The safest way to monitor an AI agent with signing authority is to limit what it can sign before you collect telemetry. Bind the agent to a dedicated wallet, enforce an explicit policy for actions and value, verify its identity and risk continuously, simulate every transaction or UserOperation, and keep a tamper-evident record from prompt through settlement. Monitoring that only displays transactions after a signature can explain a loss but cannot reliably prevent one.
An AI agent that can sign blockchain transactions combines two difficult security problems: probabilistic software that can be manipulated, and irreversible financial operations. A complete monitoring system therefore has to observe more than the wallet. It must connect the agent’s prompts and tool calls to its policy decision, signature request, RPC path, transaction execution, and resulting state change.
Define the trust boundary before you build alerts
Start with an inventory of every component that can influence a transaction or hide evidence about it:
- Agent runtime: model, prompt templates, memory, tools, plug-ins, and orchestration code.
- Wallet and key custody: signing key, smart-account owner, hardware or remote signer, session keys, and recovery credentials.
- Policy and approval services: allowlists, limits, human-approval queues, emergency revocation, and policy storage.
- Blockchain access: RPC endpoints, providers, relayers, bundlers, mempools, and chain-selection logic.
- Execution targets: token contracts, routers, bridges, lending protocols, vaults, and arbitrary destinations.
- Infrastructure: cloud hosts, containers, CI/CD, secrets managers, monitoring systems, and log storage.
- Response controls: pause switches, credential rotation, destination blocks, limit changes, and independent signers.
Threat modeling should include prompt injection, malicious tool output, compromised dependencies, stolen credentials, a hostile or altered RPC response, a manipulated host, contract upgrades, oracle failure, and an agent that is behaving as designed but has been given an unsafe objective. The same wallet may be exposed to several of these paths at once, so a dashboard limited to transaction hashes is not a sufficient control.

Constrain authority before monitoring it
Give each agent the smallest authority that can perform its job. A useful delegation record binds a specific agent identifier to a specific wallet and chain, then states exactly what the wallet may do. ERC-8196 describes a model with policy fields that can be enforced by the agent host, wallet interface, and relayer as well as by on-chain components.
Use a dedicated wallet and explicit action scope
Do not share a treasury key with an autonomous process. Use a wallet or smart account dedicated to one agent and one operating purpose. The policy should enumerate permitted action types, approved contract addresses, permitted assets, and denied destinations. If an agent only rebalances a known set of pools, a request to call an unrelated contract should fail closed rather than proceed to a human review queue by default.
Set value, time, and replay limits
Record a maximum value per transaction and a daily or rolling-period total. Define the validity start and expiry time for every delegation, and require a unique nonce for every authorization. A stale delegation, reused nonce, or request outside the validity window is a policy failure even if the signature itself is cryptographically valid. Keep the policy hash with each decision so an auditor can tell which limits were active.
Keep the owner in control
The wallet owner or an independent governance key needs a tested revoke path. Revocation should be possible without asking the agent to cooperate. For larger balances, route high-value actions to a separate signer or a multi-party approval process. ERC-8196 also recommends multiple independent hosts for high-value agents, reducing the chance that one manipulated host can fabricate requests or suppress a revocation.
Every denied action should produce an auditable reason. Useful reason codes include PolicyViolation, ValueExceedsLimit, InvalidSignature, and an expiry error, matching the failure categories described by ERC-8196.

Reference architecture for continuous monitoring
1. Identity and delegation service
Resolve the agent’s canonical registration metadata before allowing it to act. Store the agent identifier, wallet address, owner or operator, chain ID, policy hash, delegation expiry, and verification-provider result with the request. ERC-8126 calls for risk checks covering the wallet, contract deployment, code vulnerabilities, endpoint security, and relevant threat intelligence. A verification result should have a timestamp and freshness limit; a one-time approval is not adequate for an agent that can continue acting after its code, endpoint, or wallet history changes.
Reject a request when metadata cannot be resolved, the wallet does not match the registered delegation, the chain is not permitted, or the risk score exceeds the policy threshold. Preserve the provider name, score, decision, and evidence reference so a later reviewer can distinguish an identity failure from a policy failure.

