Before you pay for an online or mail-order clear-aligner plan, get written answers to eight issues: who examined you, which X-rays were taken, who is legally responsible, how often a licensed clinician reviews progress, why aligners fit your diagnosis, what the quoted fee includes, how emergencies are handled, and what happens after the last tray. Clear aligners are prescription orthodontic medical devices, not a cosmetic subscription. In the United States, FDA guidance says a dentist or orthodontist should examine you because aligners do not work for everyone. A low-friction sign-up cannot replace diagnosis, informed consent, or supervised care.

The checklist below is designed for U.S. readers and reflects FDA, American Dental Association (ADA), and American Association of Orthodontists (AAO) guidance available on October 18, 2026. State licensing, teledentistry rules, insurance, refunds, and provider obligations can differ.

1. Are online clear aligners safe for your particular case?

Safety is not a property of the box of trays alone. It depends on the diagnosis, the health of your teeth and gums, the movements prescribed, and the supervision available when your mouth responds differently than expected.

The FDA classifies braces, clear dental aligners, and other orthodontic appliances as medical devices. Its consumer guidance says clear aligners may not work for everyone and that a dentist or orthodontist should perform a dental examination before recommending treatment. That means a scan, a payment page, or an app-generated preview is not by itself a clinical decision.

The ADA has warned that moving teeth without knowing the full condition of a patient’s mouth can contribute to bone loss, lost teeth, receding gums, bite problems, jaw pain, and other damaging, potentially permanent issues. Those are possibilities, not a prediction that every remote plan will cause harm; they explain why diagnosis and follow-up are central questions to ask.

Ask these safety questions first

  • Who examined my teeth and gums, and on what date?
  • What findings make me a suitable or unsuitable candidate?
  • Which problems would require treatment before scanning, such as decay, gum disease, a broken filling, or an impacted tooth?
  • What symptoms mean I should stop wearing a tray and obtain an urgent in-person examination?

If a seller cannot give a clear clinical answer, pause the purchase rather than treating the advertised preview as a diagnosis.

2. Will a clinician perform a complete examination and order appropriate X-rays?

Ask for the exact examination process, not merely whether you will “submit photos.” An appropriate evaluation can include an in-person examination, medical and dental history, photographs, digital scans or impressions, a bite assessment, and radiographs selected for your clinical needs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why photographs and scans are not enough

AAO guidance explains that X-rays can reveal jaw position, missing teeth, root shape and position, and the jaw-bone structure beneath the gums. Photographs and digital scans show the visible crowns and surfaces but cannot provide that same information. A plan that moves visible teeth without checking roots, supporting bone, or unerupted teeth can miss conditions that alter the safe direction or amount of movement.

Questions about imaging

  1. Will I have an in-person examination before treatment begins?
  2. Are panoramic or other clinically appropriate X-rays included, and who reviews them?
  3. Will the clinician assess my roots, jaw alignment, bone support, gums, bite, and missing or impacted teeth?
  4. Can I receive copies of my X-rays, photographs, scans, treatment plan, and consent documents?
  5. If I recently had dental X-rays, will the clinician decide whether they are adequate rather than automatically repeating or ignoring them?

There is no single X-ray set that is appropriate for every person. The important issue is that a licensed clinician chooses and interprets diagnostic records for your case and explains what they show.

3. Who is the doctor of record, and where are they licensed?

Ask for the responsible dentist or orthodontist’s full name, professional degree, license state, and a direct way to contact that clinician or the clinical team. “A dentist is involved” is not enough if you cannot identify the person making decisions.

Verify responsibility rather than a logo

  • Confirm the clinician’s license through the relevant state dental or orthodontic licensing authority.
  • Ask whether the clinician is responsible for diagnosis, prescribing each movement, reviewing progress, and changing or stopping treatment.
  • Find out which state’s law governs your care if the company, scanning site, and clinician are in different states.
  • Ask who keeps your records and how you obtain them if you cancel or the company stops operating.

