancers. These are retrospective accuracy results plus a feasibility deployment, not randomized evidence that the system prevents deaths. Nature Cancer evaluation.

Can AI miss breast cancer?

Yes. Mammography interpretation is not perfect, and an AI result cannot rule cancer in or out on its own. A screening mammogram may be followed by diagnostic imaging or biopsy when a finding needs investigation; a negative result also does not guarantee that cancer is absent. The UK evaluation’s sensitivity figures show why sensitivity is not the same as certainty: even in a study reporting higher sensitivity for AI than for the first reader, the measure was below 1.0. The study’s findings should not be generalized to other AI products or populations.

AI’s place in the workflow matters. In ASSURE, selected cases received a radiologist safeguard review; other deployments use AI as a second reader or as a tool for selecting people for additional imaging. Clinicians remain responsible for interpreting results in context and arranging appropriate follow-up.

Does AI work better for dense breasts?

Dense breast tissue can make mammograms harder to interpret, so researchers are testing whether AI can help identify people who may benefit from additional screening. The evidence supports investigation of targeted approaches, not a blanket conclusion that AI works better for everyone with dense breasts.

AI selection for supplemental MRI in Sweden

In the randomized ScreenTrustMRI study, an AI score was used to select a small, high-risk group after a negative screening mammogram for a supplemental MRI trial. The paper reported 64 cancers detected per 1,000 MRI examinations, compared with 16.5 per 1,000 in an earlier trial that used breast-density measures to select participants. This was a comparison across trials, not a randomized head-to-head comparison of AI selection against density-based selection. The primary outcome—advanced cancer after follow-up—was not yet reported in the article, so the detection result does not demonstrate a survival benefit. ScreenTrustMRI study, Nature Medicine.

In ASSURE, investigators found no measured differences in cancer detection, recall, or positive predictive value across the racial and breast-density subgroups they studied. That is useful evidence about performance in those subgroups in that deployment; it does not establish that AI performs equally across all populations or health systems.

Does AI help doctors decide treatment?

AI research may eventually help doctors interpret pathology or combine patient information, but the evidence described here does not show that a mammography AI system independently produces a validated treatment plan. Screening and treatment are different stages of care, and a tool designed for one should not be assumed to perform the other.

After a diagnosis, treatment decisions draw on a wider clinical picture, including cancer stage, histology, grade, estrogen and progesterone receptor status, HER2 status, and, when appropriate, gene-profile testing. The National Cancer Institute describes AI work that predicts outcomes from digital pathology images and integrates patient data as research and potential decision support—not as proof that AI can replace clinicians in determining an individual’s treatment. Can AI reduce differences in breast cancer outcomes?

It may help improve one part of the screening process, but current evidence does not establish that AI has closed breast cancer’s broader outcome gaps. In ASSURE, the measured screening performance did not differ across the specified racial and density subgroups. The study did not show that AI changed who was screened, who received timely diagnostic follow-up, who could access treatment, or who experienced lower mortality. Those outcomes depend on care beyond image interpretation.

For the same reason, a higher detection rate should not be described as a proven reduction in deaths. Detection is an important intermediate outcome; survival and equity require evidence about what happens to patients after screening, including diagnosis, follow-up, and treatment.

How to judge an AI claim in breast cancer care

“AI for breast cancer” can refer to different software doing different jobs. When assessing a claim, check the clinical step, the workflow, the outcome measured, and the population studied.

  • Clinical step: Is the tool interpreting a screening mammogram, selecting people for supplemental MRI, analyzing pathology, estimating risk, or recommending treatment?
  • Workflow: Does AI act as a second reader, a safety-net review, or a score used to select patients for further testing? Does a clinician review the result?
  • Evidence and endpoint: Was the evidence randomized, observational, prospective, or retrospective? Did it measure detection, recalls, interval cancers, workflow burden, or patient outcomes?
  • Population and setting: Which country, screening program, age group, imaging modality, and single- or double-reading approach were studied?
  • Equity: Were results examined across density or racial and ethnic subgroups? Did the study measure only screening performance, or also follow-up, treatment access, and mortality?

These distinctions help separate evidence that a tool can improve a particular screening measure from claims that it improves a patient’s overall outcome.

What FDA oversight means for clinical AI

In the United States, regulatory status and intended use matter. FDA classifies a pathology software algorithm that analyzes digitized images and provides risk estimates to assist physicians as a medical device; the classification page says the software is not intended to determine a clinical diagnosis. FDA pathology software algorithm device classification.

FDA’s clinical decision-support policy also distinguishes tools that support health professionals from software intended for patients or caregivers. Its policy navigator states: “Software functions that are intended for patients or caregivers do not meet the criteria in section 520(o)(1)(E) of the FD&C Act since the software function is not limited to supporting or providing recommendations to health care professionals.” The agency notes that software analyzing medical images or providing a specific patient treatment plan may be subject to device oversight.