2. Policy decision point
Put a policy decision point between the agent and the signer. It should evaluate every proposed transaction or UserOperation, not just large transfers. Check action type, target contract, asset, amount, gas constraints, time window, nonce uniqueness, cumulative daily value, and emergency-revocation state. Return a deterministic decision and reason, then write that decision before any signature is created.
Keep policy evaluation separate from the agent process. If the model, tool chain, or host is compromised, it must not be able to rewrite its own allowlist or limits. Version policies, sign policy updates, and require the owner-controlled path for changes.
3. Pre-signing simulation and intent checks
Simulate the transaction or UserOperation against a recent state snapshot before asking for a signature. Decode the requested intent and compare the recipient, asset, amount, slippage, gas, contract method, and expected events with the policy and the agent’s stated task. Block or escalate if simulation reverts, produces an unexpected recipient or token movement, exceeds a limit after fees, or depends on a state change that is no longer present.

Present the decoded operation to the signer or approval service in human-readable form. A valid signature only proves that the signer approved the bytes it received. The Ethereum Foundation has warned that blind signing and changing state can still produce an economic result that conflicts with the user’s goal or a standing safety policy. Simulation does not eliminate that risk, but it exposes many dangerous outcomes before authorization.

Ethereum does not provide a general on-chain mechanism for arbitrary code to inspect every net state change and event across a transaction. Use post-execution assertions as a second control.
4. Runtime telemetry and anomaly detection
Emit structured events for prompts, tool calls, retrieved data, policy decisions, signatures, RPC requests and errors, nonce gaps, retries, rejected actions, gas changes, destination changes, and settlement results. Correlate each event with the agent identifier, wallet, host, chain, transaction hash, and policy hash.
Baseline the agent’s normal volume, timing, assets, contracts, gas usage, and transfer patterns. Alert on rapid bursts, activity outside its usual schedule, a sudden increase in failed calls, a new destination, a new contract method, rapid forwarding after receipt, unusual token approvals, nonce jumps, or a gas-price spike. Alerts should carry enough context to make a decision quickly: decoded intent, simulation result, current limits, risk score, and the exact containment action available.
5. Tamper-evident audit storage
Application logs alone are not an adequate audit trail. OWASP’s AML Sanctions AI Agent Payments guidance specifically warns against relying on console output or syslog as the sole record. Sign requests and responses, chain entries with hashes, and store the records outside storage controlled by the agent or its host.