AAO describes orthodontic treatment as a professional medical service, not simply a product or device. A named, reachable professional should be accountable for the service, not just available through a generic support inbox.

4. How often will your progress be monitored, and what triggers a change?

Every plan should explain how supervision works after the first tray. Monitoring may involve scheduled in-person visits, remote photo or video checks, or both. Ask for the interval, the reviewer’s credentials, the expected response time, and the findings that require a new scan, a pause, an in-person visit, or a revised prescription.

Get the monitoring workflow in writing

  1. Ask how frequently a licensed dentist or orthodontist reviews your progress, rather than how often an app sends reminders.
  2. Ask whether the same clinician reviews every check or whether cases are routed among providers.
  3. Ask how the team checks tracking, tooth mobility, gum condition, bite contacts, and tray fit.
  4. Ask what happens if a tray does not fit, a tooth stops tracking, or your bite changes.
  5. Ask how quickly a clinician responds to a message and where an in-person examination would occur.

Remote monitoring can be convenient, but it does not remove the need for escalation. A plan that promises an identical sequence regardless of findings is less responsive than one with explicit decision points.

5. Why are aligners appropriate instead of braces or another treatment?

Ask the clinician to explain the diagnosis and alternatives in plain language. Aligners can move many teeth, but the achievable movement depends on roots, bone, gum health, the bite, tooth shape, missing teeth, and how consistently trays are worn. Some cases are better managed with fixed braces, limited treatment, restorative work, periodontal care, or a combination.

Questions that expose the treatment scope

  • What is my bite problem or tooth-position problem called?
  • What movements are planned, and which movements are not predictable with this approach?
  • Could I need attachments, elastics, enamel reshaping, space creation, or other auxiliaries?
  • What happens if the planned movement is not tracking?
  • Would braces or another method offer a safer or more predictable result for my diagnosis?
  • Who made the decision that aligners are suitable, and can that clinician explain the alternatives?

Be cautious of a one-device promise that treats every case as a cosmetic front-teeth adjustment. Ask specifically whether the plan addresses your bite, not just whether the teeth look straighter in a rendered image.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. What exactly is included in the treatment plan?

Request a written treatment summary before paying. It should identify the planned stages, expected wear instructions, appointments or remote checks, and what happens when the initial sequence does not produce the intended result.

Items to confirm

  • Number and delivery schedule of trays.
  • Attachments, buttons, elastics, interproximal reduction, or other procedures.
  • Replacement trays for loss, damage, or a manufacturing problem.
  • Refinements when teeth do not track or the result is incomplete.
  • Shipping charges, taxes, scanning fees, and any in-person appointment fees.
  • Whether a general dentist must provide fillings, cleaning, gum treatment, or other care before or during orthodontics.

Do not interpret a short advertised sequence as a guaranteed fast cure. FDA consumer guidance states that there is no “faster” method of straightening teeth in the sense of bypassing biological limits. Teeth and supporting tissues respond over time, and a clinician may need to change the plan.

7. What will the quoted price really cover?

No authoritative source provides a universal average price for an online plan. Prices vary by country and state, diagnosis, treatment scope, supervision model, and what is bundled. Compare itemized totals rather than headline prices.

Cost item Question to ask
Diagnostics Are the examination, scans, photographs, and clinically appropriate X-rays included?
Active treatment Are all trays, attachments, elastics, and standard shipping included?
Refinements How many additional scans or trays are covered, and for how long?
Replacement trays What does a lost, cracked, or poorly fitting tray cost?
In-person care Are examinations, attachment visits, repairs, or emergency visits charged separately?
Retainers Which retainer is included, how long is it covered, and what are replacement prices?
Retreatment If teeth relapse or the result is incomplete, what service and fee apply?
Payment terms What are financing charges, cancellation rights, refunds, and the balance if you stop?
Records Can you obtain your records and transfer care without an extra administrative fee?

