Neither body oil nor stretch-mark cream has been shown to reliably prevent pregnancy stretch marks. The best available synthesis, a 2022 Cochrane review of six trials involving 800 women, found no high-quality evidence that topical preparations prevent them. Choose an oil or cream for comfort, softness, and a routine you can tolerate—not because one format has a proven preventive advantage.
Pregnancy stretch marks (striae gravidarum) are common and partly biological. The NHS estimates that around eight in 10 pregnant women develop them. They often become paler after birth, but no topical product can promise to remove them completely. A heavy moisturizer may keep skin comfortable and soft, as the American College of Obstetricians and Gynecologists (ACOG) explains, but that is different from preventing or erasing marks.
The short answer: pick the texture you will actually use
For prevention, oil and cream are effectively tied: neither has dependable evidence behind it. Trials have examined products such as cocoa butter, olive oil, vitamin-E preparations, and hyaluronic-acid products without establishing a reliable winner. A slippery oil may suit someone who likes massage and does not mind residue; a cream may be easier to spread quickly, dress over, or apply to a larger area. Those are practical differences, not proof of different clinical effects.

If your skin feels tight or itchy, a bland, fragrance-free moisturizer can be a reasonable comfort measure. Check the entire ingredient list, avoid retinoids while pregnant, and ask your obstetric clinician or dermatologist about unfamiliar active ingredients. Do not pay a premium for a product that guarantees prevention, removal, or a particular percentage reduction: the available evidence does not support those promises.

Why pregnancy stretch marks happen
Stretch marks are a type of scar-like change that develops when skin is stretched and its deeper structural fibers are altered. Pregnancy combines rapid changes in body size with hormonal and genetic factors, so susceptibility differs from one person to another. The exact contribution of each factor cannot be predicted from the label on a bottle.
The frequency estimates are high but not identical because they come from different summaries. The background to the 2022 Cochrane review cites an estimated 50% to 90% frequency of striae gravidarum. The NHS, on a page reviewed in 2024, gives the more reader-friendly estimate of around eight in 10 pregnant women. These figures describe how common marks are; they do not mean that any individual will develop them or that a particular product caused them.
New marks may look pink, red, purple, or darker than surrounding skin before gradually fading. After delivery they commonly become less noticeable, yet fading is not the same as disappearance. A product used during pregnancy cannot guarantee a mark-free outcome, and a mark that appears despite careful moisturizing is not evidence that you applied the product incorrectly.

The most comprehensive evidence located for prevention is the Cochrane review updated in 2022. It included six trials with 800 women and assessed several topical preparations. The authors concluded: “We found no high-quality evidence to support the use of any of the topical preparations in the prevention of stretch marks during pregnancy.” The preparations studied included cocoa butter, olive oil, vitamin-E products, and hyaluronic-acid products.

One pooled comparison illustrates why a firm “oil works” or “cream works” conclusion would be misleading. Active topical preparations compared with placebo or no treatment produced a risk ratio of 0.74, with a 95% confidence interval of 0.53 to 1.03, across five trials involving 474 women. Because the confidence interval includes the possibility of no difference, this result does not establish reliable prevention. The evidence was also limited by the small number of trials and their differing products and methods.
Severity results were similarly uncertain. Two trials involving 255 women reported a standardized mean difference of −0.31, with a 95% confidence interval from −1.06 to 0.44. That range includes no clear difference, so it cannot support a claim that one preparation produces milder marks.
| Evidence point | What was found | How to use the information |
|---|---|---|
| Overall review | Six trials; 800 pregnant women; Cochrane update, 2022 | Useful as the broadest synthesis, but not proof that every product or formula has been tested. |
| Active topical preparations versus placebo or no treatment | RR 0.74 (95% CI 0.53–1.03); five trials; 474 women | The estimate is compatible with no meaningful preventive benefit. |
| Severity outcome | SMD −0.31 (95% CI −1.06 to 0.44); two trials; 255 women | No clear difference in how severe marks became. |
| Preparations represented | Cocoa butter, olive oil, vitamin-E products, and hyaluronic-acid products, among others | Inclusion in a trial shows that a product was studied, not that it worked. |
ACOG’s patient guidance reaches the same practical conclusion: “There is no proof that any of these treatments work.” It adds a more realistic benefit: “Using a heavy moisturizer may help keep your skin soft, but it will not help get rid of stretch marks.” The NHS likewise states that there is limited evidence about whether oils or creams prevent marks from appearing in the first place.

| Prevention evidence | No reliable advantage over cream; no topical format has high-quality proof of prevention. | No reliable advantage over oil; marketing language should not be treated as clinical evidence. |
| Texture | Slippery and often shiny; can leave residue on hands, clothing, or bedding. | Usually thicker or emulsion-based; often feels easier to spread and layer under clothing. |
| Moisturizing experience | Can reduce the sensation of dryness or tightness for some users. | Can soften and cushion dry skin; a heavy moisturizer is the comfort benefit ACOG describes. |

When an oil may be the more comfortable choice
An oil can make sense if you prefer a massage-like application, tolerate a greasy finish, and can keep the bottle and your hands from making the bathroom floor slippery. Use only enough to cover the area lightly; more product does not equal more protection. If fragrance makes you nauseated or irritates your skin, choose an unfragranced option rather than assuming an essential-oil blend is gentler.
When a cream or lotion may fit better
A cream is often easier when you need to dress immediately, apply product to a broad area, or dislike a slick residue. A simple, fragrance-free moisturizer can meet the realistic goal of reducing dryness without carrying a “stretch-mark cure” claim. Cocoa-butter lotion is a relevant product category because cocoa-butter preparations were included in pregnancy trials, but that inclusion does not prove that cocoa butter prevents marks.
How to choose a pregnancy-appropriate product
1. Start with the full ingredient list
Do not judge a product solely by its front label, color, or “pregnancy” wording. Check the complete list for retinoids and other unfamiliar actives. If an ingredient is not clearly identified, take the packaging to your prenatal clinician or pharmacist before using it regularly. This is especially important for concentrated blends, multi-active creams, and products purchased from sellers that do not provide a complete label.

