Osteomyelitis is an infection of bone. It can cause pain, tenderness, warmth, swelling, drainage, or fever, but symptoms alone cannot confirm it. The right way to investigate and treat it depends on where the infection is, how it began, how long it has been present, and the person’s health; diabetic-foot infections and infections of the spine, for example, follow distinct clinical pathways.

What is osteomyelitis?

Osteomyelitis is the medical term for an infection in bone. It is not one uniform condition: an acute infection, a longstanding infection, infection extending from a diabetic foot wound, and infection in the vertebrae can call for different tests and treatment decisions. The suspected cause, location, duration, and a person’s other health conditions all matter.

Possible signs include localized bone pain or tenderness, warmth, redness, swelling, drainage, fever, or reduced mobility. Not everyone has the same symptoms, and these signs can occur with other conditions. A clinician needs to assess the whole picture rather than diagnose osteomyelitis from symptoms alone.

How do clinicians investigate a possible bone infection?

Assessment is tailored to the suspected site and clinical circumstances. Clinicians may use imaging to look for bone changes and determine the extent of infection, alongside blood tests and microbiological tests to help identify an organism. A normal early X-ray does not always settle the question.

Imaging and X-rays

The American College of Radiology’s 2022 imaging criteria describe MRI as highly sensitive for osteomyelitis and discuss alternatives when MRI or CT cannot be obtained. The appropriate study depends on the patient and the clinical question.

Diabetic foot wounds

For a person with diabetes and a local infection or deep or chronic foot wound, NICE advises clinicians to consider osteomyelitis. Normal inflammatory markers, a normal X-ray, or a negative probe-to-bone test does not necessarily exclude it. NICE states: “If osteomyelitis is suspected in a person with diabetes but is not confirmed by initial X‑ray, consider an MRI to confirm the diagnosis.” This recommendation is specific to diabetic-foot care, not a universal testing rule for every bone infection.

Samples to identify the organism

A blood culture, tissue sample, or bone sample may help identify the bacteria or other organism involved and guide treatment. Whether a bone biopsy is useful, and when to obtain one, depends on the site, imaging results, whether surgery is planned, and whether another suitable microbiological result is already available.

For chronic nonspinal osteomyelitis in adults, a Society of Academic Bone Radiologists consensus describes radiography and MRI as initial steps when feasible. It discusses image-guided bone biopsy particularly when MRI is positive and surgery or another microbiological source is unavailable. This is not a rule that every suspected case needs biopsy.

Infection in the spine

Native vertebral osteomyelitis is infection affecting the vertebrae in a person without a spinal implant. For adults with suspected disease, the Infectious Diseases Society of America’s 2015 guideline recommends image-guided aspiration biopsy when clinical, laboratory, and imaging findings support suspicion and an organism has not already been established through specified blood cultures or serologic tests.

The guideline says that when a diagnostic sample is needed, clinicians may defer empiric antimicrobial treatment in some stable situations until the sample is obtained. That does not apply as a blanket instruction: urgent instability or neurologic compromise changes the urgency and requires prompt clinical management. The guideline reports that microbiologic diagnosis was established after aspiration in 50%–90% of patients in cited studies when patients were off antibiotics. That range concerns aspiration for native vertebral osteomyelitis in those studies; it is not a general success rate for all bone biopsies.

Why do the type and location change the pathway?

Clinical setting What shapes evaluation and care
Acute or chronic nonspinal infection Duration, imaging findings, available cultures or tissue samples, and whether surgery is indicated influence the workup. Adult chronic nonspinal cases may involve multidisciplinary assessment.
Diabetic-foot-associated infection A deep or chronic wound or local infection can raise suspicion. Normal initial tests do not necessarily exclude osteomyelitis; NICE advises considering MRI if it remains suspected after an initial X-ray.
Native vertebral osteomyelitis in adults Blood culture and serologic results, imaging, clinical stability, and neurologic status affect whether image-guided aspiration biopsy is needed and how urgently treatment proceeds.

These categories overlap in real patients. A clinician chooses tests and treatment for the actual presentation rather than relying on the category name alone.

How is osteomyelitis treated?

Treatment is individualized. It commonly involves antimicrobial medicines selected according to the suspected or identified organism, infection site, and patient factors. Some cases also need surgical debridement or another procedure. In chronic nonspinal disease, specialist consensus supports multidisciplinary care involving the relevant clinical teams.

Antibiotic choices and durations in local protocols are not universal prescriptions. For example, NHS Highland guidance is limited to acute osteomyelitis and septic arthritis caused by hematogenous spread and excludes several other settings; it advises early specialist input and says definitive treatment should usually be guided by aspirate or bone biopsy where possible. NHS Lothian guidance calls for blood cultures and bone or tissue samples, discourages superficial swabs, and notes that debridement may be needed. These are local clinical protocols, not instructions for self-treatment or substitutes for advice from a treating clinician.

When should someone seek medical assessment?

Prompt medical assessment is appropriate for a suspected bone infection, particularly when localized pain or tenderness occurs with warmth, swelling, drainage, or fever, or when a person has a deep or chronic diabetic foot wound. Symptoms cannot establish the diagnosis, and the right next step depends on the individual’s condition. Severe or rapidly worsening symptoms, or new neurologic problems with possible spinal infection, warrant urgent care.

Osteomyelitis is assessed and treated through clinical evaluation, imaging, and sometimes cultures or biopsy, with antibiotics and occasionally surgery. Consumer products cannot diagnose or treat a bone infection.