Ask for the total expected cost under two scenarios: successful completion with the initial trays and completion requiring refinements or an in-person intervention. Also ask what happens financially if the company closes, you move to another state, or a clinician determines treatment must stop.

8. What happens when something goes wrong?

Before starting, obtain an emergency pathway that names a licensed dental professional or a local practice. App support is not a substitute for an examination when symptoms suggest injury or infection.

Seek prompt professional help for

  • Significant or worsening pain.
  • Swelling, fever, pus, or other signs of infection.
  • A loose tooth or a tooth that feels newly mobile.
  • Cut, bleeding, or persistent injury to the gums, cheeks, or tongue.
  • A tray that cannot seat, cracks, or causes concentrated pressure.
  • A sudden bite change, jaw pain, or difficulty chewing.

Ask where the visit would occur, who pays for it, and whether the treating clinician can pause, remake, or abandon the current sequence. If you have swelling, severe pain, a loose tooth, gum injury, or a major bite change, contact a licensed dental professional promptly instead of waiting for an app response.

9. What dental work and daily hygiene are required?

Orthodontic treatment should not proceed over untreated disease. Ask whether you need a cleaning, fillings, gum treatment, replacement of a failing restoration, or other dental work before scanning. Confirm who provides that work and whether its cost is separate.

Daily-care questions

  • How many hours per day should trays be worn, and what should you do if you miss wear time?
  • How should trays be removed for meals, drinks, brushing, and flossing?
  • Which cleaning products are safe for the plastic, and which can scratch or distort it?
  • How will the clinician check for cavities, gum inflammation, or enamel damage during treatment?
  • Should you continue routine visits with your general dentist, and at what interval?

FDA guidance emphasizes careful cleaning because poor hygiene while wearing aligners can contribute to tooth decay and gum problems. Clear trays do not protect teeth from plaque; they can hold food and bacterial acids against tooth surfaces when cleaning is inadequate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

10. What is the retention plan after the last tray?

Finishing active movement is not the end of treatment. AAO guidance says retainers help maintain the result, and your plan should specify the retainer type, wear schedule, follow-up, coverage period, and replacement cost.

  • Powerful Pass-Through Charging: Supports up to 85W pass-through charging so you can power up your laptop while you use the hub. Note: Pass-through charging requires a charger (not included). Note: To achieve full power for iPad, we recommend using a 45W wall charger.

  • HD Display: Connect to the HDMI port to stream or mirror content to an external monitor in resolutions of up to 4K@30Hz. Note: The USB-C ports do not support video output.
  • What You Get: Anker 332 USB-C Hub (5-in-1), welcome guide, our worry-free 18-month warranty, and friendly customer service.

Ask for concrete retention terms

  1. Which retainer is included: removable, fixed, or both?
  2. When do you switch from full-time to nighttime wear, if that is the plan?
  3. How long does the provider cover a retainer that breaks, is lost, or no longer fits?
  4. What does a replacement cost, and how quickly can it be made?
  5. Who checks your bite and retention after active treatment ends?
  6. What happens if teeth move because a retainer is not worn or no longer fits?

Budget for retention as continuing care, not as an optional accessory. A protective case is useful for carrying a removable retainer, but it does not replace the clinician’s retention instructions or a properly fitting appliance.

11. Compare plans on clinical value, not just convenience

Use the same questions for every provider and record the answers. A comparison table makes missing information visible.

Comparison axis What to verify before purchase
Diagnosis In-person examination; appropriate X-rays; assessment of bite, roots, gums, jaw, and bone.
Clinical responsibility Named licensed clinician, state license, direct contact, and clear record custodian.
Supervision In-person visits or remote reviews, review frequency, response time, and escalation rules.
Treatment scope Aligners plus alternatives; attachments, elastics, refinements, and limits of movement.
Safety and emergencies Instructions for pain, gum injury, loose teeth, poor fit, infection, or bite changes.
Total cost Diagnostics, shipping, refinements, replacements, retainers, retreatment, refunds, and financing.
Long-term outcome Retainer type, wear schedule, follow-up, replacement policy, and relapse support.
Oral health Required cleaning, fillings, periodontal care, and continuing general-dental visits.