At minimum, an audit entry should contain the agent identity, wallet, tool invoked, hashed arguments, result, timestamp, nonce, policy or delegation identifier, transaction hash when available, and the previous-entry hash. Alert immediately when the hash chain breaks, a signature does not verify, a timestamp moves backward, or an expected event is missing. Retain records for the period required by the applicable legal, contractual, and operational environment, and support machine-readable export for incident review.
6. Response and containment controller
Automate the first safe action instead of waiting for an analyst to interpret a dashboard. Depending on severity, the controller can pause the agent, revoke its delegation, quarantine or rotate credentials, block a destination, lower limits, require a human approval, or route the next operation to an independent signer. Make the pause and revoke paths available when the primary host or RPC provider is unavailable.
Implement the monitoring system step by step
- Map authority and data flows. Document every wallet, key, smart account, host, model, tool, RPC endpoint, relayer, contract, and person that can influence a transaction. Mark which components can alter policy and which can only observe.
- Create the delegation record. Assign an agent identifier, wallet address, chain ID, owner, expiry, policy version, and verification-provider requirement. Store the policy hash in a system the agent cannot modify.
- Write the allowlist and limits. Enumerate action types, approved and denied contracts, assets, per-transaction limits, cumulative daily limits, validity windows, gas rules, and nonce handling. Decide which conditions require a human or independent signer.
- Isolate signing. Keep keys in a dedicated signer or smart-account control plane. The agent should submit an unsigned intent; it should not receive unrestricted private-key material or be able to bypass the policy service.
- Add continuous verification. Resolve registration metadata and refresh wallet, contract, code, endpoint, and threat-intelligence checks on a defined schedule and before sensitive actions. Fail closed when the result is stale or outside the allowed risk range.
- Simulate and decode. Run a transaction or UserOperation simulation against a recent state, decode the call, compare expected and actual recipients and assets, and assert limits for amount, slippage, gas, and events. Send only approved requests to the signer.
- Instrument the full path. Emit correlated events from prompt receipt through tool execution, policy decision, signature, RPC submission, inclusion, confirmation, and final state. Include errors and rejected attempts, not only successful transactions.
- Protect and export the audit trail. Sign records, maintain a hash chain, replicate logs outside the agent host, alert on integrity failures, and test a machine-readable export before an incident occurs.
- Configure response automation. Define thresholds and actions for pause, revoke, credential quarantine, destination blocking, limit reduction, human approval, and independent signing. Record who can release a pause and what evidence is required.
- Exercise the controls. Run simulations of prompt injection, a stolen credential, a changed contract, an unavailable RPC provider, a nonce race, a malicious destination, and a broken log chain. Measure whether the system blocks, alerts, preserves evidence, and recovers as designed.
What to log for every attempted action
| Event group | Required fields | Why it matters |
|---|---|---|
| Identity | Agent ID, wallet, owner/operator, chain ID, host ID, registration metadata version, verification provider, score, decision time, freshness deadline | Shows who was authorized and whether the identity check was current. |
| Intent and tools | Prompt or task identifier, tool name, hashed arguments, retrieved data identifiers, model/runtime version, result hash | Connects an on-chain action to the instruction and software path that produced it without exposing unnecessary secrets. |
| Policy | Policy hash, action type, target, asset, amount, gas constraints, validity window, nonce, cumulative value, decision, reason code | Allows an auditor to reproduce the authorization decision. |
| Simulation | State-block reference, decoded call, expected events, predicted transfers, slippage, revert data, simulation result | Captures what was known before signing and identifies state-dependent risk. |
| Signing and transport | Request hash, signer identity, signature verification result, RPC endpoint, relayer or bundler, submission time, retry count, errors | Exposes blind-signing, transport, replay, and infrastructure problems. |
| Settlement | Transaction hash, block and chain, gas used, status, emitted events, token and native-asset deltas, final assertions | Proves whether execution matched the approved intent. |
| Response | Alert rule, severity, operator, automatic action, revoke or pause identifier, recovery approval, release time | Preserves the chain of custody for containment and recovery. |
Hash sensitive arguments before central storage when the raw value is not needed for investigation, but keep a controlled, separately protected copy when legal or operational requirements demand reconstruction. Never put private keys, seed phrases, or bearer credentials into ordinary telemetry.
Detection rules that catch a compromised or misdirected agent
Identity and authorization failures
- Wallet address, chain ID, owner, or delegation metadata does not match the registered record.
- Verification data is stale, cannot be resolved, or exceeds the policy’s risk threshold.
- A policy hash changes without an owner-authorized update.
- A nonce is reused, skipped unexpectedly, or appears from two hosts at once.
- A request arrives after delegation expiry or before its validity window.
Transaction and economic anomalies
- Recipient, contract, method, asset, amount, slippage, or expected event differs from the decoded task.
- Simulation reverts, predicts an unexpected approval, or shows a transfer to a new destination.
- Per-transaction or daily value limits are exceeded after fees or cumulative activity is recalculated.
- Gas usage or gas price departs sharply from the agent’s baseline without a policy-approved reason.
- Funds are forwarded rapidly after receipt, or a normally inactive asset becomes active.
Runtime and infrastructure anomalies
- Prompt or tool-call volume changes abruptly, especially when accompanied by new instructions to bypass controls.
- RPC errors, retries, endpoint changes, or inconsistent chain responses increase around signing attempts.
- Multiple hosts submit conflicting requests for the same wallet.
- Expected audit entries are delayed or missing, signatures fail verification, or the hash chain breaks.
- A contract, endpoint, dependency, or host changes outside its approved deployment process.
Use severity tiers. A mismatched recipient or broken audit chain should normally pause signing immediately. A modest volume change may create an alert for review, while a known nonce race can trigger a short hold and state refresh rather than a permanent revoke.