The NHS states that retinoid creams “should not be used if you’re pregnant as they may harm your baby.” That warning covers products marketed with names such as retinol or tretinoin. Do not add a retinoid to an oil or cream routine to treat existing marks while pregnant, even if the product is sold without a prescription in your area. Ask your obstetric clinician about alternatives and about when, if ever, a retinoid would be appropriate after pregnancy and breastfeeding.

3. Prioritize tolerability
Pregnancy can make skin more reactive and can make strong smells difficult to tolerate. A product that stings, causes a rash, or makes you stop moisturizing is not a practical choice. Look for a texture you can spread without vigorous rubbing and a scent level you can live with. If you develop persistent redness, swelling, blistering, or intense itching, stop using the product and seek medical advice.
4. Treat “natural” and “clinical” as descriptions, not guarantees
Plant oils, vitamin E, cocoa butter, and laboratory-formulated creams can all be described in persuasive ways, but the Cochrane findings do not establish a dependable preventive effect for the tested categories. “Natural” does not mean risk-free, and “dermatologist tested” does not specify what outcome was tested. Look for transparent labeling rather than a promise that a mark will never appear.
A simple routine for comfort
- Apply after washing. Pat skin dry so it is comfortable but not dripping wet. This is a convenient time to spread either a cream or a small amount of oil.
- Use a thin, even layer. Cover areas that feel dry or tight, such as the abdomen, hips, breasts, or thighs. There is no evidence that an unusually thick layer improves prevention.
- Massage gently. Use comfortable pressure and stop if rubbing increases irritation. Massage can make application pleasant, but it has not been shown to overcome biological susceptibility to stretch marks.
- Reapply for dryness, not on a schedule promising prevention. Use the product when skin feels uncomfortable or after bathing. Consistency may help you maintain softness, but it should not be presented as a guarantee against marks.
- Keep the bathroom safe. Wipe spills promptly, wash oily hands before touching handles, and avoid walking barefoot on a slick floor.
- Record reactions. If a new rash appears, note the product and its ingredients so your clinician can help identify a trigger.
What products can and cannot do
They can help with comfort
Moisturizers can reduce the unpleasant feeling of dry, tight skin by adding a softening layer. This is the benefit ACOG supports. Comfort may make sleep, clothing, and ordinary movement easier, especially as the abdomen changes.
They cannot guarantee prevention
No oil or cream has demonstrated a reliable ability to stop pregnancy stretch marks across the available trials. The pooled risk estimate and severity analysis are uncertain, and the review authors found no high-quality evidence for topical prevention.

They cannot erase established marks
Once marks are present, a moisturizer may make the surface feel smoother, but it will not get rid of them. The NHS notes that marks often fade after birth but may not disappear completely. Be skeptical of before-and-after images that do not disclose lighting, time elapsed, skin tone, or other treatments.
Common mistakes and how to avoid them
- Switching products repeatedly. Buying a new formula every few weeks can increase irritation and expense without improving the evidence-based outlook.
- Using too many actives. Combining exfoliants, essential oils, and anti-aging ingredients makes it harder to identify a reaction and may not be appropriate during pregnancy.
- Assuming a trial proves a category. A cocoa-butter lotion being studied does not prove that every cocoa-butter formula works, nor that the studied formula prevented marks.
- Blaming yourself. Stretch marks can appear despite diligent care. Their appearance is not proof of neglect or of a failed personal routine.
- Ignoring a significant reaction. A severe rash, sudden widespread marks, or concern about a particular ingredient warrants advice from an obstetric clinician or dermatologist rather than experimentation with another product.
How to think about cost and marketing claims
There is no established price-to-prevention relationship. A basic moisturizer may provide the same practical softness as a premium “maternity” product, while a premium product may be worthwhile only if you prefer its texture, packaging, or scent and can use it without irritation. Judge the purchase by those personal factors, not by a claim that the formula changes the biology of stretch marks.
Words such as “prevents,” “stops,” “erases,” or “clinically proven” need context: what product, what comparison, how many participants, and what outcome? The Cochrane review’s findings mean that a product category can be relevant to research without having a proven preventive effect. If a seller cites a study, check whether it measured new marks, severity, comfort, or simply skin hydration.

When to contact your clinician
Contact your prenatal clinician or dermatologist if you develop a severe or rapidly worsening reaction, widespread sudden marks that concern you, or symptoms that extend beyond ordinary dryness. Bring the product container or a photograph of the ingredient list. Ask before using any retinoid, prescription cream, concentrated essential-oil blend, or unfamiliar active. Medical advice is also appropriate if itching is intense, affects sleep, or occurs with other symptoms of pregnancy complications.
A realistic decision
If you enjoy oils, use a pregnancy-appropriate oil for the sensory experience and softness. If you prefer a cream, choose a tolerable moisturizer and apply it consistently enough to keep skin comfortable. Neither choice has a demonstrated prevention advantage, and neither can remove marks that have already formed. The safest, most evidence-aligned approach is to avoid retinoids, screen ingredients, ignore guarantees, and treat moisturization as comfort care rather than insurance against stretch marks.