Give more weight to a complete diagnostic and supervision plan than to a lower introductory price or a shorter projected sequence. Convenience is valuable only when the clinical process remains accountable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

12. A practical pre-purchase process

Use this sequence before authorizing a charge or signing financing documents.

  1. Collect your records. Ask your current dentist for recent examination notes and X-rays, then let the treating clinician decide whether they are adequate.
  2. Confirm the examination. Identify who performs an in-person evaluation and what it covers.
  3. Verify the clinician. Record the doctor of record, license state, direct contact method, and record custodian.
  4. Request the diagnosis and alternatives. Ask why aligners fit your case and what braces or other options would change.
  5. Read the treatment scope. Check wear instructions, attachments, elastics, refinements, and criteria for changing the plan.
  6. Map supervision. Write down review frequency, response times, appointment locations, and escalation rules.
  7. Build the full-cost estimate. Include diagnostics, dental work, shipping, replacements, refinements, retainers, retreatment, financing, and taxes or fees.
  8. Read exit terms. Check cancellation, refund, records-transfer, company-closure, and balance-due provisions.
  9. Get emergency instructions. Know exactly where to go for pain, swelling, a loose tooth, gum injury, poor fit, or a bite change.
  10. Plan retention and routine care. Confirm retainer coverage and continue visits with a general dentist.

13. Red flags that justify walking away

  • The plan starts from a home impression or photographs with no meaningful clinical examination.
  • No appropriate X-rays are taken or reviewed when the clinician believes they are needed.
  • You cannot identify the doctor of record or verify a current state license.
  • Support staff, rather than a licensed clinician, answer clinical questions without escalation.
  • The seller promises that every case is suitable, requires no dental work, or will finish unusually fast.
  • The contract does not explain refinements, replacement trays, retainers, emergencies, refunds, or records.
  • You are discouraged from seeing your regular dentist or from obtaining your diagnostic records.
  • The company cannot say who treats you locally if pain, swelling, gum injury, a loose tooth, or a bite change occurs.

AAO consumer guidance warns that mail-order or online aligners can carry significant risks when treatment begins without an in-person examination, X-rays, and ongoing supervision from a licensed orthodontist. Its FDA policy update also states that the FDA has not authorized clear aligners sold directly to consumers without a prescription and supervision by a dentist or orthodontist.

14. The questions to copy into an email or consultation

Use this condensed list when a provider gives you a sales call or digital checkout page:

  • Who is the doctor of record, where are they licensed, and how do I contact them directly?
  • Who performs my in-person examination, and what does it include?
  • Which X-rays will be taken or reviewed, and who interprets them?
  • Will you assess my jaw alignment, bite, roots, gums, bone support, missing teeth, photographs, and scans?
  • Why are aligners appropriate instead of braces or another option?
  • What dental treatment must I complete before scanning?
  • How often does a licensed clinician review progress, and what findings change the plan?
  • What are the emergency instructions for pain, swelling, gum injury, a loose tooth, poor fit, or a bite change?
  • Does the fee include diagnostics, attachments, elastics, refinements, replacements, shipping, retainers, follow-up, and retreatment?
  • What are the cancellation, refund, financing, records-transfer, and company-closure terms?
  • What retainer is included, what is the wear schedule, how long is coverage, and what do replacements cost?

If the answers are vague, unavailable in writing, or impossible to verify, do not treat the plan as ready for purchase. The safest online experience still begins with a licensed professional who can examine you, explain alternatives, monitor biological changes, and provide hands-on care when remote treatment is not enough.