Compare monitoring designs by prevention, not dashboard polish
| Design | Custody and key isolation | Policy and simulation | Outcome checks and audit | Typical use |
|---|---|---|---|---|
| Post-signing dashboard | Often leaves the agent close to the key; custody details are not necessarily enforced. | Usually displays submitted transactions without blocking unauthorized calls or simulating intent. | Can provide visibility after settlement, but prevention and containment are weak. | Low-value experiments where signing risk is deliberately accepted. |
| Signing gateway | Separates the agent from the signer and can enforce a dedicated wallet. | Supports allowlists, value caps, nonce checks, expiry, and human approval; simulation depth varies. | Centralizes signed requests and responses; add external storage and post-execution assertions. | Operational agents with bounded transaction types. |
| Policy gateway with simulation | Uses isolated custody, owner revocation, and often an independent signer. | Evaluates every request, decodes intent, simulates against recent state, and blocks mismatches. | Correlates pre-sign and post-settlement events with tamper-evident records. | Agents moving meaningful value or interacting with contracts. |
| Defense-in-depth control plane | Dedicated wallet, multiple independent hosts, credential quarantine, and an out-of-band revoke path. | Continuous ERC-8126-style verification, ERC-8196-style policy enforcement, simulation, risk scoring, and human escalation. | Full prompt-to-state telemetry, hash-chained export, automated containment, and tested recovery. | High-value or business-critical autonomous operations. |
Use the comparison axes consistently: custody and key isolation, policy granularity, pre-execution verification and simulation, post-execution outcome checks, audit integrity and exportability, alert quality and response speed, coverage of RPC/cloud/relayer infrastructure, and interoperability with ERC-8004, ERC-8126, and ERC-8196-style identity and verification. A polished interface cannot compensate for a design that observes only after signing.

Incident-response playbooks
Unexpected destination or asset movement
- Pause new signing and preserve the relevant prompt, tool, policy, simulation, signature, RPC, and settlement records.
- Compare the approved intent with the decoded call and post-execution deltas; determine whether the mismatch originated in the agent, policy service, signer, RPC path, or contract.
- Revoke the delegation, quarantine credentials, and block the destination or contract where technically possible.
- Check other hosts, wallets, nonces, allowances, and pending transactions for related activity.
- Require an independent review before rotating credentials, lowering the pause, or restoring any limit.
Compromised host or endpoint
Take the affected host and endpoint out of the signing quorum, switch to an independent verified host or RPC path, and invalidate sessions created by the suspect environment. Compare its signed records with the external audit copy. If records disagree, treat the gap as an incident rather than assuming the more convenient version is correct.
Contract or state changed after simulation
Stop the request, refresh state, and rerun identity, risk, policy, and simulation checks. If the contract was upgraded, its code hash or verified metadata changed, or an oracle moved outside its allowed range, require an explicit policy update and owner approval before resuming. Never convert a failed simulation into a blind signature merely to avoid a missed opportunity.
Measure whether monitoring is working
Track control effectiveness rather than vanity dashboard counts. Useful measures include:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Percentage of signing attempts with a fresh identity and risk result.
- Percentage with a successful simulation, decoded intent, and recorded state-block reference.
- Time from anomalous request to automatic pause or revoke.
- Rate of rejected requests by reason code, including expired delegations and value-limit failures.
- Coverage of prompts, tool calls, RPC paths, relayers, hosts, and settlement events in the audit chain.
- Time to export a complete, verifiable incident record.
- Results of replay, nonce-race, prompt-injection, RPC-failure, and log-tampering exercises.
Review false positives with the policy owner, but do not weaken a control merely because the agent is inconveniently blocked. Change the allowlist, limits, or escalation path deliberately, sign the new policy, and record why the change was made.
What current standards and evidence do—and do not—establish
ERC-8196 provides a useful vocabulary for delegations, limits, validity, nonces, verification failures, and off-chain enforcement. ERC-8126 emphasizes canonical agent metadata and continuing risk checks. OWASP guidance adds inspectability, traceability, instrumentation, runtime control, and an audit trail that cannot be replaced by ordinary application logging. ERC-8004-style identity and reputation mechanisms can help interoperability, but they do not remove the need for local policy, simulation, custody controls, and incident response.
A 2026 survey reports reviewing 317 relevant works on autonomous agents on blockchains. That is evidence of an active research field, not a reliable estimate of industry losses or incident frequency. No authoritative industry-wide statistic establishes how often AI agents signing blockchain transactions lose funds, so operational decisions should be based on the value at risk, the agent’s authority, and the strength of the controls you can verify.
The practical boundary is clear: monitoring can make actions attributable, constrain authority, expose suspicious intent, and stop many failures before settlement. It cannot make arbitrary smart-contract behavior predictable, restore funds after an irreversible transfer, or replace an owner-controlled revoke path.
